2.Diagnostic value of CT target scanning combining with changing position for pulmonary nodule in special location

Objective To evaluate the diagnostic value of CT target scanning combining with changing position for pulmonary nodules in special location .Methods CT target scanning combining with changing position was performed in 22 patients with pulmo‐nary nodules adjacent to heart or in posterior costophrenic angle ,which were found with routine spiral CT scanning .For objective analysis , the signal‐noise‐ratio (SNR) and contrast‐noise‐ratio (CNR) of lung were calculated .In terms of subjective assessment ,the image quality was rated on a 3‐point scale (0-2) for pulmonary inflation ,gravity‐dependent pulmonary perfusion and severity of artifacts , respectively .The CT features of pulmonary nodules were compared between different scanning techniques .Moreover ,the diagnostic confidence for pulmonary nodules was evaluated .The paired t test ,Wilcoxon signed‐rank test and Kappa test were used for statisti‐cal analysis .Results In comparison with conventional spiral CT scanning ,CT target scanning combining with changing position im‐proved the subjective image quality scores (P<0 .01) ,increased the signal‐to‐noise ratio (SNR) and contrast‐to‐noise ratio (CNR) , showed more detailed CT features (P<0 .05) ,and improved the confidence of diagnosis (P<0 .01) .Conclusion CT target scanning combining with changing position technique can show detailed features ,which should be recommended as the optimal scanning tech‐nique for pulmonary nodules adjacent to heart or in posterior costophrenic angle .
3.Role of 18fluoro-deoxyglucose positron emission tomography on staging and predicting outcome in patients with lymphoma

ObjectiveTo evaluate the application of 18fluoro-deoxyglucose positron emission tomography (FDG-PET) to the staging and predicting outcome in patients with lymphoma.Methods 41 patients with newly diagnosed lymphoma(median age 57 years)were explored with FDG-PET prior to and after 4 cycles of chemotherapy.With a median follow-up of 30 months (range 10-68 months),the value of FDG-PET to staging and predicting clinical outcome was assessed. Results The maximum standardized uptake value (SUVmax) of nodal and extranodal lesions was 9.7±6.9 and 8.4±6.8 respectively prior to treatment.There were significant difference (P<0.05) in aggressive non-Hodgkin's lymphoma and indolent non-Hodgkin's lymphoma,no significant difference(P>0.05)in non-Hodgkin's lymphoma and Hodgkin's lymphoma(HL), B-cell neoplasms and T-cell neoplasms,germinal center B-cell-like DLBCL and activated B-cell-like DLBCL. In 41 patients, 22 patients (54 %)were detected extranodal focus by FDG-PET before chemotherapy. FDG-PET imaging upstaged in 6(15%)of initial lymphoma patients.There were 15 patients (37 %) in stage Ⅰ and Ⅱ and 26 patients(63 %)in stage Ⅲ and Ⅳ by FDC-PET scan.1 patient (7 %) in stage Ⅰ and Ⅱ,6 patient (23 %) in stage Ⅲ and Ⅳ died of disease progression during follow-up.After 4 cycles of chemotherapy,the FDG-PET was negative in 41%(17/41),positive in 59 %(24/41) respectively.1 patient(6 %)died of disease relapse among 17 patients who were FDG-PET negative, 6 patient (25 %)died of disease progression among 24 patients who were FDG-PET positive during follow-up. Conclusion FDG-PET scanning plays an important role in the pretreatment staging and prediction of the prognosis after 4 cycles of chemotherapy in patients with lymphoma.Thus it may offer the potential for change in treatment paradigms.
4.Determination of plasma loratadine concentration with RP-HPLC and its pharmacokinetic study

