3.Clinical Value of Demonstration of Estrogen and Progesterone Receptors using Paraffin Wax Sections in Breast Carcinoma.

This study aimed at assessing the usefulness of paraffin wax sections for demonstration of ER(estrogen receptor) and PR(progesterone receptor), using brief microwave processing rather than proteolytic predigestion. The receptor status of a breast cancer is often into consideration when planning treatment nowadays. As biochemical receptor assays require large amount of fresh tumor tissue and are not always available for all tumors, pathologists are now increasingly asked to provide a service for the assessment of the receptor status in tissue routine sections. Until recently, immunohistochemical demonstration of receptors was used in frozen sections. Therefore, routinely processed paraffin wax sections of 25 cases of breast carcinoma with known ER and PR concentrations, estimated by the standard DCC(dextran-coated charcoal) biochemical assay, were examined using the ABC immunoperoxidase technique. The results were assessed semiquantitably, using a five grade scoring system. Of the 25 cases examined, with DCC cutoff point being <10 fmol,71% and 75% in positivity of each ER, PR receptor is concordant. Statistic analysis demonstrates high relationship between scoring system of IH method and DCC value in ER (R=0.6061, p=0.001) and PR (R=0.5832, p=0.001). The IH method can provide easily assessed reliable positive information about ER, PR status of breast carcinoma using routinely processed paraffin wax sections.

5.Effect of N-methyl-D-aspartate on the Serotonin Release in the Rat Hippocampus.

PURPOSE: Glutamate and aspartate are the excitatory amino acid neurotransmitters and NMDA (N-methyl-D-aspartate) is one of their major receptors. NMDA agonist may sti mulate serotonergic nervous system that inhibit the penile erection as well as induce the penile erection. We investigate the effects of NMDA agonist on serotonin release from hippocampus. MATERIALS AND METHODS: The slices of hippocampus were incubated in a buffer con taining 0.1mM [(3)H]5-hydroxytryptamine (5-HT) for uptake in the male rat. The release of 5-HT into the buffer during each 10 minutes period was measured and the radio activities in each buffer and the tissue were counted. After 50 min from the initiation, NMDA agonist were administered at 6th and 7th 10 min period respectively. The changes of 5-HT release were expressed as percent values compared to the 5th 10 min period. Tetrodotoxin was used to determine the possible involvement of interneuron on the action of these neurotransmitters. RESULTS: A steady release of 5-HT was observed up to 100 minutes after the rapid release during the first 40 minutes. Treatment of tetrodotoxin (10(-6)M) did not change the spontaneous release of 5-HT. The 5-HT released during 10 and 20 minutes of NMDA agonist (10(-4M)) treatment significantly higher than those of control group. The increase of 5-HT release by NMDA agonist was blocked by pretreatment with tetro dotoxin. The release of 5-HT was increased by NMDA agonist and this response was blocked by tetrodotoxin. CONCLUSIONS: NMDA agonist increases the release of 5-HT through the activation of the interneurons and these results suggest that NMDA agonist may stimulate the serotonergic nervous system that inhibit the penile erection as well as inducing the penile erection.

7.Molecular cloning and characterization of an antigenic protein with a repeating region from Clonorchis sinensis.

In the course of immunoscreening of Clonorchis sinensis cDNA library, a cDNA CsRP12 containing a tandem repeat was isolated. The cDNA CsRP12 encodes two putative peptides of open reading frames (ORFs) 1 and 2 (CsRP12-1 and -2). The repetitive region is composed of 15 repeats of 10 amino acids. Of the two putative peptides, CsRP12-1 was proline-rich and found to have homologues in several organisms. Recombinant proteins of the putative peptides were bacterially produced and purified by an affinity chromatography. Recombinant CsRP12-1 protein was recognized by sera of clonorchiasis patients and experimental rabbits, but recombinant CsRP12-2 was not. One of the putative peptide, CsRP12-1, is designated CsPRA, proline-rich antigen of C. sinensis. Both the C-termini of CsRP12-1 and -2 were bacterially produced and analysed to show no antigenicity. Recombinant CsPRA protein showed high sensitivity and specificity. In experimental rabbits, IgG antibodies to CsPRA was produced between 4 and 8 weeks after the infection and decreased thereafter over one year. These results indicate that CsPRA is equivalent to a natural protein and a useful antigenic protein for serodiagnosis of human clonorchiasis.

8.Large Cell Neuroendocrine Carcinoma of the Lung 2 cases including one presented as an ovarian mass.

Pulmonary tumors exhibiting neuroendocrine differentiation are classified as typical carcinoid, atypical carcinoid, and small cell lung carcinoma(SCLC). Travis et al. proposed a fourth category of large cell neuroendocrine carcinoma, which is characterized by light microscopic neuroendocrine appearance, cells of large size, polygonal shape, low nuclear cytoplasmic ratio, coarse nuclear chromatin, with prominent nucleoli high mitotic rate and frequent necrosis; and neuroendocrine features by immunohistochemistry or electron microscopy. High grade neuroendocrine carcinoma (LCAC-NE) revealed aggressive clinical course. We report two cases of neuroendocrine tumors of the lung characterized by a trabecular pattern of large pleomorphic cells with frequent mitoses and wide necrosis. The frequent metastatic sites of atypical carcinoid were liver, bone and brain. One of our case is presented, at first, as an ovarian mass, which shows multifocal rosettes and revealed metastasis from lung. Both cases expressed neuroendocrine differentiation by light microscopy and immunohistochemistry. However clinical neuroendocrine symptom were not present.

9.Canalicular Trephination with Lacrimal Trephine and Endoscopic DCR.

PURPOSE: The purpose of this study is to evaluate surgical outcomes of canalicular trephination with lacrimal trephine and endoscopic dacryocystorhinostomy in patiens with common canalicular or canalicular obstruction. METHODS: 35 patients (38 eyes) diagnosed as common canalicular or canalicular obstruction undergoing surgery between December 1999 and August 2003. Patients have been followed up more than 5 months after surgery. RESULTS: The average age of patients was 57.3 years old and the average follow-up period was 7.1 months. The overall success rate was 92.1%. With obstructions within lower canaliculus, the success rate was 75.0%. The success rate of the cases with obstructions within upper canaliculus and upper and lower canaliculus was 100%, and the success rate of the cases with obstructions within common canaliculus was 95.2%. CONCLUSIONS: The surgical success rate of canalicular trephination with lacrimal trephine and endonasal dacryocystorhinostomy was better than canaliculodacryocystorhinostomy and this seems to be adequate as an primary procedure before Jones tube bypass surgery.

10.Effects of Aromatherapy on Anxiety and Discomfort in Patients Having Colonoscopy.

PURPOSE: This study was done to determine the effects of aromatherapy on anxiety and discomfort for patients who were having colonoscopy. METHODS: The study used a nonequivalent control group non-synchronized design. The participants were 48 people who were undergoing colonoscopy in a university hospital. The experimental group inhaled a blend of essential oil with neroli, camomile, lavender and lemon through an aroma stone for 5minutes before the examination. Then the aroma stone was put on the side of the pillow to spread the aroma scent in the room. The experimental group could inhale the aroma scent as soon as they came into the room and during the examination process. The control group received only fundamental nursing care. RESULTS: Aromatherapy significantly decreased VAS anxiety. There were also significant differences in facial change and, tone change for objective discomfort. But there were no significant differences in subjective discomfort between the groups nor were there any significant differences in vital signs. CONCLUSION: Aromatherapy was perceived as an useful intervention to reduce anxiety and objective discomfort of patients receiving colonoscopy. The results of this study could be utilized as a clinical nursing intervention.
