Objective To establish the method of depict ing the left ventricular (LV) pressure-volume loop automatically by computer which incorporates LV catherization simultaneous with LV acoustic quantification echocardiography and to assess echocardiography non-invasive evaluation LV chamber stiffness. Methods Thirteen patients with hypertrophic cardiomyopathy (HCM) underwent LV catherization simultaneously with echocardiography. LV pressure and volume curves were sent to computer, and LV pressure-volume loop was depicted automatically and modulus of LV chamber stiffness (Kc) was obtained. Pulsed Doppler echocardiography of mitral intraventricular flows was obtained in patients with HCM. Results Patients with HCM had significantly higher Kc obtained from LV pressure-volume loop ( 0.43 ? 0.11 vs 0.27 ). R-E 3/R-E 0, (R-E 2)-(R-E 1), (R-A 3)-(R-A 2) were correlated to Kc(r= 0.61 , 0.57 , 0.58 ,respectively). Conclusions This method provides a simple and reliable technique for automatically tracing pressure-volume loop and should facilitate further investigation of the left ventricular diastolic function in clinical practice. R-E 3/ R-E 0, (R-E 2)-(R-E 1), (R-A 3)-(R-A 2) offer new non-invasive indices in evaluating LV chamber stiffness.

Objective To investigate the independent factors associated with the apex of left atrial appendage(LAA) spontaneous echo contrast (SEC) in patients with valvular atrial fibrillation (VAF). Methods Seventeen normal subjects and 21 patients with VAF were included. Plasma fibrinogen (Fg),vov Willebrand factor (vWF),D-dimer (DD),P-selectin,thrombin-antithrombin complexes (TAT) and plasminogen activator inhibitor-1(PAI-1) were measured. The effect of Fusarin C, a new mutagen extracted from Fusarium moniliforme,on murine peritoneal M? activation by measuring M?-mediated MTC ADCC and CS,Fusarin C could inhibit M? activation by both MAF and FDP. The inhibitory effect was dose-and time-dependent and the curves of dose-and time-response were similar in all three assays. There is, however, apparent difference in Fusarin C dose and time needed to induce significant in hibition among these assays. Significant inhibition occured at 0.4?g/ml and 0.5?g/ml for MTC and CS respectively, but 1.6?g/ml for ADCC. Similarly, the minimal period of time necessary to bring about significant inhibition was 2 hr for MTC, but 3 hr for ADCC. Finally, significance of the inhibitory effect of Fusarin C on M? anti-tumor activity in relation to carcinogenesis was discussed.

The inhibitory effect of Fusarin C,a new mutagen isolated from Fusarium moniliforme,on M? activation was investigated.The inhibitory effect of Fusarin C disappeared partially after 24 hr culture in the absence of the mycotoxin and completely after 72 hr.In addition,it could be overcome by high concentrations of M? activating factor or anti-serum to the target cells.

BACKGROUND: Hyperhomocysteinemia has been suggested to be a possible independent risk factor for stroke.OBJECTIVE: To explore the relationship between hyperhomocysteinemia and cerebral infarction and hemorrhage, and analyze the factors that affect plasma homocysteine level.DESIGN: Case-controlled clinical trial.SETTING: Department of Neurology, Second Hospital Affiliated to Medical College of Zhejiang University.PARTICIPANTS: Totally 57 patients including 21 with cerebral hemorthage and 36 with brain infarction were treated in the Department of Neurology, Second Hospital Affiliated to Medical College of Zhejiang University Between January and November 2003. Twenty-eight healthy volunteers were also recruited from the subjects coming for routine physical examination.METHODS: Two milliliters of fasting venous blood was collected from all subjects in the morning for detecting the contents of plasma homocysteine,vitamin B12, folic acid, creatinine and so on. All patients were scored for clinical neurological impairment, with the hematoma volume calculated in patients with brain hemorrhage determined on the basis of CT scanning.acid, vitamin B12, clinical neurological impairment score and hematoma volume.RESULTS: Valid results were obtained from all the 57 stroke patients and in male and female patients of both cerebral infarction group and cerebral hemorrhage group than that of the subjects of the same gender in the control group [(25.2±21.4), (18.3±10.9), (11.5±2.9) μmol/L for male subjects;(22.8±18.9), (14.7±7.4), (10.8±2.6) μmol/L for female subjects, P＜ 0.05-0.01].The level of homocysteine was similar between cerebral infarction group and cerebral hemorrhage group, homocysteic acid level showed obvious inverse correlation with folic acid level (r=-0.442, -0.531, P ＜ 0.05), but without relation to vitamin B12 level (r=-0.086, -0.111, P ＞ 0.05). Homocysteine level was not obviously correlated to the neurological impairment scores in cerebral infarction group (r=-0.139, P ＞ 0.05), nor was it related to the scores or hematoma volume in cerebral hemorrhage group (r=0.225,0.425, P ＞ 0.05).CONCLUSION: Hyperhomocysteinemia is risk factor for cerebral infarction and hemorrhage. Plasma homocysteine level is inversely correlated with folic acid level, but not obviously related to vitamin B12, clinical neurologicla impairment score or hematoma volume.

