1.Coronary Flow Doppler Profile in No-Reflex Phenomenon after Direct PTCA in Acute Myocardial Infarction.

Profound reduction of anterograde coronary flow with concomitant ischemia is seen occasionally during percutaneous coronary intervention despite technically successful procedure. We found interesting coronary flow pattern in a patient with acute myocardial infarction, showing angiographic no reflow phenomenon after direct PTCA. The coronary blood flow pattern of the angiographic no-reflow phenomenon in this case was characterized by minimal systolic flow and sharp deceleration of diastolic flow. Coronary flow reserve calculated by the ratio of adenosine induced maximal hyperemic velocity and basal velocity was reduced. The Dopplertipped guide wire was useful for observation of phasic coronary flow pattern of angiographic no-reflow phenomenon.

2.Colonization Rate and Control of Vancomycin-Resistant Enterococci in the Neonatal Intensive Care Unit.

PURPOSE: Recently, vancomycin-resistant enterococci (VRE) have become one of the major nosocomial pathogens in Korea. However, there have been few studies on the epidemiology of VRE colonization among neonates. In this study, we investigated the prevalence of VRE colonization, risk factors for VRE, and how to control the spread of VRE infection in the Neonatal Intensive Care Unit (NICU) of Pusan National University Hospital (PNUH). METHODS: We retrospectively reviewed medical records of 192 neonates who were admitted to the NICU of PNUH from March 2006 to March 2007. Surveillance cultures from rectal swabs for detecting VRE were obtained weekly during the study period. We analyzed the prevalence of VRE and various risk factors. RESULTS: The rate of VRE colonization among NICU patients was 25% (48/192). Thirty five of these VRE colonized patients were transferred to the NICU from other local hospitals. Compared with the non-VRE group, the risk factors associated with VRE colonization were lower birth weight, congenital heart disease, applied mechanical ventilation, use of a central venous catheter, chest tubing, a history of surgery, and use of antibiotics. CONCLUSION: VRE colonization among patients admitted to the NICU is rapidly increasing. Monitoring and managing premature neonates from the beginning of the birth process, avoiding many invasive procedures, avoiding antibiotics such as vancomycin and third generation cephalosporin are important for preventing the emergence and spread of VRE colonization in the NICU.

5.Comparison of Antioxidant Level of the Elderly Living in Institutions: Vegetarian and Non-vegetarian Diet.

PURPOSE: The purpose of this study was to compare the antioxidant level, serum cholesterol and skin beta-carotene, of female elderly who had been living more then 10 months in institutions which offered either vegetarian or nonvegetarian diet. METHODS: Total of 110 female elderly, 56 from institution serving vegetarian diet and 54 from nonvegetarian diet, were recruited from institutions located in S and N city in Korea. beta-carotene level was measured on their palm using Pharmanex Biophotonic Scanner and annual health evaluation data was utilized for the analysis of serum cholesterol. RESULTS: The subjects on a vegetarian diet had a significantly higher level of phosphorus, carotene, and folic acid. The elderly who were living in an institution offering a vegetarian diet had a lower level of serum cholesterol and higher level of skin beta-carotene, compared to those living in an institution offering a non-vegetarian diet. CONCLUSION: This study reveals that a vegetarian diet is beneficial in increasing skin beta-carotene level, decreasing serum total cholesterol level. The skin beta-carotene measurement appear valuable as a bio-marker of antioxidant intake. Further study on antioxidant food and effective serving strategy for elderly are recommended.

