1.Application study of multi-slice CT in distinguishing between benign parotid tumors and malignant parotid tumors

Objective To explore the clinical value of multi-slice CT in distinguishing between benign parotid tumors and malignant parotid tumors. Methods CT scanning images of 72 parotid tumor patients were analyzed. The location, shape, density, edge, cystic necrosis and degree of enhancement of the tumors were analyzed, de-gree of cervical lymph node swelling were analyzed, and all the analysis results were compared to pathological re-sults. Results 80 lesions were diagnosed in tumors, among which 59(73. 8%) lesions were benign tumors and 21 (26. 2%) lesions were malignant tumors. The CT images of benign tumors showed that 42(71. 2%) of them were located in the superficial lobe and with regular shape and uniform density;62.7% of them had clear edge;57 . 6%of them showed mild or moderate enhancement. 2 cases of benign tumors presented cervical lymph node swelling, which were proved to be pathological inflammatory mass after operation. Whereas in the CT images of malignant tumors 12( 57. 1%) of them were located in the deep lobe or in both of the superficial lobe and the deep lobe. 15 (71. 4%) of them had irregular shape and uneven density. 81% of them had unclear edge. 71. 4% of malignant tumors were significantly enhanced. 9 cases of malignant tumor were with cervical lymph node metastasis. There was statistical difference between the shape, edge, density, cystic necrosis, degree of enhancement and cervical lymph node metastasis of the two groups of tumors(P<0.05), while the location and the degree of enhancement of the two groups of tumors showed no statistical difference ( P>0.05 ) . Conclusion Most of benign and malignant parotid tumors can be correctly diagnosed by analyzing multi-slice CT images.

3.Effect of being waked up on bispectral index and auditory evoked potential index during emergence from propofol administered by TCI

Objective To investigate the effect of being waked up on bispectral index (BIS) and auditory evoked potential index (AEPI) during emergence from propofol administered by TCI.Methods Twenty ASAⅠorⅡpatients aged 31-63 yrs weighing 52-70 kg undergoing elective cholecystectomy using fiberoptic laparoscope under propofol anesthesia administered by TCI were enrolled in the study. During induction of anesthesia the initial target effect-site concentration of propofol was set at 1.0?g?ml-1 and was increased by 1.0?g?ml-1 until 6.0?g?ml-1 step by step. The BIS and AEPI values and hemodynamic variables were recorded at each target effect-site concentration during induction of anesthesia. At the end of operation the infusion of propofol was stopped and the name of the patient was called loudly every minute asking the patient to open his/her eyes. The BIS and AEPI values were recorded 1 minute before and after the eyes opened on command. Results Both BIS and AEPI correlated closely with target effect-site concentration of propofol during induction of anesthesia. During emergence from propofol at the end of operation the BIS value was 69?7 and 72?10 at 1 min before and after the patients opened eyes on command; while the mean AEPI value was 29.8?6.0 and 73.9?5.9 respectively. The increase in AEPI value was significantly larger than the increase in BIS value. Conclusion Both BIS and AEPI correlate closely with target effect-site concentration of propofol during induction of anesthesia. During emergence from propofol the AEPI is more sensitive to being waked up on command.
4.Nursing care of patients with "zero blood transfusion" type A aortic dissection in perioperative period

Objective:To summarize the nursing experience of 7 cases of "zero blood transfusion" type A acute aortic dissection during the perioperative period.Methods:Preoperative risk assessment, control of heart rate, blood pressure, analgesia and sedation to avoid dissection;fully keep warm during operation and recover blood from operation field;after operation, avoid heart rate and blood pressure fluctuations to reduce oozing, monitor coagulation indicators, observe the amount and characteristics of drainage fluid, ensure the safety of anticoagulation during Renal replacement therapy, and avoid blood waste.Results:The 7 patients all transitioned to the operation smoothly. Intraoperative and postoperative infusion of pre-storage and recovery autologous blood of 200-1 200 ml, no allogeneic blood transfusion. One patient had a lot of drainage after surgery. The drainage volume was 1 450 ml on the day and the first day. He was given autologous blood infusion combined with hemostatic drugs, and the hemoglobin could be maintained above 80 g/L; 2 patients complicated with hypoxemia, were sequential High-flow oxygen; 2 patients with acute kidney injury underwent continuous renal replacement therapy without unplanned dismissal. All 7 patients recovered and were discharged after active treatment.Conclusions:For patients with type A acute aortic dissection surgery, through careful blood protection care, you can reduce dependence on allogeneic blood.
5.Observation of clinical efficacy of S-1 combined with oxaliplatin in neoadjuvant chemotherapy for local advanced gastric carcinoma

