It has been indicated that the oncogene and anti-oncogene are closely related to the development and progression of cancer.With the development of molecular biology,detection of oncogene and anti-oncogene has become an important approach for clinical warning and diagnosing of cancer.Since the establishment of the molecular model of colon cancer by Fearon and Vogelstein in 1990,the genetics of colorectal cancer has gained major advances in early diagnosis.Early detection of related gene mutations is helpful for the early warning of colorectal cancer,especially for palients with hereditary cancer(the detection rate is fairly high).This review summarizes the progression of study on biomarkers of colorectal cancer.
2.The effect ofShenmaiinjection on cardiac systolic and diastolic function of isolated myocardial infarction model rats

ObjectiveTo observe the effect ofShenmai injection on isolated cardiac systolic and diastolic function in myocardial infarction rats.Methods32 rats were randomly divided into a control group, a propranolol group, aShenmai + propranolol group and aShenmai group. The myocardial infarction models were made by ligating the left anterior descending coronary artery of rats .The heart was taken from myocardial infarction rat after 3 days. The indexes of Isometric tension (IT), Diastolic tension (DT), heart rate (HR), the maximum pressure ascending and descending rate of left ventricle (± LVdp/dtmax) (mmHg/s) were recorded by using Langendorff isolated heart perfusion technique at 5min before and 0.5 h, 1 h, 2 h after perfusion.ResultsCompared with the control group, the IT (2.01 ± 0.02 g, 1.52 ± 0.01 g, 1.53 ± 0.02 g), the HR (326 ± 32 time/min, 331 ± 41 time/min, 307 ± 37 time/min) and the ±LVdp/dtmax (4 012 ± 193 mmHg/s, 3 729 ± 174 mmHg/s, 3 631 ± 123 mmHg/s; 3 565 ± 202 mmHg/s, 3 632 ± 154 mmHg/s, 3 601 ± 129 mmHg/s) were apparently decreased in the propranolol group at 0.5 h, 1 h, 2 h after perfusion, and the differences were statistically significant (P<0.05). Compared with the control group, the IT (2.71 ± 0.10 g, 3.74 ± 0.04 g, 3.17 ± 0.01 g), the HR (491 ± 46 time/min, 527 ± 52 time/min, 519 ± 49 time/min) and the +LV dp/dtmax (5 690 ± 2 403 mmHg/s, 5 739 ± 219 mmHg/s, 5 825 ± 186 mmHg/s) were distinctly increased in theShenmaigroup at 0.5 h, 1 h, and 2 h after perfusion, and the differences were statistically significant (P<0.05). While the IT, DT, HR and ±LVdp/dtmax in theShenmai + propranolol group had no significant effect compared with the control group(P> 0.05).ConclusionsShenmaiinjection can improve myocardial contractility in isolated heart of myocardial infarction rats, however, propranolol can completely block the strong heart effect ofShenmaiinjection. This suggests thatShenmai injection has a beta receptor agonist effect.
3.Clinical characteristics and classification of ketosis-prone diabetes

Objective To investigate the clinical characteristics and classification of ketosis-prone diabetes. Methods Firstly,according to with or without autoantibodies,patients with an initial onset of unprovoked ketosis or ketoacidosis were classified to two different groups. Secondly,based on weather or not depending on insulin,patients without autoantibodies were further divided into two groups,insulin dependent and non-insulin dependent. Clinical characteristics,biochemical parameters were compared between different groups. Results The autoantibodies negative group showed a strong male predominance and family history of diabetes.BMI was higher than type 1 diabetes. Plasma triglyceride level was highest. Fasting and postprandial C-Peptide level was between the other two group. Compared with non-insulin dependent group,insulin dependent group showed a strong male predominance,lower ratio of overweight and obesity,more severe ketosis status. There were no significant differences with regard to blood glucose level and fasting C-Peptide level of initial. But fasting C-Peptide after 3 months was higher in non-insulin dependent group . Conclusion Ketosis-prone diabetes can divided into two groups,based on autoantibody negative or positive. The latter consist of type 1A diabetes mellitus. The former can further classified as ketosis-prone type 2 diabetes and idiopathic type 1 diabetes(type 1B diabetes)according to insulin dependent or non-insulin dependent.
4.Endovascular Stent-Assisted Angioplasty of Symptomatic Intracranial Stenosis

