Objective To observe the effect of the self-made traditional Chinese medicine combined with brufen in treatment of cervical spondylosis. Methods 170 cases were divided into the treatment group(85 cases) and the control group(85 cases) randomly. The control group was treated with brufen. Patients in treatment group received routine treatment plus the self-made traditional Chinese medicine. After the treatment, the curative effect, aches and coma were analyzed. Results The total effective rate was 94. 1% in treatment group but 84. 7% in control group.Difference between the two groups had statistical significance( x2 = 3.997,P ＜ 0. 05). The integrate of 1 ahes and coma were ( 7. 8 ± 2. 4), (9. 4 ± 2. 1 ) marks and ( 2. 5 ± 0. 6), (3. 1 ± 0. 9) respectively, it was lower than before treatment(11.9 ±2.3) ,(12.3 ±2.9) marks and (5.6 ± 1.4) ,(5.3 ± 1.2) marks. Aches and coma showed a great difference after treatment from those before treatment( t = 34. 641、6. 159、6. 173、6. 080,all P ＜0. 05 ). Aches and coma were lower then those of control group(t =9. 283、3. 464,all P ＜0. 05). No severe adverse reaction occurred in two groups. Conclusion The self-made traditional Chinese medicine combined with brufen is effective to treat aches and coma in cervical spondylosis. The method was feasible for treat aches and coma in cervical spondylosis.

Objective: To evaluate the effect of gastroscopic hemostasis combined with limited fluid resuscitation in the treatment of upper gastrointestinal bleeding in patients with liver cirrhosis. Methods: From December 2016 to June 2018, 100 liver cirrhosis patients with upper gastrointestinal bleeding in Yuncheng Central Hospital were divided into control group(n=50) and observation group(n=50) according to non-randomized non-inferiority.The control group was treated with limited fluid resuscitation, and the observation group was treated with gastroscopic hemostasis combined with limited fluid resuscitation.The therapeutic effects of the two groups were compared. Results: The recovery rate, 24-hour hemostasis rate of the observation group were 98.00%(49/50), 68.00%(34/50), respectively, which were higher than those of the control group [82.00%(41/50), 16.00%(8/50)], and the clinical mortality rate of the observation group was 2.00%(1/50), which was lower than 16.00%(8/50) of the control group, the differences were statistically significant(χ²=5.444, 25.657, 4.396, all P<0.05). The time of hemostasis, ventilator detachment and ICU detachment in the observation group were shorter than those in the control group[(1.34±0.28)d vs.(2.05±0.43)d, (1.68±0.42)d vs.(2.59±0.51)d, (2.85±0.72)d vs.(5.19±1.03)d], the differences were statistically significant(t=9.784, 9.739, 13.166, all P<0.001). The incidence of complications, rehaemorrhage rate within 6 months in the observation group were 4.00%(2/50), 2.00%(1/50), respectively, which were lower than those in the control group [22.00%(11/50), 16.00%(8/50)], the differences were statistically significant(χ²=5.659, 4.396, all P<0.05). Conclusion Gastroscopic hemostasis combined with limited fluid resuscitation in the treatment of upper gastrointestinal bleeding in patients with liver cirrhosis has significant therapeutic effect, high hemostasis rate and few complications.

