Objective:To summarize the nursing experience of 7 cases of "zero blood transfusion" type A acute aortic dissection during the perioperative period.Methods:Preoperative risk assessment, control of heart rate, blood pressure, analgesia and sedation to avoid dissection;fully keep warm during operation and recover blood from operation field;after operation, avoid heart rate and blood pressure fluctuations to reduce oozing, monitor coagulation indicators, observe the amount and characteristics of drainage fluid, ensure the safety of anticoagulation during Renal replacement therapy, and avoid blood waste.Results:The 7 patients all transitioned to the operation smoothly. Intraoperative and postoperative infusion of pre-storage and recovery autologous blood of 200-1 200 ml, no allogeneic blood transfusion. One patient had a lot of drainage after surgery. The drainage volume was 1 450 ml on the day and the first day. He was given autologous blood infusion combined with hemostatic drugs, and the hemoglobin could be maintained above 80 g/L; 2 patients complicated with hypoxemia, were sequential High-flow oxygen; 2 patients with acute kidney injury underwent continuous renal replacement therapy without unplanned dismissal. All 7 patients recovered and were discharged after active treatment.Conclusions:For patients with type A acute aortic dissection surgery, through careful blood protection care, you can reduce dependence on allogeneic blood.

Objective To investigate the synergistic proliferation-inhibiting and apoptosis-inducing effects of recombinant human-derived interleukin-24 (rhIL-24) and recombinant human-derived decorin (rhDCN) on human hepatocellular carcinoma cells HepG2.Methods Cellular growth and morphological changes of HepG2 cells were observed under the inverted microscope at 48 h after being transiently transfected with pcDNA3.1 (+)-IL-24 and pcDNA3.1 (+)-DCN by Lipofectamine.The proliferation-inhibiting effects of IL-24 and DCN on HepG2 cells,respectively and jointly,were observed with MTT assay at 24 h,48 h and 72 h post-transfection.Apoptosis and cell-cycle of HepG2 cells were analyzed by flow cytometry at 48 h post-transfection.Results Compared to control groups,the cells of target gene groups presented typically changes of proliferation inhibition and apoptotic morphology,which occurred obviously in co-transfection group.The results of MTT assay showed that at 48 h and 72 h post-transfection,the profiferation-inhibiting rates in the group of cells co-transfected with IL-24 and DCN were (31.88±6.57) ％ and (36.83±3.76) ％,respectively,displaying significant difference with those of other groups (P ＜ 0.01).The results of flow cytometry showed that IL-24 and DCN can induce HepG2 cells apoptosis to some extent.Compared to the early apoptosis rate of cells of control groups,plasmid (2.98±0.72) ％,blank cell (3.50±0.92) ％,IL-24 (20.01±1.08) ％ and DCN (22.20±0.91) ％,a statistically remarkable apoptosis rate,(32.56±0.90) ％,can be seen in the group of cells treated with IL-24 and DCN jointly (P ＜ 0.01).The result of cell cycle analysis revealed that,compared to control groups,the proportion of cells was higher in the phase of G2/M in the IL-24 group (11.24±0.35) ％ and in the phase of G0/G1 in the DCN group (77.93±0.67) ％.The proportions of cells in the phases of G2/M increased to (71.36±0.60) ％ and that of G0/G1 statistically increased to (10.39±0.67) ％ in the group of cells co-transfected with IL-24 and DCN (P ＜ 0.01).Conclusions Combinatorial treatment of HepG2 cells with IL-24 and DCN can exert stronger synergistic proliferation-inhibiting effect and apoptosisinducing activity-in comparison to single therapies.IL-24 and DCN can induce cell cycle arrest on HepG2 cells,occurred in the phase of G2/M and G0/G1,respectively.Promoting effect of cell cycle arrest in the phase of both G2/M and G0/G1 can be seen on HepG2 cells co-transfected with IL-24 and DCN,which maybe the possible mechanism of the synergistic proliferation-inhibiting and apoptosis-inducing effect.

