1.Rearing Pattern of Mothers with Affective Disorder and Their Children's Behavior Problem.

OBJECTIVES: This study is to compare psychopathology and their perceived parental rearing pattern of the children of mothers with major depression, bipolar disorder and those of normal control group. The correlation was also assessed between behavior problem and perceived parenting style. METHODS: The subjects were 30 children (age range 12-18) of mothers who met the DSM-IV criteria of major depressive disorder or bipolar I disorder and 30 children of normal mother group. All completed self-report questionnaires containing Child, adolescent Behavior Problem Checklist (K-CBCL), Youth Self Report (YSR) and Parental Bonding Instrument (PBI). RESULTS: 1) The mean score of the subscales of somatic complaints, anxious/depressed, internalizing problems in K-CBCL was significantly higher in the control subjects compared with that of the children of mothers with affective disorders (p<0.05). 2) In YSR, there was a tendency of increased behavior problems among the children of mothers with major depression compared with the control subjects. Compared with children of mothers with bipolar disorder, children of mothers with major depression had significantly more behavior problems (p<0.05). 3) Mothers with affective disorders tended to display less caring and fathers and mothers with affective disorders tended to display more overprotective parenting style than the control subjects. 4) There was positive correlation between paternal overprotection and behavior problems in the children of mothers with affective disorders. CONCLUSION: In summary, the author identified that the children of mothers with affective disorders have more behavior problems in social adjustment than normal control subjects. Parental rearing style is thought to have more negative influence on the children of mothers with affective disorders than normal control. It is surmised that less caring and more overprotective rearing style could influence behavior problems of their children.

2.Allergic contact dermatitis to Tego@ in two siblings.

Tego (dodecylic aminoethyl glycine HCl) is an antiseptic detergent used abroad in hospitals, food industries, public baths, and for cleaning machinery. Allergic contact dermatitis may occur in hospital operating-room personnel, swimming instructors, and deep-sea divers. We present two cases of allergic contact dermatitis to Tego in two siblings after dressing the accidental abrasion with Tego . The patch test results showed a strong positive reaction to 0.1% Tego.

3.Health Promotion Researches Supported by the Korean Government.

BACKGROUND: This study was carried out to examine the trends of government-supported health promotion research projects conducted in Korea over the past 12 years. METHODS: Research type, area of interest, organization, and expense of 726 research projects conducted from 1998 to 2009 were examined and the health promotion content analyzed. RESULTS: In Korea, 361 health policy researches (HPR) and 365 general health researches (GHR) were supported by the government during the defined time period. A total of 60.5 health promotion research were conducted annually with a total amount of 27.1 billion won provided (2.26 billion won per year). With the average research project lasting 8.5 months, HPR (7.7 months) projects were completed sooner than GHR (9.2 months). Those who majored in preventive medicine completed 177 research (24.4%), the most number of research projects, followed by public health (22.5%), and family medicine (15.6%). There were 641 health promotion research projects done mostly on policies, legal systems, and grasping current conditions, and only 85 (11.7%) clinical test research on the development or effects of health promotion programs. CONCLUSIONS: HPR have been increasing annually. However, our study could not be certain of how close the studies were to the government's health promotion policies. Furthermore, the main health promotion area, 'healthy living', was not researched as often as should be. Additionally, to improve applicability of the research projects, interdisciplinary cooperation should be promoted.

4.A case of soft fibroma of the vulva.

We report a case of soft fibroma occured in a 27-year-old female. The lesion was multiple, pea-to walnut-sized, baglike, pedunculated growth on the vulva. The histopathologic findings of excisional biopsy specimen evaled a slightly t.hinned epidermis and loosely arranged collagen fibers in the dermis.

5.A Pitfall in the Use of Three Dimensional Computed Tomographic Angiography for Early Surgery of Ruptured Cerebral Aneurysm: Case Report.

Three dimensional computed tomographic angiography is a rapid and minimally invasive method of detecting intracranial aneurysms, however, there are some limitation to identify the associated cerebrovascular lesions. We report a case of ruptured dorsal wall aneurysm of internal carotid artery in a 50-year-old man who had underlying stenosis of cervical internal carotid artery, which was overlooked due to lesion outside the scan field of view.

7.Transcranial Doppler Examination Following EIAB in Patients with Hemodynamic Cerebral Ischemia.

