2.The effect of early enteral feeding on intestinal mucosal energy systhesis in burned rats

Objectives:To investigate the effect of early enteral feeding on intestinal mucous energy systhesis in burned rats. Mothods:Wistar rats with 30% total body surface area Ⅲ degree burns were randomly divided into 3 groups:early feeding(EF) group,delayed feeding(DF) group,control group.The ATP,ADP,AMP of intestinal mucosa were detected by HPLC and energy charge were calculated according to formula. Results:The contents of ATP,ADP and energy charge in both experimental groups were all fluctuated at much lower level than those in control group,and the above indices exhibited significant decrease in DF group as compared with EF group at all time points except post burned 6 h.On the contrary,the contents of AMP increased more significantly in DF group than in EF group. Conclusions:Early feeding could increase the energy storage and alleviate the catabolism of energy rich phosphates of intestinal mucosa.
3.Research progress on TRAIL and its related apoptosis regulating mechanism in cholangiocarcinoma

Cholangiocarcinoma (CCA) is a common cancer in hepatobiliary system .Because CCA is in sensitive to conventional chemotherapy and radiotherapy ,the current treatment is mainly depending on the surgical operation when diagnosed early .Tumor necrosis factor related apoptosis inducing ligand ( TRAIL) plays a key role in the process of controlling cell proliferation by binding with its receptor and therefore mediated cell apoptosis . Although the characteristic of selectively killing tumor cells by TRAIL makes it become the new direction of clini -cal treatment of cancer ,the condition that CCA cells are resistant to TRAIL mediated apoptosis restricts its scope of application.Nowadays many studies demonstrate that multiple signaling pathways can regulate the sensitive of CCA cells to TRAIL mediated apoptosis .In this review,we summarize the expression and effects of TRAIL and its receptor in CCA , and conclude the mechanism of multiple pathways restoring the sensitivity of CCA cells to TRAIL mediated apoptosis .

6.Thyroid functional changes of normal human fetus and newborns

Objective To study the changes of the thyroid hormones leval of human fetus and newborans Methods More than 71 cases of medically indicated cordocentesis have been done in 16 36 gestational weeks in our hospital during last three years. Among them, 71 fetus who were free of diseases and their maternal thyroid function were normal were included into the study group. The blood samples were sent to analysis of thyroxine (T 4), triiodothyroxine (T 3), free thyroxine (FT 4), free triiodothyroxine (FT 3) and thyrotropin (TSH). 140 umbilical cord blood samples taken at the time of term delivery were sent to analysis of FT 4,FT 3 and TSH as a control. Normal range of different gestational weeks was calculated. Statistical analysis was done for the changes of all these thyroid hormones before 28 weeks and after Results All the thyroid hormones can be detected in 16 weeks of pregnancy,FT 4 already reaches the top level of adults (5.8?2.6) pmol/L and will continually increase with the increase of gestational age. There was a parallel increment of all the fetal thyroid hormone concentrations with the gestational age. The concentrations of T 4,T 3 and FT 4 have a rapidly increase after 28 weeks and have a statistically significant difference from (2.8 ?1.8) nmol/L, (37.2?27.2) nmol/L and (10.6?3.1) pmol/L,respectively to (5.8?2.6) nmol/L, (55.9?33.3) nmol/L,( 13.0?4.5) pmol/L, respectively. TSH level of fetus was increased gradually along the gestation, reaching the up level of the adults at the 20 weeks and peaking at the birth time. While the T 3 and FT 3 keep in a lower level in gestation Conclusions Fetal thyroid hormones increase with the gestational age.The diagnosis of congenital fetal thyroid hormone malfunction in the second half of the pregnancy should be monitored mainly by the T 4, FT 4 and TSH levels in different gestational age. For this consideration, to set up a reliable data for normal human fetus thyroid hormone concentrations is a very important and essential step to provide a practical guide for doctors to do intra uterine diagnosis and treatment of associated high risk groups. The peaking level of TSH at the birth time will surely company the changing of other thyroid hormones, so it might not be the best time to screening the congenital thyroid malfunction at the 72 hours after birth.
7.Changes in heart geometry and function during off-pump coronary artery bypass grafting

