1.Medical voucher system in Hong Kong and Macao and its implications to mainland China

It is important and difficult to establish the market competition mechanism in the health care re-form. Medical voucher system in Hong Kong and Macao can provide policy guidelines for Mainland China to promote institutional innovation, force public hospital reform and the rational allocation of medical and health resources. This paper introduced the origin and development of medical voucher system. Based on the description of the implementa-tion background, similarities and differences and effects of medical voucher system in Hong Kong and Macao, the pa-per found that medical voucher system could help encourage the demander to make more frequent use of medical serv-ices, improve their consciousness of prevention and health care and promote family doctor system. Through analyzing the applicability of medical voucher system in mainland China, the paper pointed out it was consistent with the reform orientation and could be served as a useful supplement to the health care system to improve medical insurance, medi-cal assisstance system as well as an effective measure to develop private medical institutions.
2.Clinical research of protein-free calf blood extract eye gel for corneal epithelial defect

AlM: To evaluate the clinical effect of 200g/L protein-free calf blood extract eye gel for corneal epithelial defect.METHODS: One hundred and sixty - eight cases of corneal epithelial defect ( 58 cases with herpes simplex keratitis; 24 cases with chemical injury; 85 cases with pterygium operation injury ) were randomly divided into two groups: 84 eyes were treated with protein-free calf blood extract eye gel; 84 cases were treated with basic fibroblast growth factor eye gel ( bFGF ) . The bFGF and protein-free calf blood extract eye gels were used 4 times a day. The treatment course was 7d. Epithelial defect restoration, local symptom and sign were observed.
RESULTS: The difference between pre-treatment and post-treatment was significant ( P<0. 01 ) in the two groups. The effect of protein-free calf blood extract eye gel (83. 3%) group was prior to that of bFGF (69%) for corneal epithelial defect. The effective rate of protein-free calf blood extract eye gel in the herpes simplex keratitis, chemical injury and pterygium operation injury was 72. 4%,69. 2% and 95. 2%. Localized stimulus and adverse reaction of all over the body were not been observed.
CONCLUSlON: Protein-free calf blood extract eye gels is valuable and safe for corneal epithelial defect.
3.Research on design of hospital inspection information system and HIS interface

Objective:To develop an interface program between a hospital inspection information system and Hospital Information System(HIS). Such interface program not only guarantees the safety and independence of HIS, but achieves interoperability between HIS and other third-party system.Methods: The interface program between the inspection information system and HIS conforming to hospital demands is developed by using Visual Studio 2008 development platform and WebServices+XML technology.Results: The interface between the inspection information system and HIS operates stably, reliably and efficiently, avoiding manually input information errors, reducing the amount of registration work, and raising work efficiency.Conclusion:WebServices+XML technology is utilized to develop an interface characterized by safety, stability and reliable performance, realizing resource sharing among information systems in the hospital. Besides, the interface can be expanded in the future to adapt to other third-party system, achieving customization.
4.Study on the mechanism of action of total flavonoids of Litsea coreana for reducing blood glucose level in rat with type 2 diabetes mellitus.

OBJECTIVETo explore the possible mechanism of total flavonoids of Litsea coreana (TFLC) on reducing blood glucose level in rat with type 2 diabetes mellitus (T2DM).
METHODSMale SD rats of T2DM allocated in two groups were treated with 400 mg/kg TFLC or metformin respectively via gastrogavage for 6 weeks. Blood routine biochemical indices in rats were measured; pathology of rats' liver was examined with HE stain under transmission electron microscopy; levels of malondialdehyde (MDA) and superoxide dismutase (SOD) in liver homogenate were determined, and the expression of protein tyrosine phosphatase 1B (PTP1B) in liver was detected using RT-PCR at the terminal of the experiment.
RESULTSBiochemical measuring showed that the glucose tolerance of rats after treatment was markedly improved in both groups. Meantime, levels of fast blood glucose (FBG), glycohemoglobin (HbA1c), fast blood insulin (FINS), free fatty acid (FFA), total cholesterol (TC), triglyceride (TG) and low density lipoprotein-cholesterol (LDL-C), as well as MDA level in liver were decreased, while levels of high density lipoprotein-cholesterol (HDL-C) in blood and SOD in liver were significantly increased in both groups after treatment, showing insignificant difference between two treatment groups. Light microscopic examination showed markedly fatty degeneration of liver, and electron microscopic examination found mitochondria swelling and endoplasmic reticulum breakage in liver of T2DM rats, but these changes were ameliorated to some extent after treatment. The elevated PTP1B expression in liver of T2DM rats was decreased in the TFLC treated group, but unchanged in the metformin treated group.
CONCLUSIONTFLC can significantly decrease the blood levels of glucose and lipid and ameliorate oxidation stress in liver; its mechanism of action in improving insulin resistance might be related with its suppression on PTP1B expression in rat's liver to enhance the insulin signaling pathway.

5.Diagnostic analysis of the radiologic characteristics in osteoporotic Kiinmmel's disease.

