1.Laparoscopic Appendectomy Combined with High Ligation of the Vaginal Process in Children

Objective To summarize our experience on laparoscopic appendectomy combined with high ligation of the vaginal process in children.Methods From January 2004 to May 2007,totally 326 patients with appendicitis underwent laparoscopic appendectomy in our hospital.During the operation,by inguinal exploration,83 cases of vaginal process deformity (17 bilateral and 69 unilateral lesions) were confirmed.High ligation of the vaginal process was performed on these patients.Results The operation was successfully done in all of the 83 cases,none of them showed postoperative complications.The cases were followed up for 6 to 12 months,during which only one patient had recurrent inguinal hernia.Three patients developed latent hernia but receined no treatment. Conclusions Laparoscopic appendectomy combined with high ligation of the vaginal process is an safe,effective,and low-cost procedure,leading to quick recovery and less pain for children with appendicitis complicated with vaginal process deformity.
2.Laparoscopic Surgery of Intussusception in Children:Report of 12 Cases

Objective To evaluate the value of laparoscopic surgery application in treating intussusception.Methods Between January 2004 and December 2007,12 children with refractory (8 cases,failed air enema) or recurrent (for over 3 times) intussusception received laparoscopic surgery in our hospital.Three trocars were used during the surgery.Under a laparoscope,by interrupted suture,the terminal ileum was joined to the ascending colon (3 stitches),and the ileocecal junction was fixed to the right posterior abdominal wall (2 to 3 stitches,only for children that failed air enema).Results The intraoperative blood loss ranged from 20 to 60 ml,and the operation lasted for 45 to 90 minutes.No patient developed anastomotic leakage or abnormal gastrointestinal function.The patients were followed up for 1 to 5 years (mean,2 years and 9 months),during which none of them had recurrence. The children had normal intake and excretion,and developed well.Conclusions Children with intussusception recover quickly after laparoscopic treatment with a few postoperative complications.
3. Study on the chemical constituents of Terpinia arguta.

OBJECTIVE: To study the chemical constituents of Turpinia arguta. METHODS: The compounds were isolated by chromatography on silica gel and sephadex LH - 20 columns, and their structures were elucidated by spectral analysis. RESULTS: Thirteen compounds were obtained and identified as vanillic acid (1), E-p-hydroxoy-cinnamic acid (2) , butanedioic andydride (3), apigenin(4), pyrogallic acid(5), 2-furoic acid(6), gallic acid(7), 2-(4-hydroxybenzyl)malic acid(8), luteolin(9), apigenin-7-O-β-D-glucoside(10), luteolin-7-O-β-D-glucoside(11), apigenin-7-O-β-D-neohesperidoside(12) and apigenin-7-O-β-D-(2″, 6″- di-α-rhamnopyran-osyl)-glucopyranoside (13). CONCLUSION: Compounds 1-13 were isolated from this family for the first time. Copyright 2012 by the Chinese Pharmaceutical Association.
4.Assessing new homeostasis model assessment by Botnia clamp

objective To investigate the ability of insulin sensitivity index HOMA2-%S and secretion function index HOMA2-%B calculated by HOMA2,the new homeostasis model assessment,in clinical application. Methods Eighty female volunteers with polycystic ovary syndrome in Chongqing area [50 subjects with normal glucose tolerance(NGT group)and 30 subjects with impaired glucose regulation(IGR group)]were involved in this study.Thev underwent a 75 g oral glucose tolerance test(OGTF)and the Botnia clamp test. From the data of faming blood samples in OGTF,insulin sensitivity index HOMAI-ISI,secretion function index HOMAl-β and disposal index DI-HOMA1 were calculated by the old homeostasis model assessment(HOMA I),meanwhile insulin sensitivity index HOMA2-%S,secretion function index HOMA2-%B and disposal index(DI-HOMA2) were caleulated by the new homeostasis model assessment (HOMA2).Correlation coefficients between insulin sensitivity index and GIR (the glucose infusion rate at steady state of Botnia clamp test),and between insulin secretion function index and AIR(the acute insulin response in Botnia clamp test),were studied.Results The Pearson's linear correlation coefficient between HOMA2-%S and GIR(r=0.503),HOMA1-ISI and HOMA2-%S (r= 0.990).HOMA2-%B and AIR(r=0.382),HOMA1-B and HOMA2-%B(r=0.976) were all statistically significant(a11 P<0.01).The glucose disposal indexes calculated from the HOMA2 and HOMA1 of 1GR group were significantly lower than those from the NGT group(t=2.825,P<0.Ol;t=2.222,P<0.05). Conclusion The HOMA2 is a better model in evaluating the insulin sensitivity and secretion function and is recommended to be widely used in clinical evaluation.
5.Discussion of single-site umbilical laparoscopy in the treatment for inguinal incarcerated hernia in children