Objective To assess the changes in heart geometry and function during off-pump coronary artery bypass grafting(CABG).Methods Thirty ASAⅡorⅢpatients(24 males,6 females)aged 50-78 undergoing elective off-pump CABG were studied.After induction of anesthesia and tracheal intubation a catheter which was connected to the monitor measuring continuous cardiac output(CCO),mixed venous blood oxygen saturation(S(?)O_2)and CEDV was placed via right internal jugular vein and the probe of multi-phase transesophageal echocardiograph(TEE)(Sonos HP 2500)was inserted in the esophagus.The distance between the probe and the incisors was between 34-45 cm.The hemodynamic variables and TEE parameters were recorded when epicardium was opened(T_1,baseline)when the octopus tissue stabilizer was placed during anastomosis between the graft vessel and left anterior descending artery(LAD)(T_2)left circumflex artery(LCX)(T_3)and right coronary artery(RCA)(T_4)and when the anastomoses between the vessel grafts and aorta was completed (T_5).Results At T_2 the mitral valve deceleration time(DT_1)and the blood flow through the mitral valve(Q_1) were decreased significantly as compared with the baseline(T_1);MAP was significantly decreased while HR and CVP were increased(P<0.05).At T_3 the left and right ventricular end-diastolic diameter(LVEDD,RVEDD), tricuspid valve diameter(TVD)and the blood flow through Q_1 and tricuspid valve(Q_2)were significantly decreased as compared to the baseline values at T_1.The mitral valve E/A ratio was increased while the tricuspid valve E/A ratio was decreased(P<0.05).The MAP,S(?)O_2,right ventricular end-systolic and end-diastolic volume(RVESV,RVEDV)and right ventricular ejection fraction were significantly decreased while HR and CVP were increased at T_3 as compared to the baseline at T_1.At T_4 LVESD,LVEDD,MVD,tricuspid valve E/A ratio and Q_1 were significantly decreased as compared to the baseline at T_1(P<0.05).MAP,MPAP,SV,S(?)O_2, RVEF,RVESV and RVEDV were significantly decreased while HR and CVP were increased(P<0.05).At T_5 all the TEE and hemodynamic parameters returned to the baseline valves at T_1.Conclusion During anastomoses between graft vessels and LAD,LCX and RCA,both left and right ventricles are compared to some extent and the heart function is impaired temporarily but returns to the baseline after the anastomoses are completed.
8.MRI with multiple contrast weightings and dynamic contrast enhancement in evaluation of vulnerable atherosclerotic plaques

Objective To analyze the utility of MRI with multiple contrast and dynamic contrast weightings enhancement(DCE) in evaluation of vulnerable atherosclerotic plaques. Methods Forty male New Zealand white rabbits were fed with hypercholesterolemic diet,and right iliac arteries including the common and external iliac arteries were examined by multiple contrast and DCE MRI at intervals 6 to 20 weeks after balloon denudation.For multiple contrast weightings scanning,T1-,T1-/T2WI with fat suppression,proton density weighted and double invention recovery were used.Meanwhile,post DCE T1-with fat suppression images were obtained in 1,5,15 and 25 min after a bolus injection of Gd-DTPA contrast agent.Then a comparative analysis of plaque morphology and components to images was performed. Results There were 34(42.5%) vulnerable plaques and 46(57.5%) stable plaques amomg the 80 atherosclerotic lesions located at the right common or external iliac arteries.The accuracy,sensitivity and specificity of MRI with multiple contrast weightings and DCE for the detection of vulnerable plaques were 87.5%,94.1% and 82.6%,respectively,significantly higher than those only with multiple contrast weightings,which were 73.8%,82.3% and 67.4%,respectively(P
9.Study on clinical characteristics and therapeutic outcomes of 173 patients with Takayasu arteritis

ObjectiveTo study the clinical features, arterial involvement, therapeutic strategies and outcomes of Takayasu arteritis (TA). MethodsThe clinical symptoms, arterial images, inflammatory parameters and follow-up information of 173 patients with TA were retrospectively studied. Comparisons between groups were performed by t-test. ResultsThere were 136 female and 37 male patients in this study. The mean age at onset was(26±11 ) years. Hypertension, pulse deficit or asymmetrical pulse, and fever were present in 46.6%, 41.1%, 28.7% of patients, respectively. The distribution of arterial involvement were 64.7%in aorta, 9.8% in pulmonary artery, 19.1% in innominate artery, 65.9% in common carotid arteries, 65.3%in the subclavian artery, 36.2% in the renal artery, 12.1% in the vertebral artery, and 5.8% in coeliac axis.Elevated erythrocyte sedimentation rate(ESR) was found in 61.0% patients. Active tuberculosis or history of tuberculosis was implicated in 45 patients(26.0%). Ten patients(5.8%) were hepatitis B virus carriers.Among 105 followed-up patients, 98 patients(94.2%) achieved persistent remission, 17 patients relapsed when corticosteroids were tapered. ConclusionCorticosteroids combined with or steroid alone, supplemented with endovascular intervention procedures or surgical bypass procedures when necessary, can effectively control the clinical symptoms and inflammatory parameters and improve the quality of life of patients.
