1.Inhibition of L-type calcium currents by magnesium sulfate on the rat basilar artery smooth muscle cells

Objective: Vasospasm remains the leading cause of cerebral damage after aneurysmal subarachnoid
hemorrhage. Although magnesium regulates the calcium influx in vascular smooth muscle and
endothelial cells, it has not been reported whether L-type calcium channels are involved in magnesiuminduced
vascular relaxation in rat basilar artery. So, the effect of magnesium sulfate on L-type calcium
currents in freshly isolated smooth muscle cells from rat basilar artery was investigated. Methods:
The smooth muscle cells were isolated from rabbit basilar artery by enzyme treatment. L-type Ca2+
currents were identified using cesium chloride, a potassium channel blocker and Bay K8644, an
activator of L-type Ca2+ channel. Currents were recorded under step pulse whole cell patch clamp
technique. Results: In the presence of cesium chloride (in pipette solution), inward currents were
observed by depolarizing step pulses. The inward currents were significantly reduced by nimodipine
(n=4, p<0.05), an L-type Ca2+ channel blocker and increased by Bay K8644 (n=5, p<0.05), an L-type
Ca2+ channel activator. The L-type calcium currents (156±17.0 pA, n=12) were significantly reduced
by the application of 5 mM magnesium sulfate (53.8±7.0 pA, n=12, p<0.01).
Conclusion: These results suggest that magnesium may relax cerebral vessel of rat basilar artery
through decreasing intracellular Ca2+ ion by inhibition of L-type Ca2+ channels.

Synovial chondromatosis is a rare monoarticular disorder which often involves the knee, hip and the elbow but the shoulders are less frequently affected. The diagnosis depends on the important radiographic and operative findings as well as histopathological criteria. We have experienced the synovial chondromatosis developed in the left shoulder of a 37 year-old male who was diagnosed by CT scan, axillary angiography and microscopic findings and treated at Chonbuk National University Hospital.

4.Opacity and masking effect of the opaque shade composite resins.

The purpose of this study was to assess the background color-interceptive ability and opacity of opaque shade composites (Universal composite, Filtek Z350, Charisma, Clearfil ST, Palpaque Estelite, Esthet-X, and Metafil Flo). Twenty four background specimens (diameter 5.5 mm, thickness 3.0 mm) with Root dentin Mustard (Bisco, Schaumburg, IL, USA) were made. The CIE L*a*b* value of background specimens was measured by a spectrophotometer (Spectrolino, GretagMacbeth, Regensdorf, Switzerland). Three specimens in every group were filled on the background specimens. The surface color of samples was measured by a spectrophotometer in 3.0 mm and every thickness to 0.5 mm while grinding. The color difference in the background color along with 3.0 mm specimen gauged the masking effect in each thickness while grinding and polishing. The opacity was calculated in 1 mm thick specimens. The opacity was in the decreasing order of Clearfil ST, Metafil Flo, Filtek Z350, Palpaque Estelite, Universal composite, Charisma, and Esthet-X (p < 0.05). As the thickness get reduced, L* value showed decreasing, a* increasing tendency. The surface color difference between pair of the 3.0 mm thick specimen and after grinding in same opaque resin was above 3.3 except Clearfil ST and Metafil Flo. The color difference (DeltaE*) between pair of background specimen and opaque resin builtup specimen showed more than 10.0 regardless kinds and thickness. The variance in opacity characteristics and color of the opaque composites is dependent upon manufacturer. When using the opaque resin, the optical properties of each material must be considered as well as cavity.

5.Color stability of the resin cements with accelerated aging.

The purpose of this study was to evaluate the color stability of resin cements with accelerated test. Four dual curing resin cements: Panavia-F (KURARAY), Duolink (BISCO), Variolink-II (Ivoclar Vivadent), and RelyX Unicem (3M ESPE) and 1 self curing resin cement: Resiment CE (j. l. Blosser) were used in this study. In control group, Gradia Anterior (GC) composite resin and Tescera Dentin (Bisco) indirect composite were used. Ten disk shape specimens were made from each resin cement. The specimens were subjected to an accelerated aging process in a refrigerated bath circulator at 60degrees C for 15 and 30 days. Spectrophotometric analyses were made before and after 15 days and 30 days of accelerated aging time. The color characteristics (L*, a*, b*) and the color difference (DeltaE*) of the specimens before and after immersion were measured and computed. Regardless of type of the resin cements, L* value was decreased and a* value was increased, but there were no significant difference. But b* value was increased significantly (p < 0.05). Tescera inlay showed least color change (p < 0.05), but Gradia showed notable color change after 15 days. After 30 days on accelerated aging, DeltaE* value was increased (Panavia-F < Variolink-II < Resiment CE < Duolink < Unicem) (p < 0.05), but there were no significant difference among Panavia-F, Variolink-II, and Resiment CE groups. After 30 days of accelerated aging, DeltaE* value of all resin cements were greater than 3.0 and could be perceived by the human eye.

10.A Clinical Experience in 16 Pregnancies with Hemolysis, Elevated Liver Enzymes, and Low Platelets (HELLP Syndrome).

OBJECTIVE: Our purpose was to describe the clinical progress and the maternal and fetal outcome in 16 pregnancies complicated by the HELLP(hemolysis, elevated liver enzymes, low platelet). Material: We reviewed the maternal and neonatal charts from 16 consecutive pregnancies complicated by the HELLP syndrome among 302 pregnancies complicated by preeclamsia and eclamsia managed at our hospital during the period of 4 years from June 1994 through June 1998. The HELLP syndrome was defined by previously published laboratory criteria. We assessed the time of onset, presenting symptom, laboratory finding, mode of delivery, fetal and maternal complication in each case. We also reviewed the clinical finding in detail in the case resulted in maternal death. RESULTS: In regards to the time of onset, 15 cases (93.7%) occurred at antepartum period and only 1 case (6.2%) occurred at postpartum period. Among the 15 cases occurred at antepartum period, 13 cases (81.25%) developed at 27 to 36 weeks gestation and 2 cases (12.5%) developed at near term. In regards to the presenting symptom, twelve patients (75%) complained of right upper quardrant or epigastric pain. Of 16 patients, 12 patients (75%) experienced headache and 10 patients (62.5%) complained of nausea, or vomiting and 5 patients (31.2%) had visual disturbance. The laboratory finding of all 16 cases were as follow; the mean level of platelet: 68700/mm3 (range: 48000 to 91700), the mean level of serum asparate aminotransferase: 335 IU/L (range: 62 to 135), the mean level of lactic dehydrogenase: 910 IU/L (range: 558 to 5794), and the mean level of total bilirubin: 2.6 mg/dl (range: 0.7 to 10.4). To review the mode of delivery, cesarean sections were done on 10 patients (62.5%) including 7(43.7%) emergency and 3(18.7%) elective operations. However, 6 patients (37.5%) delivered vaginally. Maternal complications were as follow; abruptio placenta in 1 case (6.2%), DIC in 2 cases (12.5%), pulmonary edema in 3 cases (18.7%), pleural effusion in 4 cases (25%), renal failure in 4 cases (25%), and 1 case of death. Fetal and neonatal outcome was assessed; 9 cases of intrauterine growth retardation (56.2%), meconium stained in 3 cases (18.7%), 2 stillbirth (12.5%), and 2 neonatal death (12.5%). CONCLUSION: HELLP syndrome is associated with serious maternal and fetal morbidity and mortality.
