1.Study on nursing by stages for patients with acute aortic dissection during the postoperative period

Objective To explore the key points of observing and nursing for patients with acute aortic dissection during the different stages postoperatively.Methods The nursing methods by stages were performed on 25 patients with acute aortic dissection.The stages included early stage(1 to 2 days after the operation), intermediate stage(3 to 14 days after the operation) and advanced stage(14 days after the operation and later).The key points of each stage were different.In the early stage,it needs to closely observe the circulation,the amount of blooding,the respiration system,the nerve system,and the occurrence of complications;and it also needs to do the syndrome-based nursing well,including calming,dropping blood pressure,blood transfusion,suction of phlegm and et al.In the intermediate stage,it needs to guide the daily life of the patients,including taking diet and drinking,activities,excretion and et al.In the advanced stage,it needs to do directions of discharging from the hospital and to prevent the recrudescence of aortic dissection. Results A total of 25 patients were improved in term of their symptoms or cured by our nursing care.The diagnosis was timely and the operation Was suitable.The observation and nursing by stages after the operation were reasonable.All the patients were discharged from the hospital after being healed,no death and no occurrence of serious complications.Conclusions It is helpful to improve the restoration of patients with acute aortic dissection by observing and nursing care by stages,which will play an important role in promoting the smooth healing of the patients and preventing the accidents and the reoccurrence of aortic dissection.
2.Nursing of one elderly patient with multiple myeloma and the complication of interstitial pneumonia

Summarized the nursing care process of one elderly patient with multiple myeloma and interstitial pneumonia.Nursing measures included oxygen therapy,infection nursing,hormones therapy nursing,rehabilitation training,psychological nursing and so on.After treatment and nursing,the patient got the stability of myeloma,and interstitial pneumonia was cured in our department.
3.Analysis of diagnosis and treatment of 66 cases of testicular torsion

Objective To analyse the diagnosis and treatment of testicular torsion. Methods The clinical data of 66 cases of testicular torsion were retrospectively analysed. Results Among the 66 cases,32(48.5%) paid the first medical visit within 10 h,and 24(36.4%)were confirmed diagnosis at the first visit.False negative results occurred with color Doppler flow imaging(CDFI),and 8 testicles were damaged due to the false negative diagnosis.Thirty-three patients without prophylactic contralateral orchidopexy were followed up for 6 months to 20 years,and one experienced recurrent torsion. Conclusion The testicular torsion must be considered when a sudden acute scrotum pain is occurred.Testicular damage is closely related to the torsion time,and delayed medical intervention contributes to the testicular damage.Highly suspected cases should be performed surgical exploration timely due to the false negative results with CDFI.Prophylactic contralateral orchidopexy is recommended.
4.Nursing on coronary restenosis patients with depression after stent placement

Objective To learn the depression levels of coronary restenosis patients after stent placement, so as to provide corresponding nursing strategies for them.Methods A total of 71 coronary restenosis cases after stent placement hospitalized in cardiovascular surgery department of our hospital were selected from 2004 to 2007.They were investigated by self-rating depression scale (SDS) .Among all cases,59 cases had different degrees of depression and were divided into test and control group randomly.29 cases in control group were given routine nursing care.Another 30 cases in test group were provided comprehensive nursing intervention.Rescoring with SDS of both group cases were compared with each other 10 days (preoperation) after nursing intervention.Results After 10 days (pre-operation) intervention, the scorings of test group were remarkably lower than those of either control group or before nursing intervention.There were statistical significant differences between them(P<0.05).Clinical symptoms relieved in test group cases, which was better than that of control group.Conclusions Coronary restenosis cases after stent placement have higher incidence of depression feeling.We should pay attention to them and take corresponding comprehensive nursing intervention to relieve their depression, which is good for the preparation of their operation.
5.Clinical care issues and strategies of the elderly malignant blood cancer complicated by multiple organ cancer

