2.Characterization of Aminoacylase in the Mycelial Cell of Cunnighamella echinulata

The strain of Cunninghamella echinulata 9980 was first selected with high aminoacylase activity . In three submerged cultures, the aminoacylase activity in the mycelial cell was compared . A number of factors have effects on the resolution reaction. The results showed that, peptone culture gave the highest aminoacylase activity with 680U/g.The optium temperature,pH,and substrate concentration were 55℃, 7.0,and 0.2mol/L,respectively. The ions in the buffer lowered the activity,but the Co~(2+) in 10~(-4)~10~(-3 )mol/L was necessary for its activity.
3.Gene polymorphism of killer cell immunoglobulin-like receptors and HLA-Cw in patients with ankylosing spondylitis

Objective To investigate the alterations in killer cell immunoglobulin-like receptors (KIRs)2D and their specific HLA-Cw ligands in patients with ankylosing spondylitis(AS)and determine whether the changes were correlate to the pathogenesis of AS.Methods Polymerase chain reaction of sequence specific primerB(PCR-SSP)was employed for genotyping the presence or absence of five KIR2D genes(KIR2DL1,2DS1,KIR2DL2,2DL3,2DS2)as well as HLA-Cw01-08 alleles from genomic DNA in 105 individuals with AS,together with 51 individuals with osteoarthritis(OA)and 120 healthy controls.Then HLA-C10-08 was divided into two groups.HLA-Cwasn and HLA-Cwlys to calculate the frequency of KIRID genotype.HLA-Gu alleles and KIR/HLA-Cw genotypes.Results The frequencies of HLA-Cwlys genes were significandy higher in patients with AS(0.269 7)compared with those in OA controls(0.148 2)and healthy controls(0.138 8,P=0.024,P=0.001,respectively).The frequency of KIR2DS1/HLA-Cwlys combination Was also markedly higher in AS group(26.67%)than that in OA controls(11.76%)and healthy controls(13.33%,P=0.039,P=0.018,respectively).Condusion The data suggest that the HLA-Cwlys allele may be associated with genetic susceptibility to AS and moreover.in the existence of HLA-Cwlys.the individuals with KIR2DS1 gene are likely to be at increased risk of AS.
5.Study on incidence of phlebitis in patients with PICC through peripheral vein and its nursing effect

Objective To observe the incidence of phlebitis in patients with PICC through peripheral vein and its nursing effect, in order to aLleviate the patients' suffering. Methods The 60 volunteers were divided into three groups randomly, who were received PICC through basilie vein, vena mediana and cephalic vein, the incidence of of phlebitis and therapeutic effieaey were analyzed. Results 2 patients appeared phlebitis in 20 cases received PICC through basilic vein, 3 patients in 20 eases through eubital vein, and all cured;11 patients in 20 eases through cephalic vein, while 9 patients were cured and 2 patients were detubated in cephalic vein group. Conclusions It' s suggested that the prevalence of phlebitis with PICC through cephalic vein is high, with long treat time and low cure rate. It' s likely to reduce the phlebitis occurrence and patients relief from pain through eephalic vein.
6.Design and evaluation of an overall unloading knee brace

Objective To design an overall unloading knee brace, and analyze the relationship between the unloading force and the position of the brace as well as the thigh bracket-thigh pressure. Methods Based on biomechanical experimental platform, the mechanical and kinematic analysis on squatting down-standing upright motion was conducted to evaluate unloading capacity of the overall unloading knee brace. Results The unloading forces supplied by the brace ranged from 0 to 200 N. The forces were larger with the brace hinges installed on the front-top of the knee, and the unloading forces increased with the pressure between the thigh and thigh bracket increasing. Conclusions The proposed overall unloading knee brace can partly reduce knee force, and its unloading forces are related to the installation site of the brace as well as the elasticity of the belts.
7.Analysis of cardiovascular disease prevention indicators among residents with intra-urban migration in Central China

Objective:
To investigate cardiovascular disease (CVD) prevention status among residents with intra-urban migration in Central China, so as to provide insights into targeted prevention and control of CVD.
Methods:
Basic data of residents aged 35 to 75 years who participated in Early Screening and Comprehensive Intervention Project for CVD high-risk populations in Central China from September 2015 to August 2020 were collected. According to birth place, type of registered residence and current residence, residents were divided into four groups: local residents in old urban area, local residents in new urban area, other urban migrants and other rural migrants. The status of CVD primary and secondary prevention, were analysed by using a robust Poisson regression model.
Results:
A total of 76 513 residents were recruited, including 29 420 males (38.45%) and 47 093 females (61.55%), and had a mean age of (56.36±9.84) years. There were 45 087 (58.93%) local residents in old urban area, 23 868 (31.19%) local residents in new urban area, 5 668 (7.41%) other urban migrants and 1 890 (2.47%) other rural migrants. After adjusting for variables such as age, gender and educational level, the results of robust Poisson regression analysis showed that compared with local residents in old urban area, local residents in new urban area had lower compliance rates of non- or moderate-drinking (RR=0.987, 95%CI: 0.975-1.000) and healthy diet (RR=0.535, 95%CI: 0.365-0.782), lower proportion of using aspirin as primary prevention in CVD high-risk population (RR=0.616, 95%CI: 0.511-0.741), lower awareness (RR=0.873, 95%CI: 0.782-0.974) and control rates (RR=0.730, 95%CI: 0.627-0.849) of hypertension; other urban migrants had higher compliance rate of non-smoking (RR=1.045, 95%CI: 1.017-1.075); other rural migrants had lower proportion of using aspirin as primary prevention in CVD high-risk population (RR=0.826, 95%CI: 0.707-0.966).
Conclusion
The CVD primaryprevention among local residents in new urban area is relatively poor among four groups of residents in Central China, and key interventions are needed.
8.Mutation analysis of the HBV reverse transcriptase in nucleos(t)ide-treated patients with chronic HBV infection

