1.Study on the pharmacological mechanism of sodium ferulate for anti-asthmatic effect in guinea pigs.

OBJECTIVETo study the anti-asthmatic effect of sodium ferulate (SF) and its mechanism in guinea pig asthmatic model.
METHODSGuinea pigs were sensitized with ovalbumin as animal asthmatic model and treated with 3 different concentration of SF for 8 days. Levels of endothelin (ET) and nitric oxide (NO) in blood and lung tissue, and eosinophil (EOS) in blood and bronchoalveolar lavage fluid (BLAF) was counted at the end of trial.
RESULTSSF could significantly lower the ET content and increase the NO concentration in serum and lung tissue, reduce the number of EOS in blood and BLAF (P < 0.05, P < 0.01). Stronger effect was showed in the high dose group.
CONCLUSIONMechanism of anti-asthmatic action of SF might be to increase NO concentration, lower ET content, alleviate EOS infiltration.

2.Clinical analysis of patients with actue renal failure at high altitude

Objective To analyze the etiologies,clinical characteristics and prognostic factors of patients with acute renal failure(ARF)admitted to the hospital at high altitude.Method This retrospective study included clinical data of patients with acute renal failure in the General Hospital of Tibet Military Command from May 2001 to April,2006.Results There were 85 male patients and 63 female patients with mean age(42.4?18.1)years old.Among 148 patients with acquired ARF,52.7% was iatrogenic or nosoeomal origin, demonstrating a trend of increasing.The ARF included pre-renal(n=48,32.4%),renal parenchymal(n= 90,60.8%)and post-renal(n=10,6.8%)in origin.Acute high altitude sickness(n=20)was the major causes of pre-renal ARF.Renal parenchymal ARF could be classified into glomerular vascular lesions(n=24), acute tubular necrosis(n=53),acute interstitial nephritides(n=12),and contusion of unitesticle(n=1).of 90 cases of renal parenchymal ARF,39 patients(43.3%)were induced by medicines.Lithiasis was the major causes of post-renal ARF.The mortality of ARF in our study was 42.6%.The mortality of patients contracted ARF in hospital was much higher than that of patients community ARF in community(55.1 vs 23.6%;P=0.01). There was no significant differences of the mortality between the patients with and without dialysis treatment. Univariate analysis showed that prognosis was correlated with age,the presence of hematuria and oliguria or anuria Hb,and the number of organ system failures.The logistic regression showed that age,Hb and the number of organ system dysfunction were the predictors of mortality.Conlusions The major causes of ARF at high altitude were acute high altitude sickness and the use of medicines with nephrotoxicity.The morbility and mortality of nosocomisl ARF increased significantly.Prevention of MODS is a key management to decrease mortality in severe ARF.

Objective To investigate the expression and significance of tumor necorisis factor related apoptosis induled ligand receptor(TRAILR) in human craniopharyngioma.Methods The expression of TRAILR was determined by immunohistochemistry and in situ hybridization in 24 samples of craniopharyngioma and 16 samples of normal brain tissue.Results With low decoy receptor(DcR) expression in partial craniopharyngioma cells and low death receptor(DR) expression in partial normal brain cells,DR was expressed highly in all craniopharyngioma samples while DcR in most normal brain tissue. 4.Effect of two Pi deficiency syndrome models on the configuration and function of the skeletal muscle in mice.

