3.Roles of depression and C-reaction protein in early development of cardiovascular disease in the elderly diabetes patients

Objective To investigate the role of depression and C-reaction protein (CRP)in the early development of vascular disease in the elderly patients with type 2 diabetes. Methods Totally 396 patients with type 2 diabetes were recruited in the study,including 196 cases with depression and 200 cases without depression.Depression was evaluated by Hamilton depression scale (HAMD),and CRP was measured using rate nephelometry.The related parameters of wave intensity (WI) such as accelerating wave intensity (W1),decelerating wave intensity (W2),arterial stiffness β,pressure-strain elasticity modulus (Ep),arterial compliance (AC),pulse wave velocity -wave intensity(PWVWI),pulse wave velocity (PWV) and argumentation index (AI) were detected and compared between the groups. Results The levels of CRP [(8.1±2.8)mg/L vs. (3.9±3.2) mg/L,t=6.411,P=0.000],W2 [(4.82±1.33) mm Hg· m-1 · s-3 vs.(2.27±1.28)mm Hg · m-1 · s-3,t=4.256,P=0.021],arterial stiffness β(12.58±3.23 vs.8.96±2.24,t=4.634,P=0.019),Ep (157.90±52.64 vs.118.24±53.08,t=5.512,P=0.007),PWVβ(7.53± 1.41 vs.6.68±1.56,t=3.982,P=0.024),AI(19.26± 13.21 vs.15.34±10.23,t=3.213,P=0.037)and PWV-WI (7.59±1.67vs.6.63± 1.45,t =2.613,P =0.046) in cases with depression were higher than the cases without depression,while the AC value was reduced (0.70±0.19 vs.0.78±0.21,t=4.676,P =0.017).There was no difference in W1 value between the groups.The CRP level and HAMD score were positively associated with W2 (r=0.470,P=0.000),stiffness β (r=0.582,P=0.000),Ep (r=0.352,P=0.004),PWV-WI (r=0.233,P=0.019),PWVβ (r=0.340,P=0.007) and AI (r=0.265,P=0.015),but negatively related with AC(r=-0.457,P=0.000).HAMD score was positively correlated with CRP level(r=0.328,P=0.003). Conclusions The carotid artery wave intensity W2,blood vessel elasticity parameter β,Ep,AC,PWV-WI,PWVβ and AI determination can evaluate the risk of cardiovascular diseases in elderly diabetes,and higher CRP level and HAMD score may deteriorate its process.
4.The treatment of postoperative hemorrhage of pancreato - jejunal or choledochojejunal anastomotic stoma after whipple operation by the way of fibercholedochoscopy

Objective To evaluate the therapeutic effectiveness of postoperative hemorrhage of cholejejunal or pancreato - jejunal anastomotic stoma after pancreato - duodenectomy by the way of fibercholedochoscopy. Meth- ods The drainge tube was extracted out of choledochojejunal anastomotic stoma. And fibercholedochoscopy was in- serted in fistula to affirm hemorrhagic position of choledochojejunal or pancreato - jejunal anastomotic stoma. And then local coagulation and microwave solidifying was proceeded with. Local spray of Galla - chineses compound solution was performed for hemostasis. Results We succeeded in hemorrhage of four choledochojejunal and six pancreato - jejunal anastomotic stoma One failed for frequent hemorrhage in pancreato - jejunal anastomotic stoma. Conclu- sion Fibercholedochoscopy is the first selective and effective method in the diagnosis and treatment of postoperative hemorrhoge of pancreato - jejunal or choledochojejunal anastomotic stoma after whipple operation.
5.Correlation analysis of acute pancreatitis with hyperlipemia and C reactive protein

Objective To analyze the correlation of acute pancreatitis with hyperlipemia and C reactive pro-tein. Methods 42 patients with acute pancreatitis with hyperlipemia were divided into two groups of SAP group and MAP group under the diagnostic code. The blood fat and CRP were compared between the two groups, also the scores of APACHE Ⅱ , Ranson and CT were measured. Results The CRP and TG in group SAP compared with those in group MAP had significant difference(P<0.01),but the CHOL has no-difference;the scores of APACHEⅡ , Ranson and CT after control lipid in 42 patients were significantly' different( P < 0.05). Conclusion Hyperlipe-mia is one risk factors of the AP;combined with the CRP, it can be one evaluating index of the severity.
8.Mechanical device for prevention of deep vein thrombosis on patient compliance:a meta-analysis based on observational studies

