Chinese Journal of Practical Nursing 2021;37(7):511-516
Objective:To explore the hemodynamic effect of the modified Z Trendelenburg position in patients undergoing laparoscopic radical surgery of endometrial carcinoma, to provide a reference for the selection of the operative position of patients. Methods:A total of 80 patients who undergoing laparoscopic radical surgery of endometrial carcinoma from April 2017 to April 2019 in the Central Hospital of Xiangyang were recruited and assigned to experiment group and control group by random digits table method with 40 cases in each group. Patients in the experiment group were positioned in modified Z Trendelenburg position, while the patients in the control group were placed in conventional Trendelenburg position. The data of heart rate, mean arterial pressure, central venous pressure, systemic circulation resistance, cardiac output were recorded at 10 minutes before pneumoperitoneum (T 0), pneumoperitoneum immediately (T 1) and 5 minutes (T 2), 10 minutes (T 3), 30 minutes (T 4), 60 minutes (T 5) after pneumoperitoneum and 1 minute after recover horizontal position (T 6). Followed up investigation at 24 h after surgery, the degree of pain in the shoulder and the lower limbs were also recorded by Visual Analogue Scale(VAS). Results:The differences of heart rate, mean arterial pressure, systemic circulation resistance and cardiac output between the two groups had no statistical significance at T 0 to T 6 (all P>0.05). The central venous pressure were (9.86±1.95), (10.94±1.53), (11.33±2.24), (11.14±1.54),(7.75±1.59) cmH 2O(1 cmH 2O=0.098 kPa) respectively from T 2 to T 6 in the experiment group, lower than that of control group (18.83±2.82), (16.81±2.93), (16.27±3.56), (15.76±2.33), (8.92±1.68) cmH 2O at the same time, the differences were statically significant ( t values were 3.057-15.736, all P<0.05). The VAS scores of shoulder pain and low leg pain in experiment group were (3.48±1.10), (2.35±0.66) points, lower than that of control group (4.16±1.32), (2.99±1.02) points, the differences were statically significant ( t values were 2.404, 3.228, P<0.05). Conclusions:Modified Z Trendelenburg position can maintain the circulatory system hemodynamic stability, reduce the degree of shoulder pain and low leg pain after surgery in the laparoscopic radical resection for endometrial carcinoma patients.

Objective To investigate the effects of irbesartan hydrochlorothiazide combined with candesartan cilexetil in the treatment of elderly patients with hypertension and its protective effect on cardiac function. Methods 180 elderly patients with hypertension were selected, and they were randomly divided into observation group and control group according to the digital table,90 cases in each group. The observation group was given irbesartan hydrochlorothiazide combined with candesartan treatment, the control group was given candesartan. The heart function improvement after treatment, clinical efficacy and patients in hospitalization, complications, critical events and new death of the two groups were compared. Results After treatment, the heart rate,EVDD,SBP and other indicators of the two groups declined, which of the observation group[(56.14 ± 6.15)mm,(112.12 ± 20.12)mmHg,(70.45 ± 8.69/min)]were lower than the control group[(60.12 ± 6.78) mm(119.45 ± 21.45) mmHg,(82.12 ± 9.12/ min)],there were significant differences between the two groups (t1 =4.124,P =0.000;t2 =2.364,P =0.019;t3 =8.788,P =0.000).The total effective rate of the observation group was 93.33%,which was higher than 74.44% of the control group, there was significant difference between the two groups (x2 =11.879,P =0.000).Conclusion Irbesartan hydrochlorothiazide combined with candesartan in the treatment of elderly hypertensive patients can effectively improve the heart function of patients, has significant effect.

Objective: To investigate the relationship between arterial stiffness and variability of blood pressure (BPV) in senile patients with hypertension.
Methods: A total of 234 senile patients with hypertension were enrolled. The patients were examined by 24h ambulatory blood pressure monitoring (ABPM), BPV was identified by the standard deviation (SD) of blood pressure value. Based on increased percentage of baPWV, the patients were divided into 4 groups: Normal artery group, n=44, Mild arterial stiff group, n=72, Moderate arterial stiff group, n=78 and Severe arterial stiff group, n=40. The 24h ABPM results, BPV, biochemical indexes and their relationship to baPWV were compared among different groups.
Results:①The HDL-C level and 24hDBP, dDBP, 24hSBPSD, 24hDBPSD, dSBPSD, dDBPSD, nDBPSD were significantly different among each group, all P<0.05.②The increased percentage of baPWV was positively related to the age and 24hSBPSD, 24hDBPSD, dSBPSD, dDBPSD, nDBPSD, while negatively related to HDL-C level, all P<0.05.③The age and 24hSBPSD, 24hDBPSD, dSBPSD were the independent risk factors for baPWV elevation.
Conclusion: The arterial stiffness was closely related to BPV in senile patients with hypertension.

Objective To observe the earlier changes of left heart structure and function in patients with Grade 1 hypertension. Methods 65 patients with Grade 1 hypertension were measured on the left atrial diameter(LAD),left ventricular diameter, thickness of ventricular, mass index and evaluate the left ventricular systolic and diastolic function by UCG.Results Compaired to 50 normal controls, the LAD and left ventricular diastolic function of the patients(38.06?3.68 mm,0.72?0.11) was significantly different with those of 50 normal controls(29.86?3.08 mm,1.18?0.08)(P

Objective To summarize our experience on laparoscopic appendectomy combined with high ligation of the vaginal process in children. Methods From January 2004 to May 2007,totally 326 patients with appendicitis underwent laparoscopic appendectomy in our hospital.During the operation,by inguinal exploration,83 cases of vaginal process deformity (17 bilateral and 69 unilateral lesions) were confirmed.High ligation of the vaginal process was performed on these patients. Results The operation was successfully done in all of the 83 cases, none of them showed postoperative complications. The cases were followed up for 6 to 12 months, during which only one patient had recurrent inguinal hernia.Three patients developed latent hernia but receined no treatment. Conclusions Laparoscopic appendectomy combined with high ligation of the vaginal process is an safe, effective, and low-cost procedure, leading to quick recovery and less pain for children with appendicitis complicated with vaginal process deformity.

Objective To evaluate the value of laparoscopic surgery application in treating intussusception. Methods Between January 2004 and December 2007,12 children with refractory (8 cases, failed air enema) or recurrent (for over 3 times) intussusception received laparoscopic surgery in our hospital.Three trocars were used during the surgery.Under a laparoscope,by interrupted suture, the terminal ileum was joined to the ascending colon (3 stitches),and the ileocecal junction was fixed to the right posterior abdominal wall (2 to 3 stitches, only for children that failed air enema). Results The intraoperative blood loss ranged from 20 to 60 ml,and the operation lasted for 45 to 90 minutes. No patient developed anastomotic leakage or abnormal gastrointestinal function. The patients were followed up for 1 to 5 years (mean,2 years and 9 months),during which none of them had recurrence. The children had normal intake and excretion, and developed well. Conclusions Children with intussusception recover quickly after laparoscopic treatment with a few postoperative complications.
