1.Subtrochanteric Femoral Fracture during Trochanteric Nailing for the Treatment of Femoral Shaft Fracture.

We report on three cases of subtrochanteric femoral fractures during trochanteric intramedullary nailing for the treatment of femoral shaft fractures. Trochanteric intramedullary nails, which have a proximal lateral bend, are specifically designed for trochanteric insertion. When combined with the modified insertion technique, trochanteric intramedullary nails reduce iatrogenic fracture comminution and varus malalignment. We herein describe technical aspects of trochanteric intramedullary nailing for femoral shaft fractures to improve its application and prevent implant-derived complications.

2.Piperacillin-Tazobactam-Induced Linear IgA Bullous Dermatosis Supported by a T-Cell Activation Assay.

Linear immunoglobulin (Ig) A bullous dermatosis (LABD) is a rare subepidermal autoimmune blistering disease characterized by linear IgA deposits at the basement membrane zone visualized with direct immunofluorescence (DIF). Most cases of LABD are idiopathic, but some are drug-induced with vancomycin being the most common causative agent. We herein report a patient presenting with blisters and erosive lesions, primarily in the intertriginous and flexor areas, consistent with a diagnosis of piperacillin-tazobactam-induced LABD based on the patient's clinical course and histopathology, DIF, and in vitro T-cell activation assay (TAA) findings. Only one case of piperacillin-tazobactam-induced LABD has been previously reported. In addition to its rarity, our case was also unique in that the skin lesions occurred in the intertriginous and flexor areas, uncommon locations for typical adult patients with LABD, and TAA strongly suggested an association with the causative drug.

However, few studies have explored the relationship between telomere length (TL) and irritable bowel syndrome (IBS). We investigated the association between TL and IBS in 43 IBS patients using quantitative polymerase chain reaction. The clinical characteristics and severity of IBS, assessed by the visual analogue scale, were also analyzed. The average TL was 4.40 ± 3.87, with TL shortening tending to be associated with female sex and smoking. However, these associations did not reach statistical significance. Correlation analysis showed a negative correlation between IBS severity and TL (r = –0.285, p = 0.083), although this was not statistically significant. No other clinical characteristics were significantly associated with TL. This is the first study to examine the relationship between TL and IBS. Our findings suggest that TL may have potential as a predictor for IBS diagnosis.

7.Effects of Cefodizime on Phagocytosis of COS-1 Ccells.

PURPOSE: Cefodizime is a new third-generation cephalosporin which has a structure and immunomodutation properties similar to cefotaxime. Various studies on cefodizime have demonstrated the direct eradication of bacteria in cooperation with the host defense mechanism, particularly with phagocytosis. We evaluated the effects of cefodizime on the phagocytosis of COS-1 cells transfected with FcgammaRI/gammagamma or FcgammaRIIA cDNA. METHODS: Phagocytosis was measured using the in vitro COS-1 cell modeling system according to Schreiber's method. COS-1 cells, which lack endogenoous Fcgammareceptors but have phagocytic potential, were transfected with either FcgammaRI/gammagammaor FcgammaRIIA cDNA. COS-1 cells, as target cells, were treated with antibiotics for 1 or 24 hours and incubated for 30 min with IgG coated sheep RBCs. Adhered IgG coated sheep RBCs were removed after brief exposure to hypotonic phosphate buffered saline. Phagocytosis index (PI) was calculated as the number of ingested RBCs per 100 phagocytic cells after wright-Giemsa staining. RESULTS: COS-1 cells tranfected with FcgammaR (either FcgammaRI/gammagamma or FcgammaRIIA cDNA) showed the phagocytic activity against IgG coated sheep RBC, while untransfected COS-1 cells did not. After treatment with cefodizime, phagocytic activity of FcgammaRI/gammagammacDNA transfected COS-1 cells was significantly increased, while that of FcgammaRIIA cDNA transfected COS-1 cells did not. Marked enhancement of phagocytosis of COS-1 cells was observed after treatment with cefodizime, but was not observed with ceftriaxone or moxalactam. CONCLUSION: Cefodizime showed marked enhancement of phagocytic activity of FcgammaR transfected COS-1 cells. FcgammaRI seems to play an important role in the enhancement of phagocytosis. Further studies will be required.

