4.Relation between Helicobacter pylori Infection and Socioeconomic Status in Korean Adolescents.

PURPOSE: This study was conducted to evaluate the association between H. pylori infection and socioeconomic status and to determine the current prevalence of H. pylori infection in Korean adolescents. METHODS: A structured questionnaire was sent to the children's parents to obtain demographic information on the parents and environmental information. Of the 532 questionnaires sent out, 375 (70.5%; 170girls and 205boys) were returned. Their ages ranged from 10 to 15 years (mean, 12.9 years). After collecting blood samples, we measured serum IgG antibody to H. pylori using ELISA method. The association of risk factors such as age, sex, socioeconomic class, type of house, and crowding index with H. pylori infection were analyzed by multiple regression analysis. Socioeconomic status was estimated from the parents'education and occupation using a modified Hollingshead index. RESULTS: The prevalence rate of H. pylori infection was 16.8% (63/375). It increased with age (10.3% at 10~11 years, 15.9% at 12~13 years, and 20.7% at 14~15 years). The H. pylori infection was inversely related to the socioeconomic class (6.3% for the upper class, 16.0% for the middle class, and 20.0% for the lower class). Crowding condition and type of house did not affect significantly on seroprevalence of H. pylori infection. After logistic regression, we found that the odds ratio for age was 2.2 (95% confidence interval 0.9~5.4), and for socioeconomic status, 3.6 (95% confidence interval 0.5~28.9). CONCLUSION: The prevalence of H. pylori infection in Korean adolescents was 16.8%. It related inversely to socioeconomic status but was not statistically significant. Socioeconomic status based on parents' education and occupation seemed to affect more on H. pylori seroprevalence than crowding or type of house did.

We performed radionuclide ventriculography before and within 1 week after percutaneous mirtal valvuloplasty(PMV) to evaluate left ventricular(LV) function in 20 patients(3 males and 17 females, mean age of 38±10 years) who were pure mitral stenosis before PMV and less than grade 1 mitral regurgitation developed after PMV. 9 out of 20 patients had atrial fibrillation and 3 patients developed a small left-to-right shunt(Qp/Qs<1.5)after PMV using double-balloon technique resulted in a increase in mitral valve area(0.9±0.3 to 2.1±0.8mm

6.Comparison of Children's Body Weights and Eating Habits by Maternal Parenting Attitudes Perceived by Children.

Effective parenting attitudes have been known to be associated with children's health practices including dietary intake and physical activity. The objective of this study is to compare children's body weights and eating habits by maternal parenting attitudes. Data were collected at school (N = 396; 4th and 5th grade students) using self-administered questionnaires on maternal parenting attitudes, eating habits and physical activity. Parenting attitudes were categorized as 1 of 4 parenting attitudes (overprotective, authoritarian, democratic, and neglectful) using affection and control median cut points. Children's body weights, frequency of breakfast, eating out and fastfood, and physical activity were compared by maternal parenting attitudes. Children's body weights were related with mother's employment status (p < 0.05) and parenting attitudes (p < 0.01). Children of unemployed mothers were more likely to be overweight. Children of neglectful mothers (p < 0.01) were more likely to be underweight, compared with children of mothers with other parenting attitudes. Since, unfortunately, the number of children of neglectful mothers was very limited in this study, we could hardly assess eating habits of children of neglectful mothers. Children of authoritarian mothers ate breakfast more regularly (p < 0.05), but ate snacks less regularly (p < 0.01). Children of democratic mothers ate fastfood less frequently (p < 0.01) and ate snacks more regularly (p < 0.01). Meanwhile, children of overprotective mothers ate breakfast less regularly (p < 0.05) and ate out less frequently (p < 0.01). However, maternal parenting attitudes were not related to children's physical activities. In conclusion, the maternal democratic parenting attitude was associated with healthy eating habits including regular snack time and less fastfood. On the other hand, the maternal neglectful parenting attitude was associated with high risk of children's underweight. Understanding the mechanism through which parenting attitude is related with underweight risk and healthy eating habits may lead to the development of better interventions.

7.Factors Influencing Mortality in Geriatric Trauma.

