【Objective】To improve the adaptation of Plat-Ⅱceramic crown.【Method】The Plat-Ⅱcastable ceramic crowns were made by three methods,namely routine method,die spacer technique,and the model investing method.24 crowns were made by these three methods individually and adhered to the corresponding model die with zinc phosphate to measure the adhesive thickness and compare their adaptation.【Result】The average adhesive thickness at different parts of crown of these three groups were 52 μm、42 μm、30 μm and the margins discrepence were 63 μm、59 μm、42 μm,respectively.There is statistically difference in both the adhesive thickness and the margin discrepence with these three methods (P<0.01).【Conclusion】The crowns made from the die spacer technique and the model investing method could improve the adaptation significantly.

Objective To explore the relationship between plasma Tumor necrosis factor-alpha(TNF-α),Interleukin-6(IL-6) levels and brain edema caused by hypertensive intracerebral henorrhage.Methods 62 patients with hypertensive intrscerebral hemorrhage( the observation group) and 50 healthy persons( the control group) were selected.The expression of plasma TNF-α and IL-6 were determined by ELISA pre-therapy and 1d,3d,7d,14d after treatment in two groups;The volume of cerebral edema was measured by CT.The relationship between plasma TNF-α,IL-6 levels and brain edema caused by hypertensive intracerebral hemorrhage were analyzed.Results Before treatment,the levels of TNF-α and IL-6 in the observation group were( 15.62 ±9.49)μg/L and (67.47 ±6.31 )ng/L,which were significandy higher than(8.28 ± 3.36) μg/L and(31.02 ± 3.51 ) ng/L of the control group( t =9.17,64.28,P =0.01 ),and Spearman analysis showed that the levels of TNF-α and IL-6 were positively correlated with the volume of cerebral edema(r=0.934,P=0.02;r=0.922,P =0.026).Conclusion There was an up-regulation of TNF-α and IL-6 levels in the plasma of patients with hypertensive intracerebral hemorrhage.TNF-α and IL-6 may promote the formation of cerebral edema during the course of hypertensive intracerebral hemorrhage.

A guinea pig model of asthma was formed with ovalbumin.Every guinea pig of Mycobacterium vaccae vaccine group was intramuscularly injected with 22.5 ?g Mycobacterium vaccae vaccine 10 days before ovalbumin immunization.mRNA expressions of IL-4,IL-5 and IFN-? in lung tissue,total IgE and ovalbumin-specific IgE in serum were determined. Objective To investigate the application effects of quantitative and qualitative fecal occult blood (FOB) reagent in colorectal cancer screening.Methods With the platform of colorectal cancer screening for early diagnosis and treatment,the original screening methods and process were not changed,individuals underwent colorectal cancer screening,aged from 40 to 74 years old,from four villages of Haining City in Zhejiang Province were selected as study objects.Initial screening included medical history questionnaire and FOB test.The first stool samples were examined both with quantitative and qualitative FOB test,one week later the second samples were detected with qualitative FOB test.History positive or once of any type of reagent positive was considered as initial screening positive.The individuals with initial screening positive received fine screening (colon endoscope examination).The effects evaluation included the positive rate of FOB,intestinal lesion detection rate,sensitivity,specificity and Youden index.The health economic analysis was calculated.Chi-square test was performed for comparisons between the two groups.Results A total of 8 427 history survey were completed,the positive rate of history was 8.48％(715/8 427) ; 6 644 underwent quantitative detection of FOB and the positive rate was 4.44 ％ (295/6 644) ; qulitative test was conducted in 12 633 samples were conducted and the positive rate was 2.56 ％ (324/12 633),the difference was statistically significant (x2 =49.27,P＜ 0.01).A total of 854 individuals with initial screening positive underwent electronic colonoscopy examination,the total intestinal lesion detection rate was 22.72％ (194/854),of which the intestinal lesion detection rate of quantitative group was 34.55％ (66/191),and that of qualitative group was 17.73％ (36/203),the difference was statistically significant (x2 =14.53,P＜0.01).The detection rate of both quantitative and qualitative test positive was 43.75 ％ (14/32).The colorectal cancer detection rates of quantitative and qualitative FOB test were 3.66％ (7/191) and 0.99％ (2/203),advanced adenoma detection rates were 8.38 ％ (16/191) and 5.91％,there were no significant differences (x2 =2.08 and 0.96,both P＞0.05).The sensitivity of quantitative and qualitative FOB test in advanced adenoma and cancer detection rate was 45.10％ and 27.45％,respectively.The average cost of quantitative and qualitative FOB test in intestinal lesions detection was 5 874.50 and 1 432.14 yuan.Conclusion The effects of quantitative FOB test in colorectal cancer screening was better than that of qualitative reagent,however on the cost of screening,the former was 4.10 times of the latter.