Objective To explore the clinical effects of laparoscopic surgery combined with marvelon,gestrinone and leuprolide in treatment of chronic endometriosis.Methods 50 cases of endometriosis were collected.They were randomly divided into observation group and control group,each group having 25 cases.The control group underwent laparoscopic surgery and the observation group underwent laparoscopic surgery combined marvelon,gestrinone and leuprolide for 3 months.The two groups were compared in clinical effects,recurrence and pregnancy rate,as well as the operation situation.Results The effective rate was 92.0％ (23/25) in the observation group and 64.0％ (16/25) in the control group.The effective rate of observation group was obviously higher than that of the control group(P ＜ 0.05).The recurrence rate was 8.0％ (2/25)in the observation group,and 32.0％ (8/25) in the control group.The recurrence rate of combination group was obviously lower than that of the simple surgical group(P ＜ 0.05).The pregnancy rate was 80.0％ in the observation group;and 48.0％ in the control group after treatment.The pregnancy rate of the observation group was obviously higher than that of the control group(P ＜ 0.05).Liver damage or other adverse reactions didn't occur to any group after the treatment.The mean time of leaving bed was (1.4 ± 0.4) d after operation in the observation group vs (3.6 ± 1.5) d after operation in the control group.Length of hospital stay was (7.6 ± 3.5) d in the observation group vs (11.5 ± 3.9) d in the control group.The difference had statistical significance(all P ＜ 0.05).Conclusions Laparoscopic surgery combined with marvelon,gestrinone and leuprolide is superior to pure surgery group in treatment of endometriosis.It can improve the effective and pregnancy rate,and reduce the recurrence rate.No obvious adverse reactions can be found,thus it is worth of clinical promotion.

Objective To investigate the effects of extractum trametes robiniphila murr combinated with hydroxyurea on proliferation and apoptosis of K562 cells and whether the 2 drugs have synergistic effects,and to detect the antiproliferation and apoptosis ratio of cells in diffe-rent groups were detected.Methods K562 cells were cultured in vitro,and logarithmic phase cells were used for study.MTT-inhibitory test and cell morphological analysis were employed to examine the effects of extractum trametes robiniphila murr alone or in combination with hydroxyurea on proliferation of K562 cells.The effects of extractum trametes robiniphila murr alone or in combinated with hydroxyurea on apoptosis of K562 cells were examined by flow cytometry.The expressions of bcr-abl,bax and bcl-xl mRNA levels were detected by way of reverse transcription polymerase chain reaction(RT-PCR).Results Extractum trametes robiniphila murr could inhibit proliferation and induce apoptosis of K562 cells,and extractum trametes robiniphila murr combinated with hydroxyurea had a synergistic effects on cell prolife-ration and apoptosis.The RT-PCR displayed that either extractum trametes robiniphila murr alone or in combinated with hydroxyurea could down-regulate the expression of bcl-xl mRNA and up-regulate the expression of bax mRNA and the effect of the 2 drugs had a synergistic effect on bcl-xl and bax mRNA expressions.Extractum trametes robiniphila murr alone could also down-regulate the expression of bcr-abl mRNA,but hydroxyurea alone did not show any effect on the expression of bcr-abl mRNA and the 2 drugs had no synergistic effect.Conclusions The effect of extractum trametes robiniphila murr combinated with hydroxyurea on the proliferation and apoptosis of K562 cells has a synergistic effect.The mechanism of antiproliferation and inducing apoptosis of extractum trametes robiniphila murr is probably related to down-regulating the expression of bcl-xl and bcr-abl mRNA and up-regulating the expression of bax mRNA.The synergistic effect of antiproliferation and inducing apoptosis of extractum trametes robiniphila murr combinated with hydroxyurea on K562 cells is probably related to down-regulating the expression of bcl-xl mRNA and up-regulating the expression of bax mRNA.

