1.The Results and Prognostic Factors of Mitomycin C Trabeculectomy in Neovascular Glaucoma.

BACKGROUND: Neovascular glaucoma is common secondary glaucoma at high risk for failure of glaucoma filtering surgery. Recently, trabeculectomy with adjunctive mitomycin C trabeculectomy has been tried to improve the surgical success rate of conventional trabeculectomy. But, the long-term effects of mitomycin C trabeculectomy for neovascular glaucoma are unknown. Thus, we evaluated the long-term effects of mitomycin C trabeculectomy and its prognostic factors influencing the outcome. MATERIALS AND METHODS: Medical records of 62 eyes of 55 neovascular glaucoma who had undergone mitomycin C trabeculectomy were retrospectively reviewed. Surgical success was defined as intraocular pressures of 21 mmHg or less with or without glaucoma medications and no loss of light perception. Surgical failure was defined as postoperative loss of light perception in patients with preoperative vision better than light perception, additional glaucoma surgery, or phthisis bulbi in patients with preoperative vision of no light perception. RESULTS: Postoperative success was obtained in 37 (60%) out of 62 eyes after mean follow- up period of 23.9+/-16.2 months. Using Kaplan-Meier survival analysis, cumulative success rate at the 6-, 12-, 24- and 36-month intervals were 85%, 71%, 57% and 52%, respectively. Success rate was greater in eyes with diabetic retinopathy than other causes(p=0.005) and in eyes with preoperative panretinal photocoagulation(PRP) than without PRP(p=0.015). However, Cox proportional hazard regression analysis revealed that preoperative PRP was not a significant risk factor for surgical failure. CONCLUSION: Prognosis of neovascular glaucoma caused by diabetic retinopathy was better than that caused by the other disorders following mitomycin C trabeculectomy. The author would suggest that mitomycin C trabeculectomy could be effective and relatively safe as the first procedure of choice before performing glaucoma drainage device implantation or cyclodestructive procedure.

2.A Case Report of Traumatic Bilateral Bislocation of the Hip

Bilateral dislocation of the hip was observed in a 44-year-old man after the accident that he was overrun from backward by a slowly approaching goods-train. Successful manual reduction was obtained, followed by one month cast immobilization and partial weight bearing exercise thereafter. Peroneal palsy of right extremity, noted immediately after the dislocation, showed the prospect of good recovery. The mechanism of injury was also discussed.

5.A Case of Solitary Congenital Calcified Nodule of the Ear.

We report a case of solitary congenital calcified nodule of the ear in a 1-year-old female. The lesion was a 2 X 2mm-sized, elevated, slightly erythematous and whitish-centered nodule located since birth on the superior helical rim of her left ear. Histopathological findings fram the totally excised lesion showed calcium deposits as amorphous, basophilic masses in the upper dermis and focal discharge of calcium by means of transepidermal elimination. Surrounding the calcium deposits, lymphohistiocytic infiltrations were present.

7.The Patterns and Risk Factors of Smoking Relapse among People Successful in Smoking Cessation at the Smoking Cessation Clinics of Public Health Centers.

PURPOSE: The purpose of this study was to investigate the patterns of smoking relapse and to identify risk factors related to smoking relapse among those successful in smoking cessation at the smoking cessation clinics of public health centers. METHODS: Data were collected from 1,705 six-month quitters and analyzed by the Kaplan-Meier analysis and the Cox proportional hazard model. RESULTS: The rate of smoking relapse was 38.2% in 1 year, 44.4% in 2 years, and 47.8% in 5 years. The vast majority of relapse (62.3%) occurred within the first six months after quitting. The risk factors related to smoking relapse were age (HR 1.964: 95% CI 1.545, to 2.497), nicotine dependence (HR 1.293: 95% CI 1.087 to 1.539), problem drinking (HR 1.497: 95% CI 1.116 to 2.008), behavioral therapy type (HR 1.398: 95% CI 1.193 to 1.638), and nicotine replacement therapy type (HR 1.363: 95% CI 1.077 to 1.724). CONCLUSION: For reducing smoking relapse, it is necessary to develop a smoking relapse prevention program for the first six months after quitting and to strengthen behavioral therapy in the course of smoking cessations clinics.

8.A case of hyperimmunoglobuline E syndrome.

The hyperimmunoglobulin E syndrome is a primary immunodificiency disorder characterized by recurrent staphylococcal infections and markedly elevated serum IgE level. Clinical features are coarse face and severe infections of the skin-furunculosis or chronically pruritic dermatitis and sinopulmonary tract infection from infancy by coagulase positive Staphylococcus aureus, or Candida albicans etc. The patients's serum IgE level is elevated but the basic immunologic pathogenesis not fully understood. We have experienced a case of hyperimmunoglobulin E syndrome in a 26/12-year-old who had suffered from recurrent staphylococcal pneumonias and abscesses and chronically pruritic dermatitis from 1 month of age with elevated serum IgE level. A brief review of the related literature is presented.

10.Chronological observation on intestinal histopathology and intraepithelial lymphocytes in the intestine of rats infected with Metagonimus yokogawai.

The relationship between the intestinal histopathology and number and position of intraepithelial lymphocytes (IEL) was observed chronologically in the small intestine of rats experimentally infected with Metagonimus yokogawai. Fifteen Sprague-Dawley rats were orally infected each with 3,000 metacecariae, and 3 were kept uninfected for controls. Three rats each were sacrificed on the day 5, 10, 15, 24 and 70 post-infection (PI) and samples of the small intestine, 5 cm, 10 cm, 20 cm and 70 cm posterior to the pylorus were taken. The samples were processed routinely and stained with Giemsa. The intestinal histopathology was severe during the day 5-15 PI and characterized by villous atrophy, crypt hyperplasia, and decrease of villus/crypt height ratio. After the day 24 PI, the intestinal lesions showed some tendency of recovery. The number of IEL increased at the early stage of infection, but decreased thereafter to a lower level than that of controls, with progression of the pathological changes. Then, the IEL number began to increase again after the day 24 PI. In control rats, the great majority of the IEL were located at the basal region of the epithelium. During the early stage of infection, however, a considerable proportion of IEL was found to have moved to the intermediate or apical region of the epithelium. From the above results, it is suggested that the change of IEL number and position during the course of M. yokogawai infection should be closely related to the progression and recovery of the intestinal histopathology.