Objective: To evaluate the protective effects of ulinastatin combined with lung-protective ventilation on lungs in patients undergoing laparoscopic colorectal carcinoma surgery. Methods: A total of 120 patients undergoing laparoscopic radical resection of colorectal cancer in Yuncheng Central Hospital were selected in this study.According to thedigital table, they were randomly divided into conventional mechanical ventilation group(G0 group), lung protection mechanical ventilation group(G1 group), conventional mechanical ventilation combined with ulinastatin group(G2 group)and lung protection mechanical ventilation combined with ulinastatin group(G3 group), with 30 cases in each group.Ulinastatin 2 500IU was intravenously infused 10min before induction of anesthesia in G2 group and G3 group.Ventilator parameters were set as tidal volume(V_T)10mL/kg, respiratory frequency 12 times/min, inspiration-expiration ratio(I∶E)1∶2 in G0 group and G2 group.V_T 6mL/kg in G1 group and G3 group , positive end-expiratory pressure ventilation(PEEP)5-10cmH₂O, respiratory frequency 12-18 times/min, once every 30min.Before anesthesia induction(T₀), before pneumoperitoneum(T₁), 2h after pneumoperitoneum(T₂), before extubation(T₃), 24h after operation(T₄), arterial blood was collected to detect partial pressure of oxygen(PaO₂), oxygenation index(OI). And mean airway pressure(Pmean), peak airway pressure(Ppeak)at T_1-3 were recorded.Venous blood was collected at T₀, T₃ and T₄ to detect serum concentrations of interleukin-6(IL-6), interleukin-8(IL-8)and tumor necrosis factor-alpha(TNF-α). Results: At T₃, the PaO₂ of G1 group, G2 group and G3 group were (457±60)mmHg, (460±49)mmHg and (510±45)mmHg, respectively.At T₄, the PaO₂ of the three groups were (90±6)mmHg, (92±7)mmHg and (96±7)mmHg, respectively.The PaO₂ in G1 group and G2 group were lower thanthose in G3 group at T_3-4(t=3.872, 3.850, 4.186, 2.513, all P<0.05). At T₂, the OI of the three groups were (460±44), (462±37) and (481±31), respectively.At T₃, the OI of the three groups were (460±51), (475±62) and (504±38), respectively.The OI in G1 group and G2 group were lower than those in group G3 at T_2-3(t=2.142, 3.753, 2.209, 2.170, all P<0.05). At T₂, the Pmean of G1 group, G2 group and G3 group were (10.5±1.5)cmH₂O, (11.2±1.9)cmH₂O and (9.5±0.9)cmH₂O, respectively, the Ppeak of the three groups were (22.3±3.0)cmH₂O, (24.0±3.3)cmH₂O and (22.3±2.9)cmH₂O, respectively.At T₃, the Pmean of the three groups were (10.9±1.1)cmH₂O, (11.1±1.8)cmH₂O and (9.8±0.9)cmH₂O, respectively, the Ppeak of the three groups were (22.8±2.7)cmH₂O, (24.1±2.1)cmH₂O and (21.6±2.0)cmH₂O, respectively.The Pmean in G1 group and G2 group at T_2-3 were lower than those in G3 group(t=3.061, 4.580, 5.759, 4.305, 3.746, all P<0.05), the Ppeak in G2 group at T_2-3 were lower than those in G3 group(t=2.952, 4.630, all P<0.05). At T₄, the serum concentrations of IL-6 of G1 group, G2 group and G3 group were (151±28)ng/L, (152±20)ng/L and (136±18)ng/L, respectively, the IL-8 levels of the three groups were (468±28)ng/L, (359±65)ng/L and (327±51)ng/L, respectively, the TNF-α levels of the three groups were (1.09±0.20)ng/L, (1.08±0.20)ng/L and (0.94±0.20)ng/L, respectively.The concentrations of IL-6, IL-8 and TNF-α at T₄ in G1 group and G2 group were higher than those in G3 group(t=2.428, 4.094, 2.536, 4.251, 2.807, 3.128, 4.119, 2.592, 3.446, 2.555, all P<0.05). Conclusion The combination of ulinastatin and lung-protective ventilation provides protective effects on lungs and exerts better efficacy than either alone.