OBJECTIVETo explore the diagnostic value of the radiologic characteristics of osteoporotic Kummell's disease.
METHODSTotal 16 patients with pathologically confirmed osteoporotic Kummell's diseases were reviewed from May 2010 to May 2012, including 4 males and 12 females with the mean age of 73.4 years (ranged, 67 to 83 years old). Radiologic imagings of all patients, including X-ray, CT and MRI, were analyzed retrospectively.
RESULTSIntravertebral linear clefts could be seen on the AP and lateral X-ray films of vertebrae. Sagittal and axial CT scans demonstrated the vacuum cleft phenomenon with liquid and air was identified within the vertebral body. Sagittal MRI showed the callapsed vertebral segment and the area of fluid signal with clear and intact border within the vertebral body. The fluid signal was low on T1-weighted images and high on T2-weighted images and stir images, which was corresponding to an intravertebral vacuum cleft.
CONCLUSIONThe radiologic characteristics of Kurmmell's diseases can provide valuable evidences for the early diagnosis.

7.Comparative study on the correlation of penetrating keratoplasty rejection with different cornea preservation methods

AIM: To understand the relation between the penetrating keratoplasty rejection and the methods of cornea preservation.
METHODS: The 30 Wistar rats as donator and 60 SD rats as receptor were used to establish the animal models of penetrating keratoplasty rejection. And 60 SD rats were randomly divided into 3 groups. Donor cornea of Wistar rats preserved in different methods were used separately in 3 groups. The penetrating keratoplasty rejection index ( RI ) , means survival time ( MST ) of corneal grafts and pathological changes in post -operation were analyzed.
RESULTS: The MST was ( 10. 4±1. 70 ) d in moist-chamber- preserved group (Ⅰ), ( 12. 9 ± 1. 81 ) d in medium-term-preserved group (II) and (16. 1±2. 57) d in cryopreserved group ( Ⅲ) . The MST in the cryopreserved group was evidently prolonged, showing a significant correlation compared with other two groups (P<0. 01). The sections with HE staining revealed that the severity of inflammation in Ⅲ group was reduced compared with that of Ⅰ, II group after 10d of keratoplasty.
CONCLUSION: The postoperative rejection of penetrating keratoplasty in rats is decreased and rejection time is delayed in cryopreserved cornea.
8.Establishment of a mouse model of Humulus pollen allergen-induced allergic asthma

Objective To develop a mouse model of allergic airway inflammation by using crude Humulus pollen extract.Methods A total of 24 Balb/c mice were divided randomly into two groups: control group and asthma model group with 12 in each.After sensitization by intraperitoneal injection,the mice were challenged by intranasal instillation of crude Humulus pollen extract.The total cell number and eosinophile percentage of bronchoalveolar lavage fluid(BALF) samples were determined.The lungs of mice were stained with hematoxylin and eosin to evaluate the degree of allergic inflammation.The cytokines in BALF and spleen homogenate were assayed by enzyme-linked immunoassay(ELISA).The total IgE in the serum was also measured by ELISA.Results Compared with the mice in normal control group,those in asthma group developed obvious allergic inflammation in the airways.Histological scoring for pulmonary inflammation significantly differed between normal group and asthma group(P
9.Effects of acetylcholine chloride on intracellular free calcium concentration of cultured sweat gland epithelial cells

Objective To isolate and culture sweat gland epithelial cells in vitro,and to study the effects of acetylcholine (ACh) on intracellular flee calcium concentration ([Ca~(2+)]i) of cultured sweat gland epithelial cells.Methods Sweat glands epithelial cells were collected by enzymatic digestion.After ACh was added to the primary and first passage cells,[Ca~(2+)]i was examined using confocal laser scanning microscopy (CLSM) and the Ca~(2+) sensitive dye Fura 3/AM.Results The primary and first passage epithe- lial cells grew well.After ACh was added,opening of the calcium channel and significant [Ca~(2+)]i increase were observed when the primary and first passage cells were incubated with high concentration of calcium (2 mmol/L);no significant [Ca~(2+)]i increase was observed in those cultured without calcium.Conclusion Upon stimulation with ACh,calcium channels of cultured primary and first passage sweat gland epithelial cells would open,influx of extracellular Ca~(2+) occurred,which resulted in an increase of [Ca~(2+)]i.Extracellular bound calcium was therefore converted into intracellular free calcium.
10.Application of electromyographic biofeedback training in the treatment of urinary incontinence

Objective To study effect of eleetromyograghie(EMG) biofeedback training on three kinds of u- rinary ineontifienee.Methods Nineteen patients with urinary incontinence were treated by means of EMG biofeed- back training twice a day for six weeks.The treatment was performed with a device,which can detect the EMG ampli- tude of the pelvic muscle and deliver electric stimulation accordingly.Results After 6 weeks of treatment,the inci- dence of uretbrorrhea was reduced by 41%,and the frequency of micturition was decreased by 38% ,while the fre- quency of urination in one day reduced to 9 to 13.The general subjectively rated improvement rate of patients was 53% ,while the general objectively one was 58%.Conclusion Biofeedback training has significant therapeutic: eftects on patients with urinary incontinence.
