5.Preoperative evaluation of left gastric artery by 64-slice spiral CT angiography in gastric cancer patients

ObjectiveTo delineate the anatomy of left gastric artery (LGA) by CT imaging before radical gastrectomy in gastric cancer patients to facilitate intraoperative N07 lymph node dissection.MethodsPreoperative 64 spiral CT angiography (CTA) was applied to observe the origin and course of LGA in gastric cancer patients.ResultsThere were 731 gastric cancer patients undergoing preoperative image evaluation with LGA origin identified from celiac axis in 635 cases. Among those, LGA originated respectively at the crotch of splenic artery and common hepatic artery in 176 cases, from the proximal 1/3 in 292 cases, middle 1/3 in 135 cases and distal 1/3 in 32 cases. There were 9, 28, 27, 4, 4, 1, 4 and 1 cases in whom LGA originates respectively at the angle between celiac trunk and abdominal aorta, from the abdominal aorta, arteria gastrolienalis, splenic artery, common hepatic artery, left hepatic artery, artery gastrohepatica and superior mensentaric artery. LGA has not been observed in 15 cases and the origination of LGA could not be delineated in 3 cases. ConclusionsBeing familiar with the anatomy of LGA as visualized by preoperative CTA in gastric cancer patient, facilitates lymph node dissection around the LGA and decreases iatrogenic injury to vital blood vessels.
6.Comparison of standard-dose and low-dose scanning with 16-MDCT for urinary calculi

Objective To explore the feasibility to diagnose ureterolithiasis by 16-multidetector spiral computed tomography (16-MDCT) at different low doses based on body mass index (BMI).Methods A total of one hundred patients from 2009 Sep to 2010 Feb suspected of ureterolithiasis were randomly divided into 2 equal groups undergoing 16-MDCT at standard-dose (120 kV, 240 mA) or lowdose (120, 80, and 50 mA, respectively) based on the body mass index (BMI).Taking the clinical diagnosis as the standard, the sensitivity level, specificity level, and positive predictive value of these groups were compared.Results The dose length product ( DLP ) of the low-dose CT group and the average CT dose index (CTDIvol ) were 18.95 and 6.65 mGy, respectively.There was significant difference between the two groups ( t = 31.78, P < 0.01 ).The sensitivity level, specificity level, and positive predictive value of the low-dose group were 97.1% , 94.0% , and 94.3% , respectively, all not significantly different from those of the standard-dose groups (97.3% , 96.0% , and 97.3% , respectively,P >0.05).Conclusions It is feasible to diagnose ureterolithiasis by low-dose 16-MDCT based on BMI with the obtained image quality meeting the clinical diagnostic requirements.
7.Carotid Atherosclerotic Plaque:MR Imaing at 3.0T Magneton

Objective To study the capability in detection and qualitative analysis of carotid atherosclerotic plaque with 3.0T MR scanner.Methods Bilateral carotid arteriae in thirty-two patients with arterosclerosis which detected by Doppler ultrasound were examined with a 3.0T MR scanner.The pulse sequences included 3D time of flight,FLASH-3D,TSE-DIR T_1WI,TSE-DIR PDWI-FS,DIR TSE-T_2WI-FS and TSE-T_2WI.According to AHA modified criteria of atherosclerosis typing,the signal characteristics of plaques were analysed.Results Carotid plaques were homogeneous signal intensity on all sequences in 30/49,heterogeneous signal intensity on at least one sequence in 15/49,among them,7 with lipid core,3/49 plaques contained calcification ,2/49 appeared as thrombosis and resulting in vessel occulusion,28/49 plaques presented the character of thick fibrous cap,13/49 plaques showed character of thin fibrous cap,the surfaces of 5/49 plaques were rough.Conclusion High resolution multiple sequences imaging with 3.0T MR scanner can be used for detection and qualitative analysis of the carotid atherosclerotic plaque in a certain degree.

9.Survey on the secondary prevention of coronary artery disease with statins in elderly patients

Objective To probe into the usage of statins in the patients with coronary artery disease in elderly patients. Methods From Jun 1,2009 to Mar 31,2009 in our hospital, 323 elderly outpatients with coronary artery disease were included in the investigation. The data were statistically analyzed. Results 62.2% of the patients used statins. The statins usage rates (44. 8% ,61.3% ,71.3% ) among three age groups(60 -69 years old,70- 79 years old, and > 80 years old)were significantly different. The rates of statins usage were significantly different between male and female groups. Conclusion The usage of statins as secondary prevention of coronary artery disease varies and there is urgent need to standardize the usage of statins according to the treatment guideline.
10.Role of spinal cord proteasome in chronic morphine tolerance in rats

ObjectiveTo evaluate the role of the spinal cord proteasome in chronic morphine tolerance in rats.MethodsTwenty-four healthy male SD rats in which intrathecal catheters were successfully placed without complications were randomized into 4 groups ( n =6 each):normal saline group ( group NS),chronic morphine tolerance group (group M),chronic morphine tolerance + proteasome inhibitor MG-132 group (group M + MG) and MG-132 group (group MG).Normal saline 10 μl,morphine 10 μg,morphine 10 μg+ MG-132 2.5 μg and MG-132 2.5 μg were injected intrathecally twice a day for 7 consecutive days in groups NS,M,M + MG and MG respectively.Tail flick latency was measured on day 1 before intrathecal injection and on day 1,3,5 and 7 of intrathecal injection to calculate the percentage of maximum possible antinociceptive effect (MPAE).After the last intrathecal injection,5 rats were sacrificed,and L3-5 spinal cord was removed for determination of the expression of glutamate-aspartate transporter (GLAST) and excitatory amino acids carrier 1 (EAAC1)by Western blot.Results MPAE was gradually decreased during the intrathecal injection in groups M and M + MG( P < 0.05).Compared with group NS,MPAE was gradually incresed during the intrathecal injection in groups M and M + MG,and the expression of GLAST and EAAC1 in the spinal cord was significantly down-regulated in group M (P < 0.05).There was no significant difference in the parameters mentioned above between group MG and group NS ( P >0.05 ).Compared with group M,MPAE was significantly increased and the expression of GLAST and EAAC1 in the spinal cord was significantlyup-regulatedingroupM+MG(P<0.05or0.01).ConclusionSpinal cord proteasome is involved in the development of chronic morphine tolerance in rats.
