1.Effect of rigid corneal contact lens and corneal limbal stem cell transplantation for senile patients with pterygium

AIM: To investigate the effect of rigid contact lens in the treatment after pterygium excision and limbal stem cell transplantation in senile patients.METHODS: Totally 90 elderly patients diagnosed as unilateral pterygium in our hospital from March 2015 to March 2016 were selected and divided into two groups, observation group and control group, 45 case with 45 eyes in each group.Observation group was treated with limbal stem cell transplantation and rigid contact lens.Control group was treated with limbal stem cell transplantation only.The following indicators were observed and compared: corneal healing time, visual analogue score(VAS) at 1, 3, 5 and 7d after surgery and the recurrence rate of pterygium.RESULTS: The score on corneal irritation of observation group was significantly lower than that of control group (P<0.05).Control group took more time on corneal healing than observation group (P<0.05).VAS of control group were higher than that of the observation group at 1 and 2d after treatment with significant difference (P<0.05).The recurrence rates of pterygium were not statistically significant between the two groups (P>0.05).CONCLUSION: Autologous corneal stem cell transplantation combined with rigid contact lens for pterygium in elderly patients is effective with shorter healing time and less pain, and it does not increase the recurrence rate.
2.Analysis the complications after trabeculectomy in 405 cases in department of ophthalmology

AIM: To analyze and summarize the effect of glaucoma trabeculectomy in the department of ophthalmology in basic hospital.
METHODS: Postoperative intraocular pressure ( IOP ) , filtering bleb and complications of 316 cases (405 eyes) of patients with glaucoma after trabeculectomy were analyzed.
RESULTS: After follow- up 12mo, 76. 5% IOP was controlled in normal level. 42. 5% was filtration bleb typeⅠ, 33. 1% typeII, 14. 6% in typeⅢand 9. 9% in typeⅣ. Intraoperative complication rate was 2. 5%, that was 31.4% at postoperative early stage ( before discharge ) , and 6. 7% at postoperative long - term ( 6mo after discharge) .
CONCLUSION:Trabeculectomy for glaucoma can better solve the problem of high IOP. It is more mature in primary hospitals, but there are still a variety of intraoperative and postoperative complications.
3.Progress in the ligands and their complex structures of farnesoid X receptor.

Farnesoid X receptor (FXR) belongs to the nuclear receptor superfamily. It is highly related to the formation of metabolic syndrome and the glucose homeostasis, and therefore represents an important drug target against metabolic diseases and diabetes. In recent years, great progress has been made in the agonists, antagonists, and crystal structures of FXR. The diverse FXR ligands and their structure-activity relationship are reviewed in this article. The advances in the crystal structures of FXR in complex with different ligands are also introduced.
4.Sequence and timing of immunosuppressants-thymectomy combination therapy in patients with myasthenia gravis

Objective Following the prognosis of patients with myasthenia gravis(MG) who received both immunosuppressants and thymectomy, to determine whether the sequence and timing of treatment have influences on outcomes of MG and also to select significant predictors of prognosis.Methods It was a retrospective study which reviewed 105 patients with MG who underwent thymectomy and immunosuppressants between Januay, 2000 and December, 2013.All patients were divided into two subgroups based on their order of treatment.Thirty two patients were called as immunosuppressants-first group, and 73 patients were called as thymectomy-first group.The ratios of ideal status of two groups were compared with use of the ChiSquare test.Predictors of ideal status were analyzed by logistic regression.Results (1) After 1 year, 2 years and 5 years of intervention, the proportion of ideal status was significantly higher in the thymectomy-first group than that in the immunosuppressants-first group[(33%-50%) vs.(10%-20%), P <0.05].For oMG patients, after 1 year, 2 years and 5 years of treatment, thymectomy-first group had a higher ideal status rates than immunosuppressants-first group[(25%-50%) vs.(11%-18%)].But no significant difference was noted in the rates of ideal status at each follow-up time point.(2)Multivariate logistic regression analysis showed that sequence of treatment (OR =0.154, 95 % CI: 0.046-0.518, P =0.002) and symptom-thymectomy interval(OR =0.903,95 % CI: 0.835-0.976, P =0.010) were the significant predictors of ideal status.Conclusion Thymectomy prior to immunosuppressants is associated with relatively good prognosis.It is suggested that patients with gMG, or adult patients with oMG who are resistant to drug, will be better to undergo thymecotomy as early as possible.
5.Effect of combined Metformin and Esomeprazole therapy on plasma gastrin and blood glucose in elderly patients with type 2 diabetes mellitus