Objective To analyze the clinical care issues of the elderly malignant blood cancer and figure out the strategies. Methods Review and summarize clinical care issues during the care of 19 elderly patients with malignant blood cancer. Results All the patients had various degrees of psychiatric symptoms (all had sleep disorders, 2 cases of diagnosed depression), 11 cases of infection, 5 cases of malnutrition, skin problems occurred in 9 cases (various degrees of petechia, ecchymosis), 0 patients had cancer pain, 8 patients had the changes of the form and frequency stool (diarrhea, diarrhea) , 1 patient with chest drainage tube, 1 patient with abdominal drainage tube, 1 patient with chest and abdominal drainage tube, 3 patients with large venous tube, IS patients with PICC tube, 1 case with enterostomy. Through targeted clinical care measurements, the life quality of the patients was greatly improved and the life year was extended. Conclusions During the clinical care of 19 elderly patients with malignant blood cancer, we provided the intervention measurements characterized by the combination of physiology and psychology. These measures had made the patients actively cooperate with the treatment and improved their quality of life.
6.Investigation of hypnotic agents administation in hospitalized elderly patients and its nursing

Objective To investigate the status of hypnotic agents administation in hospitalized elderly patients, and the relativity between incidents and the hypnotics consuming, in order to reduce the side effects of hypnotics in the clinics. Methods Investigated the outflow status of hypnotics in the clinical pharmacy, and status of the category, volume , consuming time of the old patients, and whether or not they received evaluation of sleeping quality before they taking the hypnotics was also investigated, and the factors affect the result of hypnotics and its negative consequences were educated. Results The mainly used of hypnotics in clinics was benzodiazepines, with various category and quantity, depending on different disease the individuals suffering from. There still existed hidden danger of consuming hypnotics, such as many patients hadn' t not received sleeping quality evaluation before taking them. There were also some factors affected the quality of patients sleep after taking medicines. Conclusions The health education of doctors, nurses and patients should be strengthened. It' s also recommended that early non-medicine intervention and reduction of the circumstance affecting factors would be benefit for the less use of hypnotics therefore reduce its side effects. The evaluation of sleep quality is also essential in the instruction of hypnotics using in clinics.
7.Establishment and evaluation of a rat model of type 2 diabetes associated with depression.

OBJECTIVETo establish and evaluate a rat model of type 2 diabetes mellitus (T2DM) associated with depression for further elaborating the disease.
METHODSTwenty-four Wistar rats were randomly divided into three groups: normal group (group N), T2DM group (group T) and T2DM with depression group (group T + D), with 8 rats in each group. The T2DM rat model was induced by high fat diet and low dose of Streptozotocin (STZ) injection, and in addition, the T2DM rats were made restraint stress for 21 days. After the model was established, the insulin tolerance test (ITT) and oral glucose tolerance test (OGTT) were performed. Then the rat depression level was analyzed by open field test, and the concentration of 5-hydroxytryptamine (5-HT) and dopamine (DA)was determined by ELISA to confirm the model identity.
RESULTSThe blood glucose level in group T and group T + D didn't return to the normal level at 180 minutes in the ITT and OGTT test; Compared with the group N, the max movement distance, retaining time in the central zone and the retaining frequency within 5 minutes in the group T + D decreased; 5-HT and DA level in the serum of rats in. group T + D was reduced.
CONCLUSIONA rat model of type 2 diabetes mellitus associated with depression has been successfully established by high fat diet and injection of low dose streptozotocin in combination with restraint stress for 21 days. This rat model is useful for further relevant studies.

8.Relationship between gene polymorphism of transforming growth factor-beta and pneumoconiosis.

OBJECTIVETo explore the association between genetic polymorphisms of TGF-beta (TGF-beta) and susceptibility to pneumoconiosis.
METHODSOne hundred and seventeen patients with pneumoconiosis were selected as case. The control group was workers exposed to dust but without pneumoconiosis who had the same sex, nationality, and workshop or work site as case. The differences in the age and cumulative exposure time between the case and control group were not move than five years and two years, respectively. The case matched with the control according to 1:1. Polymerase chains reaction-restriction fragment length polymorphism was used to determine the frequencies of TGF-beta genes in the two groups.
RESULTSThe frequencies of this TGF-beta (-509) genotypes were CC (22.2%), CT (43.6%) and TT (34.2%) in cases, which was significantly different from the control group, respectively (OR = 1.390, P < 0.05). There was no significant difference for frequency of TGF-beta+869 genotypes and allelic between case and control (P > 0.05). The frequencies of the TGF-beta (+915) genotypes in case [GG (70.9%), GC (29.1%)] were significantly different from the control group (OR = 1.455, P < 0.05). The frequency of TGF-beta (+915) * C allele in the case and control was 14.5% and 8.5%, respectively (P < 0.05). The frequencies of carrying TGF-beta (-509) CC and (+915) GG genotypes were 12.8% and 29.9% in case and control. The frequencies of carrying TGF-beta (-509) * T and (+915) * C alleles were 9.8% and 5.1% in pneumoconiosis and control (P < 0.05).
CONCLUSIONSTGF-beta (-509)CC genotype may be the protective factor for the pneumoconiosis. TGF-beta (+915)GC genotype may be a susceptible factor for the pneumoconiosis. The workers of carrying TGF-beta (-509) * T and (+915) * C alleles are more susceptible to pneumoconiosis.

