1.Characteristics of treating dislocation and fracture of sacroiliac joint through anterior and posterior approches.

OBJECTIVETo investigate characteristics of treating dislocation and fracture of sacroiliac joint through anterior and posterior approaches.
METHODSBetween January 2006 and September 2012, 39 patients with dislocations and fractures of sacroiliac joint were treated with operation. There were 28 males and 11 females, aged 12 to 64 (mean, 41.3) years old. Seventeen cases were chosen through anterior approach, 13 cases through posterior and 9 cases were combined with anterior and posterior approaches. The anterior approach were made with supine position, performed open reduction, and two plates with 4 holes were used as internal fixation for arthrodesis of sacroiliac joint after reduction; while posterior approach prone position, close or open reduction was performed, and one or two partial thread lag screws, which with 7.3 mm in diameter and 60 to 75 mm in length, were penetrated via sacroiliac joint for fixation under X-ray. Postoperative complications were observed, Matta and Majeed scoring were used to evaluated currative effects.
RESULTSAll patients were followed up from 6 to 36 months. The patient could sit on the bed 2 to 4 weeks after surgery and walk with a crane 6 weeks' later. No breakage or loosening of screw occurred during follow-up. In all but one case with old sacral iliac fractures reducted poor, others obtained good opposition. According to Matta's criteria, 30 cases got excellent results, 8 good and 1 fair. According to Majeed's funtional standard, 14 patients got excellent results, 20 good 4 fair and 1 poor.
CONCLUSIONTreating dislocation and fracture of sacroiliac joint through anterior and posterior approaches can obtain good effects. However, anatomic location, fracture types, degree of displacement, the rate of preoperational reduction, vulnerability assessment of operation and fixation strength should be considered.

2.Economic evaluation on new strategy for preventing poliomyelitis in Zhejiang Province

Objective To evaluate the economic effect of new strategy for preventing poliomyelitis in Zhejiang Province. Methods Based on the population of Zhejiang Province in 2016, cost-effectiveness ratio (CER) , benefit-cost ratio (BCR), and net benefit (NB) were applied to calculate the health economic difference for the new strategy and the original strategy as compared to no vaccination strategy. Univariate sensitivity analysis was used to assess the robustness of results with main parameters; including burdens of poliomyelitis, cost of vaccines, and the vaccination program itself, and the discount rate. Results CERdisease, CERdeath, BCR and NB for the new strategy were 1:52700 RMB Yuan per case, 1:1813700 RMB Yuan per case, 12.26 and 796.6622 million and CERdisease, CERdeath, BCR and NB for the original strategy were 1 :32900 RMB Yuan per case, 1 :1133900 RMB Yuan per case, 19.58 and 823.1753 million, respectively. The results of the univariate sensitivity analysis with main parameters were robust. Conclusion The new strategy for preventing poliomyelitis is necessary for this period of the global polio eradication. Though the new strategy appears not as good as the original strategy on economic evaluation, its health economic benefit is significant.

Objective To evaluate the quality of the hepatitis B report data on a pilot surveillance in Zhejiang Province from 2013 to 2015.Methods Hepatitis B report data of 6 pilots in Zhejiang Province from 2013 to 2015 were extracted from national notifiable infectious disease reporting system,including reported cases of hepatitis B,classification of hepatitis B cases and supplementary card information etc. To evaluate the accuracy of hepatitis B classification,information of supplementary cards was used to make classification diagnose for hepatitis B.Results A total of 3214 hepatitis B cases were reported in 6 pilot surveillance counties in Zhejiang Province between 2013 and 2015. Excluded 32 duplicated cases within the year and between years,3182 hepatitis B cases were actually reported,hepatitis B cases repeated reporting rate was 1%. A total of 2717 hepatitis B cases were correctly classified,and the accuracy rate of classification was 85.39%,showing an increasing trend (P<0.05). The proportion of accurate classification of reported hepatitis B by referring to the results of positive time of HBsAg and ALT from the supplementary card were 80.86% and 97.29% respectively. Among those reported acute hepatitis B cases,90.43% of them filled with anti-HBc IgM positive in supplementary card. The proportion of accurate classification of reported hepatitis B by referring to the information for liver puncture and the HBsAg and anti-HBs transform during the recovery period in supplementary cards were 0.19% and 5.13% respectively. Among those reported acute hepatitis B cases,0.69% of them were chronic or undetermined. Among those reported chronic hepatitis B cases,13.92% of them were acute,undetermined or HPV carrier. Five out of 3182 cases were unclassified,accounting for 0.16%.Conclusion The quality of classification diagnosis for hepatitis B should be improved in Zhejiang Province. Filling in the supplementary card is very beneficial for the classification of hepatitis B.
5.Effects of maca extract on exercise endurance and blood hormone levels in the rats