Objective To study the method of selecting appropriate exposure parameter of AOP mode and reducing the radiation dose effective with the control of image quality .Methods The unique mammary phantom was mammography in three different AOP mode(DOSE mode ,STD mode and CNT mode) and compared the radiation dose .150 sample was selected randomly .50 sample each mode ,and recorded the radiation dose .All image reach the diagnostic standard .The mean dose of the each mode were computed and compared with others .Results whatever the phantom mammography and the sample mammography ,the mean dose in DOSE mode was lowest ,higher in STD mode ,and highest in CNT mode .The number of fiber cords ,mass and calcification fit the standard of quality control .The image of sample aslo fit the diagnostic standard .Conclusion In daily work ,we can lower the dose choosing appropriate mode according to the thickness and density of mammary .DOSE mode prior ,STD mode advocate limit to CNT mode .

OBJECTIVE To investigate pathogens in neonatal septicemia and the antimicrobial resistance of these bacteria in recent years to guide the clinical treatment.METHODS The data of 76 neonatal septicemia confirmed by hemoculture from Jan 2005 to Jan 2008 in our hospital were analyzed retrospectively.RESULTS Coagulase-negative Staphylococcus(CNS) were the main pathogens,among which their were 47.36% of S.epidermidis,10.53% of S.haemolyticus,9.21% of S.aureus,and the others of 7.88% Escherichia coli.Bacteria were highly resistant to penicillin,ampicillin and erythromycin.Vancomycin,imipenem,ciprofloxacin and amikacin were the most sensitive drugs.CONCLUSIONS Gram-positive cocci are main pathogens in neonatal septicemia,in which the CNS are the most common ones.To choose sensitive antibiotics bases on the drug sensitivity tests may decrease the occurrence of drug resistance to bacteria and increase the clinical curative effects.

Objective To discuss the value of ABCD2 score in predicting medium and long-term prognosis in patients with ischemic stroke.Methods 184 patients with ischemic stroke admitted to department of neurology from March 2014 to December 2014 were selected as the research object.According to the ABCD2 score points prior to admission, the patients were divided into 3 groups: 40 cases in the low risk group, 76 in the moderate group and 68 in the high risk group based on whether or to what extent he was able to do self care.The phone call reviews were made for the clinical symptoms of patients discharged from the hospital after six months..According to the modified Rankin scale classification standard, the patients discharged from the hospital after six months were divided into 2 groups: 126 cases in the self-care group and 56 cases in care-needing group.The groups were compared in terms of conditions.Results The self-care rate in the low risk group was 90.00%, the moderate group 77.63% and the high-risk group 45.59%.The care needing rates in the three groups was 5%, 19.74% and 48.53%, respectively.The care needing rate in the low risk group was obviously lower than that in the moderate-risk group and the high risk group (P<0.05).The care needing rate in the moderate risk group was obviously lower than that in the high risk group (P< 0.05).The self-care rates in the low risk group and the moderate group were higher than that in the high risk group (P<0.05).Conclusion The ABCD2 score has a higher predictive value for medium and long-term prognosis in patients with ischemic stroke.

Objective To research the value of using constant circling individual training program to improve surgery clinical skills of clinical medical specialty interns.Method 42 interns were divided into 3 groups:A(excellent),B(medium) and C(poor) according to their scores in entrance clinical skill test and each group would perform respective training program including conventional training,unified intensive training and intensive training in person.Their performance would be scored six times during the whole process of training.Results Compared with the final scores with beginning scores,the number of students scored A increased significantly meanwhile the number of students scored C decreased obviously.Conclusion The constant circling individual training program combined with effective unified test can benefit improving the surgery clinical skills of the interns of clinical medicine.

