1.An Experimental Investigation for Biologic Reactions of Carbon Fiber Implants

Ligament and tendon prostheses have all suffered because of failure of the particular material that has used in the past. Various materials, including, silk, nylon, Dacron, and combinstions of other synthetic substances, have all enjoyed temporary vogues and have vogues and have all passed out of habitual use because of collapse of the material employed. In an attempt to find an alternative material for use as a tendon or ligament prosthesis, a fundamentally new approach to the problem has been used, namely, that of tendon ligament induction. Carbon fiber has an attraction as a biologic implant because of its inherently inert nature when used in the pure form. Within recent years, it has been possible to manufacture filamentous carbon fiber of high purity and constant filament size, and this material has been examined as a tendon and ligament prosthesis. Both achilles tendons of 14 adult Korean rabbits were replsced with carbon fiber reinforced epoxies(CFRE, it was developed in department of chemical engineering, Chungnam national university, Daejeon, Korea) after artificially rupture, and serial histologic examination of neotendon formations and biologic responces in vivo were done during 8 weeks. The results obtained were as followings ;1. Hemorrhage and inflammatory reactions were observed at 2–3 weeks after operations. 2. Foreign body reactions were developed at 4 weeks after operation, also observed proliferations of foreign body giant cells and histiocytes. 3. Fibroblasts were increased at 4 weeks after operation, and most severe poliferations at 5–6 weeks. 4. Foreign body reactions and fibroblasts were markedly decreased at 7–8 weeks after operation, and examination of the collagen itself showed it to be closely similar to that found on the normsl side.5. Thus, it appeared that the carbon fiber initially acted as a tendon prosthesis.

5.Lymphangiomatosis of Bone and Soft Tissue: A Case Report

A case of Lymphangiomatosis of the skeleton and soft tissue in a 23 years old woman is presented. Lymphangiomatosis are benign tumors formed of lymphatic vessel and like hemangiomatosis are probably best thought of as harmatomatous malformation. As term imply, the endothelial space in these lesions are lymph-rather than blood containing. Bone involvement are very rare condition that affect mainly the ilium and features a largely lytic pattern, consisting of tiny elliptical radiolucents slots. Treatment and Prognosis depend on the site and extent of involvement and progression. Although irradiation may be the treatment of choice, the lesions have not been usually successfully treated.

6.Color Doppler Ultrasonography in the Evaluation of the Acute Scrotum.

Thirty seven patients with acute scrotal pain were evaluated by color doppler ultrasonography. The diagnosis was made by clinical finding in 26 cases and surgical finding in 11 cases. There were inflammatory diseases in 22 cases, testicular torsion in 5 cases, other diseases in 10 cases. 20 cases of inflammatory disease showed increased blood flow in color doppler ultrasonography and 4 cases of testicular torsion showed absent blood flow. Thus, color doppler ultrasonographic evaluation had diagnostic value that was 80% in sensitivity and 96% specificity in differentiation of testicular torsion from other acute scrotal diseases. We think that color doppler ultrasonography in acute scrotum is a valuable diagnostic method for differentiation of testicular torsion from other acute scrotal diseases.

7.The Clinical Significance of Torsion of Appendix Testis in Acute Scrotum.

The torsion of appendix testis is the second most common disease of the acute scrotal diseases in frequency. Despite recent development of diagnostic equipments, its differential diagnosis is often difficult and its treatment is controversial. We reviewed retrospectively 19 cases who were diagnosed to have torsion of appendix testis during recent 6 years. The mean age was 11.3 years. Five cases who had blue dot sign and tender mass on upper pole of testis, and one of 9 who had been examined with scrotal ultrasonography were diagnosed to have torsion of appendix testis, but other clinical finding and laboratory findings were not contributable to its diagnosis. Nine of 14 cases who were diagnosed by surgical exploration were consistent with the preoperative impression. Mean duration of scrotal pain were 1.3 days in surgical treatment group and were 3.6 days in conservative treatment group. The pathognomonic sign of torsion of appendix testis was seen only in a few cases. The diagnostic accuracy of torsion of appendix testis before exploration was low. The scrotal pain was relieved relatively more rapidly in surgical treatment group. So, we think that surgery was more desirable in diagnosis and treatment of torsion of appendix testis.

