Objective To study the histocompatibility of domestic new type nitinol alloy ventricular septal defect(VSD) occluder in order to test the feasibility for clinical application.Methods The domestic new type nitinol alloy VSD occluder was inserted into 20 healthy dogs with intravenous balanced anesthesia and tracheal intubation,assisted by animal breathing machine respiration.The dogs were given oral antibiotic and would be killed in several months after operation.According to different slaughter time(1,2,3,6 months),they were divided into 4 sections and including 5 dogs in each section.The occluder sites,the intimal coverage and the change of inflammatory cells were observed through gross anatomy,with the help of the light microscope and electron microscope.SPSS 11.0 software was used to analyze the data.Results The thickness of the endocardial enhanced with the time(P
2.Application of Pemberton's osteotomy in the treatment of congenital hip dislocation of chidren

[Objective]To summarize clinical experience of Pemberton's ilium osteotomy in developing hip dislocation and emphasize the combined and individual principles in the clinical practice. [Methods]From February 1996 to April 2007,26 patients(aged 3 to 10 years) with 28 hips were operated on together with soft tissue solution,a pericapsular osteotomy of the ilium (Pemberton) and an optional femoral rotational,shortening or varus osteotomy of the proximal femur. Pre-and post-operative indexes including those of AI angle,CE angle and deep of acetabulum were compared. According to Severin's classification and McKay clinical curative effect evaluation critera,these indexes were evaluated.[Results]According to the Severin's classification,the results were rated as 64.3% excellent,28.6% good and 7.1% fair.On the McKay's critera ,the functional results were graded as 50.0% excellent,35.8% good,7.1% fair and 7.1% poor. On the whole,the radiographic evaluation was superior to that of the functional results.[Conclusion]The Pemberton's osteotomy is safe and effective.The combined surgical treatment can improve the success rate and decrease complications.

5.Establishment functions and processes in the Wound Care Center

Objective: To investigate the reasonable establishment of functions and processes in the Wound Care Center,and discuss the duties and roles of its enterostomal therapists(ET).Methods: According to the army's related infection management rules and nosocamiol infection control criteria,our Wound Care Center is divided into 8 areas,each with its set function and corresponding equipment and materials.We have laid out the consulting process for patients,stoma and wound management processes for nurses,responsibilities for ET and an operation mode for the Wound Care Center.The targets attained in work,teaching and research in the previous two years were analyzed and compared.Results: The index of the attainment of the tangets increased dramatically,with a growth rate of more than 20% in 8 of the 9 targets,most obviously in teaching and research.Conclusion: The division of the functional areas has proved to be reasonable,each operational process capable of meeting patients' needs and technical requirements,which enables ET to play important professional roles in the specialized field.

7.Clinical and Pathological Analysis of 91 Children with Lupus Nephritis

Objective To analyze the clinical and pathological characteristics of children with lupus nephritis(LN).Methods Ninety-one children with LN were diagnosed from 1993 to 2005,according to the clinical literature and renal pathology and the data were retrospectively summarized.Results Within the cohort of 91 children,there were 69 females and 22 males(female to male ratio 3.1).Most of the sick children were at the school age ranging from 6.0 to 15.5 years old.Nephrotic syndrome(44.0%) was the most common clinical manifestation.Fifty-nine renal biopsies were performed.Class Ⅳ LN(59.3%) was the most frequent pathological findings."A full-house pattern" on immunofluorescence was found in 72.0% of biopsies.The clinical and pathological manifestations of some children were atypical.There were 3 patients characterized by predominant deposits of immunoglobulin M(IgM),1 patient with predominant deposits of IgA,and 2 children with pauci-immune LN.Three children with class Ⅱ LN in our study presented with nephrotic syndrome.LN was initially controlled by aggressive treatment in 93.7% of thse patients.Relapses of nephritis covered 27.1% of them,mostly caused by the intermittent treatment.Conclusions The clinical and pathological manifestations of LN were variable.Some atypical LN was considered to be associated with the distinct pathogenesis.Most of LN could be controlled by aggressive treatment.Long and regular treatment is necessary to improve the prognosis of LN.

Objective To evaluate the efficacy of radiosynovectomy with 32P colloid in haemophilic synovitis of adolescents. MethodsRadiosynovectomy with 32P colloid was primary performed on 26 male haemophilic patients(26 joints),whose average age was 16 years(11 to 21 years).The average dose of 32P colloid was 2.1 mCi(1.0 to 3.0 mCi). Objective To assess the value of integrated 18 F-fluorodeoxyglucose (FDG) PET/CT in differentiation of malignant and benign pericardial effusion. Methods 18F-FDG PET/CT were performed in 23 patients with pericardial effusion. The detected soft tissue tumor or nodulous lession in pericardium or the thickened pericardium, with the maximum standardized uptake value( SUVmax ) ≥2.5, was defined as PET/CT-positive. The invaded lession in pericardium with SUVmax ≥2.5 was also as the positive. The difference of SUVmax of benign and malignant lesions was analyzed with two-independent-sample test of nonparametric tests. The final diagnosis was confirmed by biopsy or post-operative pathology. Results The diagnosis were confirmed with 14 malignant and 9 benign lesions. The median of SUVmax was 6.0 in malignancy group and 2.2 in benign group (z= -3. 279, P =0.001 ). According to the pathology results, there were one false negative case and two false positive cases with PET/CT imaging interpretation. The sensitivity, specificity,accuracy, positive predictive value ( PPV ) and negative predictive value ( NPV ) of 18 F-FDG PET/CT in diagnosis of benignity or malignance of pericardium effusion were 92.9% ( 13/14), 7/9, 87.0% (20/23),86.7% (13/15) and 7/8, respectively. Conclusion For the patients with pericardium effusion 18F-FDG PET/CT may be a helpful modality for malignancy differentiation
10.Effect of Yiguan Decoction on differentiation of bone marrow mesenchymal stem cells into hepatocyte-like cells: an experimental research.

OBJECTIVETo observe the effect of Yiguan Decoction (YGD) on differentiation of bone marrow mesenchymal stem cells (BMSCs) into hepatocyte-like cells in vitro.
METHODSRat BMSCs were isolated using whole bone marrow adherent method. The properties of BMSCs were identified by analyzing the expression of surface cytokines by flow cytometry. The third passage cells were differentiated into fat cells to identify their features. BMSCs were incubated with hepatocyte growth factor (HGF) plus fibroblast growth factor 4 (FGF4) or YGD containing serum YGD for 21 days. The mRNA expression of alpha-fetoprotein (alphaAFP), albumin (Alb), and hepatocyte nuclear factor 4alpha (HNF4alpha) were detected by real time PCR. Expression of AFP and cytokeratin 18 (CK18) protein was detected by cell immunofluorescence. Glycogen synthesis was observed using periodic acid-Schiff stain (PAS). CK18, Wnt 3alpha, and alphacatenin protein expressions were detected by Western blot.
RESULTSHigh expression of CD90, CD29, and CD44, and low expression of CD34 and CD11b were observed in BMSCs isolated by whole bone mar- row adherent method, and numerous lipid droplets were observed in BMSCs using oil red O staining. Both YGD containing serum and growth factor stimulated the expression levels of Alb, AFP, HNF4alpha mRNA and CK18 protein. The down-regulated expression of Wnt 3alpha and beta-catenin could be detected at 21 days after induction. The synthesized glycogen granule could be seen. Down-regulated Wnt 3alpha and beta-catenin expression could also be observed.
CONCLUSIONYGD could induce the differentiation of rat BMSCs into hepatocyte-like cells, which was related to down-regulating Wnt/beta-catenin signal pathway.
