1.Gastric Phycomycosis Presenting as an Ulcerative Lesion within the Early Gastric Carcinoma.

Primary phycomycosis of the stomach within the ulcerative portion of early gastric carcinoma is presented in a sixty one year old male alcoholics. In the center of IIc type submucosal adenocarcinoma was a large deep ulcer (UL-IV) which was the only seat of phycomycetes infection accompanied with angioinvasion and exudative endovasculitis aside from unusual granuloma formation with Splendore Hoeppli phenomenon and eosinophilic response as in allergic granulomatous aspergillosis. The above features were reconstructed to indicate that the phycomycetes infection was confined to the type III area of IIc + III early gastric carcinoma, and that the local ulcerogenic lesion seems more contributory in its development than systemic factors including gastric carcinoma per se or alcoholic consumption.

2.Influence of Job Embeddedness Factors on Turnover Intention of Nurses in Small and Medium Sized General Hospitals.

PURPOSE: This study was done to investigate the influence of organization and community job embeddedness on turnover intention of nurses in small and medium sized general hospitals. METHODS: The participants, 333 nurses, were recruited from small and medium sized general hospitals in Korea. Data were collected by self-report questionnaires on job-embeddedness and turnover intention and were analyzed using descriptive statistics, Independent t-test, One-way ANOVA and Scheffé', Pearson correlation coefficient, and multiple linear regression analysis with the SPSS 18.0 program. RESULTS: The score for job embeddedness and it's 6 factors, and turnover intention were above 3 on a 5 point scale with the exception of organizational sacrifice. There was a significant difference in turnover intention according to age, marital status, salary, and position. There were significant negative correlations between the 6 factors of job embeddedness and turnover intention. Variables entered in multiple regression showed that organizational sacrifice, organizational fit and age were significant contributing factors to turnover intention. CONCLUSION: The findings suggest that there is a need for strategies to enhance job embeddedness, especially organization sacrifice and organization fit. These factors should be developed and used to decrease turnover intention of nurses in small and medium sized general hospitals.

3.A Case of Remission of Systemic Juvenile Rheumatoid Arthritis(Still's Disease) Treated with High-dose Intravenous Gammaglobulin.

High dose intravenous gammaglobuline (IVLG) therapy is effective in some of the autoimmune diseases. Although the exact mechanism of action of IVIG is uncertain, the action as a neutralizing antibody against unknown etiologic agents, the action of blocking of Fc receptors of effector cells, or the action as a antiidiotypic antibody are suggested. We report a case of 12 year old girl with systemic juvenile rheumatoid arthritis who was treated with high dose IVIG and got a remission. In August 1990 she was admitted to our hospital. because of intermittent fever, transient rash and multiple arthralgia. Under the diagnosis of systemic juvenile rheumatoid arthritis, aspirin (4.0g/day) had been given with symptom improvement. She was readmitted in October 1990 because of aspirin intoxication and acute fulminant hepatitis. She was discharged after recovery and any medicine was not prescribed. In November 1990 she was admitted because of epigastric pain, vomiting, intermittent fever, multiple arthritis, and mild hepatomegaly. Total parenteral alimentation had been given under the diagnosis of superior mesenteric artery syndrome and gold sodium thiomalate (Myochrysine, 5 and 10 mg, two weekly IM injection) was given in conjunction with prednisolone (30 mg/day) and naproxen (375 mg/day). She was admitted again in February 1991 due to the fever, coughing, rash, and hepatosplenomegaly. Pneumonia and leukopenia (2100/mm(3)) were found and gold sodium thiomalate injection was discontinued. Gammaglobulin 1 g/kg/day was given intravenously for 2 consecutive days with dramatic symptom improvement. Five more monthly IV gammaglobulin had been given and the side reaction of injection were nausia, fever, and headache which were controlled by the decrease of infusion rate. Four months after the last IVIG injection she had no symtom of arthritis and the hepatosplenomegaly was decreased. Hemoglobin level was increase to 12.2 mg/dL form 6.2mg/dL and ESR was decrease to 15mm/h. The oral prednisolne and ibuprofen were stopped one year after th last IVIG injection. All the laboratory parameters of arthritis and physical examinations had been normal for more than two year after the stop of all the medications until March of 1994. We suggest that high dose intravenous gammaglobulin can be one of treatments for severe systemic juvenile rheumatoid arthritis.

5.The APACHE III Score and Multiple Organ Failure(MOF) Score in Patients who were Recipients of Decision-Making Do-Not- Resuscitate.