8.Three-Dimensional Pelvic Floor Ultrasound Assessment of Pelvic Organ Prolapse: Minimal Levator Hiatus and Levator Ani Deficiency Score

Purpose:
The purpose of this study was to determine whether levator ani deficiency (LAD) scores and minimal levator hiatus (MLH) areas affect Pelvic Organ Prolapse Quantification (POP-Q) stage.
Methods:
This study was a retrospective chart review of patients with pelvic organ prolapse (POP) at Seoul Songdo Hospital between August 2019 and August 2020. Three-dimensional (3D) pelvic floor ultrasound, preoperative anal manometry, and other physiological tests were performed in 78 patients with POP symptoms. We divided the patients into mild prolapse and severe prolapse groups based on the POP-Q. We examined the LAD and MLH areas. LAD scores were categorized as mild, moderate, or severe.
Results:
There were 32 patients (41.0%) in the mild prolapse group (POP-Q stage I and II) and 46 (59.0%) in the severe prolapse group (POP-Q stage III and IV). The mean LAD score was significantly higher in severe prolapse group (13.33±2.49 vs. 8.19±2.92, P<0.001), and the rate of severe deficiency was also significantly higher in the severe prolapse group (29 [63.0%] vs. 2 [6.3%], P<0.001). The mean MLH was also significantly larger in the severe prolapse group (17.91±2.74 cm2 vs. 14.95±2.60 cm2, P<0.001). In addition, both MLH and LAD scores tended to increase at each stage.
Conclusion
There is a strong positive correlation between the POP-Q stage and the MLH and LAD scores that can be seen on 3D pelvic floor ultrasound. The findings of this study, by objectively demonstrating LAD and MLH in women with POP, are an important contribution to POP.
9.ACL Reconstruction with a Minimally Invasive Physeal Plate in Adolescence: Technical Note.

Substantial ACL ruptures are not common in adolescence or childhood. During this period, ACL tibia avulsion fractures are more common than substantial ACL ruptures are. However, recent advancements in MRI and increases in adolescent sports activity have led to increased diagnosis of substantial ACL ruptures. Many physeal-sparing ACL reconstruction methods have been reported. However, some physeal-sparing ACL reconstructions are associated with poor outcomes because of laxity in the reconstructed ligament and discordance in the isometric point. We report a 14-year-old male patient with a chronic substantial ACL rupture and a longitudinal medial meniscus tear who was treated with ACL reconstruction and all-inside meniscal repair, which spares the femoral physis and reduces tibia physeal injury with an Achilles allograft.

10.Incomplete Reduction that Influence Reduction of Sesamoid as a Cause for Recurrence of Hallux Valgus.

PURPOSE: The incomplete reduction of the sesamoid has lately been issued as cause for recurrence. In this study, we analysed factors that may influence reduction of sesamoid. MATERIALS AND METHODS: The study consists of 50 cases operated by single surgeon. Eighteen cases were done by proximal chevron osteotomy, and 32 cases were done by scarf osteotomy. Hallux valgus (HV) angle and intermetatarsal (IM) angle were measured before and three months after the surgery. Sesamoid position (SP) was classified according to Hardy and Clapham grade system. RESULTS: After the proximal chevron osteotomy, the correction of the mean HV angle was 19.5degrees, and IM angle was 6.2degrees. SP was changed from 5.6 to 3.4 grade. After the Scarf osteotomy, the correction of the mean HV angle was 25 degree, and IM angle was 9degrees. SP was changed from 5.5 to 2.8 grade. There was difference of sesamoid's correction between two different method of surgery (p=0.127). However, better correction of sesamoid was witnessed with bigger correction angle regardless of method of surgery (p=0.002, 0.001). CONCLUSION: We believe surgical method do not effect sesamoid's correction but more correction angle can result in better correction of sesamoid position.
