2.Clinical Observation of Chemotherapy Combined with Bevacizumab in the Treatment of Colorectal Cancer Liver Metastases

OBJECTIVE:To observe therapeutic efficacy and liver toxicity of chemotherapy combined with bevacizumab in the treatment of colorectal cancer liver metastases (CLMs). METHODS:126 CLMs patients were selected and randomly divided into chemotherapy group(54 cases)and combination group(72 cases). Chemotherapy group received FOLFOXIRI regimen:irinotecan 165 mg/m2+oxaliplatin 85 mg/m2+calcium folinate 200 mg/m2+fluorouracil 2 g. Combination group was additionally given bevaci-zumab 5 mg/kg one day before chemotherapy on the basis of FOLFOXIRI regimen. A treatment course of 2 groups lasted for 2 weeks,and both received 12 courses of treatment. The pathological response,survival rate and toxic reactions caused by chemother-apy were compared between 2 groups. RESULTS:Pathological complete response proportion,tumor remission rating(TRG),the proportion of patients with tumor necrosis rate ≥50% in combination group were significantly higher than chemotherapy group, with statistical significance (P<0.01). The proportion of TRG4-5,TRG 1-3 progression-free survival in combination group were significantly higher than chemotherapy group,with statistical significance (P<0.05). The incidence of liver parenchymal necrosis in combination group was significantlg higher than chemotherapy group,there was statistical significance (P>0.05). CONCLU-SIONS:Chemotherapy combined with bevacizumab can improve pathological response and the incidence of CLMs necrosis and doesn't increase liver toxicity.
3.The study on the relationship of occupational burnout and coping style among general practitioners

Objective To investigate the status of occupational burnout among general practitioners (GPs) and explore the relationship between coping style and burnout. Methods 243 GPs were assessed by Chinese Maslach burnout inventory(CMBI) and simple coping style questionnaire. Results 36.6% of GPs experienced mild burnout, 26.8% moderate burnout, and 3.3% severe burnout; 11.5% of GPs got high scores on emotional exhaustion(EE), 44.9% subjects got high scores on depersonalization(Dp) and 43.6% subjects got high scores on reduced personal accomplishment (RPA). Male GPs experienced greater personal accomplishment than female. Among technical post, subjects with senior technical post scored higher on Dp. The condition of occupational burnout among subjects in north Jiangsu was more serious. GPs with seniority of 5 to 9 years seemed to be more prone to occupational burnout. The coping style of GPs had obvious correlation with occupational burnout. Conclusion The status of occupational burnout among GPs is serious. To prevent and cure occupational burnout, more attention should be paid to female, senior technical post GPs and'persons with seniority of 5 to 9 years. And taking positive coping style can reduce GPs' occupational burnout.
4.THE ANTITUMOUR AND IMMUNOMODULATING EFFECT OF EXOPOLYSACCHARIDES PRODUCED BY LACTOBACILLUS DELBRUECCKII SSP.BULGARICUS

Objective To study the antitumour, immunomodulating and enterobacteria regulating effects of exopolysaccharides (EPS). Method The suppressive rate of growth of sarcoma 180, the index of immunity, and the intestinal microflora were determined. Results (1) EPS inhibited the growth of sarcoma 180 by oral for BALB/c mice significantly. The suppressive rate at the dose of 120mg/kg bw?d was 42.7%. (2) At the dose of 120mg/kg bw?d the index of thymus and IL-2 in serum increased significantly. There was increasing trend in respect of TNF-? and killing activation of NK cell. (3) The amounts of Lactobacillus in groups treated with EPS increased significantly, and the amounts of Enterobacteriacea and Enterococcus droped significantly. Conclusion EPS had the antitumour effect in vivo, and could modulate the immune function, improve the microbial balance in the intestinal tract of tumour bearing mice to some extent. So the antitumour mechanism was probably correlated with the immunomodulating effect.
5.Effect of "Dispersing Lung-qi and Relieving Asthma,Warming Yang to Induce Diuresis" Method on ADH of CPHD Patients during Acute Period with Peripheral Edema

Objective To evaluate the effectiveness of "dispersing lung-qi and relieving asthma, warming Yang to induce diuresis" method on CPHD patients during acute period with peripheral edema and observe the effect on ADH. Methods The CPHD patients during acute period with peripheral edema were randomly divided into treatment group and control group. The treatment group received Chinese medicine adding to regular western medicine, the principle is dispersing lung-qi and relieving asthma, resolving phlegm to stop coughing, warming Yang to induce diuresis. The control group received placebo adding to regular western medicine. Both of the groups were treated for 14 days. The effect observed indexes were as follows:general curative effect, integral of main symptoms of TCM, integral of western medical signs, serum electrolyte, etc. Mechanism index was about ADH. Results The treatment group had better effects than the control group on the general curative effect, reducing integral of symptoms and signs, rectifying electrolyte and acid-alkaline-balance disturbances, etc. The safety analysis of two groups had no significant abnormality. Meanwhile, the treatment group had more superiority than the control group on reducing serum ADH. Conclusion The comprehensive treatment of traditional Chinese and western medicine probably has more advantage in increasing the general effect, improving symptoms and signs, rectifying electrolyte and acid-alkaline-balance disturbances, etc. This therapeutic method can restrain secretion of ADH. It indicates that restraining secretion of ADH maybe part of the mechanism of using "dispersing lung-qi and relieving asthma, warming Yang to induce diuresis" method to treat CPHD patient during acute period with peripheral edema.
6.Cultural Conditions for Production of Glutathione by Mutant Saccharomyces J-X25

