2.Clinical analysis of treatment outcome for 559 patients with endometrial cancer

Background and purpose:With the recognition of prognostic factors and application of surgical- pathological staging,the treatment strategy of endometrial cancer has greatly changed.In this study,we investigated the treatment strategy,survival rate and prognostic factors of endometrial cancer.Methods:Five hundred and fifty-nine patients of endometrial cancer underwent primary surgical treatment from January 1996 to December 2006 in Cancer Hospital,Fudan University.The clinicopathologic parameters and prognosis were analyzed retrospectively.Results: Patients were followed up with the median period 36 months,25 cases experienced local relapse,24 cases were found to have distant metastases.The 3-year overall survival(OS)was 88.7%,3-year and 5-year disease free survival(DFS) was 82.7% and 80.9%.The 3-year overall survival(OS)were 95.5% in stageⅠ,95.3% in stageⅡ,75.9% in stageⅢand 32.4% in stageⅣrespectively(P
3.Effects of different doses of dexmedetomidine on cerebral oxygen saturation and pulmonary shunt fraction in patients undergoing one-lung ventilation

Objective To investigate the effects of different doses of dexmedetomidine on cerebral oxygen saturation and pulmonary shunt fraction in patients undergoing one-lung ventilation (OLV). Methods Sixty ASAⅠ-Ⅱpatients, aged 46-71 years, with body mass index (BMI)18-24 kg/m2 and scheduled for thoracotomy were randomly divided into three groups (n=20 each):high dose dexmedetomidine group (group D1), low dose dexmedetomidine group (group D2) and control group (group C). Dexmedetomidine 1μg/kg was infused in group D1 after anesthesia induction, and then a rate of 0.5μg·kg-1·h-1 was continuously infused. Dexmedetomidine 0.5μg/kg was infused in group D2 after anesthesia induction, and then a rate of 0.3μg · kg-1 · h-1 was continuously infused. Group C was received the equal volume of normal saline. Anesthesia was main?tained with propofol-remifentanil and intermittent iv boluses of rocuronium. Arterial and jugular venous blood samples were collected before anesthesia induction (T0), at 15 min after two-lung ventilation (T1), at 5 min (T2) and 30 min (T3) of OLV for blood gas analysis. Value of Qs/Qt was calculated and SctO2 was recorded at the same time. Results Compared with group C and group D2, Qs/Qt was decreased at T2 in group D1 (P<0.05). Qs/Qt was lower at T3 in group D1 and D2 than that of group C, and which was lower in group D1 than that of group D2 (P<0.05). In group C and group D1 a significant de?crease in SctO2 was observed at T2 and T3 compared to that at T0 and T1 (P<0.05). SctO2 was significantly higher at T2 and T3 in group D2 than that in group C and group D1 (P<0.05). Conclusion Dexmedetomidine given during OLV undergoing thoracotomy can improve oxygenation, decrease pulmonary shunt fraction and reduce the occurrence of low SctO2.
6.Comparison of Different PBL Effecting on the Clinical Teaching of Pediatrics

Objective To compare and evaluate the effect of PBL in clinical teaching of Pediatrics.Methods Among students of Grade 2002 in our university,two types of PBL,pre-learning and case-discussion,were used in their clinical learning of Pediatrics. And then,their effects were evaluated and compared with those of traditional learning method.Results More than 60% of the students agreed with PBL methods,and they considered PBL favorable to practice scientific logical thinking of clinical affairs,to increase their capabilities of learning,oral expression,communication and cooperation.The teachers agreed with PBL methods too for the better learning effect resulting from PBL.Conclusion PBL fits the needs of medical learning reformation.To train new type of doctors in century 21st,it is necessary to use kinds of new learning methods,including PBL methods and standardized patient (SP)in clinical teaching.
7.Clinical observation on tuina plus foot bath with Chinese medicine for diabetic foot in early stage

Objective:To observe the clinical effect of tuina plus foot bath with Chinese medicine for patients with diabetic foot (DF) in early stage.Methods:A total of 70 patients with early-stage DF were randomly allocated by the random number table into two groups,with 35 cases in each group.Patients in the control group received conventional medication,while patients in the observation group received tuina plus foot bath with Chinese medicine on the basis of conventional medication.The clinical efficacy was compared after 2 courses of treatment.Results:After treatment,intra-group comparisons of ankle-brachial index (ABI) showed statistical significance in both groups (both P<0.05).The curative rate was 83.3% in the observation group,with the total effective rate of 96.7%,versus 29.4% and 76.5% in the control group,respectively,and the between-group comparisons showed statistical significance (both P<0.05),indicating a better effect in the observation group.Conclusion:Tuina plus foot bath with Chinese medicine has a good therapeutic effect for DF patients in early stage.
8. Molecular mechanism of Danggui Beimu Kushen Pills in treating prostatic diseases based on TCM-IP