OBJECTIVETo observe the relation between Pi deficiency syndrome (PDS) and the configuration and functions of extensor digitorum longus (EDL)and soleus (SOL).
METHODSTotally 36 ICR mice were randomly divided into 3 groups according to weight matching principle, the control group, the exhausted group, and the rhubarb group, 12 in each group. Two PDS models were established by either purgation with rhubarb diarrhea (as Group A) or exhausted swimming plus sleep deprivation (as Group B).The cross sectional area (CSA) of type I and II fibers of extensor digitorum longus (EDL) and soleus (SOL), relative proportions of type I and II fibers were measured by m-ATPase histochemical method. The isotonic contraction and the maximum tetanus contraction of EDL and SOL were detected by PowerLab system.
RESULTSCompared with the control group, the body weight, body temperature, and the general health condition of PDS model rats obviously decreased; the spleen index and the thymus index were also lower; the maximal isotonic contraction and the maximum tetanus contraction obviously decreased; the cross section areas of EDL and SOL were reduced with loosely arranged cells. In EDL, the proportion of type I fibers was added and the proportion of type II fibers was lowered. In SOL, there was no change in the proportion of type I and type II fibers.
CONCLUSIONSEDL and SOL were obviously atrophied in the two PDS model mice. The type I fibers of SOL was more significantly atrophied in Group B.

5.Study on causes and treatment of repeated vulvovaginitis in girlhood

Objective To explore the causes and treatment of repeated vulvovaginitis in girlhood in order to improve its prevention and treatment.Methods Fifty-one girls with repeated vulvovaginitis(age≤10 years)admitted to The Second Affiliated Hospital of Sun Yat-sen University from Jan.1990 to Nov.2004 were reviewed retrospectively.Results We found 28 girls(55%)suffering from non-specific vulvovaginitis and 14 ones(27%)suffering from posterior recto-vaginal fistula with in 51 patients.Five girls(10%)were smitten with vulval ulcer and 3 ones(6%)had been were found with vaginal foreign bodies.One girl(2%) was smitten with adhesion of labia minora.The vaginal discharges taken from 21 girls were cultured. Seventeen cases found bacteria.The positive rate of bacteria culture in the 21 cases reached 81%,in which, E.coli accounted for 5 cases(24%),staphylococcus and streptococcus accounted for 3 cases(14%) respectively.Patients suffered from non-specific vulvovaginitis and vulval ulcer accepted external lotion, antibiotic ointment or combining with antibiotics.Patients suffered from posterior recto-vaginal fistula accepted fistulectomy.Three girls who found vaginal foreign bodies took out of foreign bodies by hysteroscopo.Fifty-one girls all were cured after appropriate therapy.Conclusions Vulvovaginitis is the most common gynecologic diagnosis in girlhood.The principal cause of repeated invasion is non-specific vulvovaginitis and the secondly one is posterior recto-vaginal fistula.It need overhaul during the diagnosis.It is very availability to use hysteroscopy and do bacteria culture+antibiotic sensitivity test for repeated pediatric vulvovaginitis.
6.Preliminary study on the value of ratio of serum luteinizing hormone/follicle-stimulating hormone in diagnosis of polycystic ovarian syndrome among women with polycystic ovary

Objective To investigate the value of ratio of luteinizing hormone (LH) to folliclestimulating hormone (FSH) in diagnosis of polycystic ovarian syndrome (PCOS) among women with ploycystic ovary (PCO) and to compare the difference of the diagnostic criteria between the Rotterdam Consensus and the Committee for Reproductive and Endocrine in Japan Society of Obstetrics and Gynecology.Methods By means of transvaginal Doppler ultrasound, 195 women with PCO were diagnosed in Nanfang Hospital of Reproductive Medicine Center and compare difference of multiple clinical indexes according to Rotterdam consensus and Japan consensus respectively. In the mean time, the ratio of LH/FSH, the level of LH, testosterone (T) and recevier operating characteristic (ROC) curve were explored to on the value of diagnosis of PCOS. Results By Rotterdam consensus, 144 women were diagnosed with PCOS and 51 women were non-PCOS, while 111 were identified as PCOS and 84 were non-PCOS according to Japan consensus. LH/FSH in PCOS and non-PCOS were 1.59 ±0. 84 and 0. 85 ±0. 47 respectively when based on Rotterdam consensus, and this ratio were 1.87 ± 0. 76 in PCOS and 0. 78 ± 0. 39 in non-PCOS based on Japan consensus. When using LH/FSH to diagnosis PCOS by Rotterdam consensus and Japan consensus,areas under ROC curve are 0. 786 and 0. 942, respectively. Conclusions The ratio of LH/FSH ≥ 1 provide the significant value in the diagnosis of PCOS. The criteria of the Committee for Reproductive and Endocrine in Japan Society of Obstetrics and Gynecology is more suitable for Chinese patients.
7.Clinical Observation of the Relationship between Retinal Vein Occlusion and Homocysteine

