1.An analysis of the questionnaire survey about the first year residency training in internal medicine in Peking Union Medical College Hospital

Objective To evaluate the effects of medical residency training program in postgraduate-year-one (PGY-1) residents at Peking Union Medical College (PUMC) Hospital.Methods PGY-I medical residents at PUMC Hospital were surveyed by a self-administered questionnaire after they completed their first year residency.Results Forty-nine residents who completed their first year residency training participated in the survey with a 100% response rate.Before training,only a few had bedside (n =18) and on-call experiences (n =10).At the end of the one-year-training,all the residents (100%) considered themselves being improved to certain degrees,especially in clinical practice capability,and passed all the examinations.However,53.1% and 8.2% of all the residents agreed that they need to improve their abilities in teaching (n =26) and doctor-patient communication (n =4),respectively.All residents hoped to get further improved by taking part in various forms of teaching activities,including experience summary/sharing and basic skills training.During the first year,95.9% of them participated in all teaching activities in medicine actively.Conclusions The tradition and current models of medical residency training program at PUMC Hospital had a significant impact on professional development of the 1st year resident physicians.It is critical to focus on basic skill training and multi-level teaching to improve residents' clinical competency.

4.The effect of epicardial fat volume in patients with metabolic syndrome on insulin resistance and coronary heart disease

Objective Epicardial fat volume ( EFV) was a risk factor for coronary heart disease ( CHD) , but there is little research regarding the relationship of EFV with insulin resistance ( IR) and CHD in patients with metabolic syndrome ( MS) .The aim of the article was to explore the effect of EFV in patients with MS on CHD and IR . Methods Patients with MS treated by percutane-ous coronary angiography ( CAG) in our hospital from February 2013 to August 2013 were recruited in this study .The data of height , weight, waist circumference(WC) and hip circumference(HP) were recorded.EFV were measured by MSCT.Fasting blood samples were collected for blood biochemical test . Results EFV in patients with MS was in positive relation with IR index (IRI)(r=0.335, P<0.001) and CHD (r=0.321, P<0.05), and the correlation still remained when the influences of WC and body mass index (BMI) were excluded.Logistic regression analysis showed that EFV was an independent risk factor for CHD (P<0.05), while linear regression analysis indicated EFV , BMI and LDL-C were the risk factors for IRI .ROC curves analysis proved EFV and BMI had diag-nosis value for IRI, and the areas under curve of EFV were 0.755 and 0.679 (P<0.05) respectively. Conclusion EFV is an in-dependent risk factor for CHD and IRI in patients with MS , and EFV has an advantage over BMI in the diagnosis value of IRI .

7.Mycological profile of cryptococcal meningitis in patients with non-acquired immune deficiency syndrome during treatment and follow-up

Objective To examine mycological profile of eryptococcal meningitis in patients with non-acquired immune deficiency syndrome (AIDS) during treatment and follow-up so that to support clinical therapy. Methods Data of 28 cuhure-confirmed cryptoeoccal meningitis patients with non-AIDS were retrospectively analyzed. Fungat smear, count, culture and latex agglutination test of cerebrospinal fluid (CSF) were done during treatment and follow-up. Initial treatment included intravenous amphotericin B plus oral flucytosine or f;uconazole for at least 6 weeks, and consolidation treatment included oral fluconazole and (or) itraeonazole for at least 2 months. All 28 patients were cured. The data were analyzed by rank-sum test. Results The positive rate of CSF fungal smear was 92.9% before treatment and gradually decreased, and the fungal count was significantly reduced over time after treatment. While fungal smears of some patients were still positive after initial treatment. Fungal growth time in culture was gradually extended, and fungal culture turned to be negative in all patients after 2 weeks of treatment. The positive rate of latex agglutination test of CSF was 100%. Cryptococcal antigen titer decreased steadily after treatment, which was not correlated with the decrease of fungal count. Conclusion Mycological tests of patients with eryptococcal meningitis should be interpreted comprehensively during treatment, and result of each test should be specifically analyzed.
8.Combination of laser-point cloud and reverse engineering to rapidly establish a three-dimensional soft tissue model in cosmetic surgery

BACKGROUND:The design and communication of cosmetic operation is more refined than that of traumatic operation. Thus, more accurate data should be acquired and more precise three-dimensional models should be reconstructed.
OBJECTIVE:To investigate the prospect of 3D laser scanner and matched graphics-processing software which acquire and analyze facial morphological data in the modern cosmetic surgery.
METHODS: We obtained the point clouds of the facial surface in two cases of facial soft tissue defects or dissatisfied facial appearance using a hand-held laser scanner to reconstruct a visualized facial model, which was used for measurement of facial profile, simulation of cosmetic operation and evaluation of the therapeutic effects.
RESULTS AND CONCLUSION:We simulated and designed upon the NURBS model before the operation primarily. Then the three-dimensional comparison was done between the simulate model and the postoperative model. Deviation analysis showed that the ratio of variation degree of the two models within 1 mm was more than 80%. It enables the rapid three-dimensional reconstruction of the facial surface by the combination of reverse engineering and laser scanning, thus providing accurate three-dimensional data for cosmetic design and quantitative evaluation of therapeutic effects in cosmetic surgery.
9.The Influence of TFPI-2 on the Growth and the Invasion of K562 Cells in vitro

Objective To evaluate the influence of tissue factor pathway inhibitor 2(TFPI-2)gene on the proliferation and invasion of K562.Methods The expression vector pcDNA3.0/TFPI-2 was transfected into human leukemia line K562 cells(K562-T)by using liposome,then the mRNA and protein TFPI-2 were detected by real-time RT-PCR and western blot separately.The growth curve and the colony-forming unit assay were used to measure the ability of cells growth and transwell chamber model was employed to test the ability of cell invasion in vitro.Results Expression of mRNA and protein of TFPI-2 was detected in transfected cells.The growth rate and self-replication ability of K562-T cells were lower than those of the two control groups obviously.The number of K562-T cells to traverse a Matrigel-coated membrane was dramatically decreased compared with that of non-expressing cells.Conclusion The gene of TFPI-2 can inhibit the growth,proliferation and invasion of the K562 cells.
10.Effects of total flavonoids in Gingko Biloba on glucose and lipid metabolism and liver function in rats with insulin resistance

Objective To investigate the effects of total flavonoids in Gingko Biloba on glucose and lipid metabolism and liver function in rats with insulin resistance. Methods Forty SD rats were randomly divided into normal control group,model group,total flavonoids in Gingko Biloba group and rosiglitazone group(positive drug control group)(n=10).Models of insulin resistance were established by high glucose and high fat diet in model group,total flavonoids in Gingko Biloba group and rosiglitazone group.After treatment for 12 weeks,serum glucose,serum lipids,and parameters of insulin resistance,liver function and anti-oxidation capability were detected in each group,and histologic observations of liver tissues were conducted with adipose staining.