OBJECTIVE: The aim of this study is to evaluate the effects of intracranial-extracranial arterial bypass(EIAB) surgery on cerebrovascular reserve capacity(CRC) in patients with hemodynamic cerebral ischemia and to assess the significance of transcranial doppler(TCD) examination before and after EIAB surgery. METHODS: In 29 consecutive patients who underwent EIAB surgery due to symptomatic internal carotid artery or middle cerebral artery(MCA) occlusion, 21 patients were studied using preoperative and postoperative digital subtraction angiogram(DSA), SPECT, and TCD examination. After measuring mean radioactivity count of a region of interest, relative regional cerebral blood flow(rrCBF) was quantitated by the following formula: lesional radioactivity count/contralateral radioactivity count x 100%. CRC was calculated as the percentage change from baseline flow after a vasodilatory challenge. RESULTS: Postoperative DSA showed good collateral circulation through the bypass except 4 patients. Preoperative mean value of rCRC improved significantly after EIAB surgery from -14.8+/-2.6% to 6.9+/-2.7%(p<0.01). Intraoperatively, flow velocity of superficial temporal artery(STA) and MCA just after anastomoses increased remarkably in comparison with the values just before anastomoses(p<0.01). There was no correlation between the change of pre- and postoperative flow velocity and the change of rrCBF. In patients with the increase of flow velocity after surgery manifested good collateral circulation through the bypass. The difference was statistically significant(repeated measures ANOVA, p<0.05). Over a mean follow-up period of almost 3 years, no patient had another episode of brain ischemia. CONCLUSION: We believe that the measurement of flow velocity is expected a good method to evaluate the degree of collateral circulation through the bypass.

8.Effect of Postpartum Outcomes in Mother's Upright Position During the Second Stage of Labor: Systematic Review.

PURPOSE: The purpose of this study was to determine whether upright position is effective in labor through systematic review in randomized controlled trials. METHODS: We established the PICO (Patient-Intervention-Comparator-Outcome) strategy, and reviewed 282 literatures from national and international electronic databases, and finally selected 9 references based on inclusion and exclusion criteria. We evaluated the quality of references and carried out a meta-analysis. RESULTS: The maternal outcomes showed that the duration of their second-stage labor was 2.29 minutes shorter than that of the women in the recumbent position, and were less likely to have episiotomy. The other outcomes, including the mode of delivery, blood loss, hemoglobin level, use of oxytocin, use of analgesics, and perineal laceration, did not differ between the groups. The fetal heart rate abnormality occurred less than in the control group. The Apgar scores of the groups did not differ. CONCLUSION: There is evidence that an upright position in the second stage of labor reduces the duration of the second stage of labor, the incidence of episiotomy, and an abnormal fetal heart rate.

9.Four-layer technique for tracheostomy scar treatment

One important complication of the tracheostomy procedure is the depressed scar left after the tube is removed. A depressed tracheostomy scar can be aesthetically and functionally unacceptable. Tracheostomy scar treatment aims to fill lost soft tissue volume and correct tracheal skin tug. There are various techniques described to manage post-tracheostomy scars, including the use of autologous tissue or allogenic material and the creation of muscle flaps. In this article, the authors introduce a surgical method using four layers: the scar, the strap muscles, the platysma muscle, and the skin. This procedure has been used in two patients with depressed scar after prolonged tracheostomy placement. The tracheal tug was eliminated in each patient, and an imperceptible cutaneous scar remained. In each case, patient satisfaction was complete. The authors recommend this technique as a simple and effective method of closure for these troublesome tracheostomy scars.
10.Antiobesity effects of the water-soluble fraction of the ethanol extract of Smilax china L. leaf in 3T3-L1 adipocytes.

BACKGROUND/OBJECTIVES: Several medicinal properties of Smilax china L. have been studied including antioxidant, anti-inflammatory, and anti-cancer effects. However, the antiobesity activity and mechanism by which the water-soluble fraction of this plant mediates its effects are not clear. In the present study, we investigated the lipolytic actions of the water-soluble fraction of Smilax china L. leaf ethanol extract (wsSCLE) in 3T3-L1 adipocytes. MATERIALS/METHODS: The wsSCLE was identified by measuring the total polyphenol and flavonoid content. The wsSCLE was evaluated for its effects on cell viability, lipid accumulation, glycerol, and cyclic adenosine monophosphate (cAMP) contents. In addition, western blot analysis was used to evaluate the effects on protein kinase A (PKA), PKA substrates (PKAs), and hormone-sensitive lipase (HSL). For the lipid accumulation assay, 3T3-L1 adipocytes were treated with different doses of wsSCLE for 9 days starting 2 days post-confluence. In other cell experiments, mature 3T3-L1 adipocytes were treated for 24 h with wsSCLE. RESULTS: Results showed that treatment with wsSCLE at 0.05, 0.1, and 0.25 mg/mL had no effect on cell morphology and viability. Without evidence of toxicity, wsSCLE treatment decreased lipid accumulation compared with the untreated adipocyte controls as shown by the lower absorbance of Oil Red O stain. The wsSCLE significantly induced glycerol release and cAMP production in mature 3T3-L1 cells. Furthermore, protein levels of phosphorylated PKA, PKAs, and HSL significantly increased following wsSCLE treatment. CONCLUSION: These results demonstrate that the potential antiobesity activity of wsSCLE is at least in part due to the stimulation of cAMP-PKA-HSL signaling. In addition, the wsSCLE-stimulated lipolysis induced by the signaling is mediated via activation of the beta-adrenergic receptor.
