1.Roles of depression and C-reaction protein in early development of cardiovascular disease in the elderly diabetes patients

Objective To investigate the role of depression and C-reaction protein (CRP)in the early development of vascular disease in the elderly patients with type 2 diabetes. Methods Totally 396 patients with type 2 diabetes were recruited in the study,including 196 cases with depression and 200 cases without depression.Depression was evaluated by Hamilton depression scale (HAMD),and CRP was measured using rate nephelometry.The related parameters of wave intensity (WI) such as accelerating wave intensity (W1),decelerating wave intensity (W2),arterial stiffness β,pressure-strain elasticity modulus (Ep),arterial compliance (AC),pulse wave velocity -wave intensity(PWVWI),pulse wave velocity (PWV) and argumentation index (AI) were detected and compared between the groups. Results The levels of CRP [(8.1±2.8)mg/L vs. (3.9±3.2) mg/L,t=6.411,P=0.000],W2 [(4.82±1.33) mm Hg· m-1 · s-3 vs.(2.27±1.28)mm Hg · m-1 · s-3,t=4.256,P=0.021],arterial stiffness β(12.58±3.23 vs.8.96±2.24,t=4.634,P=0.019),Ep (157.90±52.64 vs.118.24±53.08,t=5.512,P=0.007),PWVβ(7.53± 1.41 vs.6.68±1.56,t=3.982,P=0.024),AI(19.26± 13.21 vs.15.34±10.23,t=3.213,P=0.037)and PWV-WI (7.59±1.67vs.6.63± 1.45,t =2.613,P =0.046) in cases with depression were higher than the cases without depression,while the AC value was reduced (0.70±0.19 vs.0.78±0.21,t=4.676,P =0.017).There was no difference in W1 value between the groups.The CRP level and HAMD score were positively associated with W2 (r=0.470,P=0.000),stiffness β (r=0.582,P=0.000),Ep (r=0.352,P=0.004),PWV-WI (r=0.233,P=0.019),PWVβ (r=0.340,P=0.007) and AI (r=0.265,P=0.015),but negatively related with AC(r=-0.457,P=0.000).HAMD score was positively correlated with CRP level(r=0.328,P=0.003). Conclusions The carotid artery wave intensity W2,blood vessel elasticity parameter β,Ep,AC,PWV-WI,PWVβ and AI determination can evaluate the risk of cardiovascular diseases in elderly diabetes,and higher CRP level and HAMD score may deteriorate its process.
2.Micro satellite instability in the development of gastric cancer

Objective To detect the micro satellite instability (MSI) from the precancerous and cancerous lesions (GC) and its effect on carcinogenesis.Methods Silver staining single strand conformation polymorphism polymerize chain reaction (PCR SSCP) was used to screen MSI markers at 5 loci in formalin fixed,paraffin embedded tissues of GC (n=30), dysplasia (n=30), intestinal metaplasia (IM) ( n =40) and corresponding normal gastric tissues.Results The abnormal shifting of the single strand DNA was identified in 7(23 3%)out of GC, in 9 (30%) out of dysplasia and in 8(20%) out of IM samples respectively. GC with MSI was associated with distal location of the tumors ( P =0 044). MSI was not detected in low grade IM tissues.Conclusion MSI may be of an early event during gastric carcinogenesis. It contributes probably to the acquisition of a transformed cell phenotype and to the development of gastric cancer.

Red blood cell sorbitol contents were determined with a enzymatic method in 37 diabetics and 19 controls. The fasting blood glucose (FBG) levels and RBC sorbitol contents in the diabetics were significantly higher than that in the controls. RBC sorbitol was positively correlated to FBG, but not to the age of the patients or the type and duration of diabetes mellitus. 4.Analysis of monitoring results of coal-burning borne endemic fluorosis in Fuyuan county Qujing city in 2009

ObjectiveTo explore the prevalence state of children in coal-burning borne endemic fluorosis in Fuyuan county Qujing city,to know the implementation of preventing measures,and to provide a scientific basis for prevention of endemic fluorosis.MethodsIn 2009,105 families were sampled to check the service condition of defluoridation stove and the effect of health education (the awareness of use defluoridation stove reasons) in Xiaoyuwang,Tuoga and Shaliang villages,Fucun town of Fuyuan county.Food fluorine of indoor stored peppers and corns was monitored of 20 families in Xiaoyuwang village,5 household were randomly selected from east,west,south,north and middle places of the village.Air fluorine was monitored from indoor air samples of 5families,dental and urinary fluorine was monitored of the native-born children aged 8 to 12,and dental fluorosis of these children was checked.Clinical endemic fluorosis and X-ray endemic fluorosis was monitored of local adult residents over the age of 30 that had been lived in the area for more than 5 years.ResultsThe rate of correct use of the defluoridation stove was 100%,and adult awareness rate of use defluoridation stove reasons was 82.86%(87/105).Of the 19 pepper samples,fluorine content were 9.98 - 538.40 mg/kg and median was 32.38 mg/kg,all exceeded the standard(1.5 mg/kg).Of the 25 corn samples,fluorine content were 0.42 - 101.54 mg/kg and median was 6.32 mg/kg,with 18 samples exceeded the standard(1.0 mg/kg).Of the 6 air samples,fluorine content were 0.000 98 - 0.003 33 mg/kg and median was 0.001 45 mg/kg.Detection rate of dental fluorosis of children aged 8 - 12 was 42.3%,urinary fluorine content were 0.13 - 1.80 mg/L,geometric mean was 0.58 mg/L,4 adults were diagnosed clinical skeletal fluorosis,detection rate was 2.46%(4/162),5 adults were found by X-ray with endemic fluorosis,detection rate was 9.61%(5/52).ConclusionPeppers and cons fluorine pollution is serious,general public should be guided to change their lifestyles to minimize the fluorine pollution.