Purpose Ovarian dysgerminoma has no specific symptoms or physical signs, which is thus prone to misdiagnosis before treatment. This paper aims to investigate CT and MRI clinical features of ovarian dysgerminoma, so as to provide an accurate basis for its clinical treatment and also improve patients'' life, quality.Materials and MethodsThe CT and MRI data of 13 patients with ovarian dysgerminoma proved by pathology were retrospectively analyzed. Three patients underwent plain and enhanced CT scan, 6 took plain and enhanced MRI scan on T1WI and T2WI with different angles of views, and 4 other patients were performed both CT and MRI scan.Results①Twelve patients had single lesions, and 1 was bilateral multiple ones. The diameter of the lesions ranged from 5.5 cm to 16.5 cm, with mean of (9.5±3.8) cm; in terms of pathological type, 11 belonged to single type and 2 were mixed.②The CT results showed that 6 were solid mass and 1 was cystic. On enhanced CT scan, 6 cases showed heterogeneous light-to-moderate enhancement, 4 had tumors with significantly enhanced blood vessels, and 1 showed heterogeneous significant enhancement at arterial phase; all cases presented continuous enhancement at venous phase.③The MRI manifestations demonstrated 8 were with solid mass, which displayed iso- or slightly hypointense signal on T1WI, and iso- or slightly hyperintense signal on T2WI. The enhanced scan showed that 5 cases had heterogeneous mild enhancement, 3 had moderate enhancement, and 4 were seen with tortuous tumor vessels at arterial phase; all displayed continuous enhancement at venous phase. In the one case of cystic mass, solid component had moderate enhancement at arterial phase and continuous enhancement at venous phase. In another case of multilocular cystic mass, the tumor separate and cyst were significantly enhanced at arterial phase, and were continuous enhanced with hyperintense at venous phase.Conclusion Ovarian dysgerminoma tumors present features such as large lesions with clear boundaries. Tumors of single type are likely to be solid mass with central necrosis; the plain CT scan shows isodensity; the plain MRI scan mainly shows isodensity on T1WI, and iso- or slight hyperintensity on T2WI; the enhanced scan presents slight to moderate enhancement. Tumors of mixed type demonstrate cystic mass; the solid and separate parts are significantly enhanced. The comprehensive analysis of imaging features of ovarian neoplasms could help to make a correct diagnosis before surgery.

Objective:To investigate the expression of miR-203a in bladder cancer (BC) cell lines (RT-112, T24, 5637, UM-UC-3) and evaluate the effects on BC cell proliferation and radiosensitivity.Methods:Mir-203a mimics, mir-203a inhibitor, CDK6 siRNA, CDK6 expression plasmid and corresponding negative controls were transfected into BC cells. Quantitative real-time PCR was used to detect the expression of miR-203a in BC cell lines and human bladder epithelial immortalized cell line (SV-HUC-1). CCK8 assay was used to investigate the regulation of miR-203a and cyclin-dependent kinases 6(CDK6) on the proliferation of BC cells. Colony formation assay was performed to assess the effect of miR-203a and CDK6 on the radiosensitivity of BC cells. The target gene of miR-203a was confirmed by luciferase reporter assay. The effect of miR-203a on CDK6 protein expression was detected by Western blot. Multi-group comparison was performed by one-way ANOVA and two-group comparison was conducted by t-test. Results:Compared with the SV-HUC-1 cells, the expression levels of miR-203a in RT-112, T24, 5637 and UM-UC-3 cells were significantly down-regulated (all P<0.05). Compared with NC group, overexpression of miR-203a significantly inhibited the proliferation of BC cells, whereas knockdown of miR-203a significantly promoted the proliferation of BC cells (both P<0.05). Compared with NC group, overexpression of miR-203a significantly increased the sensitivity of BC cells to radiotherapy, whereas knockdown of miR-203a significantly weakened the sensitivity of BC cells to radiotherapy (both P<0.05). CDK6 was the target of miR-203a. Compared with NC group, overexpression of miR-203a significantly down-regulated the expression level of CDK6 protein, whereas knockdown of miR-203a significantly up-regulated the expression level of CDK6 protein (both P<0.05). After overexpression of CDK6 in T24 and UM-UC-3 cells transfected with miR-203a mimics, the cell proliferation ability was significantly increased, whereas the sensitivity to radiotherapy was significantly decreased compared with mir-203a mimics (both P<0.05). After CDK6 was silenced in RT-112 and 5637 cells transfected with miR-203a inhibitor, the proliferation ability of cells was significantly decreased, whereas the sensitivity to radiotherapy was remarkably increased compared with miR-203a inhibitor group (both P<0.05). Conclusion:miR-203a can serves as a tumor suppressor gene to inhibit the proliferation of BC cells and enhance the radiosensitivity of BC cells.
