1.Features of Event-Related Potentials P300 in Children with Narcolepsy

Objective To explore the clinical value of depression and event related potentials P300 in children with narcolepsy(NRL).Methods Event related potentials P300 test and self-rating depressive scale(SDS)test were performed in 39 children with NRL and 38 normal healthy children were selected as controls.Denmark Keypoint 4 path myoelectricity induced potential equipment and short sound stimulus were applied to determine event related potentials P300 test.Participants were required to hit the key for target stimulus,and reaction time and percentage of hits were recorded automatically.Long P300 and(or) low P3 amplitude of wave exceeded control group 2.5 s were abnormal.Zung SDS was adopted to evaluate somatization disorder,psychomotor disturbance,depression obstruction factor value.SPSS 12.0 software and t test were done to analyze the data.Results The abnormal rate of P300 in children with NRL of children group was 82.1%(32/39 cases),the latency of N2(260.7?22.1)ms and P3(353.1?25.9) ms of event related potentials of children with NRL group were longer than those in control group(241.0?23.2)ms,(341.3?26.1)ms,and there was significant difference between two groups(t=2.77,4.82 Pa
2.Fiberbronchoscopy for Right Middle Lobe Syndrome: A Report of 42 Cases

ive] To assess the diagnostic value of fiberbronchoscopy for right middle lobe syndrome (RMLS) and to explore the etiology of RMLS. [Methods] Fiberbronchoscopy was carried out in 42 casesof RMLS diagnosed by chest X-ray or CT. [Results] Patients with RMLS due to non-specific inflamma tion accounted for 57.1 % , lung cancer 26.2%and pulmonary tuberculosis 11.9%. Congestion, edema, stenosis, neoplasm, purulent secretion, and mucosa thickening were the main manifestations under fiber-bronchoscope. The diagnostic accordance rate of fiberbronchoscopy for lung cancer, non-specific inflam mation and pulmonary tuberculosis was 90.9% , 87.5% and 60% respectively. No significant differences of the incidences of non-specific inflammation and lung cancer were found in the teenage, the adult and the aged. [Conclusion] Fiberbronchoscopy is an important method of the diagnosis of RMLS. Lung can cer, non-specific inflammation and pulmonary tuberculosis are the common pathogenic factors of RMLS. Male cases over 40 should take fiberbronchoscopy to exclude the possibility of lung cancer.
3.Nursing management in the treatment of large number of infants with urinary calculi

To explore the measures of nursing management in the treatment of large number of infants with urinary calculi. The nursing management measures included launching the preparedness and response project for sudden public health events, formulating scientific and standardized nursing management system,optimizing work flow,strengthening nurse training,focusing on the details in nursing management,implementing disinfection and isolation system seriously,and paying close attention to health education for the parents of minority infants. Scientific nursing management can ensure the treatment effectiveness and nursing safety for the infants with urinary calculi.
4.A cross-sectional study on prevalence of scoliosis among primary and middle school students in Jiaxing City

Objective:
To investigate the prevalence of scoliosis among primary and middle school students in Jiaxing City, Zhejiang Province, so as to provide insights into scoliosis control among children and adolescents.
Methods:
Grade 4 to 6 primary school students and grade 1 to 3 junior high school students were recruited using a stratified cluster sampling method in Jiaxing City in 2019. Participants' demographic characteristics, dietary habits and nutritional status, physical activity, learning environments, reading and writing habits were collected using questionnaires. Scoliosis was screened through general examinations, forward bend test and scoliometer, and scoliosis was diagnosed with whole-spine X-ray scans in an erect position. The prevalence of scoliosis was descriptively analyzed among primary and middle school students.
Results:
A total of 8 026 primary and middle school students were included, 7 304 valid questionnaires were recovered, with an effective recovery rate of 91.00%. The respondents included 3 667 primary school students (50.21%) and 3 637 junior high school students (49.79%), and included 3 776 boys (51.70%) and 3 528 girls (48.30%). There were 659 participants with initial screening positive for scoliosis (9.02%), and the percentages of positive initial screening of thoracic, thoracolumbar and lumbar scoliosis were 2.93%, 4.56% and 4.56%, respectively. A higher percentage of positive initial screening of scoliosis was diagnosed among participants living in Pinghu City (10.45%), junior high school students (11.74%), girls (11.96%), students with a medical history of anemia (22.44%), students with less than 3 days of moderate-intensity physical activity in the past week (9.46%), students with less than 3 days of walking duration of over 10 minutes in the past week (10.18%), students with daily sitting duration of 5 hours and more in the past week (10.74%), students with their class seats exchanged every semester or month (10.28%), students with daily reading and writing duration of 3 hours and more after school (10.93%) and students with less than 10 cm distance from the chest at reading or writing to the edge of the table (9.67%) (all P<0.05). A total of 218 students received whole-spine X-ray scans in an erect position, 132 participants were definitively diagnosed as scoliosis (60.55%), and the estimated prevalence of scoliosis was 5.46%.
Conclusion
The percentage of positive initial screening of scoliosis was 9.02% among primary and middle school students in Jiaxing City. Gender, stage of learning, nutritional status, exercise frequency and habits of reading and writing may be factors affecting the development of scoliosis.
5.The analysis of risk factor and the establishment of pre-operation warming score system for infection after intracavity lithotripsy in upper urinary tract calculi