Objective To study the effects of maca extract on exercise endurance and blood hormone levels in the rats. Methods Wistar rats treated with maca extract (2. 0, 4. 0, 8. 0 g/kg body weight) were freely swimming in the cir-culating water flow daily for 15 days. On the 16th day of experiment, the exercise endurance and blood noradrenaline (NA), estradiol (E2), and testosterone (T) levels of the rats were determined. Results The rats administered with maca extract at the doses of 2. 0, 4. 0, 8. 0 g/kg body weight for 15 d showed that the swimming time before sinking was in-creased by 32. 51%, 60. 04%, 106. 52%, the total swimming time was extended by 16. 99%, 56. 50%, and 101. 73%respectively ( P<0. 01 ); while the number of sinking was decreased by 18. 89%, 35. 89%, and 58. 06%, respectively (P< 0. 01), compared with those swimming rats without maca extract treatment. The noradrenaline level in the blood of rats treated with maca extract 2. 0, 4. 0, 8. 0 g/kg body weight was increased by 3. 30%, 6. 60%, and 16. 50%, respec-tively, compared with the control group, and increased by 42. 49%,47. 05%, and 60. 70%, respectively, compared with the swimming rats without extract treatment;the E2 level was increased by 132. 83%,102. 72%, and 62. 26% (P<0. 01) respectively, compared with the control group, while decreased by 23. 88%, 33. 72%, and 46. 95% (P<0. 01) respec-tively, compared with the swimming rats without extract treatment. The blood testosterone level was increased by 5. 11%, 37. 65%, and 123. 16% (P<0. 01) respectively, compared with the control group, and increased by 28. 98%, 68. 92%, and 173. 85%, (P<0. 01) respectively, compared with the swimming rats without extract treatment. Conclusions The results of this study demonstrate that maca extract has effect to resist fatigue and enhance exercise capacity in rats. The mechanism is associated with reduced blood E2 , and increased noradrenaline and testosterone levels in the blood of rats.

Objective To investigate the growth inhibition mechanism of octadecenoic acid on human gastric carcinoma transplanted tumor.Methods The cancer tissue was isolated from adult patients with gastric cancer and transplantation in severe combined immunodeficiency (SCID) mice.And SCID mice were treated with concentrations of 0.2,0.4,0.8 g · kg-1 octadecenoic acid,respectively.The SCID mice in control group treated with concentration of 100 mg · kg-1 fluorouracil(5-FU).The SCID mice in model group treated with equal amounts of rapeseed oil.The duration was 4 weeks.The cyclic adenosine monophosphate (cAMP),tumor suppressor gene p53 (P53),phosphatidylinositol 3-kinase(P13K) levels in tumor tissue were detected by the method of enzyme linked irnmunosorbent assay.Results Compared to the model group,growth inhibition rate of transplantation tumor of human gastric cancer in low,middle,high experimental groups and control group were increased 26.52%,58.75%,67.80% and 46.25% respectively with significace(P < 0.01);the cAMP levels were increased by 35.61%,74.93%,130.48% and 54.98% inthe four groups,respectively with significance also (P < 0.01);the P53 and P13K levels were reduced by 57.22%,79.45%,95.71%,80.10% and 33.11%,44.39%,97.42%,51.69% respectively in the four groups with significance too(P < 0.01).Conclusion Growth inhibition mechanism of octadecenoic acid on transplantation tumor of human gastric cancer,mainly related to elevated cAMP level within the tumor tissue,inhibit the activity of P53 and P13K signal pathway.
7.Regulating action of unsaturated fatty acids on human gastric cancer cells protein kinase B signaling pathways

Objective To study the regulating action of unsaturated fatty acids on human gastric cancer cells protein kinase B (PKB/Akt)signaling pathways.Methods The gastric cancer tissue of adult patients with gastric cancer was isolated and transplantation in severe combined immunodeficiency (SCID) mice.Separation cells from gastric cancer transplanted tumor by digestive enzyme were cultured.They were divided into five groups:three doses experimental groups (unsaturated fatty acid 0.3,0.6,1.2 mg · L-1),control group (5-fluorouracil 10 mg · L-1),solvent control group (dimethyl sulfoxide 30 μL · L-1),cultivate 48 h after the treatment.The toxic effects of the unsaturated fatty acid on human gastric cancer cells were detected by 3-(4,5-dimethyl-2-thiazolyl) -2,5-diphenyl-2-H-tetrazolium bromide (MTT) method.The levels of p21,murine double minute2 (MDM2),PKB/Akt in tumor cells were detected by the method of enzyme linked immunosorbent assay.Results Compared to the control group,the human gastric cancer cells proliferation inhibition rate in high,moderate,low,doses experimental groups were increased by 31.7%,55.51%,68.28%,respectively.The levels of p21 and MDM2 in that three groups were increased by 4.39%,80.12%,161.36% and 19.81%,97.76%,207.35%,respectively.The level of PKB/Akt in that three groups was reduced by 13.31%,57.69%,78.71%,respectively.Conclusion The unsaturated fatty acids on human gastric cancer cells proliferation inhibition mainly related to elevated p21,MDM2 levels within the tumor cells,inhibit the activity of PKB/Akt signal pathway.
8.Toxicity mechanism of the unsaturated fatty acids on human cervical carcinoma of ME-180 cells