Objective: To explore the relationship between the expression of serum B-type natriuretic peptide(BNP) levels and cardiac function, heart failure(HF) in children with ventricular septal defect of congenital heart disease(CHD). Methods: From October 2013 to July 2017, 150 children with ventricular septal defect of CHD who were treated in Yuncheng Central Hospital were selected, and they were divided into no HF group(n=40), mild HF group(n=46) and moderately severe HF group(n=64) according to the New York university pediatric heart failure index(NYU PHFI) evaluation standard.Meanwhile, 148 healthy children were selected as control group.The serum BNP levels, left ventricular ejection fraction(LVEF), left ventricular ejection fraction shortening rate(LVFS), left ventricular end diastolic diameter index(LVEDDI) and cardiac output index(CI) were measured in all subjects.The levels of BNP, LVFS, LVEF, LVEDDI and CI in the study group were compared with those in the control group.The levels of BNP, LVEF, LVFS, LVEDDI and CI in children with CHD ventricular septal defect were compared.The relationship between the level of BNP and LVEF, LVFS, LVEDDI and CI in patients with ventricular septal defect of CHD was analyzed. Results: The serum level of BNP in the study group was (268.47±25.48)ng/L, the levels of LVEF, LVFS, LVEDDI and CI in the study group were (55.56±4.23)%, (27.96±2.74)%, (77.56±6.54)mm/m², (3.35±0.41)L·min^-1·(m²)^-1, respectively.The serum level of BNP in the control group was (33.41±6.21)ng/L, and the levels of LVEF, LVFS, LVEDDI and CI in the control group were (65.43±5.12)%, (36.24±3.23)%, (57.32±5.31)mm/m², (4.27±0.46)L·min^-1·(m²)^-1, respectively.The serum levels of BNP and LVEDDI in the study group were significantly higher than those in the control group(t=109.077, 29.306, all P<0.05), while the levels of LVEF, LVFS and CI in the study group were significantly lower than those in the control group(t=18.152, 23.875, 18.231, all P<0.05). With the increased severity of HF, the levels of BNP and LVEDDI were increased, while the levels of LVEF, LVFS and CI were gradually decreased(F=332.232, 245.400, all P<0.05). The level of serum BNP was negatively correlated with LVEF, LVFS, CI(r=-0.705, -0.692, -0.718, all P<0.05), but it was positively correlated with LVEDDI and HF(r=0.721, 0.735, all P<0.05). Conclusion The level of serum BNP in patients with ventricular septal defect of CHD is correlated with cardiac function indicators, and serum BNP is positively correlated with HF in children with ventricular septal defect of CHD, which plays an important role for the diagnosis, treatment and prognosis of HF.

AIM:To explore the effect of base curve aspheric orthokeratology lens in the treatment of adolescents with myopia and moderate to high astigmatism, and its influence on ocular surface morphology and tears.METHODS:A total of 232 adolescents(464 eyes)with myopia and moderate to high astigmatism treated in the hospital from December 2018 to March 2020 were selected as the research subjects. They were randomly divided into control group(116 patients, 232 eyes)and observation group(116 patients, 232 eyes). The control group was treated with base curve spheric orthokeratology lens, while the observation group was treated with base curve aspheric orthokeratology lens. Comparison was made between the two groups in terms of the correction effect, objective visual quality, ocular surface morphology, tears related indicators before and after wearing the lenses, and the incidence of complications after wearing the lenses for 12mo.RESULTS:After wearing the lenses, the uncorrected visual acuity(UCVA)and diopter of the two groups were significantly improved. The UCVA and diopter of the observation group were significantly better than those of the control group(all P<0.05). After wearing the lenses, the whole-eye and corneal coma, spherical aberrations and high-order aberrations were significantly increased, while Strehl ratio and modulation transfer function were decreased in the two groups. Trefoil aberrations was significantly increased(all P<0.05). These indicators in the observation group were better than those in the control group(all P<0.05). After wearing the lenses, the epithelium fluorescence staining scores, ocular surface disease index(OSDI)scores were significantly decreased in the two groups(P<0.05), but there was no statistically significant difference between the two groups(P>0.05); The noninvasive tear breakup time(NI-BUT)of the two groups were significantly decreased after wearing the lenses(P<0.05), but there was no statistically significant difference between the two groups(P>0.05). The Schirmer Ⅰ test and tear meniscus height of the two groups showed no statistically significant difference before and after wearing the lenses(P>0.05). The incidence rates of complications in the observation group and the control group after wearing the lenses were close(6.9% vs 6.0%, P >0.05).CONCLUSION:Base curve aspheric orthokeratology lens is superior to base curve spheric orthokeratology lens in the treatment of adolescents with myopia and moderate to high astigmatism in terms of correction effect and objective visual quality. The two lenses have similar influence on ocular surface morphology and tears.

To analyze the main factors influencing the ethical review work of municipal hospitals in Shanxi Province by investigating the current situation of the construction and operation of ethics committees, and to put forward reasonable suggestions for improving the work of ethics committees. Questionnaire surveys and interviews were used to investigate the composition of ethics committees, systems and standard operating procedures, personnel training, review project methods, time limits, etc. of 24 hospitals at municipal levels in Shanxi Province. The establishment and system of ethics committees in tertiary hospitals at prefecture and city level were basically reasonable, and they could actively play the role of ethics committees. At present, most hospitals do not pay enough attention to the work of ethics committees, lack of full-time staff and systematic training, resulting in insufficient ethics review capabilities. The construction of the ethics committee of the second-level hospital is a mere formality, and no substantive work has been carried out. It is recommended to strengthen the supervision, increase the training of ethics committees and researchers, improve the information management of ethics committees, and establish regional ethics committees to further improve the ability and efficiency of ethics review.