8.Routine chest computed tomography as a preoperative work-up for primary colorectal cancer: is there any benefit in short-term outcome?.

PURPOSE: The aim of this study was to assess the role of pre-operative chest computed tomography (CT) compared with abdominopelvic CT (AP-CT) and chest radiography (CXR) for detecting pulmonary metastasis in patients with primary colorectal cancer (CRC). METHODS: We retrospectively analyzed the data of 619 patients with primary CRC who simultaneously received a preoperative chest CT (chest CT group), AP-CT with hilar extension, and CXR (CXR group). RESULTS: In the chest CT group, there were 297 (48.0%) normal, 198 (32%) benign, 96 (15.5%) indeterminate, 26 (4.2%) metastasis, and two lung cancers. Eighteen patients (2.9%) in the CXR group who had no pulmonary metastasis were diagnosed with pulmonary metastasis on a chest CT. The sensitivity and accuracy were 83.9% and 99.0% in the chest CT group, respectively, and 29.0% and 91.5% in the CXR group, respectively (P < 0.0001 and P = 0.0003). CONCLUSION: Chest CT appears to improve the accuracy of pre-operative staging in patients with CRC and is useful for the early detection of pulmonary metastasis as a baseline study for abnormal lung nodules.

9.Detection of Occludable Angles with the Pentacam and the Anterior Segment Optical Coherence Tomography.

PURPOSE: To assess efficacy of the Pentacam (PTC) and the anterior segment optical coherence tomography (AOCT) for detection of occludable angles. MATERIALS AND METHODS: Fourty-one eyes with gonioscopically diagnosed occludable angles and 32 normal open-angle eyes were included. Anterior chamber angle (ACA) and anterior chamber depth (ACD) were measured with PTC and AOCT. Receiver operating characteristic (ROC) curve was constructed for each parameter and the area under the ROC curve (AUC) was calculated. RESULTS: Values of ACA and ACD measured by PTC and AOCT were similar not only in normal open angle eyes but also in occludable angle eyes. For detection of occludable angle, the AUCs of PTC with ACA and ACD were 0.935 and 0.969, respectively. The AUCs of AOCT with ACA and ACD were 0.904 and 0.947, respectively. CONCLUSION: Both PTC and AOCT allow accurate discrimination between open and occludable angle eyes, so that they may aid to screening the occludable angles.

10.Ruptured Cerebral Aneurysm without Subarachnoid Hemorrhage: Who needs angiography?.

OBJECTIVE: Ruptured intracranial aneurysms usually present as a subarachnoid hemorrhage (SAH), but are sometimes associated with intracerebral hemorrhage (ICH), intraventricular hemorrhage (IVH), or subdural hematoma (SDH). However, the presentation of a ruptured aneurysm without a SAH is quite unusual. We describe nine such cases and highlight some easily overlooked, but important clinical features. METHODS: Among 341 patients diagnosed with ruptured cerebral aneurysms during the past 4 years, 9 patients exhibited non-SAH bleeding on admission, as revealed by brain computed tomograms (CT). On these 9 patients, the characteristic features were reviewed using medical charts, emergency room notes, and radiographic findings. RESULTS: The incidence of aneurysmal rupture without SAH was 2.6%. Eight patients exhibited ICH, and among them, an IVH occurred in one patient and a SDH in two patients. The initial clinical grade was grave in 8 patients, and a favorable outcome occurred in 4 patients. All of these aneurysms arose from the anterior circulation (the circle of Willis in two patients, and distal aneurysms in seven patients). The causes of the aneurysms were spontaneous in four patients, trauma in two patients, infective endocarditis in two patients, and moyamoya syndrome with a history of craniotomy and clipping in one patient. In three patients, additional intervention was required because the initial radiographic images did not reveal a ruptured aneurysm. CONCLUSION: Ruptured aneurysms should be suspected in cases of unexplained intracranial bleeding, even if SAH is not present on the initial CT scan, because most patients exhibit a poor neurologic grade. Therefore, careful interpretation of the clinical and radiologic culprits and timely management should be provided to achieve total occlusion.