Objective To observe the clinical efficacy of S-1 plus oxaliplatin (SOX) in neoadjuvant chemotherapy for patients with local advanced gastric carcinoma.Methods Clinicopathological data of 68 patients with local advanced gastric cancer were retrospectively analyzed and enrolled as treatment group.All the patients in the treatment group achieved 3-4 cycles chemotherapy of SOX regimen preoperatively.Outcomes of tumor remission after neoadjuvant chemotherapy were evaluated according to RECISI1.1 criteria.Meanwhile,70 patients who underwent operation directly after pathological diagnosis of local advanced gastric cancer during the same period were recruited as control group.Resection rate,R0 resection rate,postoperative complications rate and postoperative progression free survival(PPFS) time were compared between the two groups.Results In the treatment group,after neoadjuvant chemotherapy,4 cases (5.9%) achieved complete renission,36 cases (52.9%) achieved partial remission.Meanwhile,the numbers of cases with stable disease and progressive disease were 15 cases(22.1%) and 13 cases (19.1%) respectively.The objective remission rate and disease control rate in the treatment group were 58.8% and 80.9% respectively.The tumor resection rate,R0 resection rate,postoperative complications rate in the treatment group were 92.6%,85.3% and 14.7% respectively.The R0 resection rate in the treatment group was higher than that in the control group,the difference was statistically significant(x2 =8.042,P < 0.05).However,the differences of tumor resection rate and postoperative complications rate between the two groups were not statistically significant (x2 =1.711,0.673,all P > 0.05).The median time of PPFS in the treatment group and the control group was 36 months and 30 months respectively,there was statistically significant difference in PPFS between the two groups (x2 =4.049,P < 0.05).Conclusion The neoadjuvant chemotherapy of S-1 plus oxaliplatin for local advanced gastric carcminoma has satisfactory short-term effect and can also improve PPFS.However,further investigation is required to observe the effect of this neoadjuvant chemotherapy regimen on postoperative overall survival.

Adiponectin is one of the adipocytokines that are derived from adipose tissue. It has anti-inflammatory, anti-diabetic, anti-atherogenic and insulin-sensitizing effects. Its receptors AdipoR1 and (AdipoR2) mediate the function of adiponectin. Recent researches show that AdipoR1 is abundantly expressed in skeletal muscles, whereas AdipoR2 is predominantly expressed in the liver. Both AdipoR1 and AdipoR2 are found to be expressed in human and rat pancreatic ? cells and the expression levels are similar to those in liver and higher than those in muscles; AdipoR1 and AdipoR2 can also be found in monocytes and macrophages. GH and agonists of PPAR-?, PPAR-? and LXR also affect the exprssion and regulation of adiponectin receptors.
7.IN VITRO STUDY ON EFFECTS OF ZINC ON BONE DEVELOPMENT

Objective: To explore the effects of zinc on bone development in vitro. Methods: Mice of 16 gestation were killed by snapping cervical vertebra, and the front limbs were cut off and cultured in basic medium. Which contained 20 ?mol/L Zn 2+ (control group). In Zn deprived group, the basic medium was added TPEN to final concentration 20 ?mol/L. In other three groups, the basic medium was added ZnSO 4 to final concentration of Zn 2+ at 45, 70, and 120 ?mol/L respectively. Results: 1. Compared with control limb(uncultivated),the test limb(cultivated for 6 d)showed longer bone length and higher bone density.Histological analysis showed active bone cell differentiation, proliferation, increased bone trabecula.2.Compared with control group, the Zn-deprived and Zn 2+ 120 ?mol/L group expressed decreased osteocalcin (OC) and 45 Ca content and lower activity of AKP; Zn 2+ 45 ?mol/L and 70 ?mol/L group presented increased OC and 45 Ca content and higher activity of AKP.3.Compared with the control group, Zn-deprived and Zn 2+ 120?mol/L group showed shorter bone length and lower bone density; Zn 2+ 45 ?mol/L and 70?mol/L group showed longer bone length and higher bone density. Conclusion: Zinc is involved in bone metabolism and growth; both zinc-deficiency and zinc-excess can alter bone growth and normal metabolism.
