1.Relations of Self-Esteem with Paranoia in Healthy Controls, Individuals at Ultra-High Risk for Psychosis and with Recent Onset Schizophrenia.

OBJECTIVES: Some emotional components, such as self-esteem, depression and anxiety, have been reported to be associated with paranoia in non-clinical population and schizophrenia patients. The aim of this study was to investigate the correlation between self-esteem and paranoia in healthy controls, in ultra-high risk for psychosis and schizophrenia patients. METHODS: 34 subjects with recent onset schizophrenia, 36 subjects with ultra-high risk for psychosis, and 44 healthy volunteers participated in this study. A detailed assessment was made of the paranoia, self-esteem, depression, and anxiety. RESULTS: In all three groups, there were a negative correlation between paranoia and self-esteem, and positive correlations between paranoia and depression and anxiety. In healthy control, lower self-esteem showed a trend to predict higher paranoia, and in ultra-high risk for psychosis, this trend tern on statistically significant level, and in recent onset schizophrenia group, this correlation was disappeared. CONCLUSION: The individual who have lower self-esteem showed higher paranoia tendency under delusional level, but after formation of persecutory delusion, the tendency was disappeared. This result supports the hypothesis that persecutory delusions are a defense against negative affective process.

2.Prognostic factors affecting the results of pyeloplasty in infants and children.

This study was performed to evaluate the influence of the possible preoperative prognostic factors on the results of pyeloplasty and to predict the postoperative prognosis according to these factors. Of 56 cases under the age of 15 years admitted to Yonsei University Medical Center from 1979 to 1988 with the diagnosis of ureteropelvic junction obstruction, 30 cases, who could be followed up after dismembered pyeloplasty, were analyzed for the preoperative prognostic factors. The possible preoperative prognostic factors affecting postoperative results after pyeloplasty are the age at the time of surgery. the length of obstruction and the state of growth and development. But the pyuria, the urinary tract infection, the severity of hydronephrosis, the presence of extrarenal pelvis, the function and degree of obstruction of the diseased kidney on renography, the thickness of renal parenchyme and the presence of the obstruction of contralateral kidney did not affect the postoperative results after pyeloplasty.

3.Gastrointestinal Cytomegalovirus Infection: A clinicopathologic analysis of 8 cases.

We analysed 8 cases of gastrointestinal cytomegalovirus(CMV) infection including one autopsy, three surgically resected and four endoscopic biopsy cases. Involved sites were colon in four,stomach in three and small intestine in one. Three of them were associated with immu-nosuppressed condition but others had no clinical evidence of immune deficiency. Multiple aphthous mucosal ulceration was a common presentation, but one revealed a mucosal ulcer with segmental narrowing and thickening of wall. Microscopically, six showed cytomegalic inclusions in endothelial cells and fibroblasts, one in mucosal epithelial cells and the remaining one in both endothelial cells and mucosal epithelial cells. Immunohistochemical staining using monoclonal antibody against CMV confirmed postive result in seven cases. Serum IgM anti-CMV antibody was elevated in one case. We conclude that gastrointestinal CMV infection is currently not a rare condition and frequently associated with non-immunosuppressed condition, and thus a thorough histologic examination is required especially in the gastrointestinal ulcerative lesion. Once cytomegalic inclusion is suspected, immunohistochemical identification of CMV seems essential for specific diagnosis.

4.Effects of Oral Care with Essential Oil on Improvement in Oral Health Status of Hospice Patients.

PURPOSE: This study was done to examine the effects of oral care with essential oil in improving the oral health status of hospice patients with terminal cancer. METHODS: The participants were 43 patients with terminal cancer admitted to K hospital in G city, Korea. Twenty-two patients were assigned to the experimental group and 21 to the control group. Participants in the experimental group received special mouth care with essential oil (application of essential oil mixture consisting of geranium, lavender, tea tree, and peppermint). The control group received special mouth care with 0.9% saline. The special mouth care was performed twice daily for one week in both groups. The scores for subjective oral comfortness, objective oral state, and numbers of colonizing Candida albicans were measured before and after the treatment. RESULTS: The score for subjective oral comfortness and objective oral state were significantly higher in the experimental group compared to the control group. The numbers of colonizing Candida albicans significantly decreased in the experimental group compared to the control group. CONCLUSION: Oral care with essential oil could be an effective oral health nursing intervention for hospice patients with terminal cancer.

5.Effect of Pancuronium into the Lateral Ventricle on the Repiratory and Circulatory Functions in Rabbit.

In this study the action of pancuronium given directly into the lateral ventricle on the mean blood pressure, heart rate, respiratory frequency and skeletal muscular contractility of the rabbit were investigated. Pancuronium, which a dose of 10ug/kg and 30kg/kg were given into the lateral ventricle of rabbit. 1) Intraventricular injection of 10ug/kg of pancuronium increased pulse rate, respiratory frequency and muscle contraction and the differences were not statiscally significant. 2) Intraventricular injection of 30ug/kg of pancuronium produced a gradual increase of respiratory frequency which began at 5 minutes after drug injection and continued over a half hour. The difference between control and experimental value was statistically significant (p<0.005) while pulse rate and muscular contractility were increased but not significant. 3) Mean blood pressure was not changed after intraventricular injection of the pancuronium.

