After the background, main contents and paper writers of the medical journal Yi Chao founded in Nanjing during the late period of Republic of China were analyzed with stress laid on its characteristics of nursing experiences and surgical treatment procedures for diseases, its historical values in studying the debate on traditional Chinese and Western medicine and the history of public health and medical cause during the period of Republic of China were elaborated.
2.Evidence-Based Medicine in Coronary Artery Bypass Grafting Using Arterial Conduits

To promote the understanding and practice of evidence-based medicine (EBM) in coronary artery bypass grafting (CABG). The transition of CABG graft materials from saphenous vein to arterial conduits were perceived using the principles of EBM. The arterial conduits were increasingly performed based on the principles and practice of EBM, therefore the postoperatively long-term graft patency rate, survival rate, and free cardiac event rate were significantly improved. CABG outcomes can be significantly improved by using arterial conduits based on principles of EBM.
3.Clinical study of OCM minimally invasive total hip arthroplasty

[Objective]To investigate the clinical value of OCM approach of minimally invasive total hip arthroplasty(MIS-THA).[Method]From February 2005 to December 2006,18 cases of MIS-THA were performed with OCM approach technique.The mini-incision through the intermuscular interval provided a good exposure for total hip replacement,preserved muscle integrity and kept the posterior capsule intact.A two-stage osteotomy of femoral neck was conducted.Specialized acetabular and femoral instruments were applied to fix the prosthesis.[Result]The average length of skin incision was 9.2 cm,the blood loss and drainage were 230 ml and 90 ml,respectively.At 6 months after operation,the mean Harris scores of hip increased from 46.2 to 87.1 for all cases.One case of femoral fracture around prosthesis and one of excessive anteversion were observed.[Conclusion]OCM approach brought satisfactory clinical outcomes when applied in MIS-THA.This surgical technique shows an obvious learning curve,but requires specialized instruments.
4.Inhibitory effects and mechanisms of snake venom tripeptide pENW on platelet adhesion.

This study was designed to investigate inhibitory effects and possible mechanisms of snake venom tripeptide (pENW) on platelet adhesion in order to promote the development of a novel anti-platelet therapy. To study the inhibitory effects of pENW on platelet adhesion, washed platelets pre-incubated with pENW (116.5-466.2 μmol x L(-1)) were used to test the ability of platelet adhesion to fibrinogen. Effect of pENW on fibrin clot retraction was also tested. Effect of pENW on platelets viability was tested by MTT assay. Effect of pENW on reactive-oxygen species (ROS) levels of platelet was studied by flow cytometry assay. Calcium mobilization in Fura-2/AM-loaded platelets was monitored with a spectrofluorimeter. Cyclic guanosine monophosphate (cGMP) and cyclic adenosine monophosphate (cAMP), thromboxane A2 (determined as its metabolite thromboxane B2) were measured using enzyme immunoassay kits. Akt, ERK and p38 phosphorylation were tested by Western blot. The results showed that pENW inhibited platelet adhesion and fibrin clot retraction in a concentration-dependent manner without cytotoxicity. Intracellular cGMP and cAMP in both resting and thrombin-activated platelets were increased by pENW. In addition, pENW attenuated intracellular Ca2+ mobilization and TXA2 production in platelets stimulated by thrombin. As shown by Western blot assay, Akt, ERK and p38 phosphorylation in thrombin-induced platelet were attenuated by pENW. However, inhibitory effects of pENW had nothing to do with ROS. Thus, pENW exhibited a significant inhibition on platelet adhesion to fibrinogen, which means pENW could block the first step of thrombosis as while as retard the more stable clot formation. The mechanisms of pENW on inhibition platelet adhesion might be related to instant regulations, such as protein kinases.

6.Meta-analysis of the effect of low-frequency repetitive transcranial magnetic stimulation on paretic hand recovery after stroke.