Objective To characterize genotypic resistance within HBV RT region in chronic hepatitis B (CHB) patients with nucleos(t) ide analogue (NA) treatment.Methods Serum samples of 229 CHB patients with NA treatment were obtained.Full-length HBV RT sequences were amplified,sequenced and analyzed,on the following NA resistant (NAr) mutations belonging to different NAr pathways.Results Among 229 HBV isolates,14.41% (33/229) and 85.59% (196/229) were genotype B and C,respectively;and the patients with HBV genotype C may be more susceptible to develope resistant mutations than patients with HBV genotype B (x2 =2.95,P < 0.05).NAr mutations were detected in 63 CHB patients.Mutations were not found at rtI169,rtT184,rtA194 or rtS202.RtM204 mutations were detected at the highest frequency among 63 mutants (40/63,63.49%) and found to display 11 combination mutation patterns,in which rtM204I were associated with rtL80I/V and rtL180M,and rtM204V were associated with rtL180M,respectively.Conclusions There are complicated mutation patterns in the HBV RT region for chronic hepatitis B (CHB) patients with nucleos(t) ide analogue (NA) treatment.RtM204V/I mutation was the highest.
9.Diffuse panbronchiolitis in a child: case report and literature review.

OBJECTIVEDiffuse panbronchiolitis (DPB) is a chronic progressive disease of the lower respiratory tract, which is prevalent in Asian population. So far, many DPB cases have been found in adults in China. To our knowledge, no pediatric DPB case has ever been reported in China. We describe the first pediatric DPB case in Chinese literature and the second case in the English-language literature.
METHODThe clinical manifestations, characteristic imaging and histological features of this DPB case were summarized.
RESULTSThe patient was a 13-year old girl complained of chronic productive cough with wheezing. Chest auscultation revealed fine moist rales and wheezing over both lung fields. The chest X-ray showed small nodules and reticular opacities in left lower lobe. High-resolution thorax computerized tomography (HRCT) demonstrated bilateral diffuse small centrilobuler nodules and bronchial wall thickening or bronchiectasis in some parts of the lungs. Histopathological examination of transbronchial biopsy specimen revealed lymphocytes and foamy histocytes infiltrated in the walls of bronchi, respiratory bronchioles and adjacent alveoli. Lymphoid follicles were present around some bronchi. Sinus radiographs revealed sinusitis. Lung function studies showed obstruction and restriction. PaO2 was 65 mm Hg. The diagnosis of DPB was made according to the current diagnostic criteria. Low-dose erythromycin [5 - 10 mg/(kg.d)] was effective.
CONCLUSIONDPB could occur in children in China. The major diagnostic clues may include the following: (1) persistent cough, sputum, and dyspnea; (2) coexistent chronic sinusitis; (3) bilateral diffuse small nodular opacities on HRCT. Low-dose erythromycin was effective in treatment of the case with DPB.

10.Comparative study on rat in situ nasal absorption of geniposide of Xingnaojing nasal drop and Xingnaojing microemulsion.

Xingnaojing (XNJ) is an effective clinical drug used to treat acute stroke. Compared with injection administration, its nasal administration has better brain targeting. Therefore, through nasal administration, XNJ microemulsion could help solve the drug load of compound components of different polarities contained in large-dose and high-concentration traditional Chinese medicines, and reduce irritation to nasal mucosa In this study, the modified volume correction method and the improved rat in situ nasal perfusion model were adopted to compare the nasal absorption of geniposide contained in different XNJ preparations. The results showed that the constant absorption rate of geniposide (GE) in XNJ-D was (2.95 +/- 0.25) x 10(-3) min(-1), whereas the constant absorption rate of GE in XNJ-M was (2.16 +/- 0.21) x 10(-3) min(-1). This indicated that the rat nasal absorption of GE in different XNJ preparations complied with the first-order process and could be considered as passive absorption. GE in XNJ-D was absorbed faster than that in XNJ-M, which provided basis for the development of nasal preparations of XNJ.