Objective To investigate the cytotoxic effect mechanism of the unsaturated fatty acids on human cervical carcinoma of ME-180 cells.Methods The 2 × 108 cell · L-1 ME-180 cells were moved into the 96 hole culture plate,per hole 90 μL.Three concentrations of the drug groups:0.3,0.6,0.9 mg· L-1 unsaturated fatty acid (prepared with DMSO) per hole 90 μL.The 5-fluorouracil(5-FU) group:10mg · L-1 5-FU (prepared with phosphate buffer) per hole 10 μL.Control group:30 μL · L-1 DMSO per hole 10 μL.Each group set of four repeat hole,in saturated humidity condition of 37 ℃ and 5% CO2 in 48 h.The inhibition rate and apoptosis rate of unsaturated fatty acids were detected with a trypan blue,thiazole blue (MTT),and flow cytometry method.Results When ME-180 cells were administered with the unsaturated fatty acids at the doses of 0.3,0.6,0.9 mg · L-1 for 48 h,ME -180 living cell count with the increase of drug concentration and action time is reduced.The cells proliferation inhibition rate were 43.35%,51.75%,66.45% in three concentrations of the drug groups;apoptosis rate were 18.21%,23.11%,28.06% in the three groups,respectively.Compared with the drug groups and the control group on the number of living cells,inhibition rate and apoptosis rate,the differences were statistically significant (P <0.01).Conclusion The unsaturated fatty acids has significant toxic effects on ME-180 cells.The toxic effect mechanism of the unsaturated fatty acids is related to inhibite proliferation on cell and induce-cell apoptosis.
9.Genome-wide DNA methylation patterns in monocytes derived from patients with primary Sjogren syndrome.

BACKGROUND:
Epigenetics, especially DNA methylation, plays an important role in the pathogenesis of primary Sjogren syndrome (pSS). Our study aimed to reveal the role of DNA methylation in peripheral monocytes of pSS patients.
METHODS:
A total of 11 pSS patients and five age-matched healthy controls (HCs) were included in this study. Monocytes were isolated from peripheral blood mononuclear cells using magnetic microbeads. DNA methylation profiles were generated using Human Methylation 850K BeadChips.
RESULTS:
In monocytes from pSS patients, we identified 2819 differentially methylated positions (DMPs), comprising 1977 hypomethylated- and 842 hypermethylated-DMPs, corresponding to 1313 unique genes when compared with HCs. IFI44L, MX1, PAARP9, and IFITM1, which influence the interferon (IFN) signaling pathway, were among the genes hypomethylated in pSS. Functional analysis of genes with a minimum of two DMPs showed involvement in antigen binding, transcriptional regulation, cell adhesion, IFN-γ pathway, type I IFN pathway, antigen presentation, Epstein-Barr virus infection, human T-lymphotropic virus type 1 virus infection, and metabolic disease-related pathways. In addition, patients with higher serum IgG levels exhibited enrichment in Notch signaling and metabolic-related pathways. Upon comparing monocytes with salivary gland epithelial cells, an important overlap was observed in the cell cycle, cell senescence, and interleukin-17 signaling pathways. The differentially methylated genes were more enriched in the ribosome- and AMP-activated protein kinase signaling pathway in anti-Ro/SSA and anti-La/SSB autoantibodies double-positive patients.
CONCLUSION
Genome-wide DNA methylation profiling revealed significant differences in DNA methylation in monocytes isolated from patients with pSS.

10.Immunogenicity and safety of DTaP-IPV//PRP-T combined vaccine in infants in China

Objective The aim of this study was to demonstrate the immunogenicity and safety of diphtheria, tetanus, pertussis (acellular, component) , poliomyelitis (inactivated) vaccine (adsorbed) and Haemophilus influenzae type b conjugate vaccine (DTaP-IPV//PRP-T) combined vaccine compared with commercially available DTaP (diphtheria, tetanus and pertussis), Haemophilus influenzae type b (Hib), tetanus conjugate and IPV monovalent vaccine. Methods Subjects were randomly divided into three groups, Group A and Group B were DTaP-IPV//PRP-T combined vaccine (PENTAXIMTM) vaccinated at 2,3,4 months of age or 3,4, 5 months of age respectively; Group C was commercially available DTaP. Hib tetanus conjugate (Act-HIBTM) and IPV (IMOVAX PolioTM) vaccines vaccinated at 3,4, 5 months of age. All groups received booster dose at 18 to 20 months of age, with antibody titers tested. Non-inferiority analysis was demonstrated in terms of seroprotection / seroconversion rates between Group A, Group B respectively and Group C. Safety information was collected after each vaccination to assess the safety of investigational vaccines. Results The non-inferiority of DTaP-IPV//PRP-T combined vaccine vaccinated at 2,3,4 or 3,4, 5 months of age versus DTaP, Hib tetanus conjugate and IPV vaccine was demonstrated for all vaccine antigens in both primary and booster phases in terms of seroprotection/seroconversion rates. DTaP-IPV//PRP-T combined vaccine was well tolerated. The rate of solicited/unsoliciated severe adverse reactions was very low and similar to the control vaccines. Conclusion DTaP-IPV//PRP-T combined vaccine was highly immunogenic with good safety profile in Chinese infants, which was comparable to the commercially available control vaccines.