The application of intracranial stent-assisted angioplasty has been in controversy.In recent years, the development novel stents, preoperative evaluation, and accumulation of periprocedural management experiences have significantly improved the success rate of this operation.The incidence of complications has decreased significantly. However, the efficacy and safety of intracranial stent-assisted angioplasty are still lack of support from prospective multicenter studies.This article reviews the recent advances in this field.
5.Anesthetic management for video-assisted laser operation under rigid bronchoscopy

Objective To discuss the management of anesthesia for video-assisted laser operation under rigid bronchoscopy.Methods A retrospective review was made on anesthetic records of 15 cases of video-assisted operation under rigid laser bronchoscopy in this hospital from September 2002 to December 2006.According to the ASA classification,there were 7 cases of class Ⅱ,7 cases of class Ⅲ,and 1 case of class Ⅳ.The diagnosis included 13 cases of tumor and 2 cases of cicatricial stricture.All the patients had a forced expiratory volume in 1 second(FEV1) predicted lower than 70%. By using intravenous fast induction,the propofol,fentanyl,and vecuronium or rocuronium were injected successively.Then a rigid bronchoscope was inserted into the trachea under orthoptic laryngoscope.During the operation a total intravenous anesthesia(TIVA) was maintained.Before induction and 5~10 min after operation,we monitored artery blood gas(ABG) levels.Immediately following the operation the end-tidal CO2 pressure(PETCO2) was measured.Results Hemodynamics parameters were not significantly different between preoperatively and intraoperatively.During the operation,the pH value was 45 mm Hg in 5.All the patients presented normal blood pressures and heart rates. Conclusions During video-assisted laser operation under rigid bronchoscope,anesthesia with normal frequency jet-ventilation and artery blood gas monitoring is safe and reliable.
6.The expression difference of serum bilirubin and uric acid in carotid artery plaque formation of patients with ischemic cerebral vascular disease

Objective To investigate the possible roles of serum bilirubin and uric acid in the course of carotid artery intima thicken/plaque formation. Methods Patients with ischemic cerebral vascular disease were divided into the control group, intima thicken group and plaque formation group according to the carotid artery intimamedia thickness (IMT) examination by B-mode ultrasound. The serum bilirubin and uric acid were detected by Automatic Chemistry Analyzer. Results The serum level of uric acid of (391.22 ± 27.52) μmol/L in intima thicken group was significantly higher than that in the control group and plaque formation group((307.32 ± 13.68)μmol/L and (327.84 ± 17.96)μmol/L, P < 0.05). The serum level of indirect bilirubin and total bilirubin in plaque formation group was (10.96 ± 0.58) μmol/L and (15.91 ± 0.71) μmol/L respecitively, which were significantly lower than those in the control group ((15.09 ± 2.21) μmol/L and (20.59 ± 2.43) μmol/L,respectively) and the intima thicken group((15.09 ± 2.21) μmol/L and (20.59 ± 2.43) μmol/L, respectively) (Psignificantly lower than that of (5.70 ± 0.28) μmol/L in the control group(P < 0.05). Conclusions Bilirubin and uric acid play different roles in carotid artery plaque formation during different stage.
7.Antimicrobial Resistance of Staphylococcus:Analysis by Shandong Provincial Nosocomial Infection Surveillance System