Low-frequency repetitive transcranial magnetic stimulation (rTMS) is a non-invasive brain stimulation method that has been used in the treatments of stroke patients. Our purpose was to investigate the effects of low-frequency rTMS on the hand motor function recovery for stroke patients. After searching from electronic database from January 1990 to April 2012, we collected randomized controlled trials explored the low-frequency rTMS effects on the hand motor function rehabilitation. A total of 7 comparisons (N = 216) from 5 articles were included in this study. We summarized the effect size by calculating the standardized mean difference (SMD). Summary of effect size (SES) indicated positive effects of low-frequency rTMS on the finger motor ability (SMD = 0.58) and hand function (SMD = -0.98). Few adverse effects occurred. The evidence showed that low-frequency rTMS could improve the paretic hand recovery after stroke.

8.Changes of serum leptin levels and the influential factors in peritoneal dialysis patients

Objective To investigate the changes of serum leptin levels and the influential factors in maintenance peritoneal dialysis patients.Methods Seventy-six peritoneal dialysis patients were chosen at the time before,and 3 months,6 months,12 months,18 months and 24 months after they began the peritoneal dialysis therapy,to examine body mass index (BMI),triceps skinfold thickness (TSF),abdominal circumference,homeostasis model assessment of insulin resistance (HOMA-IR),the plasma lipid profile,and leptin in the same situation.Results For 24 months,these patients showed higher serum leptin level than the values before commencing peritoneal dialysis treatment (P < 0.01).The level of leptin was positively correlated with the BMI(r =0.412,P < 0.01),TSF(r =0.308,P < 0.01),abdominal circumference(r =0.284,P < 0.01),HOMA-IR(r =0.184,P < 0.01) and TG(r =0.288,P < 0.01),negatively corelated with the high-density lipoprotein cholesterol(HDL-C)(r =-0.285,P < 0.01).Multiple logistic regression analysis showed that BMI (β =0.339,P < 0.01),TGβ =0.157,P < 0.01) and HDL (β =-0.126,P < 0.05)were significant predictive factors for the changes of serum leptin levels.Conclusion Leptin maybe involve in the occurrence and the development of cardiovascular events like other metabolic parameters in peritoneal dialysis therapy.
9.Advancement on the pharmacogenomics of immunosuppressant

At present,transplantation has been the predominant way to solve most of the end-stage diseases,ensued by the use of immunosuppressive drugs.Since the immunosuppressive drugs have narrow therapeutic index,the blood drug concentration is needed to mornior.Pharmacogenetics is one subject which focuses on the interaction between gene and the metabolism of the drug,providing great help for designing the regime of achieving the target drug concentration.Meanwhile,it facilitates the realization of individual therapy.This review thus focuses on the latest advancement on the pharmacogenetics of those immunosupprressants,hoping to provide help for the treatment.
10.Protecting myocardium by inhibiting the activation of nuclear factor kappa B in open heart surgery

Objective To investigate the relationship of the activation of nuclear factor kappa B(NF-kB) with myocardial neutrophil infiltration and injury in human open heart surgery,and to observe the inhibiting effect on the activation of NF-kB and protecting effect on myocardium of the coenzyme Q10,a scavenger of oxygen free radicals.Methods Forty-seven adult patients undergoing open heart surgery were randomly divided into two groups,the control and the treatment.CoenzymeQ10 tablets were given to the treatment group 5 days before operation.Biopsy of right atrium for myocardial pathology,activated NF-kB detection and ultrastructure observation were done prior to cardiopulmonary bypass,45 minutes of ischemia and 45 minutes of reperfusion.The dynamic indexes,vasomotor drug dosage,and outcomes were observed postoperatively.Results Upon 45 minutes of ischemia and 45 minutes of reperfusion,in control group there were neutrophil accumulation and adhesion of vascular endothelium,ultrastructural damages,and positive expression of NF-kB both in nuclei and cytoplasm,in myocardium.In treatment group,there were only mild neutrophil infiltration and ultrastructural damages,and weak positive expression of NF-kB both in nuclei and cytoplasm.However,the dynamic indexes,vasomotor drug dosage,and outcomes of two groups were not significantly different.Conclusion NF-kB plays an important role in pathophysiological process of myocardial ischemia and reperfusion in open heart surgery.CoenzymeQ10 has obvious inhibiting effect on activation of NF-kB and protecting effect on myocardium.
