3.A Study on the Perinatal Outcomes of Natural Childbirth Led by Midwives: A Retrospective Study

Purpose:
The purpose of this study was to suggest the importance of natural childbirth by investigating the results of midwife-led perinatal periods and whether they were positive experiences for both individual women and their families.
Methods:
This retrospective study aimed to confirm the perinatal outcomes of midwife-attended vaginal births. Among 1,127 women who gave midwife-led birth in Gyeonggi-do, Seoul between January 1, 2019 and December 31, 2021, data were collected for 829 women. Records were excluded that were missing necessary data. Data on childbirth were analyzed using frequency, percentage, mean, standard deviation, and the chi-square test.
Results:
Among the mothers who underwent natural childbirth, 549 (66.2%) were multiparous. In addition, all 829 patients (100%) breast-fed their infants. First-degree perineal lacerations occurred in 29.2% of patients, and second-degree lacerations occurred in 48%. Non–high-risk subjects made up 56.0% of the study sample. The average bleeding amount was 281.44±4.50, and the average 1-minute Apgar score for infants was 8.25±0.02.
Conclusion
Compared to childbirth in a hospital, natural childbirth was associated with a lower risk of birth canal laceration, postpartum bleeding, and Cesarean section. In addition, the health statuses of the newborns were confirmed to be good . Based on the results of this study, there is a need for a strategy in which midwife-led natural childbirth is integrated into the local medical system, as well as a need to raise women's awareness of natural childbirth and positively influence the experience.
4.A Case of Adrenocortical Carcinoma with Remarkably High Concentrations of 11-dexycortisol.

The adrenocartical carcinoma is a rare tumor with an estimated incidence of 1 case per 1,700,000 population. Despite its rarity, a large number of investigators have studied this neoplasm for the following two reasons. The First is the occasional difficulty of differentiation between careinoma and adenoma at the time of initial surgery, even by histopathologic examination. The other is its unique feature of corticosteroidogenesis. Steroid metabolism of adrenocortieal carcinoma is characterized by its low efficiency of steroid production due to deficiency of steroidogenic enzyme. The deficieney of 11 B-hydroxylase has been indieated in case of adrenoeortical carcinoma by several investigators. In this study, the serum level of cortisol was within normal range, but the serum level of 11-deoxycortisol was 50 times higher than normal. After the removal of tumor, the serum level of ll-deoxycortisol was markedly decm. In conclusion, the results from the this case suggest that measurement of serum 11-deoxy- cortisol may be a useful tool in the diagnosis and follow-up of adrenocortical carcinoma.

5.The Morphometric Study of the Pons and Cerebellum in Korean using MRI.

PURPOSE: To evaluate the size of normal pons and cerebellum in vivo and the change in size according to age, and to compare those with measurement of the diseased pons and cerebellum. MATERIALS AND METHODS: 121 normal adults(M:F=54:67), 5 patients with OPCD and 19 patients with WallerJan degeneration were studied. The normal group was divided into 5 subgroups according to the age (ranged from 20 to 72 years). 1.5T GE Signa MR unit was used. On axial plane, the AP(A) and transverse(B) diameters of the pons, the size of the middle cerebellar peduncle(C), and transverse diameter of the posterior fossa(D) and the cerebellum(E) were measured. On midsagittal plane, the longitudinal(F) and AP(G) diameters of the basis pontis were measured. The ratios of E/D and F/G were calculated. The student t test was used for statistical analysis. RESULTS: C, E and F/G were 15.5mm+/-1.3, 99.8mm+/-4.3 and 1.63+/-.10, respectively. F/G, H/I, and H/J were larger in male(p<.01). All data of the pons showed no statistically significant differences among age groups. E of the seventh decades was shorter than that of the third decades(p<.05). C(12.7 mm+/-1.4) in OPCD and F/G(1. 81+/-.10) in Wallerian degeneration(p<.01) showed the most significant differences when they were compared to the normal. CONCLUSION: Although the cerebellum decreased in size with age, the pons maintained its size up to eighth' decades. The measurement of middle cerebellar peduncle on axial plane (C) and the ratio of basis pontis on midsagittal plane (F/G) were important in the evaluation of OPCD and WallerJan degeneration, respectively.

6.Effects of Tailored Case Management using a Gatekeeper for the Depressed Single-household Elderly Population: Focusing on the Mental Health Case Management.

PURPOSE: The aim of this study was to evaluate the effects of tailored case management using a gatekeeper on depression and life satisfaction in the single-household elderly population. METHODS: The design of Quasi experiment was applied to compare the variables before and after the management. Ninety-seven people who had depression categorized by the Korean Geriatric Depression Scale. RESULTS: Depression (t=11.22, p<.001) and life satisfaction (t=-5.36, p<.001) were improved after management in comparison to the results of pre-tests, and the differences were statistically significant. The difference in the pre-test and post-test scores of the support system (chi2=13.89, p<.001) were significant, while the differences in the perception of depression (chi2=.02, p=.891) and coping methods (chi2=.34, p=.558) were not statistically significant. CONCLUSION: Tailored case management using a gatekeeper is effective to reduce the degree of depression and improve life satisfaction in the single-household elderly population having depression. This study offers a model of individualized as well as systemic mental health care for the community of single-house hold elderly people as an effective means for prevention of and early intervention in depression.

