2.Clinical observation on effect of jingling oral liquid in treating infertile patients with varicocele after varicocelectomy.

OBJECTIVETo observe the clinical effect of Jingling oral liquid (JLOL) in treating infertile patients with varicocele after varicocelectomy.
METHODSSixty patients were randomly divided into two groups, the 30 patients in the treated group treated by JLOL, and the 30 in the control group treated with intramuscular injection of human chorionic gonadotropine (hCG).
RESULTSThe pregnant rate of patient's wife in the treated group was 76.6%, while that in the control group was 40.0%, showing significant difference between them (P < 0.05). The quality of semen was improved in both groups (P < 0.05 or P < 0.01), and the improvement was better in the treated group than that in the control group (P < 0.05). The reproductive hormones were also improved in both groups (P < 0.01). In the treated group, levels of superoxide dismutase (SOD) and zinc in semen increased, and that of cadmium decreased after treatment, as compared with those before treatment, the difference was significant (P < 0.05).
CONCLUSIONJLOL could improve and regulate the reproductive hormone disturbance in infertile patients with varicocele after varicocelectomy, enhance their quality of semen and sperm to increase the pregnancy rate of their spouses.

AIMTo approach the effects of multi-site synchronous ventricular pacing on myocardial mechanics and cardiac work.
METHODSFive modes of multi-site synchronous ventricular pacing were randomly performed in 12 dogs with anesthetized, opened chest and artificial-ventilation. Some parameters were measured simultaneously including: the peak of left ventricular pressure rise and fall (+/- dp/ dt(max)), the time constant of left ventricular relaxation(tau), the muscle tensile force in left/right ventricular wall (V-tensile force, V-TF), SV, LVSW and RVSW.
RESULTSThe myocardial systolic mechanical parameters: +dp/dt(max) and LV-TF of cHisB-LVPL and RVA-LVPL pacing by biventricular pacing modes were increased than that of cHisB-RVA pacing in right ventricular bifocal pacing mode. +dp/dt(max) in above two groups of biventricular pacing was increased than that in cHisB-RVA pacing. Tau value of cHisB-LVPL and RVA-LVPL pacing modes were shorted than that of cHisB-RVA pacing. The above parameters of cHisB-RVA-LVPL and cHisB-RVA-LVA biventricular trifocal pacing were superior to that of cHisB-LVPL and RVA-LVPL biventricular pacing. The +dp/dt(max), LV-TF and RV-TF of cHisB-RVA-LVPL pacing were increased as compared with that of cHisB-RVA-LVA pacing (P > 0.05). The -dp/dt(max) in cHisB-RVA-LVPL pacing were increased by 6.0% and tau value was shorted by 3.7% compared with those in cHisB-RVA-LVA pacing (P > 0.05). SV, LVSW and RVSW of cHisB-LVPL and RVA-LVPL biventricular pacing were increased than those of cHisB-RVA bifocal pacing. The above parameters of cHisB-RVA-LVPL pacing were increased than that of cHisB-RVA-LVA and cHisB-LVPL pacing.
CONCLUSIONIt was explained that the cHisB-RVA-LVPL biventricular trifocal sites synchronous pacing mode would increase the velocity of ejection and filling during myocardial contraction and relaxation and enhance cardiac work by maintaining normal VSS.

OBJECTIVETo explore the effect of anti-VEGF hairpin ribozyme gene on the tumor cell growth and tumor angiogenesis in nude mice.
METHODSThe recombinant eukaryotic expression plasmid pcDNA-RZ containing anti-VEGF hairpin ribozyme gene and the empty vector plasmid pcDNA were introduced separately into K562 cells by lipofectamine mediation and positive clones were screened by G418. Ribozyme gene in K562 cells was confirmed by PCR. Fluorescent real time RT-PCR and Western blot were used to detect the expression of VEGF mRNA and protein in the leukemia cells. The tumorigenicity of transfected K562 cells were transplanted in nude mice and tumor microvascular density (MVD) were observed by morphology and vWF immunohistochemistry stain.
RESULTSStable expression of the ribozyme gene in K562 cells was confirmed by PCR. The level of VEGF mRNA and protein decreased dramatically in K562/RZ cells when compared with K562 or K562/PC (K562 cells transfected with empty vector) cells. The tumor volumes were (4.43 +/- 0.87), (3.96 +/- 0.94), (2.24 +/- 0.56) cm3; tumor weight was (4.43 +/- 0.87), (3.96 +/- 0.94), (2.24 +/- 0.56)g; and tumor microvascular density was 4.70 +/- 1.25, 4.67 +/- 1.31, 1.80 +/- 1.55 in K562, K562/PC and K562/RZ cell groups, respectively.
CONCLUSIONTransfection with anti-VEGF ribozyme gene can inhibit tumor growth and vessel formation by down-regulating the VEGF gene expression in K562 cells.