PURPOSE: The purpose of this study was to identify characteristics of patients who were recipients of decision-making DNR, to describe the situations of DNR, and to analyze the APACHE III and MOF scores. METHOD: Data collection was conducted through reviews of medical records of 51 patients and through interviews with families of patients who were decision-makers for DNR at C university K Hospital located in Seoul from April to September 2002. RESULTS: The men's APACHE III and MOF scores were higher than the women's and the non cancer patients were higher than cancer patients. Some 80.4% of DNR orders was by communication, while 11.8% of consents were written. Each of APACHE III and MOF scores of patients in the intensive care unit was higher than the patients in general ward at both points of admission and decision-making of DNR. APACHE III and MOF scores positively correlated statistically with each other. CONCLUSIONS: The findings of this study suggest that APACHE III and MOF scores be useful for decision-making of DNR as a tool measuring severity.

6.Factors Influencing of Evidence based Practice Competency and Evidence based Practice Readiness in General Hospital Nurses.

PURPOSE: The study was done to verify evidence-based practice (EBP) readiness and factors contributing to EBP competency in general hospital nurses. METHODS: Participants were 219 nurses working in 7 general hospitals in G-city and J Province. Data were collected from May 12 to May 28, 2015 and analyzed using one-way ANOVA, Pearson's correlation and stepwise multiple regression. RESULTS: Mean score for EBP competency was 3.89±0.83, for critical thinking disposition, 3.35±0.36, for EBP attitude, 3.53±0.66, for beliefs about value of EBP, 3.42±0.56, for ability in research performance, 2.86±0.57, for time to devote to EBP, 2.57±0.69 and for organization support for EBP, 3.36±0.46. EBP competency was significantly correlated with critical thinking disposition (r=.37, p<.001), beliefs about value of EBP (r=.43, p<.001), ability in research performance (r=.44, p<.001), and time to devote to EBP(r=.33, p<.001). Factors influencing EBP competency in general hospital nurses were research performance (β=.29, p<.001), beliefs about value of EBP (β=.24, p<.001), critical thinking disposition (β=.15, p=.014), recognition of EBP (β=-.18, p=.002), which together explained 35% of total variance. CONCLUSION: Findings indicate that introduction and activation of EBP in general hospitals is essential and development of systematic training programs to strengthen critical thinking disposition and EBP competency are needed.

7.Trends in Evidence-based Nursing Research in South Korea.

PURPOSE: The purpose of this study was to analyze the current status of research related to evidence-based nursing and to suggest directions for the future of evidence-based nursing practice, education and research. METHODS: A search was done of 20 research publications, including domestic nursing journals, nursing master's theses and doctoral dissertations before November 2016. Finally, 183 studies were selected. The selected papers were analyzed using descriptive statistics and χ² test with the SPSS/WIN 18.0 program. RESULTS: Most of papers examined in this study were journal articles (80.9%). Meta-analysis (35.0%) was the most common study design. Methodological characteristics were as follows: before 2010 about 5% were documents that suggested Priori' design, generation of PICO, search strategy, quality assessment and description of quality assessment outcome, but after 2011, these designs increased to 30.8%, 73.1%, 41.0%, 91.0% and 65.4%, respectively. The most frequent topics for evidence-based nursing implementation were evidence-based nursing readiness (16 papers). Highest frequency topics in systematic reviews and meta-analysis were studies that confirmed the intervention effect of exercise programs. The highest frequency topics in guideline were temperature control. CONCLUSION: Researchers' perceptions to improve research methodological quality and education to strengthen the research capability are necessary.

9.MRI of the temporomandibular joint using flip back spin echo technique.

The authors introduced flip back spin echo (FBSE) technique for MR imaging of the temporomandibular joint (TMJ) instead of conventional spin echo (SE) technique, and evaluated whether FBSE technique in MRI of TMJ is adequate for the diagnosis of the disorders or not. FBSE T1 parasagittal images in closed mouth state and sequential opening and closing mouth states using patient's own finger(s) were obtained and then FBSE T1 paracoronal image in closed mouth state and STAGE(short tip angle gradient echo) parasagittal T2WI were followed. All 30 images of the symptomatic TMJs using FBSE technique were excellent in the visualization of articular and their displacement, and we could easily diagnose the internal derangement. FBSE technique was adequate for the diagnosis of internal derangement of the TMJ, and the modified cine display from images using FBSE technique was advantageous in the diagnosis of the disorders by visualization of the dynamic motion of the TMJ.

10.A Case of Allergic Contact Dermatitis due to Lanolin.

Lanolin is a complex material consisting of a mixture of numerous esters and polyesters of high-moleculer-weight alcohols and fatty acids, Lanolin allergy is important because lanolin is widely used as an emulsifier in topical medicaments and in numerous cosmetic preparations. We report a case of allergic contact dermatitis in a 27-year-old female. An erythematous pruritic rash develgood around the eyelid developed after applying a new eye cream and was aggravated after applying Cambison ophthalmic ointment. Patch testing showed positive reactions to Cambison ophthalmic ointment, 30% wool alcohol, Amerchol L101 and Nivea cream. A use test with 100% Cambison ophthalmic ointment base was positive
