1.Stent Implantation for Teatment of Intracranial Wide-Necked Aneurysms: 12 Cases Report

ObjectiveTo investigate the effect of endovascular stent with or without coil graft on intracranial wide-necked aneurysms. Methods12 cases with intracranial wide-necked aneurysms, among whom, 10 cases were treated with stents and coils, 2 C4 aneurysms were treated with graft stent, were analyzed retrospectively. ResultsOf the 10 cases who were treated with stents and coils, 8 patients were completely occluded and other 2 patients were incompletely (>95%) occluded. 2 cases accepted graft stent were obliterated. Follow-up with angiography in 10 patients for 3~12 months revealed that all the aneurysms were completely occluded. 1 patient among them died. ConclusionEndovascular stent with or without coil graft is effective on intracranial wide-necked aneurysms.
2.Clinical trial of docetaxel in the treatment of advanced gastric cancer

Objective To evaluate the clinical efficacy and safety of do-cetaxel in the treatment of advanced gastric cancer .Methods Eighty-five patients with advanced gastric cancer were randomly divided into control group (n=42) and treatment group (n=43).Control group was given irinotecan hydrochloride 60 mg? m-2 , day 1 +60 mg? m-2 cisplatin, intravenous infusion, day 1.Treatment group was given 40 mg? m-2 docetaxel , intravenous infusion , day 1.Two groups were trea-ted for 4 courses with 7 d per course.The total effective rate , levels of carcinoembryonic antigen ( CEA ) and carbohydrate antigen 199 ( CA199 ) , incidence of adverse drug reactions were compared between two groups.Results After treatment, the total effective rate in treatment group was 93.02%(40/43), which was significantly higher than that in control group [69.05%(29/42), P <0.05] .After treatment, the serum levels of CEA were ( 37.61 ±5.87 ) , ( 59.73 ±6.97 )μg? L-1 and CA199 were ( 49.25 ±6.83 ) , ( 98.23 ±14.63 ) U? L-1 in the treatment and control group .And those indexes after treatment in treat-ment group were significantly lower than those in control group (P<0.05).The adverse drug reactions were based on gastrointestinal reactions and bone marrow suppression for two groups .Also, the inci-dence of adverse drug reactions was not statistically significant in the two groups ( 9.30% vs 16.67%, P >0.05 ) .Conclusion Docetaxel has a definitive clinical efficacy for the treatment of advanced gastric cancer , which was better than irinotecan hydrochloride combined with cisplatin , without increasing the incidence of adverse drug reactions .
3.Clinical trial of thymosin α 1 injection combined with entecavir capsules in the treatment of cirrhosis with decompensated liver function

Objective To observe the clinical efficacy and safety of thymosin α1 injection combined with entecavir capsules in the treatment of cirrhosis with decompensated liver function.Methods Eighty patients with hepatitis B virus (HBV) e antigen (HBeAg) positive patients with decompensated liver function were randomly divided into control group (n =40 cases) and treatment group (n =40 cases).Control group was received oral entecavir 0.5 mg qd.Treatment group was given subcutaneous injection of thymosin α1 1.6 mg biw,on the basis of control group.Two groups were treated for 24 weeks.The clinical efficacy,alanine aminotransferase (ALT),aspartate aminotransferase (AST),total bilirubin,negative rates of HBV-DNA and HBeAg,adverse drug reactions were compared in two groups.Results After treatment,the total rates of treatment and control groups were 92.50% (37/40 cases) and 75.00% (30/40 cases) with significant difference (P < 0.05).After treatment,the main indexes in treatment and control groups were compared,ALT were (51.39 ± 8.17),(64.12 ± 6.24) U·L-1;AST were (33.49 ± 5.14),(40.05 ± 5.28)U · L-1;total bilirubin were (17.59 ± 1.59),(16.32 ±1.32)mmol · L-1;negative rates of HBV-DNA were 77.50%,40.00%;negative rates of HBeAg were 70.00%,32.50%;conversion rates of HBeAg/anti-HBe were 35.00%,22.50%,there were statistically significant difference (P < 0.05).Adverse drug reactions in treatment group were nausea,vomiting and dizziness,which in control group were nausea,vomiting,dizziness and allergies.The incidence of adverse drug reactions were 7.50% and 17.50% in treatment and control groups,with significant difference (P <0.05).Conclusion Thymosin α1 injection combined with entecavir capsules has a definitive clinical efficacy and safety for the treatment of HBeAg positive cirrhosis with decompensated liver function.
4.Comparison of two kinds of cationic vectors-mediated gene delivery.