6.Clinical study thoracolumbar Chance fractures: a report of 21 cases.

OBJECTIVETo discuss the mechanism, clinical characteristics, diagnosis and treatment of thoracolumbar Chance fractures.
METHODSAmong all the 21 patients, 13 patients were male and 8 patients were female, ranging in age from 23 to 57 years, with an average of 32 years. The injury was at the level of T11 in 3 patients, at the level of T12 in 9 patients, at the level of L1 in 8 patients and at the level of L2 in 1 patient. According to Denis classification, 10 patients were type A, 2 type B, 5 type C, and 4 type D. All the patients were treated with pedicle screw internal fixation and posterior lateral fusion at the level of injury.
RESULTSTwenty-one patients had been followed up for an average of 18 months (13 to 28 months). According to ASIA neurologic grading system, 4 cases in grade C or D recovered to grade E postoperatively and one case in grade B was not improved. Postoperative X-ray showed good reduction and fixation in all cases and no looseness or breakage in the fixation system. The results of postoperative back pain and function assessment were mostly satisfactory.
CONCLUSIONThoracolumbar Chance fractures are due to flexion-distraction injuries and regarded as unstable, which are uncommon and usually not present with a neurologic deficit. Reduction and stabilization with posterior pedicle screw internal fixation is suggested in patients with thoracolumbar Chance fractures.

7.CD36 expression in leukemia cells checked with multi-parameter flow cytometry and its significance.

The aim of study was to investigate the expression of CD36 in leukemia cells and to explore its significance in diagnosis and differential diagnosis for leukemia in patients. Blood samples from 133 cases of leukemias were analyzed by CD45/SSC double parameters and multi-color flow cytometry in order to determine the CD36 and other leukocyte differentiation antigens. The results show that the CD36 positive rate was 21.8% (29/133) in 133 cases of leukemia, 41.9% (26/62) in 62 cases of AML (acute myeloid leukemia), and none of the 54 cases of lymphocytic leukemia was positive for this antigen. The positive rate of CD36 in M(4) (8/10), M(5) (12/12) and M(6) (3/3) was significantly higher than that in M(1) (0/9), M(2) (3/12), M(3) (0/16) (all P < 0.001). The percent of positive cells of CD36 in M(5a) was significantly higher than in M(5b) (P = 0.001). A significantly negative regression was found between CD36 and CD117 in AML (r = -0.751, P = 0.005). And a significantly positive regression was found between CD36 and CD14 in M(4) and M(5b) (r = 0.870, P = 0.011). In monocyte lineage involved leukemia (MLIL), the positive rate of CD36 (92.6%, 25/27) was significantly higher than that of CD14 (48.1%, 13/27)(P = 0.001). None of the 7 cases with M(5a) was positive for CD14, but 4 of 5 cases of M(5b) were positive. The positive rate of CD117 in M(5a) was significantly higher than that of in M(5b)(P = 0.01). The positive rate of CD34 in M(5) was low (33.3%, 4/12). It is concluded that the combination of CD36 with lymphoid and myeloid antigens is helpful to the diagnosis and differential diagnosis of lymphoid, myeloid and MLIL. The positive rate of CD36 is higher than that of CD14 in MLIL.

8.Influence of intracoronary administration of anisodamine on no-reflow, ventricular function and systolic synchrony in acute myocardial infarction patients undergoing percutaneous coronary intervention.

