1.Clinical effects of a good control of blood sugar during perioperative period on surgical tolerance and postoperative recovery in elderly patients with gastric cancer and diabetes

Objective:To investigate clinical effects of a good control of blood sugar during perioperative period on surgical tolerance and postoperative recovery in elderly patients with gastric cancer and diabetes.Methods:A total of 92 elderly patients with gastric cancer and diabetes undergoing surgery in our hospital from August 2018 to March 2020 were randomly divided into a control group(taking conventional perioperative care, n=46)and an observation group(taking conventional perioperative care and good glycemic control, n=46). The changes of perioperative blood glucose level, surgical conditions and postoperative recovery in the two groups were observed and analyzed.Results:Fasting blood glucose level of observation group and control group was[(7.4±1.1)mmol/L vs.(7.9±1.3)mmol/L, t=-1.991, P=0.025]at 1 d before operation, [(7.2±0.9)mmol/L vs.(8.7±1.2)mmol/L, t=-6.782, P=0.000]during operation, and[(6.9±1.5)mmol/L vs.(8.5±1.1)mmol/L, t=-5.834, P=0.000]after operation.Anyway, fasting blood glucose level was lower in the observation group than in the control group.In observation vs control group, time of incision healing[(7.5±1.6)d vs.(9.7±2.2)d, t=-5.485, P=0.000], time to get out of bed[(3.3±0.8)d vs.(4.7±1.3)d, t=-6.221, P=0.000], length of stay in hospital[(10.5±2.7)d vs.(15.1±3.5)d, t=-7.058, P=0.000]were shorter in observation group than in control group.The incidence of postoperative complications was lower in the observation group than in the control group(8.70% vs.26.09%, χ2=4.484, P=0.028). Conclusions:The application of perioperative glycemic control can effectively maintain the perioperative blood glucose at a reasonable and stable level, improve the surgical tolerance and promote the postoperative recovery in elderly patients with gastric cancer and diabetes.And it plays a very positive role in ensuring the safety of surgery

This paper was aimed to investigate the effects of icariin on the expression of p38 mitogen activated protein kinase(MAPK)protein in rat osteoblasts cultured in vitro, to elucidate the signal transduction of icariin in preventing and treating osteoporosis. The results showed that p38MAPK could be activated by icariin within 5min, and Cbfal could also be upregulated, and SB203580, an inhibitor of p38MAPK pathway, could partly inhibit Cbfal upregulation by icariin.
3.The effect of radial extracorporeal shockwave therapy on scapulohumeral periarthritis in the elderly

Objective:To investigate the clinical efficacy of radial extracorporeal shockwave therapy(ESWT)for scapulohumeral periarthritis in the elderly.Methods:A total of 82 patients diagnosed with scapulohumeral periarthritis at the rehabilitation medicine department of our hospital from July 2016 to January 2019 were retrospectively analyzed.The visual analogue score(VAS), Constant-Murley score(CMS), range of motion(ROM), adverse reactions, curative effectiveness and survival analysis were examined before treatment and at several time points within 6 months of treatment.Results:Compared with pre-treatment, the VAS, CMS, ROM, curative effects and survival analysis of patients with primary and secondary scapulohumeral periarthritis improved after treatment in both the conventional rehabilitation therapy group and the conventional rehabilitation therapy combined with ESWT group(all P<0.05). The short-term effects(2 weeks and 1 month after treatment)were better in the conventional rehabilitation therapy combined with ESWT group than in the conventional rehabilitation therapy group( P<0.05). There was no significant difference in long-term effects(2, 4 and 6 months after treatment)between the two groups( P>0.05). For patients with primary scapulohumeral periarthritis, conventional rehabilitation therapy combined with ESWT had advantages over conventional rehabilitation therapy in reducing the VAS score and improving shoulder function(CMS score and ROM)( P<0.05). For patients with secondary scapulohumeral periarthritis, conventional rehabilitation therapy combined with ESWT was better in reducing VAS than conventional rehabilitation therapy( P<0.05), while there was no significant difference in shoulder function improvement between the two groups(all P>0.05). Conclusions:Conventional rehabilitation combined with ESWT can relieve shoulder pain, improve range of motion and has a good safety profile in the treatment of scapulohumeral periarthritis in the elderly, but there is no significant difference in long-term efficacy between conventional rehabilitation therapy and conventional rehabilitation therapy combined with ESWT.
4.Efficacy and Safety of Autologous Adipose-derived Stem Cells Transplantation in Patients with Type 2 Diabetes Mellitus

Objective to evaluate the clinical efficacy and safety of ADSCs infusion in patients with type 2 diabetes mellitus(t2DM). Methods Autologous ADSCs were cultured and identified. totally 48 cases of type 2 diabetes patients were divided into two groups:24 cases in A group re-ceived ADSCs and conventional treatment,while the other 24 cases in B group received conventional treatment. With 15 months′follow-up,FPG,2h-PGh,HbA1c,FC-P,AUCC,insulin medication changes and security on post-treatment 3,6,12 and 15 months were calculated. Results the AD-SCs which cultured met the clinical application of standards. FPG,2h-PGh and HbA1c in both groups were significantly decreased,and the values in A group were more impressive than B. there were significant improvement in FC-P,AUCC and insulin of A group compared with B group. there was not treatment related adverse events. Conclusion the clinical data demonstrate that the treatment with ADSCs can improve metabolic control and beta cell function in patients with t2DM. Moreover,the safety of the treatment method was considerable.
5.Perioperative observation and nursing of male patients with breast cancer