OBJECTIVE To investigate the trends of susceptibility of Staphylococcus isolates to antimicrobial agents analyzed by the Shandong provincial nosocomial infection surveillance system.METHODS The data from Jan 2002 to Dec 2005 were summarized and analyzed by the Shandong provincial nosocomial infection surveillance system.RESULTS During this period,1644 strains of first isolate from each patient were collected.Major specimens were sputum,wounds,skin soft tissue and urine,which were 39.72%,22.08%,9.43% and 7.54% in total specimens,respectively.The drug-resistant rates of Staphylococcus isolates to antimicrobials were high,except to vancomycin.CONCLUSIONS Antimicrobial resistance of clinical isolates threatens the success of infectious disease treatment.It is very important to promote the rational use of antimicrobial agents for control of resistance and to take effective measures for the prevention of infections.
8.Treating lupus nephritis by a drug pair of radix astragali and rehmanniae radix combined with glucocorticoid: a preliminary clinical study.

OBJECTIVETo observe the therapeutic effect of a drug pair of Radix Astragali and Rehmanniae Radix combined with glucocorticoid (GC) in treating lupus nephritis (LN) patients and its influence on some experimental indices.
METHODSTotally 52 LN patients were randomly assigned to the treatment group (treated by routine Western medicine and a drug pair of Radix Astragali and Rehmanniae Radix, 25 cases) and the control group (treated by Western medicine, mainly by GC and cyclophosphamide, 27 cases). All patients received 6-month therapy. The GC dosage, the withdrawal and reduction dosage of GC, clinical efficacy, systemic lupus erythematosus disease activity index (SLEDAI) score, adverse reactions, and laboratory indicators were recorded.
RESULTS(1) All patients got relieved to some degree with the dosage of GC reduced. The total withdrawal and reduction dosage of GC was slightly higher in the treatment group than in the control group [(50.23 +/- 12.43) mg vs (48.76 +/- 13.61) mg, P > 0.05]. Besides, the prednisone dosage in the treatment group was lower than that in the control group, but without statistical difference (P > 0.05). The ratio of patients in need of adding prednisone for aggravating disease was 24.0%, significantly lower than that in the control group (44.44%, P < 0.05). (2) There was no statistical difference in the SLEDAI score, inflammatory indicators, liver and renal functions, blood electrolytes, blood glucose, blood and urine routines between the two groups (P > 0.05). The 24-h urinary protein count was (1.06 +/- 0.22) g/L in the treatment group, obviously lower than that in the control group (1.43 +/- 0.55 g/L, P < 0.05). (3) There was no statistical difference in the incidence rate of infection, gastrointestinal hemorrhage, psychoneuroses, Cushing's syndrome, cardiovascular anomalies, and femoral head necrosis between the two groups (P > 0.05). But the incidence of adverse reactions such as insomnia, tidal fever, spontaneous sweat, and obesity was less in the treatment group than in the control group (P < 0.05).
CONCLUSIONSUsing a drug pair of Radix Astragali and Rehmanniae Radix combined with GC in treating LN could withdraw the dosage of GC and relieve symptoms it induced. It was advantageous in reducing the dosage of GC and stabilizing patients' conditions.

9.Mid-term follow-ups of hydroxyapatite extensively-coated femoral prosthesis for elderly patients

Objective To evaluate the effects of hydroxyapatite (HA) extensively-coated prosthesis on elderly patients undergoing hip arthroplasty.Methods From February 1997 to May 1999,96 patients Were treated with hydroxyapatite (HA) coated prosthesis and followed up for their hip arthroplasty.Their results were studied clinically and roentgengraphically to evaluate chiefly their Harris scores,complications and thigh pain.The survival of prosthesis was evaluated according to Kaplan-Meier methods.Results The 60 cases were followed up for eight years on average(range,sevon to nine years).Their mean Harris scores were improved from 42 preoperatively to 90 postoperatively.Fifty-eihgt cases felt no pain at their hip,but two did at their thigh.No radiolucent lines were observed.All the prostheses achieved bony stabilization and remolding.Conclusions HA coated prosthesis can achieve fine fixation,because it helps bone conduction,resists axial force and torsion,and reduces operative risks.