BACKGROUND:Compliance of mechanical device reduced in the prevention of deep venous thrombosis.
OBJECTIVE:To evaluate the compliance on mechanical prevention measures in the prevention of deep venous thrombosis, and to analyze the reason for compliance reduction.
METHODS:We searched Cochrane Library, PubMed, Embase, and Medline for articles on compliance of inpatients on mechanical device for prevention of deep vein thrombosis published from inception to December 2015. At the same time, the“snowbal”method was used to screen the references. After the literatures were screened, using the Newcastle Ottawa Scale tools and Agency for Healthcare Research and Quality standard, two researchers evaluated the quality of the literatures independently and strictly. Data were extracted if meet the discharge standard. The software of Meta-analyst Beta3.13 was used for heterogeneity analysis and publication bias test in the extracted data. According to the heterogeneity analysis results and literature types, a reasonable statistical analysis method was selected to col ate, summarize and discuss.
RESULTS AND CONCLUSION:Final y, 12 papers were included, 9 of which were cross-sectional studies, 2 cohort studies, and 1 case-control study. The odds ratio about compliance rate of inpatients with mechanical devices was 53.2%(95%CI:38.7-67.2). Subgroup analysis showed that the odds ratio about compliance rate of pneumatic compression devices was 54.9%(95%CI:34.9-73.4). The odds ratio about compliance rate on elastic stockings was 47.9%(95%CI:34.3-61.8). The detection of mechanical device of surgical patients showed that odds ratio about compliance rate was 66.8%(95%CI:47.4-81.7). Our results confirm that the compliance rate of the use of mechanical devices for the prevention of deep venous thrombosis in clinical inpatients requests to be improved. The reason for reduced compliance rate should be further investigated to improve the compliance rate.
9.Correlation of Cognitive Impairment and Areas of Middle Cerebral Artery Territory Infarction

Objective To explore the correlation of cognitive impairment and areas of middle cerebral artery (MCA) territory infarction.Methods The Montreal Cognitive Assessment (MoCA) was used to evaluate neuropsychological statuses in the patients who developed first-time acute cerebral infarction for 2 weeks. MoCA scores and sub-scores were recorded. 132 patients were selected with MCA occlusion,scores of MoCA were more than 15, and cognitive impairment in at least one domain, and then subgrouped by infarction site. The correlation between the results of neuropsychological cognitive assessment and the sites of infarction was analyzed. Results MCA occlusion was correlated with the impairments of visual spatial/executive, attention, language and memory (B=-1.875~-1.094, P<0.05). Infarction in frontal lobe was correlated with the impairments of visual spatial/executive, attention, abstract and memory (B=-1.760~-1.329, P<0.05),temporal lobe with visual spatial/executive and memory impairment (B=-1.849~-1.735, P<0.05), parietal lobe with visual spatial/executive, attention and memory impairment (B=-1.695~-1.482, P<0.05), basal ganglia with visual spatial/executive, attention, language and memory impairment (B=-1.932~-1.041, P<0.01). Conclusion The characteristics of impairment in cognitive function is different with infarction sites in MCA territory.
10.Preliminary study of energy loss in left ventricle obtained by vector flow mapping in patients with hypertension

Objective To quantitatively analyze the energy loss (EL) in left ventricle with vector flow mapping (VFM) in hypertensive patients,and try to reveal the correlation of EL with cardiac structure and function.Methods Hypertension group:52 hypertensive individuals were enrolled in our study,divided into 4 subgroups:①14 patients with normal geometry(NG);②20 patients with concentric remodeling(CR);③15 patients with concentric hypertrophy(CH);④3 patients with eccentric hypertrophy(EH)(not enrolled in subgroup analysis).Health control group were composed of 32 healthy volunteers.LVEF,LVMI and E/e′ were measured.And the EL in left ventricle was measured at different phases and different segments based on VFM images.Results ①EL comparison:in the phrase of late diastole(DL),isovolumic contraction(IVC) and rapid ejection(RE),compared with the control group,EL in entirety and each segment of left ventricle in hypertension group increased (P<0.05);Compared with the control group,EL in entirety and each segment of left ventricle in each subgroup of hypertension increased (P<0.05);EL among different subgroups showed no statistically significant difference(P>0.05);②Correlation analysis:in the phrase of DL,IVC and RE,EL in entirety and each segment of left ventricle showed significant positive correlations with LVMI and with E/e′ (P<0.05).In the phrase of DL,EL in entirety and each segment of left ventricle showed significant negative correlations with LVEF(P<0.05).Conclusions EL measured quantitatively by VFM is more sensitive to reflect the changes of hemodynamic in hypertensive individuals.EL is closely correlated with cardiac structure and function.