Objective : To establish a RP-HPLC method for the determination and pharmacokinetic study of loratadine in Chinese healthy volunteers. Methods; Chrornatography was performed on a Hypersil-BDS with acetonitrile-water (65 : 35, pH was adjusted to 3. 8 with H3PO4) as mobile phase. The flow rate was 1. 0 ml/min and the UV wavelength was set at 248 nm. Results: Good linearity was found within 0. 506 0-50. 60 ng/ml of loratadine in human plasma(r = 0. 999 5). The mean relative recovery rate was more than 95% ; intra-day and inter-day RSD were less than 15. 0%. The limit of detection was 0. 506 ng/ml. Conclusion: Our method has a good selectivity and sensitivity and can be used for further clinical pharmacokinetic study of loratadine.
5.Multi-detector computed tomography features of peripheral lung cancer associated with cystic airspace

Objective To evaluate multi-detector computed tomography(MDCT)features of lung cancer associated with cystic airspace,and to improve diagnostic accuracy.Methods 21 pathologically or clinically confirmed lung cancer associated with cystic airspace were retrospectively analyzed with regard to clinical data,pathological types and TNM stage,SUMmax and MDCT features.For dynamic follow-up CT scans,the lesion dynamic change was evaluated.Results There were 21 cases (1 1 adenocarcinoma,6 squamous carcinoma)in total. The lesion was predominant cysts in 2 cases(9.52%),cysts with GGO mixed lesions in 3 cases(14.29%),cysts with solid mixed lesions in 14 cases(66.67%),cysts with GGO and solid mixed lesions in 2 cases(9.52%).Four morphological patterns,20 cases had solid nodules,solid nodule protruding externally from the cyst wall was in 5 cases (23.81%),solid nodule protruding internally from the cyst wall was in 1 case (4.76%),solid tissue intermixed within clusters of cysts was in 14 cases(66.67%),the lesion presenting as circumferential thickening of the cyst wall was in 1 case (4.76%).The frequency of following features accounted for more than 60% of all MDCT signs,including round shape (66.67%),lobulation (80.95%),blood vessel passing through the cyst (76.1 9%),pleural indentation (80.95%);while the frequency of multiple cysts,irregular inner wall and septum in cyst in 19 lesions(90.48%)was more than 90%.With respect to the relationship between lesion and bronchus,bronchus passing through the cyst was the most common sign,accounting for 38.10%(8/21).Average CT enhancement value was 28.27 HU±7.27 HU(range 14.2-40 HU).Average SUVmax was 6.05(range 4.5-9.8),indicating marked FDG uptake.Two lesions manifested as progressive wall thickening and increased size of the cyst,and one lesion showed decreased size of the cyst and enlarged nodules in follow up CT.Conclusion The irregular inner wall,septum in cyst and blood vessel passing through the cyst are the most three important signs for the diagnosis of malignant lesions.
6.Application of T-SPOT.TB in the Early Diagnosis of Tuberculosis