The production conditions of glutathione with shaking flask fermentation by the mutant Saccharomyces J-X25, a methionine-defected strain were studied, and the optimum culture conditions are as follows: initial pH6. 0, temperature 30℃, 100ml in 500ml flask, the inoculum size 10% and agitation rate 220r/min. The emphasis was on the stimulating effect on the cells by dioxogen and the sodium lactate as surfactant. Both of which were added at the logarithmic phase of fermentation, and the GSH production was up to 0. 253g/L , 52% higher than the control without the additions. Compared with the production of GSH initial strain that by the mutant in optimum conditions was increased by 79%.
7.Biological ingredient analysis of traditional Chinese medicines utilizing metagenomic approach based on high-throughput-sequencing and big-data-mining.

The quality of traditional Chinese medicines (TCMs) has been mainly evaluated based on chemical ingredients, yet recently more attentions have been paid on biological ingredients, especially for pill-based preparations. It is a key approach to establish a fast, accurate and systematic method of biological ingredient analysis for realization of modernization, industrialization and internationalization of TCMs. The biological ingredient analysis of TCM preparations could be abstracted as the identification of multiple species from a biological mixture. The metagenomic approach based on high-throughput-sequencing (HTS) and big-data-mining has been considered as one of the most effective methods for multiple species analysis of a biological mixture, which would also be helpful for the analysis of biological ingredients in TCMs. Simultaneous identification of diverse species, including the prescribed species, adulterants, toxic species, protected species and even the biological impurities introduced through production process, could be achieved by selecting appropriate DNA biomarkers, as well as applying large-scale sequence comparison and data mining. By this approach, it is prospective to offer an evaluation basis for the effectiveness, safety and legality of TCM preparations.

She regards the basic pathogenesis of breast cancer is not only on traditional liver-kidney yin deficiency and liver-qi stagnation with spleen deficiency, but also three mechanisms which are breakdown of the normal physiological coordination be-tween the liver and kidney, heart and the kidney and Shaoyang. Professor Y ang gradually formed Tri-Traffic Dredg-ing Liver And Nourishing Kidney Formula base on tonification and purgation in combination. Y ang Y ufei thinks highly of combining treatment of TCM and western medicine, especially combined with endocrine therapy. 9.Relationship among HLA-A alleles,supertype and the susceptibility of cervical cancer

Objective To study the relationship among HLA-A alleles,supertype,HPV infection and cervical cancer in Tu Nationality of Hubei province.Methods As a case-control surevy. The comparisons included the comparison between HPV positive cases and HPV positive women in control group,and the comparison between HPV positive cases and HPV negative women in control group. Number of cases was 100(HPV positive in 86),and control was 187 (HPV positive in 95 and HPV negative in 92). The most polymorphism of 2 and 3 exons of the HLA-A alleles were analyzed by the high-resolution typing method-sequence-based typing(SBT).Results Comparison between HPV positive cases and HPV positive control women. Supertype HLA-A3(Pcorrected=0.005,OR=2.36,95% CI=1.45~3.85) was risk factors. Comparison between HPV positive cases and HPV negative control women,HLA-A*0206 alleles (Pcorrected=0.025,OR=0.20,95% CI=0.07~0.58)supertype HLA-A2 (Pcorrected=0.005,OR=0.57,95% CI=0.37~0.88)was protective factor. Supertype HLA-A3 (Pcorrected=0.005,OR=2.36,95% CI=1.45~3.85) was also related to the susceptibility of cervical carcinoma.Conclusion Supertype HLA-A3 is a risk factor of cervical cancer.
10.Analysis of BRAF V600E mutation in bilateral well-differentiated thyroid carcinoma

Purpose To investigate the correlation between BRAF V600E mutation and clinicopathologic parameters in bilateral welldifferentiated thyroid carcinoma.Method Collected 80 patients with papillary thyoid carcinoma (PTC) (67 patients with classical type,8 patients with follicular variant,3 patients with oncocytic variant,2 patients with tall cell variant),5 patients with follicular carcinoma and 30 normal tissues near the tumor,BRAF V600E mutation was detected via polymerase chain reaction.Result The presence of BRAF V600E mutation was found in 52 patients with PTC (65.0%),the mutation was not in the 5 patients with ollicular carcinoma as well as in the 30 normal tissues near the tumor.The BRAF V600E mutation was positively correlated with age,capsule invasion,lymph node metastasis,tumor clinical stage.In the histological variant of PTC,the BRAF V600E mutation rate of classical type and tall cell variant is very high (70.1%,100.0%),but the BRAF V600E mutation rate of follicular variant is very low (33.3%).Conclusion The BRAF V600E mutation rates are different in different age groups.The BRAF V600E mutation was positively correlated with capsule invasion,lymph node metastasis,tumor clinical stage.The BRAF V600E mutation rate of classical type and tall cell variant is very higher than follicular variant.