6.Health Promotion Behavior, Self-efficacy, Marital Intimacy, and Successful Aging in Middle-aged

Purpose:
This study is a descriptive research that aimed to investigate the degree of successful aging, health promotion behavior, self-efficacy and marital intimacy, and to identity the factors affecting their successful aging.
Methods:
The subjects in this study were 110 middle-aged. Data were analyzed using Independent t-test, one-way ANOVA, Pearson’s correlation coefficients, and Multiple Linear Regression.
Results:
Participants scored 81.7±12.25 on health promotion behavior, 29.1±2.64 on self-efficacy, 53.6±9.29 on marital intimacy and 73.8±8.51 on successful aging. There were significant positive relationships between successful aging and health promotion behavior (r=.59,p<.001), and self-efficacy (r=.31,p=.002), and marital intimacy (r=.34, p=.001). Factors influencing successful aging were perceived health status (β=4.59), health promotion behavior (β=0.34). The explanation power of this regression model was 44% and it was statistically significant (F=20.68,p<.001).
Conclusion
The perceived health status and health promotion behavior may help the successful aging of middle-aged. Therefore, it is necessary to improve the health status and health promotion behavior of middle-aged in order to prepare their successful aging.
7.Factors Influencing Self-Efficacy, Grit, Symptom Clusters on Self-Care Agency in Brain Neoplasms Patients.

PURPOSE: The purpose of this study was to investigate the level of self-efficacy, grit (consistency of interest-CI, perseverance of effort-PE), symptom clusters and self-care agency in brain neoplasm patients and to identify factors influencing their self-care agency. METHODS: A descriptive correlational design was used, and the participants were a convenience sample of 152 brain neoplasm patients from C national university hospital in Hwasun. Data were collected using a structured questionnaire from October 1, 2017 to February 15, 2018. The collected data were analyzed using descriptive statistics, t-test, ANOVA, factor analysis, Pearson correlations and multiple regression analysis with the SPSS 22 program. RESULTS: The most frequently reported symptoms included sadness (57.9%), numbness (50.7%), lack of energy (49.3%), nervousness (45.4%), worry (40.8%). There were significant positive correlations between self-care agency and self-efficacy (r=.54, p < .001), CI (r=.58, p < .001), and PE (r=.50, p < .001). There were significant negative correlations between self-care agency and cluster I (r=−.56, p < .001), cluster II (r=−.31, p < .001), cluster III (r=−.49, p < .001) cluster IV (r=−.30, p < .001). The significant factors influencing self-care agency were self-efficacy, grit, cluster I and cluster IV. These variables explained 55.4% of the variance in self-care agency. CONCLUSION: The results suggest that intervention programs to increase the level of self-efficacy and grit, to reduce the level of symptoms among patients would improve the self-care agency of brain neoplasms patients.

8.A Case of Halo Congenital Nevus.

The term halo congenital nevus is a condition in which halo formation is associated with congenital melanocytic nevus. The regression of congenital melanocytic nevi is usually accompanied by the halo phenomenon and this is considered to be a rare event, although this may be underestimated. We describe here a 9-year-old boy with verrucous congenital nevus surrounded by a depigmented halo on the right upper arm. Histologically, the nevus lesion revealed a dense lymphocytic infiltration in the upper dermis intermingled with some remnants of nevus cell nests. Most of the infiltrating cells were CD8+ T lymphocytes. At the edge of the lesion, corresponding to the area of the halo, neither melanocytes nor melanin were found in any significant amounts.

9.Atrophying Pityriasis Versicolor: Is This a New Variant of Pityriasis Versicolor?.

An atypical clinical form of pityriasis versicolor has been infrequently reported, in which cutaneous atrophy is associated with individual pityriasis versicolor lesions. The pathogenesis of this atrophy remains unclear, but is believed to be a delayed-type hypersensitivity reaction to antigens derived from the Malassezia species. A 60-year-old man presented with multiple, slightly scaly, and depressed maculopatches or plaques on the trunk and extremities. Our microscopic examination of the skin scrapings on a KOH preparation revealed numerous short hyphae and spores. The patient was treated daily with 200 mg of itraconazole in combination with topical antifungals, achieving clinical improvement and mycological recovery, which was confirmed upon follow-up 1 month later. This is the first case report of atrophying pityriasis versicolor in Korea. It needs to be differentiated from other atrophying disorders of the skin.