Objective To summarize the perioperative nursing of male patients with breast cancer.Methods Retrospectively, the clinical data of 9 male patients with breast cancer undergoing radical mastectomy from January 1990 to July 2011 were analyzed to summarize the perioperative nursing strategies.Result The radical mastectomy for the 9 patients was successful.2 patients contracted complications as hemorrhage in 1 case and flap necrosis in 1 case.Conclusion The perioperative nursing should be performed based on the characteristics of male patients with breast cancer.
6.Experimental study about absorbable balloon vertebroplasty for the treatment of thoracolumbar burst fracture

[Objective]To study the changes of biomechanical efficiency of absorbable balloon vertebraplasty associated with pedicle screw system for treatment of thoracolumbar fracture.[Method]Six fresh adult thoracolumbar specimens were harvested and 10 segmental specimens(T_(11)~L_1、 L_(2~4)、 L_(1~3)) were processed.After burst fracture created by free-drop test,the specimens were braced and reduced by APF pedicle screw,then absorbable balloon were placed through the traumatic vertebral pedicle and were infused with autosolidification calcium phosphate cement to process vertebroplasty.The stiffness in middle vertebrae were measured under flexion,extension,lateral flexion and torsion stress before fracture and after absorbable balloon vertebroplasty respectively.[Result]The crushing strength in traumatic vertebrae after vertebroplasty with absorbable saeculi increased showed 8% under flexion,15% under extension and 9.8% under lateral flexion,but with significant increasing under extension(P0.05).[Conclusion](1)absorbable balloon vertebraplasty associated with pedicle screw system conduces to the reconstruction of traumatic vertebrae,and making postoperative spinal biomechanical characteristics approach the level before fracture;(2)Vertebroplasty with absorbable sacculus for treatment of thoracolumbar burst fracture based on the premise that avoid the potential danger induced by leakage of bone cement,and can reestablish intact corporeal structure of traumatic vertebrae and restore their height essentially,so that spinal physiologic curve can be gained to restore and maintain,tardive lumbodorsal pain and spinal cord injury and nerve injury caused by secondary kyphosis can be prevented,this can fundamentally solve complications left over by thoracolumbar burst fracture treated using pedicle screw system simply.The new method prorides a rational method for clinical treatment of thoracolumbar burst fractures.
7.The less invasive stabilization system in treatment of complex proximal femoral fractures

Objective To discuss the feasibility,indications,surgical techniques,and outcome con- cerning the application of less invasive stabilization system (LISS) for complex proximal femoral fractures.Methods We treated 12 complex proximal femoral fractures using femoral LISS reversely from June 2005 to May 2006. They were five complex intertrochanteric fractures and seven subtrochanteric ones.By AO classification,two were type 31-A2.2,two type 31-A2.3,one type 31-A3.3,one type 32-A3.1,three type 32-B1.1,one type 32-B2.1, and two type 32-B3.I.The patients were treated with closed or indirect reduction and fixation with percutaneous LISS plate reversely.Results There were no major postoperative complications in this study.The mean operation time was 65 minutes (range,50 to 90 minutes),the mean intra-operative blood loss was 142 milliliters (range,50 to 400 milliliters),and the mean postoperative hospital stay was 9.3 days (range,6 to 15 days).All patients had a clinical follow-up;the mean follow-up time was 7.2 months (range,3 to 14 months).Ten cases healed clinically three months postoperatively,one periprosthetic fracture healed four months and one pathological fracture healed six months after operation.At the final radiographic follow-up,no collapse,varus deformity,cutting-out,hardware failure,or avascular necrosis was found.Conclusions Femoral LISS used reversely can provide secure fixation for proximal femoral fractures biomechanically and anatomically.It is also easy and safe.It is particularly suitable for old patients with intertrochanteric fractures complicated with disorders of internal organs and severe osteoporosis or patients with complex proximal femoral fractures.It is important to be skillful in indirect reduction and positioning of guide wire into hole A,and to avoid immediate weight-bearing postoperatively.

9.Roles of TPX2 in cell mitotic and the relation with tumorigenesis

TPX2(targeting protein for Xklp2)is a microtubule-associated protein in a new family of vertebrate spindle pole components.TPX2 is diffusely distributed all over the nucleus during the cell cycle phases S and G_2 and play important roles in mitotic spindle formation.TPX2 over expression was found in a variety of malignant tumors,that are asssociated with the centrosome amplification,aneuploidy,cell transfor- mation and the biological behavior of tumor.Blocking the expression of TPX2 can inhibit the growth of tumor cells.TPX2 may serve as a new candidate target for tumor therapy.The study progress of its structure and function,expression levels in cell cycle,molecular mechanism etc.was reviewed.
10.Effect of Various Kind of Dyslipidaemia on Insulin Resistance and Pancreatic Islet Function in Newly Diagnosed Type 2 Diabetic Patients

Objective To evaluate the effect of various kinds of dyslipidaemia on insulin resistance and pancreatic islet ? cell function in newly diagnosed type 2 diabetic patients. Methods 263 patients with newly diagnosed type 2 diabetes were divided into normal lipidemia group and dyslipidaemia group according to ATPⅢ standard. The dyslipidaemia group included 3 subgroups as the following: high TG group, high TC group, and mix group with high TG and TC. Homeostasis model assessment (HOMA) was applied to estimate insulin resistance (IR) and pancreatic islet ? cell function.