BACKGROUND: The goal of this study is to identify the factors that predict mortality in elderly trauma patients. METHOD: We reviewed retrospectively the medical record of 144 cases of geriatric trauma admitted to Kyungpook National University Hospital firm January 1998 to December 1998. We evaluated the general characteristics, mechanisms of injury, Revised Trauma Score(RTS), Injury Severity Score(ISS), Probability of survival(Ps) by TRISS(Trauma and Injury Severity Score) method, amount of blood transfused, preexisting disease, complications, length of stay, and mortality. RESULTS: 1. The mean age was 75.39+/-7.89 years old, and male to female ratio was 0.89 : 1. 2. The mechanisms of injury were primarily frills(56.3%) followed by bicycle or motorcycle(13.9%), and pedestrian injuries(13.2%) and motor vehicle accidents(6.9%). 3. The mean Glasgow Coma Scale(GCS), RTS and ISS ate 13.3+/-3.5, 7.2+/-1.4 and 14.2 +/-11.6 respectively. 4. The actual mortality rate was 18.1% (26/144). But by TRISS method, predicted mortality rate was 9.3%(12.5/144), excess mortality rate was 108% and Z score was 3.99 indicating that actual number of death exceed predicted number of death. 5. Between the survivors and nonsurvivors, the insults were significantly different as follows ; systolic blood pressure(141.9+/-28.3 vs. 116.8+/-48.7 mmHg), GCS(14.3+/-2.0 vs. 9.0 +/-5.1), RTS(7.8+/-0.7 vs 5.4+/-2.3), ISS(11.3+/-5.6 vs 27.2+/-20.2), Ps by TRISS(0.97+/-0.06 vs 0.65+/-0.37), preexisting diseases(50.8 vs 69.8%). CONCLUSION: Geriatric patients are more likely to die after trauma than other age groups. The cause of higher actual mortality rate compared to predicted mortality rate was considered as the higher incidence of delayed death due to sepsis or multiple organ failure. In order to reduce the mortality, even with relatively stable initial vile sign, invasive hemodynamic monitoring and intensive treatment are recommended and also, prevention and treatment of nosocomial infection are very important.

8.Two Cases of Bortezomib-induced Drug Eruption Presenting as Multiple Plaques on the Trunk.

Bortezomib (Velcade(R)) is proteasome inhibitor that is used as a first-line therapy for multiple myeloma. It can cause gastrointestinal, hematologic, and neuromuscular side effects, and a cutaneous reaction is one of its common adverse reactions. To date, several bortezomib-induced cutaneous adverse reactions have been reported, including folliculitis-like rash, pruriginous rash, purpuric rash, mouth swelling, stomatitis-mucositis, edema in the lower limbs, telogen effluvium, and vasculitis. In the Korean literature, only one case of vasculitis has been reported earlier. Two patients have presented with multiple plaques on the trunk at our clinic. The lesions developed several days after bortezomib chemotherapy, and disappeared spontaneously in about 1 week. Herein, we report bortezomib-induced drug eruption presenting as multiple plaques on the trunk with a review of the relevant literature.

10.Analysis of Diagnostic Techniques Used in Patients with Pemphigus over the Past 20 Years at a Single Institution

Background:
Pemphigus is an autoimmune blistering disease characterized by the production of autoantibodies against desmosomal proteins. Owing to its high mortality rate, prompt and accurate diagnosis of pemphigus is crucial.
Objective:
To determine the positivity rates of immunoblotting in patients diagnosed with pemphigus using clinical, histopathological, direct immunofluorescence (DIF), and enzyme-linked immunosorbent assay (ELISA).
Methods:
We retrospectively evaluated 84 patients diagnosed with pemphigus between 2001 and 2021 at the Chonnam National University Hospital in Gwangju, Korea.
Results:
Among the 55 patients with pemphigus vulgaris included in the immunoblotting study, 46 (83.6%) exhibited positive findings for the 130 kDa band. Of the 29 patients with pemphigus foliaceus, 17 (58.6%) showed positive findings for the 160 kDa band. In cases where autoantibodies were difficult to detect with negative DIF and ELISA results, immunoblotting aided in the diagnosis of 18.2% of pemphigus vulgaris cases and 6.9% of pemphigus foliaceus cases.
Conclusion
Immunoblotting can serve as an alternative diagnostic method for patients with mild-to-moderate disease or those who have undergone treatment, especially when DIF or ELISA tests yield negative or atypical results.