AIM:To evaluate the clinical efficacy and safety of balloon dilatation in infants with lacrimal passage obstruction.METHODS:Totally 86 patients (116 eyes) with lacrimal duct obstruction from July 1, 2015 to June 30, 2016 were randomly divided into two groups according to the digital table.The observation group (43 cases, 60 eyes) were operated with balloon dilatation and the control group (43 cases, 56 eyes) were treated with duct exploratory operation.The patients were followed up for 6mo to compare the efficacy.RESULTS:At the 6mo postoperatively, the primary cure rate and total cure rate in the observation group were significantly higher than those in the control group.There was significant difference between the two groups (P<0.05).CONCLUSION:Balloon dilatation operation is safe, and its clinical efficacy is better than lacrimal duct exploratory operation, is an effective way to treat lacrimal duct obstruction in infants.

Objective To summarize the experiences of ultrasonic liposuction in 204 cases, and to analyze the advantages and disadvantages of this technique. Methods The ultrasonic system (SILBERGEE E.U.A. TM ) and the liposuction equipment (HEROULES TM , American) were applied with the tumescent technique in 204 cases. Results Satisfactory rate was 90.69 %. After operation the highest sucked fat quantity was 6 800 ml, and the fewest quantity 250 ml, with an average of 1 907.6 ml for each case. Average volume of sucked fat in each position was 1 005.5 ml, which exceeded that by the traditional method. Advantages of this technique were larger fat quantity, obvious effect, less bleeding and pain, and safety. Disadvantages and complications were rough and uneven surface, accumulation of the liquid, bilateral asymmetry, slow wound healing, subcutaneous ecchymosis and hardening nodule, and even decreased blood pressure. The corresponding processing technique was recommended after liposuction. Conclusion This method is a simple and convenient, safe and reliable technique. It is suggested that multiple times and less portions of liposuction may be of benefit to patients.

Objective:To explore the antibacterial effect of Belamcanda Chinensis DC and Por tulaca oleracece L on P.aeruginosa (PA) in vitro.Methods:Fourty six strains PA were tested for minimum inhibition concentrati on ( MIC) by water decoct agents of the two drugs.MIC50 and MIC90 were st atistically studied.Results:For Belamcanda Chinensis DC,MIC was 31.25～3.90 g/L;MIC50 was 7 .81 g/L,and MIC90 was 15.62 g/L;whereas， for portulaca oleracece L,MIC was 31 .25～7.81 g/L, MIC50 was 15.62 g/L,and MIC90 was 31.25 g/L.Conclusion:Both of the two drugs have stronger antibacterial effects on P.ae ruginsosa in vitro.

Objective To explore the secretion of chemokine regulated upon activation normal T cell expressed and secreted(RANTES)influenced by the complex microenvironment in the peritoneal cavity of women with endometriosis and investigate chemotaxis of RANTES on the peritoneal monocytes.Methods The contact and non-contact co-culture systems including three target cells of ectopic tissue were established.The three target cells were endometrial stromal cells(ESC),human peritoneal mesothelial cells(HPMC)and monocytes.After collection of the supernatant of co-culture systems,the levels of RANTES were detected by enzyme linked immunosorbent assay(EUSA).Migration of U937 cell,a monocyte line,was detected by chemotaxis assay.Results ESC,HPMC,and U937 cultured alone secreted slight RANTES,(5.0±0.5),(4.0±0.3),and (254±40)ng/L. Compared with the culture of the target cell alone,the levels of RANTES in each co-culture system increased significantly,with the highest level in the contact culture system of E-H-U(2250±96)ng/L. RANTES secretion of non-contact co-culture of three cells were higher than contact co-culture of two cells(P＜0.01):U/E-H(912±93) vs E-H(50±40)ng/L,H/E-U(1201±93) vs E-U(243±192)ng/L,and E/H-U(1519±96) vs H-U(1251±73)ng/L. ESC,HPMC,and ESC-HPMC co-culture improved significantly migration of U937 cells [ number of cell migration respectively(6.0±0.3),(6.2±0.3),(10.0±0.3)×103,P＜0.01],which could be inhibited efficiently by anti-RANTES neutralizing antibody.Condusion The target cells in the peritoneal cavity of patients with endometriosis promote the secretion of RANTES in autocrine and paracrine manners and migration of monocytes.