Objective This study aims to explore the application value of tuberculosis T lymphocytes enzyme-linked immune SPOT test (T-SPOT.TB) on early diagnosis of tuberculosis.Methods The TB infection in 189 inpatients suspected tuberculosis in pneumology department of Shaanxi Provincial People's Hospital was detected with T-SPOT.TB,fluorescence RQPCR,tuberculosis (TB-Ab)protein chip and PPD methods.Results The sensitivity of four methods was 91.54% (119/130),73.85%(96/130),63.08%(82/130) and 57.69% (75/130) respectively and the specificity of those was 89.83% (53/59),86.44%(51/59),67.79%(40/59) and 66.10%(39/59),respectively.The sensitivity of T-SPOT.TB method was statistically higher than those of other three tests,respectively (P<0.05).The specificity of T-SPOT.TB was significantly higher than those of TB-AB and PPD (P<0.05),but there was no statistical difference between RQ-PCR and T-SPOT.TB (P>0.05).The positive predictive values of T-SPOT.TB,fluorescent quantitative PCR,TB-Ab and PPD assays were 95.2% (119/1250),92.3% (96/104),81.2% (82/101) and 78.9% (75/95) respectively while the negative predictive values of those were 82.8% (53/64),60% (51/85),45.5% (40/88) and 41.5% (39/94),respectively.The false-positive rates (misdiagnosis rate) of four assays were 10.2% (6/59),13.6% (8/59),32.2% (19/59) and 33.9% (20/59) respectively and the false-negative rates (rates of missed diagnosis) of those were 8.5% (11/130),26.2% (34/130),36.9% (48/130)and 42.3 % (55/130),respectively.The negative likelihood ratios of T SPOT.TB,fluorescent quantitative PCR,TB-Ab and PPD assays were 0.11,0.16,0.48 and 0.51 respectively,meanwhile the positive likelihood ratios of T-SPOT.TB,fluorescent quantitative PCR,TB-Ab andPPD assays were 9.0,5.4,2.0 and 1.7,respectively.What' s more,the diagnostic accordance rates of the four assays were 91.0% (172 189),77.8% (147 189).64.6% (122/189) and 60.3% (114/189),respectively.Conclusion T-SPOT.TB test is a more sensitive and specific method and of great significance to the early diagnosis of TB,which has more clinical value in different stages of tuberculosis diagnosis.
7.Multi-slice CT features and pathological correlation of peripheral lung cancer associated with thin-walled airspace

Objective To evaluate multi-slice CT (MSCT) features and pathological basis of lung cancer containing thin-walled airspace. Methods Thirty?five cases of pathologically confirmed lung cancer containing thin-walled airspace were retrospectively analysed with regard to clinical data, pathological types and MSCT features between 2012 and 2015.There were 35 cases(25 adenocarcinoma, 9 squamous carcinoma, 1 spindle cell tumor) in total. MSCT features were compared between the lesions with or without solid component .Fisher exact test was used for the statistical analysis. For dynamic follow-up CT scans, the lesion dynamic change was evaluated .Correlations between the pathological section and CT images of the 11 cases were analysed. Results These features accounted for more than 60% of all MSCT signs in 35 cases, including round shape in 28 cases(80.0%),lobulation in 32 cases(91.4%),multiple cysts in 27 cases(77.1%), irregular inner wall in 33 cases(94.3%)and septum in airspace in 31 cases(88.6%). Shape, spiculation, bronchus cut-off, blood vessel and bronchus passing through the airspace, and ground-glass opacity were significantly different between the lesions with or without solid component(P<0.05).The frequency of spiculation(11 cases) and bronchus cut-off(12 cases) in mixed solid lesions was higher than that in lesions without solid component(1 case, respectively).The frequency of irregular shape(6 cases),blood vessel passing through the airspace(12 cases),ground-glass opacity(13 cases)and bronchus passing through the airspace(7 cases) in lesions without solid were higher than that in solid mixed lesions(1, 1, 5, 3 cases respectively).The pathological basis of the formation of thin-walled airspace was obvious central necrosis in solid lesions and emphysematous change due to the tumor cells diffused along the inner airspace wall and the alveolar wall destruction.Five lesions were with progressive wall thickening and increased size of the airspace,and two lesions were with decreased size of the airspace and enlarged nodules in followed CT.One case of lung cancer with thin-walled airspace evolved from ground glass nodule. Conclusions The CT manifestation of lung cancer containing thin-walled airspace was characteristic.The pathological basis of the thin-walled airspace was various.
8.Effect of overexpression ALEX1 on proliferation and apoptosis