9.Construction of an aging model of renal tubular epithelial cells

Objective:To compare aging models for renal tubular epithelial cells induced by different drugs.Methods:Different concentrations of D-galactose(D-gal), hydrogen peroxide(H 2O 2)and cisplatin(CDDP)were administered to the human proximal tubular epithelial cell line(HK2). Cell activity and the half maximal inhibitory concentration(IC50)were measured by the CCK-8 assay; cell senescence was assessed by senescence-related β-galactosidase staining(SA-β-gal); senescence-related gene expression was detected by Western blotting; cell cycle distribution and apoptosis were determined by flow cytometry.Pathological changes in renal tubules and interstitial tissues were examined in D-gal-induced and naturally aging mice using HE staining, and p16 expression was detected using immunohistochemistry. Results:CCK-8 assay results showed that HK2 cell activity was inhibited treatment with each of the three compounds.The 48-hour IC50 values were(365.8±9.7)mmol/L for D-gal, (385.4±20.8)μmol/L for H 2O 2 and(8.4±1.6)μmol/L for CDDP.Light microscopic observation revealed slowed growth of HK2 cells in the three groups.The rate of SA-β-gal-positive cells increased, compared with the control group( P<0.05). Treatment resulted in an increase in G0/G1 phase cells by(22.9±1.0)% in the 400 mmol/L D-gal group and by(13.0±4.4)% in the 400 μmol/L H 2O 2 group, while G2/M phase cells increased by(14.4±1.9)%( t=48.07, 6.40, 16.53, P<0.05)in the 8 μmol/L CDDP group, compared with the control group.Also, compared with the control group, HK2 cell apoptosis increased by(50.3±1.0)% in the 400 μmol/L H 2O 2 group and by(41.9±2.0)% in the 8 μmol/L CDDP group, which was significantly higher than(7.7±0.4)% in the 400 mmol/L D-gal group( t=77.47, 33.73, 28.35, all P<0.05). Western blotting results indicated that the expression of CCND1 was down-regulated after any of the three drugs reached a certain concentration.The expression of p16 in the D-gal group was up-regulated( F=92.88, P<0.05), but there was no statistical difference in the expression of p16 after H 2O 2 or CDDP treatment.Mice of the D-gal model showed a decline in renal tubular cells, thickened basement membrane, widened interstitial spaces and increased expression of p16 in renal tubules similar to those observed in naturally aging mice. Conclusions:For HK2 cell senescence models induced by three different drugs, the renal tubular epithelial cell senescence model induced by D-gal is relatively close to the natural senescence model.
10.Analysis of association between the IRF5 gene single nucleotide polymorphism and allergic rhinitis

Objective To study the possible association between interferon regulatory factor 5 (IRF5) gene polymorphism and allergic rhinitis (AR).Methods Six independent single nucleotide polymorphism (SNP,rs729302,rs4728142,rs3807306,rs2070197,rs11770589,rs2280714) were analyzed.The genotype and allele frequencies were detected in 110 AR patients and 101 healthy controls in Singapore Chinese population by polymerase chain reaction (PCR) and restriction fragment length polymorphism(RFLP).Results Rs729302 was rejected as it was not polymorphic.For others SNP,no statistically significant difference was detected in genotype between AR and healthy control group (x2 value were 0.21,5.02,0.01,2.91,0.37,all P > 0.05).No statistically significant difference was detected in allele frequencies between AR and healthy control group (x2 value were 0.00,2.78,0.01,2.31,0.00,all P > 0.05).Conclusion No association is observed between IRF5 and AR in Singapore Chinese population.