Objective To prospectively evaluate the value of diffusion-weighted imaging with background signal suppression (DWIBS) in the detection and diagnoses of metastatic cervical lymph nodes. Methods Twenty patients with head and neck squamous cell carcinomas underwent conventional MRI and DWIBS. The ADC values were measured and compared between metastatic and benign lymph nodes. The abilities of DWIBS and conventional MRI in the evaluation of metastatic cervical lymph nodes were compared. Results Conventional MRI demonstrated 101 lymph nodes, in which 58 were diagnosed as metastatic nodes. When MRI criteria were used for discrimination between metastatic and benign nodes, the obtained sensitivity, specificity, accuracy, positive and negative predictive value were 78% (47/60), 81%(44/54), 80% (91/114), 82% (47/57) and 77% (44/57), respectively. The mean ADC values of metastatic nodes (n =60) and benign nodes (n =54) were (0.945 ±0.122) × 10-3 mm2/s and (1. 210 ±0. 151 ) × 10-3 mm2/s ( t = - 10. 354 ,P ＜0. 01 ). When an ADC value of 1. 090 × 10 -3 mm2/s was used as the threshold for discrimination between metastatic and benign nodes, the obtained sensitivity, specificity,accuracy, positive and negative predictive value were 88% (53/60), 80% (43/54), 84% (96/114), 83%(53/64) and 86% (43/50), respectively. Conclusion DWIBS is helpful in diagnosing metastatic cervical lymph nodes of head and neck squamous cell carcinoma, and is superior to conventional MRI in their detection.

Objective To compare the effect of active compression-decompression cardiopulmonary resuscitation(ACD-CPR) with standard- cardiopulmonary resuscitation(S-CPR) on ventricular function in a canine ventricular fibrillation model. Methods Ventricular fibrillation was induced in anesthetized and instrumented canine. Twenty-four dogs were randomly assigned to either ACD-CPR group or S-CPR group.After 4 minutes of untreated VF,two-dimension echocardiography was used to evaluate the left ventricular end-diastolic volume(LVEDV),left ventricular end-systolic volume(LVESV) and left ventricular ejection fraction (LVEF) of every canine of the two groups when they were undergoing cardiopulmonary resuscitation. Results During ventricular fibrillation, both ACD-CPR group and S-CPR group showed decreased LVEDV compared with pre-ventricular fibrillation, but not statistically significant( P ＞0.05).LVEDV was increased in ACD-CPR group compared with S-CPR group, but not statistically significant (P＞ 0. 05). Both ACD-CPR group and S-CPR group showed significantly increased LVESV compared with pre-ventricular fibrillation,of which the difference was statistically significant ( P ＜0. 001). Both ACD-CPRgroup and S-CPR group showed significantly decreased LVEF compared with pre-ventricular fibrillation,of which the difference was statistically significant (P ＜0. 001). LVEF was increased in ACD-CPR group compared with S-CPR group,of which the difference was statistically significant ( P = 0.019). Conclusions Compared with S-CPR,ACD-CPR resulted in higher LVEF.

HuangDi NeiJing, the Yellow Emperor's Classic of Internal Medicine, a foundational classic of Chinese medical theory, is ranked as the first among the four most important classics of traditional Chinese medicine. Translation of culture-specific Lexicon is a pivot, also a nodus in translating Huangdi Neijing, so, chewing over it is of important meaning for English translation of the Chinese classic or even for that of whole Chinese medicine. In expectation of somewhat contribution to the development of the Chinese medical English discipline, the classification and translation strategy for culture-specific lexicon were discussed by the authors in this paper through an empirical study on comparing the different terms translated in two versions of Huangdi Neijing translation.

Joint contracture is currently one of the very common diseases in rehabilitation medicine department.Once it occurs,it likely leads to such functional disturbances in the patients as restricted joint activities,decreased muscle strength around a joint and abnormal gait,seriously affecting the quality of life of the patients.Studies on its pathogenesis can help doctors better understand and treat the disease.In this paper,pathogenesis of the disease and progress in its therapy are summarized.
