Objective Tetralogy of Fallo( TOF) is one of cyanotic congenital heart disease, which is difficultly diagonosed because of its complicated texture.The aim of this study was to evaluate the manifestation and value of echocardiography in diagnosis of TOF. Methods The fetal sonograms of 99 fetuses with prenatal diagnosis of TOF in 5791 gravida were reviewed and analyzed from January 2010 to December 2012. Results Ninty nine fetuses with TOF were all diagnosed accurately by prenatal ultrasonography, including 11 cases with pulmonary atresia, 2 cases with complete atrio-ventricular septal defect and 4 cases with permanent left superior vena cava.The key echocardiographic features of TOF were ventricular septal defect, overriding aorta and pulmonary stenosis.TOF was accurately diagnosed by prenatal echoeardiography combined with visualization of TOF in four chamber view, outflow tract view and three vessels view.Sixty eight fetuses were confirmed to be TOF by autopsy or operation after birth, 12 cases refused to autopsy and 19 cases lost of follow up in the total 99 cases.None of TOF was reported to us in other 5692 gravidas. Conclusion The echocardio-graphy has important clinic value in diagnosis of fetal TOF with high sensitivity and specificity.

OBJECTIVE To explore the value of the continuous quality improvement(CQI) in the quality control of disinfection supply center.METHODS The CQI was applied to every aspect of disinfection supply center.We analyzed and identified the reason of the existence of quality problems,and took CQI measures and the implementaion of quality improvement.RESULTS After 5 years CQI,quality control of disinfection supply center had been remarkably improved.The harmony had increased among staff year by year.The professional knowledge rose from 60% to 100%,the rate of monitoring raised from 70% to 100%,the pass rate of sterilized package raised from 55% to 95%,the satisfaction rate of the relevant sections raised from 85% to 100%.The errors and accidents were eliminated or reduced owing to pay attention to quality control.CONCLUSIONS CQI plays an important role on medical service safety and can effectively improve the medical safety,medical quality and service quality.

Objective To investigate the role of depression and C-reaction protein (CRP)in the early development of vascular disease in the elderly patients with type 2 diabetes. Methods Totally 396 patients with type 2 diabetes were recruited in the study,including 196 cases with depression and 200 cases without depression.Depression was evaluated by Hamilton depression scale (HAMD),and CRP was measured using rate nephelometry.The related parameters of wave intensity (WI) such as accelerating wave intensity (W1),decelerating wave intensity (W2),arterial stiffness β,pressure-strain elasticity modulus (Ep),arterial compliance (AC),pulse wave velocity -wave intensity(PWVWI),pulse wave velocity (PWV) and argumentation index (AI) were detected and compared between the groups. Results The levels of CRP [(8.1±2.8)mg/L vs. (3.9±3.2) mg/L,t=6.411,P=0.000],W2 [(4.82±1.33) mm Hg· m-1 · s-3 vs.(2.27±1.28)mm Hg · m-1 · s-3,t=4.256,P=0.021],arterial stiffness β(12.58±3.23 vs.8.96±2.24,t=4.634,P=0.019),Ep (157.90±52.64 vs.118.24±53.08,t=5.512,P=0.007),PWVβ(7.53± 1.41 vs.6.68±1.56,t=3.982,P=0.024),AI(19.26± 13.21 vs.15.34±10.23,t=3.213,P=0.037)and PWV-WI (7.59±1.67vs.6.63± 1.45,t =2.613,P =0.046) in cases with depression were higher than the cases without depression,while the AC value was reduced (0.70±0.19 vs.0.78±0.21,t=4.676,P =0.017).There was no difference in W1 value between the groups.The CRP level and HAMD score were positively associated with W2 (r=0.470,P=0.000),stiffness β (r=0.582,P=0.000),Ep (r=0.352,P=0.004),PWV-WI (r=0.233,P=0.019),PWVβ (r=0.340,P=0.007) and AI (r=0.265,P=0.015),but negatively related with AC(r=-0.457,P=0.000).HAMD score was positively correlated with CRP level(r=0.328,P=0.003). Conclusions The carotid artery wave intensity W2,blood vessel elasticity parameter β,Ep,AC,PWV-WI,PWVβ and AI determination can evaluate the risk of cardiovascular diseases in elderly diabetes,and higher CRP level and HAMD score may deteriorate its process.

