1.The analysis of survival factors of patients with unresectable pancreatic cancer aged 70 years and over

Objective To explore the effect of related factors on survival of patients with unresectable pancreatic cancer aged 70 years and over. Methods Fifty-seven patients with unresectable locally advanced or metastatic pancreatic cancer aged 70 years and over were enrolled.Their survival time were analyzed with SPSS 13.0 by taking account of gender, age, smoking history,alcohol history, pancreatic disease history, diabetes mellitus history, Eastern Collaborative Oncology Group (ECOG) scoring, chemotherapy, radiotherapy, CEA and CA199 levels. Results Gender,ECOG scoring, chemotherapy and radiotherapy had relationship with overall survival. The median survival time was 8.9 months and one-year survival rate was 28.1%. The median survival was (10.7±5.4) months in male group and (5.5±2.3) months in female group (P=0.000). The median survival was(10.1±5.8) months in patients with ECOG 0~1 group and(7.3±3.8)months in patients with ECOG 2 group (P=0.040). The median survival was(7.76±5.27) months in nochemotherapy group and(11.5±5.0)months in chemotherapy group (P=0.038). The median survival was(8.87±5.36)months in no radiotherapy group and (13.7±3.8) months in radiotherapy group (P=0.048). Conclusions The patients who have better ECOG performance status and receive chemotherapy or radiotherapy show better survival.
3.Probucol prevention against restenosis by regulating functional vascular remodeling after percutaneous transluminal angioplasty in rabbits

AIM To investigate the relationship between the prevention of probucol against restenosis and functional vascular remodeling after percutaneous transluminal angioplasty(PTA) in rabbits. METHODS New Zealand rabbit thoracic aorta atherosclerosis was induced by 3 5 F balloon catheter injury following a 4 week feeding of high cholesterol diet, and percutaneous transluminal angioplasty(PTA) was performed by using 3 5 F ballon catheter. Probucol(1 g?d -1 ) or vitamin E(400 mg?d -1 ) was administrated one week before PTA. Two weeks after PTA, lumen area,neointima area(NEA),medial area(MA) and NEA/MA were analyzed by computerimage system.The relaxation response of restenotic arteries to acetylcholine and the contractive response to norepinephrine, potassium chloride and serotonin of thoracic aorta rings were measured by bioassay. Nitric oxide of serum was measured by means of biochemical assay.RESULTS After two weeks of PTA, probucol increased lumen area and decreased neointima area significantly. 4.Analysis of driver gene mutations in colorectal cancer by using next-generation sequencing

Objective:To investigate the characteristics of gene mutations in colorectal cancer(CRC)patients by using next-generation generation sequencing(NGS).Methods:Blood and tissue samples were collected from 90 CRC patients admitted to Beijing Hospital between August 5, 2016 and December 29, 2020.Analysis of driver gene mutations was performed by using a 1021-gene NGS panel.Results:There were 43 tissue samples and 83 blood samples.Also, 36 patients had both tissue and blood samples.The frequency rates of KRAS and BRAF mutations were 51.2%(22/43)and 20.9%(9/43)in tissue samples, and 3 rare concomitant KRAS/ BRAF mutations were detected.The frequency rates of KRAS and BRAF mutations were 26.5%(22/83)and 10.8%(9/83)in blood samples.In patients with tissue and blood samples, the rates of KRAS and BRAF mutations were 52.8%(19/36)and 10.8%(8/36). Conclusions:The rate of KRAS mutations in tissue samples from colorectal cancer patients is similar to rates reported in the literature, but the rate of BRAF mutation and the rate of rare KRAS and BRAF co-mutations are higher than those reported from other countries.

8.Effects of low-calorie diet intervention and aerobic exercise intervention on insulin levels and body composition in obese patients with type 2 diabetes

Objective:To explore the effects of low-calorie diet intervention and aerobic exercise intervention on insulin levels and body composition in obese patients with type 2 diabetes.Methods:A total of 300 obese patients with early type 2 diabetes who were diagnosed and treated in the Department of Endocrinology, Xuanwu Hospital of Capital Medical University from July 2016 to July 2019 were selected as the research objects. According to the random sampling method, they were divided into a control group and an observation group with 150 cases each. Calorie diet intervention was given to control group, while the observation group was given low-calorie diet intervention and concentrated aerobic exercise intervention. Observation and evaluation of fasting blood glucose (FBG), fasting insulin (FINS), homeostasis model assessment of insulin resistance (HOMA-IR), body mass index (BMI), body fat percentage, body weight, triacylglycerol (TG), total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C) were conducted.Results:After 4 weeks of intervention, FBG, FINS, HOMA-IR, BMI, body fat percentage, body mass, TG, TC, HDL-C, LDL-C indicators of the observation group were (6.15±0.92) mmol/L and (14.12±1.11) mU/L, 2.67±0.32, (25.01±1.75) kg/m 2, (27.45±1.92)%, (70.01±3.56) kg, (3.01±0.30) mmol/L, (5.25±0.88) mmol/L, (2.25 ±0.42) mmol/L, (3.15±0.41) mmol/L. The control group were (8.18±1.28) mmol/L, (16.78±1.85) mU/L, 3.78±0.78, (27.36±2.45) kg/m 2, (29.78±2.39)%, (72.98±5.62) kg, (3.49±0.52) mmol/L, (6.23±1.08) mmol/L, (1.88±0.30) mmol/L, (3.98±0.89)mmol/L. The difference between the two groups was statistically significant ( t values were 5.47-16.13, all P<0.05). After 8 weeks of intervention, FBG, FINS, HOMA-IR, BMI, body fat percentage, body mass, TG, TC, HDL-C, LDL-C indicators of the observation were (5.06±0.45) mmol/L, (12.78±0.69) mU/L, 2.01±0.12, (23.25±1.18) kg/m 2, (25.05±1.19)%, (66.02±2.45) kg, (2.21±0.12) mmol/L, (4.03±0.41) mmol/L, (3.08 ±0.72) mmol/L, (2.65±0.15) mmol/L,while in the control group were (6.07±0.88) mmol/L, (14.09±1.05) mU/L, 2.95±0.45, (26.98±2.08) kg/m2, (27.18±2.06)%, (70.98±4.02) kg, (2.98±0.28) mmol/L, (5.16±0.71) mmol/L, (2.41±0.51) mmol/L, (3.29±0.39) mmol/L. The difference between the two groups was statistically significant ( t values were 5.47-30.96, all P<0.05). Conclusions:Low-calorie diet intervention combined with concentrated aerobic exercise intervention is more conducive to improving patients′ blood sugar and blood lipid levels, as well as reducing body weight.
9.Relationship between the Epidemiology of Chronic Rhinitis and Atmospheric Environmental Factors in Wuhan