OBJECTIVE: To analyze the clinical significance of combined newborn hearing and deafness gene screening in Yuncheng area of Shanxi Province. METHODS: Results of audiological examinations, including transient evoked otoacoustic emission and automatic discriminative auditory brainstem evoked potentials, for 6 723 newborns born in Yuncheng area from January 1, 2021 to December 31, 2021, were retrospectively analyzed. Those who failed one of the tests were considered to have failed the examination. A deafness-related gene testing kit was used to detect 15 hot spot variants of common deafness-associated genes in China including GJB2, SLC26A4, GJB3, and mtDNA12S rRNA. Neonates who had passed the audiological examinations and those who had not were compared using a chi-square test. RESULTS: Among the 6 723 neonates, 363 (5.40%) were found to carry variants. These have included 166 cases (2.47%) with GJB2 gene variants, 136 cases (2.03%) with SLC26A4 gene variants, 26 cases (0.39%) with mitochondrial 12S rRNA gene variants, and 33 cases (0.49%) with GJB3 gene variants. Among the 6 723 neonates, 267 had failed initial hearing screening, among which 244 had accepted a re-examination, for which 14 cases (5.73%) had failed again. This has yielded an approximate prevalence of hearing disorder of 0.21% (14/6 723). Among 230 newborns who had passed the re-examination, 10 (4.34%) were found to have carried a variant. By contrast, 4 out of the 14 neonates (28.57%) who had failed the re-examination had carried a variant, and there was a significant difference between the two groups (P < 0.05). CONCLUSION Genetic screening can provide an effective supplement to newborn hearing screening, and the combined screening can provide a best model for the prevention of hearing loss, which can enable early detection of deafness risks, targeted prevention measures, and genetic counseling to provide accurate prognosis for the newborns.

AIM: Anti-atherosclerosis effects of Momordica charantia L was further studied in a New Zealand rabbit atherosclerotic model at the basis of anti-inflammation and antioxidant effects. METHODS: Animals were divided into 3 groups: normal group (normal rabbit diet), atherosclerosis group(diet containing 2% cholesterol), and Momordica charantia L group(diet containing 2% cholesterol and 1 5% sarcocarp of Momordica charantia L ). Ninety days later, all animals were sacrificed. The effect of Momordica charantia L on atherosclerosis was evaluated by measuring serum lipid and total cholesterol content of artery wall, observing fatty liver degree, aorta arteriosclerotic area, and the thickness of intima. RESULTS: The level of total serum cholesterol and LDL-C in Momordica charantia L treatment group were obviously lower than those in atherosclerosis group, so were the total cholesterol content of artery wall, fatty liver degree, atherosclerotic area, intima thickness and I/M ratio, but no significantly difference was found between the two groups in TG level. The level of HDL-C in Momordica charantia L treatment group was evidently lower than that in normal control group. CONCLUSION: Momordica charantia L has an anti-atherosclerosis action in rabbits.

OBJECTIVETo introduce to a new clinical therapy Internal Classic.

METHODSAccording to the theory about soft tissue diseases of twelve regular channels in The Yellow Emperor's Internal Classic, "The pressure pain point was selected as needled point", deeply needling with many needles to reach the bone and tendon combined with moxibustion at surroundings of the joint of lesion was adopted and the mechanism of silver needle warming therapy in treatment of soft tissue diseases was investigated.

RESULTSThis therapy had functions of warmly dredging channels, activating blood circulation and alleviating pain, relaxing muscles and tendons, and promoting activity of joints, which were stronger and lasted longer timeas compared with general simple acupuncture. Particularly, it had obvious analgesic effect on pain syndrome induced by chronic refractory soft tissue diseases. It is indicated that the mechanism is mainly eliminating aseptic inflammation, increasing local blood supply, relaxing muscular spasm, so as to stop pain caused by inflammation, ischemia and spasm.