This article reviews the health-related home safety assessment methods for the elderly, and briefly introduces the development process of the "Person-environment fit" model from the concept, to the risk assessment method of environmental factors and then to the interactive risk assessment method of human-environment factors.Especially, this article introduces several risk assessment methods of residential environment, such as "The Enabler" human function and environmental obstacle assessment tool, the Safety Assessment of Function and the Environment for Rehabilitation(SAFER), the Safety Assessment of Function and the Environment for Rehabilitation-Health Outcome Measurement and Evaluation(SAFER-HOME), the Westmead Home Safety Assessment(WeHSA), the Home Falls and Accident Screening Tool(HOME FAST), the Cougar Home Safety Assessment(CHSA), the in-Home Occupational Performance Evaluation(I-HOPE), the In-Home Occupational Performance Evaluation for Providing Assistance(I-HOPE Assist), the In-Home Medication Management Performance Evaluation(HOME-Rx).
8.Study on treatment of dopamine extravasation-induced injury by lidocaine combining dexametha sone

Objective To discuss the treatment of dopamine extravasation-induced injury by lidocaine combining dexamethasone. Methods Animal models of dopamine extravasation-induced injury were made using 24 limbs of 6 white mice. These models were divided into 4 groups and given different treatments. Group A was treated with lidocaine combining dexamethasone, group B with lidocaine, group C with normal saline and group D was set as control. The treatment effect was observed. Results The disappearance time of edema in group A was the shortest among the 4 groups and was significantly shorter than that of group C,P<0.05.Moreover, the pathological changes degree of local tissues in group A were also obviously lower than those of group B,C and D. Conclusion Measures must be taken timely to cope with dopamine extravasation-induced injury. Treatment with lidocaine combining dexamethasone proved to be a prompt, simple and safe method.
9.Survey on activities of daily living and analysis of the influencing factors among elder hospitalization patients

Objectives To investigate the activities of daily living and analysis of the influencing factors among elder hospitalization patients.Methods A series of scales including Activity of Daily Living Scale,Mini-Nutritional Assessment,Mini Mental State Examination,Geriatric Depression Scale 15,Falls Risk Assessment Tool and Euro Qol-5D Questionnaire were used to assess the physical and psychological health status among elder patients at first hospitalization,as well as their social-economic status,and to analyze the activities of daily living and influencing factors.Results Interviewees with less than nine years educational level had 5.93 (95 % CI:1.14-33.23)times higher risk for impaired activities of daily living than those with higher education levels.Patients with EQ-5D score ≥1 scale or with cognitive disorder showed 2.86 (95% CI:1.04-8.05) times or 18.14 (95 % CI:1.64-540.63) times higher risks for impaired activities of daily living than their normal controls,respectively.The risks for the decreasing activities of daily living were increased along with ageing (P =0.025).There was a marginal correlation between regular exercise and activities of daily living(P=0.104).Conclusions Educational level,health status and cognitive function are important factors for activities of daily living among elder hospitalization cases and enlarged sample size is required for validation.
10.Efficacy of puerarin for prevention of brain injury in patients undergoing open heart surgery under cardiopulmonary bypass

Objective To evaluate the efficacy of puerarin for prevention of brain injury in the patients undergoing open heart surgery under cardiopulmonary bypass (CPB).Methods Thirty ASA physical status Ⅲ patients of both sexes,aged 50-64 yr,weighing 50-75 kg,scheduled for elective cardiac valve replacement under general anesthesia,were randomly divided into 2 groups (n =15 each) using a random number table:control group(group C) and puerarin group (group P).Puerarin 400 mg was infused intravenously over 30 min immediately at skin incision in group P and the equal volume of 5% glucose-sodium chloride injection was given in group C.Before induction of anesthesia (T0),at 30 min of CPB (T1),immediately after aortic unclamping (T2),and at 6,12 and 24 h after termination of CPB (T3.5),blood samples were collected from the jugular venous bulb for determination of the plasma concentration of neuron-specific enolase (NSE) and S-100 β protein.P300 eventrelated potential latency and amplitude were determined and cognitive function was assessed by Mini-Mental State Examination (MMSE) score at 1 day before surgery and 7 days after surgery.The patients were diagnosed as having post-operative cognitive dysfunction when MMSE score before surgery-MMSE score after surgery ≥ 2,or when M MSE score≤ 24.Results Compared with group C,the concentrations of plasma NSE and S-100 β protein were significantly decreased at T15,and P300 event-related potential latency was shortened,P300 event-related potential amplitude was increased,and the incidence of postoperative cognitive dysfunction was decreased at 7 days after operation in group P.Conclusion Puerarin can prevent the brain injury in the patients undergoing open heart surgery under CPB.