Objective To investigate the effect of combined Metformin and esomeprazole therapy on plasma levels of gastrin,blood glucose,glycosylated hemoglobin(HbA1c) and insulin in elderly patients with type 2 diabetes.Methods A randomized,double-blind,placebo-controlled study of 41 elderly patients with type 2 diabetes was conducted.Patients were randomly assigned into treatment group(combination therapy with Metformin 0.5 g,bid or tid and Esomeprazole 20 mg,qd,for 12 weeks)and placebo group(Metformin monotherapy 0.5 g,bid or tid,for 12 weeks).Fasting blood samples were taken from vein before and after treatment.Fasting serum levels of gastrin,glucose,HbA1c,insulin,lipids,liver and renal functions were compared between the two groups.The homeostasis model of assessment for insulin resistance index(HOMA-IR) and insulin secretion index (HOMA-β)were calculated,and complications were recorded.Results There were no significant differences in body mass index and waist circumference between the two groups.Serum gastrin level was slightly increased in the treatment group 12 weeks after treatment,but without statistically significance [(127.20±9.21)ng/L vs.(131.53±7.84)ng/L,P>0.05],and serum gastrin level had no significant differences in the placebo group before and after treatment [(128.42±4.58)ng/L vs.(127.51±3.47)ng/L,P>0.05].However,there were no significant differences in the changes of blood glucose,HbA1c,insulin,HOMA-β and HOMA-IR before versus after therapy,and between the two group(all P>0.05).Conclusions Combined Metformin and insulin therapy cannot increase serum gastrin and insulin levels and has no significant effect on reducing blood glucose and HbA1c levels in elderly patients with type 2 diabetes.
6.Bioinformatical analysis of the specific allergen pTSX1 cDNA clone of Humulus pollen in allergic asthma

Objective To make a bioinformatical analysis of the specific allergen pTSX1 cDNA clone obtained from Humulus pollen in allergic asthma by immunological screening and gene cloning. Methods Bioinformatical approaches, including sequence analysis, sequence alignment, genetic makeup, protein structure and function prediction, were used. Results The analysis of the specific allergen pTSX1 cDNA showed that the gene had no reading frame and coded a protein with 204 amine acids. The analysis of amine acids sequence coded by the pTSX1 cDNA showed that the protein identity of this gene product might be one of ribosomal proteins. Conclusion The pTSX1 cDNA cloned from Humulus pollen in allergic asthma is a new gene, and the protein coded by pTSX1 gene may be one of ribosomal proteins.
7.Degeneration and establishment of occlusal wear facets

BACKGROUND:Tooth wear is the physiological change that accompanied by a person’s life, type and nature, maxil ary and mandibular occlusion status, physical status, and living environment may affect it.
OBJECTIVE:To review the occurrence and development of tooth wear, as wel as research on the morphology of wear surface.
METHODS:The PubMed database was searched by the first author for the articles from January 1952 to
December 2012. The articles on the generation of tooth wear facets, macroscopic and microscopic studies of occlusal wear facets, wear measurement method, relationship between tooth wear and oral diseases, and
application of tooth wear facets in forensic research were included. The key words were“tooth, wear, diet,
information, dentistry, anthropology”in English. A total of 59 articles were included according to the inclusion criteria for review.
RESUTLS AND CONCLUSION:With the development of wearing, some oral clinical symptoms may occur. Just as there are no same fingerprints in the world, the individual tooth wear facet is unique. Therefore, the research on occlusal wear facets has great significance for understanding the diet habits, cultural development, age and other information, as wel as for dentistry, anthropology and forensic science. The macroscopic and microscopic study of tooth wear surface can provide a large amount of individual information. The current studies on tooth wear facets cannot ful y exhibit al the implicated information, so the further studies are needed.
8.Clinical features and drug sensitivity of lower respiratory tract infection induced by Streptococcus pneumoniae in children

Objective To explore clinical features and drug sensitivity of Streptococcus pneumoniae (SP) isolated from pediatric patients with lower respiratory tract infection, and to provide evidence for clinical use of antibiotics. Methods A total of 6 358 clinical SP isolates from children with lower respiratory tract infection from January 2008 to December 2012 were col-lected and retrospectively analyzed. The antibiotic sensitivity was done by Kirby-Bauer method and E-test, and all results were in strict accordance with the rules of CLSI. Results The isolated SP strains were mainly from different departments of pediatrics. All clinical cases with SP infection mainly included pneumonia and bronchitis. The resistance rates of 6 358 SP strains to penicil-lin, cefotaxime, erythromycin, clindamycin, cotrimoxazole, tetracycline, chloramphenicol, levolfoxacin, vancomycin were 5.0%, 12.9%, 98.7%, 96.0%, 92.2%, 7.3%, 5.6%, 0.2%and 0.0%respectively, and the resistance rate to penicillin and cefotaxime was signiifcantly different in every years (all P<0.05). The resistance rates of the 318 penicillin-resistant SP strains to the above anti-biotics were 100.0%, 42.6%, 100.0%, 100.0%, 99.2%, 23.6%, 6.8%, 0.6%, 0.0%respectively, and the resistance rate to penicillin and cefotaxime was signiifcantly different (P=0.001). Conclusions The antibiotic resistance rates of SP strains isolated from children with lower respiratory tract infection were higher to erythromycin, clindamycin, cotrimoxazole and tetracycline, and an increasing rate in drug resistance to cefotaxime was observed in recent years. Appropriate antibiotics should be selected for the treatment of infection according to drug sensitivity.