Objective To evaluate the clinical effect of single-site umbilical laparoscopy in the treatment for inguinal incarcerated hernia in children. Methods Retrospective reviews were conducted for the clinical data of the 105 children with inguinal incarcerated hernia during January 2010 to August 2014. According to different surgical approaches, data were divided into single-site laparoscopic surgery group (SLS, n=56) and open surgery group (OS,n=49). The follow-up period was 6-36 months. Results Two groups of patients were successfully completed surgery. The average operative duration was significantly longer in OS group (33.73±4.41)min than that of SLS group (26.14±5.17) min (P<0.01). The average operative blood loss was (3.18 ± 2.90) mL for OS group versus (1.73 ± 0.88) mL for SLS group (P<0.01). The hospitalization duration was (4.22±1.16) d for OS group versus (3.50±0.97) d for SLS group (P<0.01). The postoperative complications were 24.5%(12/49) for OS group and 8.9%(5/56) for SLS group. There was no recurrence of hernia in two groups. Conclusion Single-site umbilical laparoscopy is safe and efficacious for treatment of inguinal incarcerated hernia in children, which offers a new choice of treatment for occult inguinal hernia.
6.Progress in Neural Differentiation of Umbilical Cord Blood -Derived Stem Cells

The ability of repair and regeneration of central nervous system (CNS) is limited. So many researchers applied themselves to search a valuable cell resource for treating severe diseases of the CNS. Several studies from different laboratories have recently reported that stem cells derived from human umbilical cord blood under certain in vitro conditions can manifest neural features that resemble features of neural-derived cells. In vivo transplantation studies have shown that these stem cells persistently engraft in the CNS, some engrafted cells acquire the characteristics of neurons and glia, and improve functional recovery after central nervous system injury. The existence of stem/progenitor cells with previously unappreciated proliferation and differentiation potential in umbilical cord blood raise the possibility that cord blood may provide an efficient source of cells differentiating into the neural lineage, with a potential to be employed in the therapy of human CNS diseases. The achievement and focuses on the mechanisms and modulation of induction of differentiation and in vitro and in vivo studies in this field was reviewed.
7.Relationship between EGFR and KRAS mutations and clinicopathologic features of non-small cell lung cancers

Purpose To explore the relationship between the mutations of epidermal growth factor receptor ( EGFR) and KRAS genes and clinicopathological characteristics in patients with non-small cell lung cancers (NSCLC). Methods Clinical samples from 431 NSCLC patients were obtained for EGFR and KRAS gene analysis. PCR based direct DNA sequencing was used to investigate mutations in exon 18-21 of EGFR gene and codon 12 and 13 of exon 2 of KRAS gene. Results The overall EGFR mutation rate of primary NSCLC was 53. 6% (231/431) in this study cohort and eight cases showed double EGFR mutations. Mutation rates in female and male were 65. 2% (122/187) and 46. 9% (98/209), respectively. The mutation rate was higher in patients with non-smokers and adeno-carcinoma and adenosquamous carcinoma subtypes than in their counterparts (P<0. 05), with the percentage of 57. 2% (124/216), 60. 3% (199/330), 42. 9% (6/14), respectively. In squamous cell carcinomas and other subtypes, EGFR mutation rates were 11. 6% (5/43) and 11. 1% (1/9), respectively. The EGFR mutation types included exon 18 point mutations (4. 0%, 9/227), exon 19 deletion mutations (4. 5%, 101/227), exon 20 insert or point mutations (9. 7%, 22/227) and exon 21 point mutations (41. 4%, 94/227). Activating mutations of KRAS gene were detected in 7. 8%(31/396) of NSCLC. Twenty-eight patients showed codon 12 mutations ( G>T, G>A, G>C) , and three patients had codon 13 mutations ( G>A, G>T) . Most of these mutations were G to T transversion (64. 5%, 20/31). Conclusion Polymerase chain reaction-direct sequencing is a reliable and effective method for the detection of the EGFR and KRAS gene mutation in NSCLC patients. The mutation rate of EGFR is higher in Chinese patients, especial-ly in non-smoking female patients with adenocarcinoma.

9.Effect of budesonide on eosinophils and histamine in nasal mucosa of guinea pig with allergic rhinitis

AIM: To build an allergic rhinitis model to evaluate the effect of budesonide on the eosinophils and histamine in nasal mucosa in allergic rhinitis guinea pig.METHODS: 33 guinea pigs were randomly divided into three groups: natural controlled group,allergic rhinitis(AR) group and budesonide treatment group.Allergic rhinitis model in guinea pigs were built by using toluene-2,4-disocyanate(TDI) nasal immunization and challenge.The indexes of clinical symptoms,pathomorphological diagnosis and content of histamine in mucosa were used to evaluate the potency of budesonide when used to treat allergic rhinitis.RESULTS: The expression of eosinophils and the content of histamine in nasal mucosa in AR group both were significantly higher than that of in natural controlled group(respectively,P(0.05)).CONCLUSION: Budesonide could effectively reduce the expression of eosinophils and content of histamine in nasal mucosa,but it is invalid in releasing the symptoms of AR in guinea pig.The reason of this phenomenon may be concerned with the method of model building.
10.Progress in study on toxin of Conus textile

Conus textile is a kind of highly toxic and abundantly existing conus in the South China Sea. The toxin from C.textile could act on sodium channels(δ-conotoxins)and calcium channels (ω-,ε-conotoxins), respectively. Their specific chemical structure and biological activity have attracted a lot of attention in recent years. This article briefly reviews their biochemical characteristics, isolation, gene cloning, biological and neuropharmacological activities, as well as their potential applications.