Objective To discuss the risk factor of infection after intracavity lithotripsy in upper urinary tract calculi,and establish a pre-operation warming score system.Methods From Jan.2013 to May 2016,412 upper urinary calculi patients who underwent intracavity lithotripsy were analyzed to evaluate the associated risk factors before operation and infection after operationg by non-conditional logistic regression analysis.The pre-operation warming score system was established by giving those risk factor 1-4 point based on OR value.The best threshold was then determined by ROC curve.Results Diabetes mellitus,infection history,renal calculus and uretero-pelvic junction calculus,stone burden,the degree of hydronephrosis and the gender of female were high-risk factors contributed to infection after intracavity lithotripsy,which were given 3,3,3,2,2,2point respectively based on their OR value(8.660,7.046,3.723,2.675,2.256,1.891),and the patients who got high socre were more likely to suffered infection.The sensitivity and specificity of the wanning score system for infection after intracavity lithotripsy were 74.3% and 84.0% respectively when its truncation point was 7.5 point(total score was 15 piont).Conclusions Patients who got more than 7.5 point according to the wanning score system were high risk groups of infection after intracavity lithotripsy.
6.Clinical Study on Total Anomalous Pulmonary Venous Return.

Clinical study was performed on 42 patients diagnosed as TARVR at the Departmet of pediatrics, Seoul National University Hospital, from January, 1966 to June, 1982. The results are as follows, 1) Of the 25 cases with isolated TAPVR, age distribution was from 10 days to 10 years. Seventeen cases were male and eight cases were female. Of the 24 cases with complicated TAPVR, age distribution was from 2 days to 22 years. Eighteen cases were male and six cases were female. 2) The type of TAPVR was supracardiac in 29 cases (59.1%), cardiac in 10 cases (24.4%), infradiaphragmatic in 3 cases (6.1%) and mixed in 5 cases (10.2%). 3) Birth weights of patients were within normal limits except 1 case. 4) The most freuent complaints at diagnosis were tachypnea and cyanosis. 5) PDA was the most frequently associated intracariac anomaly (54.1%) which was followed by pulmonary stenosis (16.6%), single atrium (16.6%) and single ventricle (16.6%). 6) Of the 25 cases with isolated TAPVR, chest roentgenographic findings were cardiomegaly in 22 cases (88%) and increased pulmonary vascularity in 23 cases (92%). 7) Of the 25 cases with isolated TAPVR, the ECG showed right axis deviation in 22 cases (88%), right atrial enlargement in 13 cases(52%), right ventricular hypertrophy in 24 cases (96%) and combined ventricular hypertrophy in 1 cases (4%). 8) Of the 23 cases with isolated TAPVR, echocardiographic finding showed increased dimension of the right ventricle in all cases. Of the 39 cases with TAPVR, the common venous chamber was visualized posterior to the left atrium in 14 cases(35.8%). 9) Cardiac catheterization was performed in 24 cases with isolated TAPVR. Pulmonary hypertension was observed in 19 cases (79.1%). 10) Fourteen patients with isolated TAPVR were operated. Three patients died.

8.Effects of combined therapy of human leucocyte antigen haploidentical related bone marrow transplantation and Chinese medicine in treating leukaemia.

OBJECTIVETo explore the feasibility of human leucocyte antigen (HLA) haploidentical related T-cell undepleted allogeneic bone marrow transplantation (Allo-BMT) combined with Chinese medicine for the treatment of leukaemia.
METHODSFour patients with chronic myeloblastic leukemia (CML) and 4 with acute myeloid leukemia (AML) received allo-BMT with graft from 1 - 3 HLA-mismatched related donors. All patients were pre-treated with standardized conditioning regimen consisting of high dose Ara-c, cyclophosphamide (CY) and total body irradiation (TBI) or busulfan. Donors were given G-CSF 250 microg/d for 7 days prior to marrow harvest. To prevent GVHD, besides application of CSA and MTX, ATG 2.5 mg/kg was given everyday for 4 days before transplantation, and MMF 1.0 g per day starting from the 7th day after transplantation. Chinese medicine for replenishing qi and yin and strengthening Pi and Wei was administrated orally after transplantation.
RESULTSSuccessful haematopoietic reconstruction was seen in all patients. The median days for reaching of granulocyte >0.5 X 10(9)/L and platelet > 20 x 10(9)/L were 12 (range 10-14) and 20 (range 18-25) days respectively. GVHD of skin in various grade was seen in 7 patients, among whom only one advanced to grade IV; besides, 2 accompanied with GVHD of gut, one with hemorrhagic cystitis, one died for concurrent infection on the 81st day, and one left hospital of his own accord on the 46th day. The median follow-up duration was 18 (range 2-32) months, during this period six patients were alive in a disease-free situation.
CONCLUSIONCombined therapy of HLA haploidentical related T-cell undepleted Allo-BMT and Chinese medicine plus immunosuppressants with pre-harvest G-CSF application in doner could effectively reduce the incidence of acute severe GVHD and raise the disease-free survival rate in treating leukaemia.
