1.An Outbreak of Seasonal Influenza Viruses A(H1N1) in Changsha Was Diagnosed by Laboratory and the HA Gene Characteristic Was Analyzed

To determine the etiologic agent of an outbreak of influenza viruses from Changsha Foothill Mountain International School in 2009, and to analyze the HA Gene Characteristic of the H1N1 influenza viruses. Twenty-five nasopharyngeal swab specimens from the outbreak of influenza viruses were tested by conventional RT-PCR and influenza viruses isolated simultaneously. Virus isolated (A/Yuelu/314/2009) from the outbreak was sequenced by CEQTM 8000 Genetic Analysis System and the sequencing results submitted to GenBank (Accession No: FJ912843), then the sequencing data was analyzed by ClustalX and Mega4.1softwares. Results showed the influenza viruses A(H1N1) of positive were 18 cases by influenza viruses isolated tests and 21 cases by conventional RT-PCR, respectively. The nucleotide and amino acid sequence homology of the HA gene of A/Yuelu/314/2009 are 99% compare with the vaccine strain (A/Brisbane/59/2007) in 2008~2009 years. The HA sequence data also showed that had 6 amino acid mutations (V148A, S158N, G202A, I203D, A206T, W435R), and the S158N located at antigenic site B of HA protein. Nine potential glycosylation sites (27, 28, 40, 71, 151, 176, 303, 497, 536) in the HA sequence of A/Yuelu/314/2009 is the same with A/Brisbane/59/2007, and the sequences of potential glycosylation sites were conserved. In this study, laboratory evidence diagnosed seasonal influenza A virus (H1N1) as the etiologic agent of the outbreak. The virus isolated (A/Yuelu/314/2009) strain of H1N1 subtype is not a new variant in Changsha in 2009 compare with the vaccine strain (A/Brisbane/59/2007), the outbreak of influenza A virus (H1N1) from Changsha Foothill Mountain International School maybe are caused by the change in genetic characteristics between vaccine strains and the decreased of immunity to influenza A virus (H1N1) in the crowd.
2.Research on Comparison of Imaging Parameters in Intravascular Ultrasound Elastography

Objective To make the validation of intravascular ultrasound elastography (IVUSE) for detecting the plaque.Methods A plain strain model of vascular plaque was established and then the RF signals of IVUS were produced.The radial displacement distribution and strain distribution were calculated through 1D numerical optimization based on the RF signals.Then the elastic moduli distribution was reconstructed by finite element method (FEM) and iteration method.Results The three elastic parameters,displacement/strain/moduli,all could detect the plaque.Elastic moduli presented the information of the shape and size of the plaque,meanwhile,the displacement and strain distributions could not give the information clearly.Conclusion Elastic modulus is proved to be superior to the other two parameters in detecting the plaque,whereas it is subject to the accuracy of the strain's estimation.
4.Transport of limonin in rat intestine in situ and Caco-2 cells in vitro.

Limonin existed in citrus fruits has been shown to have anti-bacterial, anti-viral, anti-feedant, anti-nociceptive, anti-inflammatory activities and anti-carcinogenic activities. But the clinical use is limited by its low bioavailability. The aim of this study is to observe the absorption and secretion transport mechanisms of limonin in intestine which can pave the way for the further study and clinical use. The transport characteristics and mechanisms of limonin in rat were studied by in situ intestine perfusion and in vitro Caco-2 cells method. The intestinal absorption of limonin was probably via a facilitated diffusion pathway which was poor and without segment-selection. Verapamil and ketoconazole improved the absorption remarkably according to the result of in vitro Caco-2 cells study; however, probenecid had no significant effect on the absorption. The P-gp efflux and CYP3A4 metabolism were involved in the poor intestinal absorption and low bioavailability of limonin. The exploration of the intestinal absorption mechanism is crucial to the design of dosage form and clinical use of limonin.