7.A Phenomenological Study on Breast Cancer Survivors' Experiences of Mind Subtraction Meditation.

PURPOSE: The purpose of this study was to understand the meditation experiences of breast cancer survivors. METHODS: The participants were 20 breast cancer survivors who participated in 8 weeks of mind subtraction meditation at a tertiary hospital in Korea 2013. The data was collected by in-depth individual interviews, personal logs about the meditation experiences, and field diaries. Data were analyzed using Giorgi's phenomenological analysis. RESULTS: A total of 13 subthemes and 4 themes were derived from the analysis. Participants began to meditate with some doubts initially but were soon comfortable as they became familiar with the meditation method. As they discarded their negativity, they experienced positive changes in their mindset. The experiences of discarded negativity were summarized as "discarding the hate, resentment, and obsession," "discarding the depression, anxiety, and stress," and "discarding the false me." The change after meditation was named "from restraint to freedom," "from negativism to positivism," "from closed doors to the wide open world." and "looking forward to the true happiness." CONCLUSION: Breast cancer survivors reflected on their lives and experienced positive changes as they discarded their negativity through meditation. It is recommended to perform the nursing interventions utilizing the meditation and conduct further studies to examine its beneficial effects.

8.Clinical Observation of Neonatal Group B B-Hemolytic Streptococcal Meningitis.

Neonatal meningitis caused by the Group B beta-hemolytic streptococcus was first reported in 1958. In recent years this organism has assumed major importance in the etiology of neonatal infection. In Korea, this organism is likely to play an increasing role in neonatal infection. So this investigation was designed to report the 9 cases of Group Bbeta-hemolytic streptococcus infected neonates admitted to the NICU, department of Pediatrics, Hanil Hospital from January 1990 to June 1993. The results were as followings. 1) Onset of disease was between 12days and 32 days of age and male to female ratio is 1:2. 2) Two cases were delivered by c-section, seven cases were by NFSD and all of the infants had normal birth weight with full term at delivery. Maternal obstetric and neonatal complications were not detected in all cases. 3) The common presenting symptoms were fever in all cases, seizure in 6 cases, feeding difficulty, lethargy, vomiting, etc. 4) Group Bbeta-hemolytic streptococcus was isolated in CSF of all cases and in blood of 6 cases. 5) There were abnormal brain CT findings in 4 cases of group Bbeta-hemolytic streptococcal meningitis. 6) The sensitive antibiotics were Cefotaxime, Cefazoline, Erythromycin, Ampicillin, Chloramphenicol, Clindamyucin and Penicillin. 7) All cases were reconvered except one case that expired on the 1st day of admission and 6 cases were followed up for the period of 2months to 33months without neurologic sequelae, of which 1 case revealed decreased wave on Lt. hemisphere in ABR.

9.clinical Evaluation for the Progrosis after the Fontan Operation.

A procedure descried by Fontan and Baudet in 1971 successfully bypassed the right ventricle in Tricuspid Atresia patients, after then many modification of the Fontan operation had been described and applied to many cyanotic complex heart patients. Forty patients with a variety of cardiac malformation underwent the Fontan operation at Seoul National University hospital (September 1978 to June 1986). The age at operation ranged 2 months to 18 years. Each number of cases according to basic cardiac anomaly was as follows; 17 in Tricuspid Atresia, 17 in Univertricular Heart, 2 in Double outlet of Right Ventricle, 2 in Transposition of the Great Arteries and 2 in Criss-cross heart. Total mortality rate after the Fontan operation was 50%. There was only one late death (>30 days). Mortality rate under 4 years of age (67%) was higher than that between 4 and 18 years of age (40%). we observed a significantly higher mortality for patients who, in the immediate postoperative period, had central venous pressure greater than 25cm H2O. 45% among survivals did not require further medication. Although mortality rate after the Fontan operation is much higher than that in the foreign literature, operative mortality will decline with the increased expirence of surgeon and the effective patients selection.

10.Effects of Pumpkin Water Extract Supplement on Serum Lipid and Fasting Glucose Levels in Female Collegians.

To elucidate the effect of a 4 week dietary supplementation with pumpkin water extract on serum lipid and fasting glucose levels, 41 female collegians residing in the Gyeonggi area were recruited. The subjects were divided into three groups: placebo-control (PC, n=11), treatment A (TA, 100 ml/day, n=13), and treatment B (TB, 300 ml/day, n=17). Total serum cholesterol, high-density lipoprotein (HDL)-cholesterol, low-density lipoprotein (LDL)-cholesterol, triglyceride (TG), and fasting glucose were analyzed. The average age, height, weight, and body mass index of the subjects were 21.3 years, 161.1 cm, 53.0 kg, and 20.4 kg/m2, respectively. The average total cholesterol, HDL-cholesterol, LDL-cholesterol, TG, and fasting glucose level (mg/dl) were 171.4+/-26.1, 63.5+/-11.4, 92.6+/-22.9, 76.8+/-32.0, and 89.1+/-8.2, respectively. There were no significant differences among the three groups in any measured parameters, and between the pre- and post-study values of the all parameters. The results indicated that the 4 week pumpkin water extract supplementation (100 ml, 300 ml) did not influence the serum total cholesterol, HDL-cholesterol, LDL-cholesterol, TG and fasting glucose level in female collegians.
