1.Feasibility of serum galactomannan assay in hematologic malignancy patients with invasive fungal infections

Objective To evaluate the feasibility of the serum galactomannan (GM) assay as a rapid detection method for the early diagnosis of invasive fungal infections (IFI) in haematological malignancy patients. Methods Thirty-nine patients with haematological malignancies at high risk of IFI were enrolled into this study. The criteria of high risk included fever (≥ 38℃) lasted for more than 96 hours, fever didn' t response to proper broad spectrum antibiotic or recurrenced after short period of response, and no antifungal drug was used in the last week. The GM assay in serum specimens were performed twice a week for 3 weeks. Thirty health donor were selected as control group to perform the serum GM assay. The sensitivity and specificity, the positive and negative predictive value (PV+, PV-) of GM assay in serum specimens were calculated and compared with traditional diagnosis methods. Results In the 39 patients, 31 patients were diagnosed as IFI by clinical evidence and the other 8 patients were diagnosed as bacteria infection. The cut-off of GM assay was 0.5. GM assay results showed that the positive rate, sensitivity, specificity, total consistent rate, PV+ and PV- were 80.6 %, 87.1%, 62.5%, 82.0%, 90.0 % and 55.6%, respectively. The Kappa rate was 0.474. In the 8 patients without IFI, 3 cases were GM positive and 2 BG positive. The time of the first positive GM assay was (2.8±4.8) d (ranged from 24 d before to 3 d after clinical diagnosis) before IFI was diagnosed. During antifungal treatment, the level of GM maintained highly in the patients with aggravation of IFI, and dropped with the IFI improving. Conclusion The results of GM assay were consistent with that of traditional IFI diagnosis. Compared with classical IFI diagnosis, the GM assay has the advantages of the early, rapid, and high sensitivity and specificity.

The paper uses computer multimedia technology to collect and switch audio and video signals, constructs the remote High Definition ( HD) surgery demonstration system which supports live broadcast, recorded broadcast, rebroadcast and video-on-demand depending on the network communication routing technology.It introduces functions, design scheme and composition of the system and points out that the system can realize HD video transmission and video talkback between the surgery scene and the classroom.Thus, tech-nical level of clinical surgery teaching can be enhanced.
4.Development and application of private cloud storage system

Objective To design a private cloud storage system to guarantee intranet and internet data security and realize data sharing between multi platforms.Methods The idea and solution for building the system were proposed by analyzing the difficulty during hospital informatization.The infrastructure based on the idle computing resources in the existing virtual pool and the application platform by private cloud storage software were involved in the development of the system,and related software and hardware were integrated after adjustment and customization,and then the system came into being with easy operation and high performance-cost ratio.Results The system developed realized freely data storage and sharing,and implemented file encryption to control the applied range.Conclusion The system has the functions of malicious code scanning,cooperative office system and data auto-backup,synchronization,transmission,sharing and etc,which executes intranet and intemet data sharing based on physical isolation.

7.Effects of different types and concentration of oral sweet solution on reducing neonatal pain during heel lance procedures.

OBJECTIVETo compare the effect of different types and concentrations of sweet solutions on neonatal pain during heel lance procedure.
METHODTotally 560 full term neonates (male 295, female 265) were randomized into 7 groups:placebo group (plain water), 10% glucose, 25% glucose, 50% glucose, 12% sucrose, 24% sucrose and 30% sucrose groups.In each group, 2 ml corresponding oral solutions were administered through a syringe by dripping into the neonate's mouth 2 minute before heel lance. The procedure process was recorded by videos, from which to collect heart rate, oxygen saturation and pain score 1 min before puncture, 3, 5 and 10 min after puncture.
RESULTThe average heart rate increase 3, 5 and 10 min after procedure in the 25% and 50% glucose groups, 12% and 24% and 30% sucrose groups was significantly lower than those in the placebo group (P < 0.01 or 0.05). The average heart rate increase 3 min after procedure in the sucrose group was lower than that in the glucose group (P < 0.01).Neonates who received 30% sucrose has a significantly lower average heart rate increase than those who received 12% and 24% sucrose 3 min after heel lance (both P < 0.05) . The average oxygen saturation decrease 3, 5, 10 min after procedure was significantly lower than those in the placebo group (P < 0.01). The average oxygen saturation decrease 3 min after procedure in the sucrose groups was significantly lower than that in the glucose groups (P < 0.01). The average pain score 3, 5, 10 min after procedure was significantly lower than those in the placebo group (P < 0.01). The average pain score 3 min after procedure in the sucrose groups was significantly lower than that in the glucose groups (P < 0.01).
CONCLUSIONOral administration of sweet solutions is an effective way to relieve neonatal pain on procedure, and sucrose has a better pain relief action than glucose, moreover, 30% sucrose provides better effect in control of heart rate increase 3 min after heel lance, but the best concentration of sucrose for pain relief needs further study.

