1.Study on the correlation between serum homocysteine levels with pediatric idiopathic epilepsy

ObjectiveTo study the clinical significance of determination on homocysteine in serum of pediatric idiopathic epilepsy. MethodsThe homocysteine level in serum of 81 patients with pediatric idiopathic epilepsy was determined,and the value of normal children was tested and compared as control. ResultsThe concentration of Hey in research group was from 7.6mol/L to 22.0μmol/L,the mean value was(15.91 ±2.38)μmol/L,The concentration of Hcy in control group was from 3μmol/L to 9μmol/L,and the mean value was(6.87 ± 1.75)μmol/L.which showed a significant difference(P<0.05).By comparison,the high concentration(>15 μmol/L)of Hcy in control group was only 6 cases,while it was 73 cases in research group,which also showed a significant difference (P<0.05). ConclusionHcy level was apparently relevant with occurring rate of pediatric idiopathic epilepsy.In the diagnosed clinical treatment of idiopathic epilepsy,the closely monitor of serum level of homocysteine,and timely promoting Hcy metabolism.should be carried out.
2.Discussion on the relationship between platelet-associated antibody and effect of platelet transfusion

Objective To investigate effect of platelet-associated antibody to the platelet transfusion.Methods Platelet-associated antibody in 120 patients who needed repeated platelet transfusion for long-time was determined by enzyme-linked immunosorbent assay (ELISA),and platelet count was assayed before and after blood transfusion,and the effect of platelet transfusion was investigated by corrected platelet count.Crossmatch approcah was used to screen compatible platelet for patients with platelet relative antibody positive.Results The ratio of positive platelet-aasociated antibody in patients who needed repeated platelet transfusion was 65.0%,and the ratio of platelet transfusion resistance was 61.7%.The ratios of platelet transfusion resistance were 84.6% and 19.0% respectively in patients with positive platelet-associated antibody and in patients with negative platelet-associated antibody,which were different in statistic in both groups( x2 =,46.914,P <0.0l ).The effective ratio of platelet transfusion in patients with positive platelet-associated antibody after crossmatch approach was 85.9%.Conclusion Positive platelet-associated antibody was related to platelet transfusion resistance with high risk,and crossmatch approach could increase the effect of platelet transfusion.
3.The curative effect of couple therapy for patients with postpartum depression

Objective To investigate curative effect thymoleptics with couple therapy for the patients with postpartum depression (PPD) Methods 27 patients first time diagnosed with PPD consistent with DSM-Ⅳwere randomly divided into study group and control group.The study group received regular thymoleptics with couple therapy,a control group were administered solo thymoleptics.Hamilton Depression Rating Scale(HAMD-17) and Edinburgh Postnatal Depression Scale (EPDS-10) and OLSON marriage quality questionnaire assessed the efficacy of each group before treatment and after 12 weeks treatment.Results (1) According to the total effective rate,there was no significantly differences in two groups after 12 weeks treatment from evaluation of HAMD-17>(P0.05),but complete remission of study group was higher than control group (78.6% ,46.2% ;P<0.05).(2) After 12 weeks treatment there were significantly differences in mean scores of HAMD-17 and EPDS-10 between two groups (P<0.05).The most decreased scores of HAMD-17 and EPDS-10 was found in the study group,and mean scores of each group was lower than pretherapy(P<0.05).(3) There was no significant difference on the scores of eight factors in OLSON marriage quality questionnaire between the two groups before treatment (P>0.05).After 12 weeks treatment the study group had higher score than control in seven factors in addition to sexual life factor (P<0.05).Conclusion Thymoleptics with couple therapy can improve the marriage quality and increase curative effects outstrip solo thymoleptics for the patients with postpartum depression.
4.Inhibition of visceral hypersensitivity in irritable bowel syndrome model rat by intrathecal administration of corticotrophin releasing factor

Objective To investigate whether intrathecal administration of corticotrophin releasing factor(CRF) has an efficacy on visceral hypersensitivity in irritable bowel syndrome model rat.Methods Thirty rats were randomly divided into three groups(n=10 ).After establishment of irritable bowel syndrome rat model were intrathecal injected with CRF or preemptive peritoneal injected with CP-154526,which is inhibitor of CRF-1 receptor,and the control group to give saline.After experiment all rat with the method of rectal balloon distention,the perception thresholds and the number of abdominal withdrawal reflexes (AWR) of different balloon volume were observed.Results The perception threshold of intrathecal administration of CRF group was(0.62±0.10)ml and higher than other two groups [(0.52±0.09)ml,(0.56±0.08)ml;F=3.25,P<0.05].At the same time,the number of AWR to the lower balloon content (1.0 ml) was(9.10±1.97)in intrathecal administration of CRF group slightly lower than other two groups[(14.4±1.71),(15.6±2.32);F=29.4,P<0.01],but no difference was found to the higher balloon content (1.5 ml and 2.0 ml).Besides,the areas and OD value of c-fos positive neurons in cornu posterius medullae spinalis in group with intrathecal administration of CRF were significantly lower than other two groups (P<0.05).Conclusion Intrathecal administration of CRF can degrade the visceral hyperensitivity in irritable bowel syndrome model rat,and lower expression of c-fos in CNS is probably involved in the process.
5.The effect of different transfusion measures on the clinical outcome in preterm infants

