OBJECTIVESTo study the morphological feature of complicated thoracolumbar fractures and the fixation technology of injured vertebra.
METHODSFrom January 2005 to December 2007, 61 patients with type B and C thoracolumbar fractures according to AO classification were treated. There were 53 males and 8 females, with a mean age of 33.2 years (range, 9 to 65 years). Based on ASIA grading system of neurologic deficit, Grade A was found in 41 cases, grade B in 5 cases, Grade C in 5 cases, Grade D in 6 cases and Grade E in 4 cases. The injured vertebra located at thoracic region in 13, thoracolumbar in 39 and lumbar in 9 cases. The patients were divided into two groups: 32 cases with pedicle screws in the injured vertebra and 29 cases without pedicle screw in the injured vertebra. The analyses on the morphological features of injured vertebra were performed, and comparisons were carried out on the outcome regarding the reduction and correction of the injured vertebra, and the difference of Denis scale in pain domain and work status domain, between the two groups.
RESULTSAll the patients were followed up for an average of 3 years (range, 1.5 to 4.5 years). There were significance statistical difference in the correction loss of Cobb angle, sagittal index, correction of dislocation index, and the Denis pain scale between the two groups (P < 0.05). A better outcome was observed in the injured vertebra fixation group than the injured vertebra non-fixation group.
CONCLUSIONSInserting pedicle screws in the injured vertebra is effective and useful in the correction of complicated thoracolumbar fractures. It improves biomechanical stability of the spine after the operation.

Objective To investigate the significance of TGF-β1, TGFRl and TGFR2 in the pathogenesis and prognosis in patients with myelofibrosis. Methods The expression of TGF-β1 and its receptors (TGFR1 and TGFR2 ) in bone marrow tissues and the level of TGF-β1 in the blood of 23 patients with myelofibrosis were detected by SABC immunocytochemistry and ELISA repectively. Results Expression of TGF-β1 and TGFR 1 was significantly higher in primary and secondary myelofibrosis patients than that of the control. No significant difference of TGFR2 expression was found between the groups of myelofibrosis and the control (P>0.05). The level of TGF-β1 in the blood of the patients with myelofibrosis was significantly higher than that of the control (P<0.01) and more obvious in secondary cases while TGF-β1 decreased nearly to the normal level when patients were in clinical remission. Conclusion TGF-β1 and it's receptors may be involved in the pathogenesis of myelofibrosis and might be of importance for the prognosis of the patients with myelofibrosis.

Objective To explore the plasma level of retinol binding protein 4 (RBP4) in patients with es-sential hypertension (EH) of Xinjiang Kazakans and the relations between plasma RBP4 level and blood pressure, blood lipids, blood glucose, insulin, and HOMA-IR. Methods 81 Kazakans cases who were investigated between August and November of 2008 were selected as subjects. They were divided into EH group(n=41)and control group (n=40). Blood pressure was measured. Fasting venous blood was drawn to detect the plasma RBP4, fasting insulin (FINS), fasting plasma glucose(FPG), total cholesterol(TC), triglyceride(TG), high density lipoprotein cholesterol (HDL-C)、low density lipoprotein cholesterol (LDL-C), apoprotein-A1 (APO-A1) and apoprotein-B (APO-B), and concentration in EH group was significantly higher than that in control group[ (32.74±9.93) mg/L vs (21.92± pressure (DBP),FPG,and HOMA-IR in EH group(r,=0.618,0.775,0.565,0.317,0.369,P<0.01 or P< showed that SBP was independent factor to affect the RBP4 level in EH group(Standardized Coefficients is 0.591, t=5. 695, P=0.000). Conclusion The plasma RBP4 concentration is increased in patients with EH of Kazakans, and is positively correlated with SBP,DBP,FPG,HOMA-IR in EH group. RBP4 may involve in the development of EH by promoting insulin resistance.