8.Improvement on flame atomic absorption spectrometry for determining Tin concentration in air of workplaces.

OBJECTIVETo explore the influence factors on determining Tin concentration in air of workplaces with flame atomic absorption spectrometry and to establish an accurate, sensitive and high-efficient method.
METHODSThe different reagents were used to digest the sampling filter membranes and the determining conditions of flame atomic absorption spectrometry were adjusted, then the determining results were compared.
RESULTSWhen 3 ml hydrochloric acid and 0.5 ml nitric acid served as the digesting reagents and the determining conditions of flame atomic absorption spectrometry were adjusted to the best conditions, there was the good linearity in the tested concentration range of Tin, the correlation coefficient was larger than 0.9990. The limit of quantification was 1.0 p.g/ml. The extraction recovery was between 99.6%-102.6%, and the RSD were all less than 5.0%.
CONCLUSIONThe proper kinds and quantity of digestive reagents in pretreatment of the samples should be chosen for the accuracy and precision of the determination according to the influence factors of determination.

9.Expression of low-density lipoprotein receptor after exposure to hepato-cyte growth factor in human Tenon' s capsule fibroblasts

AIM:To study the dynamic alteration of low-density lipoprotein receptor ( LDLr) expression after exposure to hepatocyte growth factor (HGF) in human Tenon's capsule fibroblasts (HTFs).METHODS: HTFs were stimulated with HGF at different concentrations (0, 10, 20, 40, 80 and 160μg/L) for 12, 24, and 48 h.The viability of HTFs was analyzed by MTT assay .The expression of LDLr at mRNA and protein levels were analyzed by real-time PCR and Western blot .RESULTS:The expression of LDLr at mRNA and protein levels was positively correlated with the viability of HTFs.HGF promoted the viability of HTFs in a time-and concentration-dependent manner .At the same time , HGF pro-moted the expression of LDLr in the same manner .CONCLUSION:Exposure of HTFs to HGF induces LDLr expression at high level , suggesting that over-expression of LDLr on the HTFs may be a target receptor for controlled drug delivery , par-ticularly in anti-scarring therapy after glaucoma filtration surgery .
10. The value of muscle biopsy in rhabdomyolysis

Objective:
To analyze the diagnostic value of skeletal muscle biopsy in patients with rhabdomyolysis.
Methods:
Clinical and pathological data of 26 patients with rhabdomyolysis from January 2002 to December 2018 undergoing muscle biopsy were collected.
Results:
Eighteen males and 8 females were finally recruited with median age of 6-73 (37.3±19.6) years. The average time from onset to biopsy was 44 days (median course was 30 days). All patients had acute manifestations with muscle pain and/or weakness. Serum creatine kinase was between 1 648-92 660 U/L. Muscle biopsies showed nonspecific changes in 12 cases (a few with type 2 muscle fiber atrophy, slight deposition of lipid droplets), 10 cases with necrotizing myopathy (muscle fiber necrosis and regeneration). Toxic neurogenic damages were seen in 2 cases (type 1 and type 2 angular atrophic muscle fibers with group change), lipid storage disease in 1 case (lipid droplets deposit significantly) and idiopathic inflammatory myopathy in 1 case (muscle fiber necrosis and regeneration, with lymphocyte infiltration). The etiology of non-specific pathological changes included short-term strenuous exercise in 6 patients, poisoning in two, chronic kidney disease in one, viral infection in one, hypothyroidism in one and unknown reason in one. As to patients with necrotizing myopathy, seven were poisoning or drug-related, one with hyperthyroidism, two with unknown reason.
Conclusions
Among the numerous causes of rhabdomyolysis, exercise usually links nonspecific skeletal muscle changes and poisoning or drug-related disorders are commonly associated with necrotic myopathy. Rhabdomyolysis induced by primary myopathy is rare.