6.Quality of Life in Patients with Hematopoietic Stem Cell Transplant.

PURPOSE: This descriptive study was to investigate the quality of life in patients with hematopoietic stem cell transplantation (HSCT) from June 1 to October 13, 2007. METHOD: The survey was conducted in 6 different university hospitals which located in Seoul and Jellanamdo province using the Functional Assessment of Cancer Therapy-BMT Scale (FACT-BMT) version 4. We collected a total of 155 questionnaires and analyzed 149 among them. RESULTS: The average score of quality of life was 2.53 out of 5. Physical well being score was highest among sub-domains, followed by emotional well-being, additional concerns, social/family well-being, and functional well-being. Study subjects worried that their conditions would get worse. However study subjects didn't regret having been received HSCT. Age, duration from HSCT, age at diagnosis, income, readmission, HSCT type, educational background, marital status, and the level of activities of daily living were related to quality of life. CONCLUSIONS: The findings of this study indicates that the HSCT survivor's quality of life issue is still important and have to be investigated repeatedly in the future. That is necessary for generalizing QOL outcomes for clinical use. We also suggest to develop interventions to improve QOL.

7.The expression of TGF-beta1 mRNA in the maxillary sutures of puberty rhesus loaded with the class III intermaxillary orthopedic force.

OBJECTIVEThe purpose of the study is to detect the expression of TGF-beta1 mRNA in the maxillary sutures of puberty rhesus during different periods of the loading of intermaxillary class III orthopedic force.
METHODSThe animal model was established with 6 puberty female rhesus, which were randomly divided into experimental group (wearing class III twin-block magnet appliance, each 2 rhesus for 3 and 6 month respectively) and control group (not wearing any appliance, each 1 rhesus for 3 and 6 month respectively). Tissue sections were obtained perpendicular to the sutures. In situ hybridization was used to the expression of TGF-beta1 mRNA, and the expression intensity was measured and statistics was performed by SPSS 11.0.
RESULTSThere were no statistic differences of cellular density between palatomaxillary suture in vertical group and pterygomaxillary suture in horizontal group, but statistic differences were found between other groups. The expression of TGF-beta1 mRNA was detected in the control group, but the expression intensity was obviously increased after the load of intermaxillary class III orthopedic force. Statistically significant differences were found among all groups except the six months experimental group and control group of temporozygomatic sutures and pterygomaxillary sutures. Experimental groups were more intensive than the control group and three months group was more intensive than the six months group.
CONCLUSIONThe active tissue remodeling happened in the circummaxillary sutures by the effect of class III intermaxillary orthopedic force. Cell proliferation, extracellular matrix synthesis and bone formation were accelerated. During the different remodeling periods, the expression intensities were different, which may be related to the different force loading manners, the different reaction of sutures to the orthopedic force and the different biological function of TGF-beta1 in the different periods.

8.Surgical Treatment of Supra and Intercondylar Fracture of the Femur.

We performed a retrospective analysis of the results of surgical treatment of sixteen patient with comminuted fracture of the distal femur from September 1993 to August 1995 and evaluated the efficacy of the angled blade plate as one of the fixation devices used. The fractures were limited to C2 and C3 fracture classifed according to the AO classification. After an average follow-up of 13 months (range, 9 to 24 months), all of the fractures had healed. Postoperatively, the arc of flexion was limited between 90degrees and 125degrees in eight, normal arc in five, and severely restricted arc of joint motion was present unexceptionally in three patients with deep infection. In angular deformity, average 5.8degrees of valgus and 2.4degrees posterior angulation were showed. The functional results by Sanders-Swiontkowski-Rosen-Helfet rating system were showed excellent in 3, good in 7, fair in 3, and poor in 3 cases. The overall results were seen to be excellent to good in ten cases (62.5%) but in twelve patients using the angled blade plate, excellent to good results were showed 10 patients (83.3%). We attribute the favorable results in our series to restoration of the articular surface through the modified extensile approach, sufficient stable fixation using the angled blade plate with conforming to the concept of bridge plate, routinely performing the bone graft for medial buttress and bone healing, and early mobilization of the extremity.

9.Chemical constituents from Chenopodium ambrosioides.

Twelve compounds were isolated from the herb of Chenopodium ambrosioides, and their structures were identified by spectroscopic methods as kaempferol-7-O-alpha-L-rhamnopyranoside (1), kaempferol-3,7-di-O-alpha-L-rhamnopyranoside (2), patuletin (3), quercetin-7-O-alpha-L-rhamnopyranoside (4), grasshopper ketone (5), 4-hydroxy-4-methyl-2-cyclohexen-1-one (6), syringaresinol (7), benzyl beta-D-glucopyranoside (8), dendranthemoside B (9), N-trans-feruloyl tyramine (10), N-trans-feruloyl 4'-O-methyldopamine (11), and 4-hydroxy-N-[2-(4-hydroxyphenyl) ethyl] benzamide (12). Among them,compounds 3, 6-8,10, and 12 were isolated from the genus Chenopodium for the first time, and compounds 2-12 were isolated from this plant for the first time.