Objective To analyze the material basis and molecular mechanisms of Danggui Beimu Kushen (DBK) Pills in treating prostatic diseases based on the method of integrated pharmacology. Method The platform of Integrative Pharmacology of Traditional Chinese Medicine (TCM-IP, www.tcmip.cn) was utilized to predict the main active ingredients and functional targets of DBK Pills in treating prostatic disease, key targets were screened for enrichment analysis of pathways, and the network of “herb-core component-key target-main pathway” was constructed, and the possible mechanisms of DBK Pills in treating prostatic diseases were explored. Results A total of 532 candidate key targets for the treatment of prostatic diseases by DBK Pills were predicted, and 1 840 terms of gene function and 194 signal pathways were analyzed by gene ontology (GO) and KEGG, respectively. The network analysis of “herb-core component-key target-main pathway” showed that 65 core components were predicted, including 29 ingredients from Angelica sinensis, 11 from Fritillaria thunbergii and 26 from Sophora flavescens. Those predicted components acted on the key targets of prostatic diseases, such as transcription factor binding, negative regulation of apoptosis, et al, through the estrogen, apoptosis, chemokines and other signal pathways, and thus played a role in the regulation of cell cycle, apoptosis and proliferation imbalance, which might be the molecular mechanisms of DBK Pills for the treatment of prostatic disease. Conclusion DBK Pills regulate the development of BPH, prostate cancer and other diseases through multiple pathways with multi-component interacting with multiple targets.

Objective:To express Tumstatin_(183-230)-TRAIL fusion protein and to observe its biological functions.Methods: SOE-ing PCR was employed to amplify the recombinant sequence of Tumstatin_(183-230)and TNF-related apoptosis-inducing ligand (TRAIL_(114-281)).An expression vector pMAL-Tu-T was constructed by inserting Tu-T sequence into pMAL-c_2;the vector was used to transfect E.coli BL21(DE3)and expression of MBP-Tu-T fusion protein was induced by IPTG.Amylose Resin columns were employed to purify the fusion protein.The biological functions of MBP-Tu-T protein was examined by inhibitory test of endothelial cell proliferation,standard tumor cell cytotoxic assay,in vitro tube formation inhibition,and electron microscopic observation(apoptosis).Results:The expression rate of MBP-Tu-T fusion protein in E.coli was about 20%. Purified recombinant protein obviously inhibited endothelial cell proliferation(IC_(50)12.5?g/ml),induced apoptosis of pancreatic cancer cells,and inhibited tube formation.Conclusion:Constructed MBP-Tu-T fusion protein is bifunctional,which lays a solid foundation for further investigation of antitumor effect of Tumstatin_(183-230)-TRAIL in vivo.
10.Current Situation of TCM Human Resources Distribution and Equity Research in China

Objective To discuss existing problems in talent team construction; To provide evidence for the government to enact proper strategies based on the analysis of the current situation of TCM practitioners (assistants) distribution in China from quantity, quality and distribution equity. Methods The research is studied through conducting a descriptive analysis of TCM practitioners (assistants) from quantity, education, job titles, ages, and other basic conditions and changes. Gini coefficient was used to evaluate the distribution of TCM practitioners (assistants). Results The quantity of TCM practitioners (assistants) increased by 53.7% from 2010 to 2015. Concerning the quality of personnel, the ratio of the personnel with bachelors degree or above increased from 38.6% to 44.2%, and a slight increase was observed with the personnel with senior title. In terms of age structure, the proportion of the age of TCM practitioners (assistants) under 25 years old, 35 to 59 years old decreased, and the proportion of the age of TCM practitioners (assistants) above 60 years old, 25 to 34 years old increased. From the perspective of equity, there is a difference between the provincial-level administration of human resources distribution. The Gini coefficient as per population distribution was around 0.1, which indicated good equity. From the geographical area configuration, the Gini coefficient was approximately 0.58, which showed high level of disparity. Conclusion There are problems of insufficient quantity, low quality and distribution disparity among regions in TCM practitioners (assistant) in China.