Objective:To investigate the effects of ALEX1 overexpression on cell proliferation and apoptosis of human breast cancer cell line MCF-7.Methods: MCF-7 cells were infected recombinant lentivirus LV5-ALEX1 and the negative control lentivirus LV5-NC,respectively.After 72 h, the expression of ALEX1 was detected by Real-Time PCR and Western blot.CCK8 assay were performed to observe the proliferation ability after 24 h, 48 h, 72 h, 96 h.The effect of overexpression ALEX1 on apoptosis was determined by flow cytometry.The level of Bax,BCL-2 and active caspase3 was detected by Western blot.Results:The mRNA level of ALEX1 markedly increased after 72 h(165.81±12.14 vs 52.29±2.32,P<0.01).In CCK8 assay,the results revealed that the cell pro-liferation was inhibited compared with control group in 48 h,72 h,96 h( P<0.05).The results revealed that overexpression of ALEX1 enhanced MCF-7 apoptosis(20.55%±2.50 % vs 3.60%±1.614%,P<0.05).The results by Western blot showed that the protein levels of Bax and active caspase were increased in LV5-ALEX1 group compared with LV5-NC group.However,the protein levels of BCL-2 was decreased in LV5-ALEX1 group compared with LV5-NC group.Conclusion:Overexpression of ALEX1 significantly reduced MCF-7 cancer cells proliferation and induced MCF-7 cells apoptosis.
9.Imaging of 99Tcm-cycllc RGD tetramer in nude mice bearing U87MG human glioma xenografts

Objective Multimeric cyclic RGD (Arg-Gly-Asp) peptides are capable of improving the integrin αvβ3-binding affinity due to the polyvalence effect.In this study,the authors prepare 99Tcm-la-bearing cyclic RGD tetramer E{E[c(RGDfK)]2}2,and evaluate its biodistribution and imaging in nude mice beating U87 MG human glioma xenografts with integrinαvβ3-positive.Methods 99Tcm-hydrazino-nictinamide (HYNIC)-E{E[c(RGDfK)]2}2 was prepared by two-step method,while HYNIC wag chosen as bifunctional chelator,and tricine and trisodium triphenylphosphine-3,3,3-trisuifonate (TPPTS) as coligands.The af-finity of c (RGDyK) monomer,HYNIC-E[c(RGDfK)]2 dimer and HYNIC-E{E[c(RGDfK)]2}2 tetramer to integrin αvβ3 was compared by in vitro competitive assay against binding of 125I-c(RGDyK)to integrin αvβ3.positive U87 MG human glioma cells.The biodistribution [the percentage of injection dose per gram of tissue(%ID/g)] and imaging were performed in nude mice bearing UB7MG human glioma xenografts.Re-suits The labeling yield of 99Tcm-HYNIC-E{E[c(RGDfK)2}2 was over 95%,and the radiochemical purity was more than 99%after purification with Sop-Pak C18 cartridge.The 50%inhibiting concentration (IC30) val-ues of c(RGDyk),HYNIC-E[c(RGDfK)]2 and HYNIC-E{E[c(RGDfK)]2}2 were 85.9,9.5 and 4.5 nmol/L, respectively.The result indicated that RGD tetramer possessed a significantly higher affinity to in-tegrinαvβ3.The biodistribution data showed that 99Tcm-HYNIC-E{E[c(RGDfK)]2}2 was excreted mainly through kidneys.The tumor uptake of 99Tcm-HYNIC-E{E[c(RGDfK)]2}2 was two times higher than 99Tcm- HYNIC-E[c(RGDfK)]2,at 1h postinjection,with the uptake of(10.32±0.07)%ID/g and(5.15±0.52)%ID/g,respectively,which was consistent with the in vitro competitive binding data.The tumor up-tale of 99Tcm-HYNIC.E{E[c(RGDfK)]2}2 was still as higher as(9.35±1.35)%ID/g at 4 h postinjec-tion, which demonstrated that the retention time of radiotracer in tumor was long enough.The imaging showed that tumor was clearly visualized at 1h postinjection,and the image at 4 h postinjection Was better.Conclusion The higher tumor uptake and longer retention time in tumor make 99Tem-HYNIC-E{E[c(RG-DfK)J 2}2 a promising radiotracer for integrinαvβ3-positive tumors imaging,furthermore,suggest that radi-onuelides(such as 90Y).1abeled RGD tetramer is more suitable for the therapy of integrin αvβ3-positive tumors.