OBJECTIVETo evaluate the influence of intracoronary administration of anisodamine on myocardial blush grade (MBG) and left ventricular regional and global systolic function and synchrony in the acute myocardial infarction (AMI) patients with no-reflow phenomenon post percutaneous coronary intervention (PCI).
METHODSForty-seven AMI patients who underwent PCI within 12 hours of onset and MBG was 0 - 1 were randomized to receive standard therapy [group B, n = 23, 18 males, mean age (62.72 +/- 11.48) years] or standard therapy plus intracoronary administration of anisodamine [200 microg/ml, group A, n = 24, 18 males, mean age (64.23 +/- 12.27) years]. The left ventriculography (LVG) was performed immediately and 6 months after PCI to measure the ventricular volume, LVEDP and wall motion score (WMS). Equilibrium radionuclide angiography (ERNA) was performed 1 week and 6 months after PCI to determine the parameters of left ventricular regional, global systolic function and systolic synchrony. Incidence of major adverse cardiac events (MACE) during the follow-up was analyzed.
RESULTSAnisodamine [(2530 +/- 340) microg/person)] was well tolerated by patients. The MBG remained unchanged in group B and significantly increased from grade 0.74 +/- 0.32 to grade 2.33 +/- 0.28 10 min after anisodamine injection in group B. Six months post PCI, LVESVI [(40.53 +/- 8.12) ml/m(2) vs. (50.32 +/- 8.26) ml/m(2)], LVEDVI [(80.13 +/- 9.74) ml/m(2) vs. (87.17 +/- 10.25) ml/m(2)], WMS [(8.24 +/- 1.31) vs. (10.23 +/- 1.82)] and LVEDP [(13.36 +/- 4.21) vs. (16.38 +/- 3.21) mm Hg, 1 mm Hg = 0.133 kPa] were significantly lower in group A compared with that in group B (all P < 0.05) while LVEF [(44.02 +/- 5.86)% vs. (38.52 +/- 5.18)%], PER [(1.86 +/- 0.09) EDV/s vs. (1.61 +/- 0.09) EDV/s] and PFR [(2.19 +/- 0.32) EDV/s vs. (1.78 +/- 0.17) EDV/s] measured by ERNA were significantly increased in group A compared with that in group B (all P < 0.05). (2) LrEF(2)-LrEF(8) in group A were higher by 13.96%, 25.02%, 30.36%, 22.86%, 27.67%, 22.07% and 18.71% respectively compared with that in group B. (3) Phase analysis showed that the left ventricular systolic synchrony parameters PS [(46.04 +/- 8.93) degrees vs. (53.19 +/- 162) degrees ], FWHM [(23.02 +/- 6.27) degrees vs. (25.02 +/- 5.31) degrees ] and PSD [(7.92 +/- 4.12) degrees vs. (11.76 +/- 4.11) degrees ] were also significantly lower in group A than that in group B (all P < 0.05). (4) During the 6 months of follow-up, the incidence of MACE in group A was significantly lower than that in group B (P < 0.05).
CONCLUSIONIntracoronary administration of anisodamine is safe and could partly attenuate the no-reflow phenomenon, improve the left ventricular systolic function and synchrony and reduce the incidence of MACE in patients with no-reflow phenomenon post AMI-PCI.

10.Study of biosensor technology on the detection of endotoxin-neutralizing materials.

OBJECTIVETo explore the application of biosensor technology in the determination of endotoxin-neutralizing materials.
METHODSAfter mixing polymyxin B (PMB) with endotoxin in certain concentration, the neutralizing ratio of PMB to endotoxin was assessed by biosensor technique and limulus amebocyte lysate test respectively. The results from the two methods were compared.
RESULTSThe neutralizing ratio of PMB to endotoxin as assessed by biosensor technology was 0.35 microg to 1 ng, while that by dynamic turbidimetric and chromogenic limulus amebocyte lysate (LAL) technique was 0.5 mg to 1 ng and 1 mg to 1 ng, respectively. The results obtained by biotechnology were similar to that by biosensor technique.
CONCLUSIONBiosensor technology was an accurate, convenient and rapid method for the determination of potency of endotoxin-neutralizing materials.