Objective The discussion of plasma homocysteine in retinal vein occlusion (homocysteine,Hcy) level changes,to study whether elevated plasma Hcy is a risk factor for RVO,and provide objective basis for the prevention and treatment of the disease.Methods A case control study of 65 cases were collected after unified ophthalmic examination standard diagnosis of RVO and after disease screening criteria of patients as case group [central retinal vein occlusion (CRVO) in 46 patients,branch retinal vein occlusion (BRVO) in 19 healthy people],and no previous history of retinal vascular diseases.65 cases as the control group,the two groups showed no significant difference in age and sex.The content of plasma Hey after statistical comparison detection.Results The mean plasma Hcy in the RVO case group was significantly higher than that in the control group (t=6.192,P<0.05).There was no significant difference in plasma Hey index between CRVO and BRVO patients in the case group (t=1.536,P>0.05).Conclusion Plasma Hcy is a risk factor for RVO,and a drug that reduces Hcy can be used in the prevention and treatment of RVO.

Objective To explore the effects of modified tubo-uterine implantations performed on women with proximal tubal occlusive infertility after femal sterilization with mucilago phenol.Methods Two hundred and eight infertile women who were admitted to the Second Affiliated Hospital of Sun Yat-sen University between 1986 and 2004 were included.They all accepted modified tubo-uterine implantation after occlusion of fallopian tubes with mucilago phenol.Results It was found that the occlusions were all located in the interstitial portion or isthmic portion of the fallopian tubes.Different degrees of pelvic adhesions were found in 65 cases.Fifty-seven cases were slightly adhesive,seven cases were of moderate degree and one case was severe.One hundred and ninety-nine cases were followed up after operations(95.7%).One hundred and ninety-three women accepted hydrotubation in the following month just after the operation and 185 women were found to be unobstructed(95.8%).One hundred and forty-three women became pregnant, the pregnant rate being 71.9%(143/199).One hundred and twenty-five women had term deliveries (87.4%),three women were in early pregnancy and two in midtrimester pregnancy.Eleven women had spontaneous abortion(7.7%).Two women had tubal pregnancy(1.0%).None of the 199 cases had any signs of endometriosis.Conelusions Modified tubo-uterine implantations are quite effective for proximal tubal occlusive infertility.It may be a favorable method for such kind of tubal occlusions.
10.CT evaluation and classification of the Le Fort fracture

Objective To evaluate the CT findings and classification of the Le Fort fracture. Methods Sixty-two cases with Le Fort fractures were studied with thin-slice high-resolution CT scanning and analyzed with three-dimensional(3D)imaging reconstruction.Results Of the 62 patients,10 had Le Fort type Ⅰ fracture,9 had Le Fort type Ⅱ fracture,8 had Le Fort type Ⅲ fracture,and 35 had various combinations of the three types of Le Fort fractures,including 18 Le Fort Ⅰ+Ⅱ fracture,7 Le Fort Ⅰ+ Ⅱ+Ⅲ fracture and 10 Le Fort Ⅱ+Ⅲ fracture.Fifty-five cases had associated multiple fractures in the maxillofacial region.On 2D CT images,Le Fort fracture manifested as multiple and complex fractures. Though 2D image was better than 3D image in accurately defining tiny fractures and fractures of deep structures,the diagnosis of Le Fort fracture could not be correctly made solely on 2D image.3D CT clearly and stereoscopically demonstrated the entire shape and orientation of Le Fort fracture,thus facilitating the correct classification of Le Fort fracture.Conclusion 3D CT image is important in providing information about the space relationship of Le Fort fracture,thus very valuable for the preoperative planning.