Objective To study the effect of different transfusion measures on the clinical outcome in preterm infants.Methods 100 cases of blood transfusion in preterm infants were chosen.They were divided into restrictive transfusion group and non-restrictive transfusion group.The clinical outcomes of infants were recorded.Results The Hct of children with restrictive transfusion group pre-transfusion was(27.97±4.86) %,which was significantly lower than pre-transfusion Hct of non-restrictive transfusion group(34.51±6.34) %.The difference was statistically significant(t =3.78,P < 0.05).The differences of each transfusion value,each blood transfusion and transfusion times for the two groups of infants were not statistically significant(t =0.59,0.37,0.68,all P > 0.05).The invasive ventilator of the non-restrictive transfusion group was(5.73±4.95)d,significantly less than the restrictive transfusion group(7.61±5.72) d.The difference was statistically significant(t =2.36,P < 0.05).The differences for the rest of the clinical outcomes for the two groups of infants were not statistically significant(P > 0.05).Conclusion The reducing of non-restrictive transfusion invasive ventilator time may be more conducive to clinical recovery in preterm infants.The clinical should not blindly adopt conservative restrictive transfusion countermeasures.
6.RP-HPLC gradient elution analysis for the determination of pantoprazole soldium and its related substances

Aim: To establish a method for the determination of pantoprazole sodium and its related substances. Methods: A column packed with octadecylsiance bonded silica gel (250 mm × 4. 6 mm, 5 pjn) was used. The 0. 01 mol/L monopotassium phosphate solution( adjusted with phosphoric acid to pH 7. 0) -acetonitrile were adopted as the mobile phase, a gradient elution was programmed as follows: 0→30 min(90:10-60:40), 30→45 min(60: 40→15: 85); the detection wavelength was 289 nm; the column temperature was 40 ℃; the flow rate was 1. 0 mL/min. Results: Pantoprazole sodium, the intermediates and its related substances could be well separated. A good linear relationship was obtained over the range of 6. 96-48. 72 μg/mL( r =0. 999 9). The limit detection and quantisation of pantoprazole sodium were 8.51 ng and 17.0 ng, respectively. Conclusion: This method can be applied to control the related substances of pantoprazole sodium and determine pantoprazole sodi-um.
7.Histological evaluation of the rhesus monkeys' craniofacial sutures after class Ⅲ functional orthopedics

Objective:To detect the influence of the early Class Ⅲ orthopedic treatment on the development of the maxillary.Methods:Four adolescence rhesus monkeys were fitted with casting Twin-block magnetic appliance(TMA),two were put to death after 1.5 months and another two after 3 months,zygomaticofrontal sutures were harvested,and performed histological examination using optical and electron microscope.Another two monkeys without appliance were served as control.Results:The maxillary sutures were rebuilt,after Class Ⅲ functional orthopedic,and the zygomaticomaxillary suture,zygomaticotemporal suture and transverse palatine suture were especially active.Electron microscopy results showed that osteoblasts and fibroblasts were active,and fibroblasts showed more active effects.Conclusion:During the treatment of the Class III functional orthopedics,maxillary sutures take part in the rebuilding actively,and there are lots of new bone formation along the sutures.
8.Effect of Multiple Interventions on Therapy Efficacy to the Patients with Epilepsy

Objective To observe the effect of multiple interventions on secondary epilepsy patients.Methods 114 secondary epilepsy patients were randomly divided into the intervention group and control group with 57 cases in each group.The cases in the intervention group received multiple interventions including heath education,psychological treatment,safety nursing,overdose administration,early rehabilitation,discharge guide and so on besides usual nursing.While,the cases in the control group received usual nursing.The follow-up was performed for 6 months and the therapeutic efficacy of two groups was compared.Results After multiple interventions,the seizure frequency and trauma incidence rate decreased,compliance and quality of life increased in patients of the intervention group,and there was a significant difference compared with the control group(P<0.05).Conclusion Multiple interventions can decrease seizure frequency and increase compliance of treatment and quality of life of secondary epilepsy patients.

10.Primary progressive aphasia: a case study

BACKGROUND: Primary progressive aphasia(PPA) is a degenerative disease of nervous system, which is very rare in clinics. Only 3 cases have been reported in our country. There is very little clinical information regarding the characteristics of PPA in linguistics and imageology.OBJECTIVE: To report the language disorder and the characteristics of imageology of one PPA patient for the clarification of the clinical features of PPA.DESIGN: A case analysis.SETTING: Department of Neurology, Beijing Tiantan Hospital; Faculty of Psychology, Beijing Normal University.PATICIPANT: Male, 56 years old, senior high school graduate, businessman before the onset of the disease. The case visited the department of Neurology,Tiantan Hospital, Beijing due to the complaint of three years of progressive decreasing in language skills, which was then diagnosed with PPA.METH ODS: Spoken language fluency evaluation in Chinese Aphasia Examination Set of the First Affiliated Hospital of Beijing Medical University was used to evaluate this patient, the type of Aphasia was judged by Western Aphasia Examination Set, and the severity gradation was classified by the Boston Diagnostic Aphasia Examination(BDAE) . Memory, intelligence screening and imageology examination were performed as well.MAIN OUTCOME MEASURES: The fluency of spoken language, the type of aphasia, and the severity of aphasia of the patient were judged, and whether the patient suffered from memory and intelligence disorder were observed as well as the features of imageology.RESULTS: The patient had fluent spoken language, which was evaluated as sensory aphasia(SA) with the aphasia severity of level 4, and had no memory or intelligence disorder. MRI showed atrophy in left frontal lobe and temporal lobe, which was more significant in left side. MRI also showed that left temporal and frontal lobes had low metabolism and low perfusion.CONCLUSION: PPA is a disease with language disorder as its dominant clinical manifestation, which no other cognitive disorder at its initial stage,and with its main pathological changes in left temporal and frontal lobes.