In order to study the important factors involved in cationic liposome-mediated gene transfer, Lipofectamine 2000 or DOTAP was evaluated using three types of cells (Hep-2, MCF-7 and SW-480) in vitro transfection efficiencies. Different properties of the two reagents were analyzed and compared by DNA arrearage assay and MTT assay. Both Lipofectamine 2000 and DOTAP had strong capability to combine with DNA; Lipofectamine 2000 can get higher transfection efficiency of the three cells by using GFP as report gene, meanwhile, DOTAP can also get higher transfection efficiency against Hep-2 cell. However, DOTAP showed lower transfection efficiency against MCF-7 and SW-480 cell. On the other hand, the cytotoxicity assay showed that over 85% cell viability of MCF-7 cell could be achieved both by Lipofectamine 2000 and DOTAP under the optimal transfection condition. Relatively speaking, Lipofectamine 2000 has very high transfection efficiency in a broad range of cell lines, but because of the special selectivity of cell type on liposome, DOTAP also has a broad application prospect.

5.Analysis on Nutritional Risk Screening and Influencing Factors of Hospitalized Patients in Central Urban Area

Rational nutritional support shall be based on nutritional screening and nutritional assessment.This study is aimed to explore nutritional risk screening and its influencing factors of hospitalized patients in central urban area.It is helpful for the early detection of problems in nutritional supports,nutrition management and the implementation of intervention measures,which will contribute a lot to improving the patient's poor clinical outcome.A total of three tertiary medical institutions were enrolled in this study.From October 2015 to June 2016,1202 hospitalized patients aged ≥18 years were enrolled in Nutrition Risk Screening 2002 (NRS2002) for nutritional risk screening,including 8 cases who refused to participate,5 cases of same-day surgery and 5 cases of coma.A single-factor chi-square test was performed on 312 patients with nutritional risk and 872 hospitalized patients without nutritional risk.Logistic regression analysis was performed with univariate analysis (P<0.05),to investigate the incidence of nutritional risk and influencing factors.The incidence of nutritional risk was 26.35% in the inpatients,25.90% in male and 26.84% in female,respectively.The single-factor analysis showed that the age ≥60,sleeping disorder,fasting,intraoperative bleeding,the surgery in recent month,digestive diseases,metabolic diseases and endocrine system diseases had significant effects on nutritional risk (P<0.05).Having considered the above-mentioned factors as independent variables and nutritional risk (Y=1,N=0)as dependent variable,logistic regression analysis revealed that the age ≥60,fasting,sleeping disorders,the surgery in recent month and digestive diseases are hazardous factors for nutritional risk.Nutritional risk exists in hospitalized patients in central urban areas.Nutritional risk screening should be conducted for inpatients.Nutritional intervention programs should be formulated in consideration of those influencing factors,which enable to reduce the nutritional risk and to promote the rehabilitation of inpatients.
6.The association between common genetic variation in the FTO gene and metabolic syndrome in Han Chinese.