Objective To investigate the relationship between the epidemiology of chronic rhinitis and atmospheric environmental factors in Wuhan. Methods A total of 3 371 patients with chronic rhinitis in Wuhan seventh hospital from January 2017 to January 2020 were enrolled. The incidence of chronic rhinitis in different years and seasons was compared. Then the influence of atmospheric environmental factors such as air pollution index (API), fine particulate matter (PM2.5), inhalable particulate matter (PM10), nitrogen dioxide (NO2), sulfur dioxide (SO2), diurnal patterns of relative humidity, wind speed, air temperature, air pressure wind speed and temperature range on occurrence of chronic rhinitis was analyzed. Results The incidence of chronic rhinitis in Wuhan showed an annual decreasing trend from 2017 to 2020, so the incidence rate was the highest in 2017 (38.83%, 1309/3371) and was the lowest in 2020 (28.98%, 977/3371). In terms of different seasons, the incidence rate was the highest in winter (33.67%, 1135/3371), and was the lowest in summer (16.11%, 543/3371). The main factors affecting the incidence of chronic rhinitis were API, PM2.5, and PM10 in spring, which were NO2, SO2, and daily average relative humidity in summer, those were API, PM2.5, PM10, NO2, and SO2 in autumn, and were API, PM2.5, PM10, NO2, SO2, and daily range of air temperature in winter. The lag response of patients with chronic rhinitis to atmospheric environmental factors was 2-5 days in spring, 1-6 days in summer, 0-5 days in autumn and 3-6 days in winter. Patients with chronic rhinitis have the fastest response to API and PM2.5 in spring, the fastest response to API in summer, and the fastest response to API and PM2.5 in autumn, respectively. Conclusion The incidence of chronic rhinitis in Wuhan is decreasing year by year, and the incidence is related to environmental and meteorological factors, and the disease onset has the characteristics of lag due to seasonal factors.
10.Application of three-dimensional visualization technique based on CT data in the analysis of renal vascular anatomical variation

Objective:To explore the application value of three-dimensional visualization technique based on CT data in the analysis of renal vascular anatomical variation.Methods:The clinical data of 210 patients with renal tumors, adrenal tumors and renal cysts who underwent renal enhanced CT from October 2020 to June 2021 were retrospectively analyzed. Among the patients, there were 114 males and 96 females with an average age of (56.5±13.2) years. The CT data were reconstructed by 3D slicer software. According to the three-dimensional visualization model, the renal vascular anatomy was analyzed from the perspective of whether it needs to be treated during laparoscopic radical nephrectomy and laparoscopic partial nephrectomy. The variation of renal artery can be divided into multiple renal arteries, premature branches of renal artery and the mixed type with the above two variations. Renal vein variation can be divided into multiple renal veins, late confluence of renal veins and mixed type with the above two variations.Results:Among the 210 patients in this study, there were no statistically significant differences in anatomical variations of renal arteries and veins between males and females ( P=0.914 and P=0.121). Among 420 kidneys, renal artery variation (174/420, 41.4%) was more common than renal vein variation (121/420, 28.8%) ( P<0.01). 32 (7.6%) right kidneys and 38 (9.0%) left kidneys have multiple renal arteries ( P=0.432). Eighty-nine cases (42.4%) had premature branches in the right renal artery, while 37 cases (17.6%) in the left kidney ( P<0.01). 24 kidneys (5.7%) showed mixed renal artery variation. 53 (12.6%) right kidneys and 3 (0.7%) left kidneys had multiple renal veins ( P<0.01). Late confluence of renal veins was found in 41 right kidneys (9.8%) and 33 left kidneys (7.9%), respectively ( P=0.306). 8 (1.9%) mixed renal vein variants were all right kidneys.. From the perspective of laparoscopic renal surgery, there were 71 cases (33.8%) of the left kidney to deal with ≥ 2 renal arteries, as well as 103 cases (49.1%) of the right kidney ( P<0.01). There were 44 cases (21.0%) of the left kidney to deal with ≥ 2 renal veins, as well as 78 cases (37.1%) of the right kidney ( P<0.01). Conclusions:The three-dimensional visualization technique based on renal CT data is helpful to accurately evaluate the renal vascular anatomy before operation. Right renal vascular variants are more common.