5.Clinical value of the current serum creatinine reference interval

Objective We aim to evaluate the value of the current serum creatinine reference interval ( RI ) provided by Industry Standard WS /T 404.5 in clinical practice.Methods The first time serum creatinine levels and urinary albumin /creatinine ratio were obtained from 67 605 adult patients ( <60 years old) who were treated in the First Hospital of China Medical University between October 1, 2014 and September 30, 2015 in this cross-sectional study.Estimated glomerular filtration rate ( eGFR ) calculated by chronic kidney disease epidemiology collaboration (CKD-EPI) equation and urinary albumin /creatinine ratio (ACR) were used to evaluate the clinical practical significance of current and old serum creatinine RIs as the criteria.Results 4.3% of normal subjects based on current RI were showed decreased eGFR, 98% of abnormal subjects based on the current RI were founded to have decreased eGFR . 1378 subjects were evaluated as increased based on current RI but as normal based on old RI , and 93.5% of these subjects were showed decreased eGFR .In addition, ACR was measured in 26 cases, and 18 out of 26 cases (69.2%) were confirmed to have elevated ACR (≥30 mg/g) and proteinuria.On the other hand of analysis, screening positive rates of declined eGFR were 43.6% by old RI and 61.9% by current RI in the subjects with eGFR under 90 ml(min ×1.73 m 2 ), and the performance of the current RI was obviously improved(χ2 =212.648,P <0.001).Conclusions The current reference interval of serum creatinine is favorable for the detection of renal dysfunction in patients .It is recommended that the current reference interval can be applied in the clinical laboratories as early as possible .
6.Pelvic lipomatosis: urodynamic characters and clinical significance

Objective To study the urodynamic characters of pelvic lipomatosis(PL) and its clinic significance.Methods From Sep.2013 to Feb.2016,7 patients,including 6 males and one female,were diagnosed as PL by image examinations and related lower urinary tract syndrome.Their mean age was 46 years,ranged from 30 to 52 years.All the patients(study group) have normal renal function.However,4 patients were noticed bilateral hydronephrosis.Other patients with similar LUTS but were confirmed as normal by image and urodynamic examinations were concluded in control group,which was paired designed to match the study group in visiting time,gender,age,and number of patients.The uroflowmetry and pressure/flow cystometry were performed in all patients.The urodynamic data of two groups including maximum flow rate (Qmax),time to maximum flow,post-voiding residual (PVR),first sensation capacity,first desire to void capacity,cystometric capacity,intravesical pressure at terminal of filling phase,compliance,detrusor pressure at maximum flow,detrusor opening pressure,bladder outlet obstruction index (BOOI) and bladder contractility index (BCI),were retrospectively analyized.Results There were no significant different between study aud control groups respect to Qmax [(18.71 ± 10.31) ml/min vs.(13.29 ± 6.55) ml/min],time to maximum flow [(6.71 ±2.50)s vs.(7.43±2.76)s],PVR [(28.14±27.81)ml vs.(3.14± 3.48) ml],first sensation capacity[(138.86 ± 77.01) ml vs.(142.29 ± 89.84) ml],first desire to void capacity [(203.71 ± 131.09) ml vs.(216.57 ± 72.20) ml],cystometric capacity [(271.0 ± 151.95) ml vs.(323.29± 92.87)ml],i ntravesical pressure at terminal of filling phase [(30.29 ± 16.45)cmH2O vs.(18.71 ± 9.57) cmH2 O],and compliance [(90.53 ± 126.46) cmH2O/ml vs.(129.57 ± 136.25) cmH2O/ml].The study group was significant higher than control group in terms of pressure at maximum flow [(69.57 ±25.06) cmH2O vs.(33.14 ± 11.63) cmH2O,P =0.004],opening pressure [(42.57 ± 22.16)cmH2O vs.(18.00±13.18) cmH2O,P=0.027],BOOI [(34.00±15.92)vs.(16.50±5.96),P=0.030] and BCI [(133.17 ± 27.37) vs.(87.67 ± 20.16),P =0.008],respectively.Moreover,the BCI of PL patients with hydronephrosis were significant higher compare with PL patients without hydronephrosis [(155.67 ±14.98) vs.(110.67 ± 1 1.37),P =0.014].Conclusions The urodynamic characters of PL were increased pressure at maximum flow and opening pressure,which implied obstruction.
7.Clinical Value of ITMAIS in the EPLAD Assessment for Profound Hearing Impaired Children with Hearing Aids