Objective To detect the micro satellite instability (MSI) from the precancerous and cancerous lesions (GC) and its effect on carcinogenesis.Methods Silver staining single strand conformation polymorphism polymerize chain reaction (PCR SSCP) was used to screen MSI markers at 5 loci in formalin fixed,paraffin embedded tissues of GC (n=30), dysplasia (n=30), intestinal metaplasia (IM) ( n =40) and corresponding normal gastric tissues.Results The abnormal shifting of the single strand DNA was identified in 7(23 3%)out of GC, in 9 (30%) out of dysplasia and in 8(20%) out of IM samples respectively. GC with MSI was associated with distal location of the tumors ( P =0 044). MSI was not detected in low grade IM tissues.Conclusion MSI may be of an early event during gastric carcinogenesis. It contributes probably to the acquisition of a transformed cell phenotype and to the development of gastric cancer.

Red blood cell sorbitol contents were determined with a enzymatic method in 37 diabetics and 19 controls. The fasting blood glucose (FBG) levels and RBC sorbitol contents in the diabetics were significantly higher than that in the controls. RBC sorbitol was positively correlated to FBG, but not to the age of the patients or the type and duration of diabetes mellitus. Objective To elucidate the deleterious diabetogenic effect of tacrolimus on Chinese Han people.Methods According to patients' whole blood trough levels,the concentration of tacroli- mus was 5,15 and 25?g/L.Human islet cells cultured in vitro were exposed to various concentra- tions of tacrolimus for 72 h respectively,then stimulated by glucose at low (2.8 mmol/L) and high (16.7 mmol/L) levels.Glucose-stimulating insulin secretion during subsequent static incubation was measured using the ultrasensitive human insulin ELISA kit.The assessment of islet cell viability was studied with acridine orange (AO)-propidium iodide (PI).According to fasting plasma glucose,461 subjects were divided into normal glucosetolerance group (FPG＜5.55 mmol/L),impaired fasting glu- cose group (5.55 mmol/L＜FPG＜6.88 mmol/L) and posttransplant diabetes mellitus group (FPG≥6.89 mmol/L).A retrospective review of 461 non-diabetic kidney recipients completing at least 12 months of follow-up was performed to determine risk factors,incidence,and characteristics of post- transplant diabetes mellitus (PTDM).Results In vitro,lower tacrolimus concentration had not dele- terious effect on insulin secretion and viability of islet;high tacrolimus impaired?-cell survival as well as insulin secretion.Of the 461 patients with no history of diabetes at transplantation over the study period of 12 months,123 received tacrolimus and 338 received cyclosporine A (CsA).For patients receiving tacrolimus the cumulative incidence of PTDM was 13.8%,compared with 7.7% for pa- tients receiving CsA,showing no significant difference between them.The reduced tacrolimus level within the therapeutic window resulted in an increase in better glucose metabolism.Older age,family history of diabetes,acute rejection episodes and pre-transplant impaired fasting glucose were identified as risk factors for PTDM.Conclusion The diabetogenic effect of TAC is dose-dependent,and in the majority is reversible.Low initial doses,more rapidly tapering and lower maintenance levels of tacroli- mus decrease the incidence of PTDM in patients receiving tacrolimus.

Objective To study the histocompatibility of domestic new type nitinol alloy ventricular septal defect(VSD) occluder in order to test the feasibility for clinical application.Methods The domestic new type nitinol alloy VSD occluder was inserted into 20 healthy dogs with intravenous balanced anesthesia and tracheal intubation,assisted by animal breathing machine respiration.The dogs were given oral antibiotic and would be killed in several months after operation.According to different slaughter time(1,2,3,6 months),they were divided into 4 sections and including 5 dogs in each section.The occluder sites,the intimal coverage and the change of inflammatory cells were observed through gross anatomy,with the help of the light microscope and electron microscope.SPSS 11.0 software was used to analyze the data.Results The thickness of the endocardial enhanced with the time(P