BACKGROUNDGenome-wide association studies for type 2 diabetes mellitus (T2DM) identified FTO gene as a locus conferring increased risk for common obesity in many populations with European ancestry. However, the involvement of FTO gene in obesity or T2DM related metabolic traits has not been consistently established in Chinese populations. The objective of this study was to investigate the association of FTO genetic polymorphisms with metabolic syndrome (MetS) in Han Chinese.
METHODSWe tested 41 FTO single nucleotide polymorphisms (SNPs) for association between FTO and MetS-related traits. There were a total of 236 unrelated subjects (108 cases and 128 controls), grouped according to the International Diabetes Federation (IDF) criteria.
RESULTSOf the 41 SNPs examined, only SNP rs8047395 exhibited statistical significance (P = 0.026) under a recessive model, after Bonferroni adjustment for multiple testing (OR 1.64, 95%CI 1.11-2.42; P = 0.014). The common distributions of this polymorphism among Chinese--with a minor allele frequency (MAF) of 36% in the control group versus 48% in the MetS group--greatly improved our test power in a relatively small sample size for an association study. Previously identified obesity- (or T2DM-) associated FTO SNPs were less common in Han Chinese and were not associated with MetS in this study. No significant associations were found between our FTO SNPs and any endophenotypes of MetS.
CONCLUSIONSA more common risk-conferring variant of FTO for MetS was identified in Han Chinese. Our study substantiated that genetic variations in FTO locus are involved in the pathogenesis of MetS.

7.Development of a real-time PCR method for the detection of bacterial colonization in rat models of severe acute pancreatitis.

BACKGROUNDTechniques for the fast and accurate detection of bacterial infection are critical for early diagnosis, prevention and treatment of bacterial translocation in clinical severe acute pancreatitis (SAP). In this study, the availability of a real-time PCR method in detection of bacterial colonization in SAP rat models was investigated.
METHODSSamples of blood, mesenteric lymph nodes (MLN), pancreas and liver from 24 specific pathogen-free rats (8 in a control group, 16 in a SAP group) were detected for bacterial infection rates both by agar plate culture and a real-time PCR method, and the results were made contrast.
RESULTSBacterial infection rates of the blood, MLN, pancreas and liver in the SAP group and the control group by the two different methods were almost the same, which were 5/16, 12/16, 15/16, 12/16 in the SAP group compared with 0/8, 1/8, 0/8, 0/8 in the control group by agar plate culture, while 5/16, 10/16, 13/16, 12/16 and 0/8, 1/8, 0/8, 0/8 respectively by a real-time PCR method. Bacterial number was estimated by real-time PCR, which showed that in the same mass of tissues, the pancreas contained more bacteria than the other three kinds of organs in SAP rats (P < 0.01), that may be due to the edema, necrosis and hemorrhage existing in the pancreas, making it easier for bacteria to invade and breed.
CONCLUSIONFast and accurate detection of bacterial translocation in SAP rat models could be carried out by a real-time PCR procedure.

8.Direct modulation of firing activity by dopamine Dlike receptors in the globus pallidus of both normal and parkinsonian rats.

The globus pallidus occupies a critical position in the indirect pathway of the basal ganglia circuit, which regulates movement under both normal and pathological conditions. Previous studies have shown that the globus pallidus receives dopaminergic innervation from the axonal collaterals of nigrostriatal fibers. Both dopamine Dand Dlike receptors are expressed in the globus pallidus. The present study was aimed to investigate the direct in vivo electrophysiological effects of dopamine Dlike receptors in the globus pallidus of both normal and parkinsonian rats. Extracellular recordings of multi-barreled microelectrode were used in the present study. In normal rats, micro-pressure ejection of dopamine Dlike receptor agonist quinpirole induced different effects on the firing rate of globus pallidus neurons. In 24 out of the 61 pallidal neurons, quinpirole significantly increased the firing rate by (62.7 ± 11.2)%. In another 16 neurons, quinpirole decreased the spontaneous firing rate by (37.5 ± 2.9)%. Furthermore, co-application of dopamine Dlike receptor antagonist, sulpride, blocked quinpirole-induced modulation of the firing rate of pallidal neurons. On the 6-hydroxydopamine (6-OHDA) lesioned side of parkinsonian rats, quinpirole increased the firing rate in 25 out of the 47 pallidal neurons by (64.2 ± 10.1)%, while decreased the firing rate in 11 neurons by (51.9 ± 6.2)%. Our findings suggest that activation of pallidal dopamine Dlike receptors may bidirectionally modulate the spontaneous firing of globus pallidus neurons in both normal and parkinsonian rats.