Objective To explore the clinical value of ITMAIS in the early prelingual auditory development (EPLAD)assessment for bilateral profound hearing impaired children with hearing aids.Methods Profound sensorineural hearing impaired children fitted with hearing aids during October 2008 and July 2011 in our center were recruited in this retrospective study, and they would be tested with the ITMAIS at baseline, 3 months, and 6 months after hearing aids fitting.A total of 51 children were recruited in the study, and divided into 2 age groups according to the age at fitting, 1 yr group (28 children) and 2 yrs group (23 children).Results The EPLAD of children in both groups improved significantly in the first half of year after fitting (P<0.05).The mean EPLAD levels of children in both groups were close to or achieved the mean level of severe sensorineural hearing impaired children without intervention when they had had the hearing aids for 6 months.Conclusion Hearing aids could help the children with profound sensorineural hearing loss to improve the abilities of EPLAD.ITMAIS is a simple, an effective tool for assessing the early outcome of children with hearing aids, especially for the children who could not cooperate with the behavioral hearing test.
8.Effect of Different Proportions of Mixed Blood Exchange Transfusion on Blood Internal Environment in Neonates with Hemolytic Disease

Objective To explore the effect of different proportions of mixed blood exchange transfusion on blood circulation in neonates with hemolytic disease.Methods Thirty-one newborn infants with hemolytic disease were treated by peripheral arteriovenous synchronization of exchange transfusion with different proportions mixed blood.AB type plasma was mixed with O type red blood cell(RBC) washing.The proportion for the treatment group was 1:1(the O type RBCs 2 U:the AB type plasma 200 mL),by exchange transfusion of haplotypes,in accordance with 80?mL/kg;the proportion for control group was 2:1(the O type RBC 4 U:the AB type plasma 200 mL),by exchange transfusion of double in accordance with 150-180 mL/kg.The indicators were detected,such as the exchange rate of neonatal serum bilirubin,RBC,hemoglobin(Hb),hematocrit(HCT),and the exchange transfusion quantity and days of hospitalization before and after the exchange transfusion were analyzed.Results The exchange rate of serum bilirubin of treatment group and control group was (44.92?3.99)% and (45.69?5.06)%,respectively,there was no significant difference between 2 groups(P=0.639),there was no significant difference of hospitalization days[(8.13?1.13) d vs(8.19?0.91) d]between 2 groups(P=0.884).After exchange transfusion in treatment group,the average level of the RBC,Hb and HCT were increased(P
9.Effect of LY294002 and its combination with chemotherapy drugs on the proliferation of human leukemia K562 cell line and its possible mechanism

Background and purpose:Remarkable advances have been made in cancer chemotherapy by developing new anticancer drugs and therapy strategies.However,multidrug resistance in human cancers remains a major clinical challenge for cancer treatment.Attempts in several clinical studies to reverse multidrug resistance protein (MDR) by using MDR modulators have not yet generated promising results.Our aim was to explored the mechanism of reversal of multidrug resistance in human leukemia K562 cells by PI3-K inhibitor.Methods:Trypanblue dye exclusion method was used to observe the drug sensitivity and the effect of LY294002 on the drug resistance.Western blot to analyze P-gp and p-Akt phenotypes,and flow cytometer was used to measure the intracellular drug accumulation. Results:K562/D induced by DNR was cross resistant to DNR,ADR,VCR and VP16 (Resistance Index:65,52,134 and 50 respectively).DNR induced over-expressions of P-gp and p-Akt in K562/D cells;LY294002 increased the intracellular drug accumulation,and then reversed the drug resistance to DNR,ADR,VCR and VPI6 in K562/D cells(Resistance Index:23,21,63 and 29 respectively),but not in the sensitive cells (K562/S).Conclusion:The multidrug resistance of K562/D cells can be induced by DNR which is related to the P-gp and p-Akt over-expressions, and LY294002 can reverse multidrug resistance in human leukemia cells in vitro via inhibits PI3-K/Akt pathway.
