1.Right Single Lung Transplantation in Pulmonary Emphysema Patient: A report of case.

Lung transplantation has been successfully employed for variety of obstructive lung disease. Single lung transplantation has become a therapeutic option for end-stage obstructive lung disease. The patient, a 57 year old man with emphysema, suffered from severe dyspnea, which progressively aggravated him for the last three years. A single lung transplantation was performed from a young brain-dead donor on April 7th, 1999 in the department of thoracic surgery, Respiratory Center, Yongdong Severance hospital, yonsei University. The immunosuppressive regimen was based on cyclosporine A and azathioprine from beginning, adding steroid. Single lung transplantation was feasible and beneficial in patients with end-stage emphysema.

2.Evaluation of the Iron Status in the Adolescents in Seoul.

BACKGROUND: During the adolescent growths spurt, iron deficiency becomes more common, particularly with the additional factors of menstrual blood loss in females and the androgen-related rise in hemoglobin concentration in males. This study was conducted to investigate the iron status in the adolescent students. METHODS: Venous peripheral blood was sampled from 178 males(79 of 12-13years, 99 of 15-16years) who were 1st grade students of two boy's middle schools and two high schools and from 186 females(87 of 12-13years, 99 of 15-16years) who were 1st grade students of two girl's middle schools and two high schools. Laboratory procedures included a hemoglobin, hematocrit, serum iron, TIBC and serum ferritin. Transferrln saturation was calculated. RESULTS: 1) Mean values were hemoglobin 13.7+/-0.78 g/dl, hematocrit 40.6+/-2.47%, serum iron 127.6+/-43.02 ug/dl, TIBC 387.9+/-47.73 ug/dl, transferrin saturation 33.3+/-11.38%, and serum ferritin 26.1+/-16.90 ng/ml in 12-13years old males. 2) Mean values were hemoglobin 13.7+/-0.85 g/dl, hematocrit 40.9+/-3.79%, serum iron 99.4+/-34.93 ug/dl, TIBC 387.8+/-45.53 ug/dl, transferrin saturation 26.2+/-9.99%, and serum ferritin in 22.7+/-14.18 ng/ml in 12-13years old females. 3) Mean values were hemoglobin 15.2+/-0.84 g/dl, hematocrit 45.9+/-2.77%, serum iron 144.9+/-51.97 ug/dl, TIBC 419.8+/-52.46 ug/dl, transferrin saturation 34.2+/-10.44%, and serum ferritin 31.2+/-20.91 ng/dl in 15-16years old males. 4) Mean values were hemoglobin 13.2+/-0.89 g/dl, hematocrit 39.9+/-2.78%, serum iron 111.1+/-39.78 ug/dl, TIBC 392.8+/-50.06 ug/dl, transferrin saturation 28.9+/-10.85%, and serum ferritin 21.4+/-17.11 ng/ml in 15-16years old females. 5) The prevalence of iron deficiency was 8.9% and 10.1% in 12-13years and 15-16years old males respectively. In 12-13years and 15-16years old females, the prevalence was 16.1% and 29.3% respectively. The prevalence of iron deficincy anemia was 1.1% and 2.0% in 12-13years and 15-16years old female and absent in male. CONCLUSIONS: Although hemoglobin is normal, we recommend to check other parameters to optimize the identification of individuals with iron deficiency.

3.Three Cases of Baboon Syndrome.

The Baboon syndrome is used to denote a characteristic distribution pattern of systernic allergic contact dermatitis. Diffuse erythema of the buttock, upper inner surface of the thighs and axillae are characteristic features. We report three cases of Eaboon syndrome developed after contact with mercury through breaking a clinical thermometer in a 30 year-old, 16 year-old, 32 year-old females, respectively. Mercury sensitivity was confirmed by a patch test in all three patients.

5.A 10 Year Period ( 1984 - 1993 ) of Clinical Observation of Cutaneous Malignant Tumors.

BACKGROUND: Skin cancers arc the most common malignancies in human. The increasing incidence of skin cancer has been attributed to increasing exposure of the papulation to sunlight and other factors, such as chemicals or. viruses. OBJECTIVE: The purpose of this study is to investigate a recent 10 year period(1984-1993) of clinical observation of cutaneous malignant tumors. METHODS: A statistical study was made with 60 cases of malignant tumors that visited the Departrnent of Dermatology, Chung Ang University during a 10 year period(1984-1993). RESULTS: 1. The mean incidence of cutlneous malignant tumors in the total number of outpatients was 0.16%(60/36,416). The incidence of malignant turnors has tended to increase in the last 4 years. 2. The age of onset was quite varied but the majority of cases, 51 cases(85.0%), were over 40 years old. The ratio of rnen to women was 1.3: 1.3. Among the 60 cases of malignant cutaneous tumors, the most frequently observed tumor was basal cell carcinoma, 20 cases(33.3% ). The next most frequent tumors were squamous cell carcinoma, 9 cases(15.0%), metastatic carcinoma, 9 cases(15.0%) and malignant melanoma, 6 cases(10.0%). 4. Among the 20 cases of basal cell carcinoma, 19 cases(95.0%) were over 40 years old. The most frequent age was the 7th decade(30.0%). The most common predilection sites were the nose (30.0%), the cheek(20.0%) and the eyelid(10.0%). 5. Among the 9 cases of squamous cell carcinoma, 8 cases(88.9% ) were over 40 years old. The ratio of men to women was 1: 2. The most frequent age was the 8th decade(44.4% ). The most common predilection sites were the head(55.6%), lower extremity(22.2%), upper extremity (11.1%) and trunk(11.1%). 6. All patients with malignant melanoma were over 40 years old. The most frequent age was the 5th decade(50.0%). The most cornmon predilection sites were the upper and lower extremities (66.7% ). 7. Among the 9 cases of metastatic carcinoma, 7 cases(77.8%) were over 40 years old. The ratio of men to women was 1: 1.25. The most frequent primary tumor was stomach cancer in men and lung cancer in women. 8. Cutaneous malignant tumor. usually had no detectable metastasis but there were 2 cases of extramammary Pagets disease with metastasis in the regional lymph nodes. CONCLUSIONS : This study revealed that the incidence of malignant tumors has tended to increase over the last few years, the most frequent age was over 40 years old, the most observed turnor was basal cell carcinoma and any type of cutaneous malignant tumors was rarely metastasis.

6.Malignant Solitary Fibrous Tumor of the Pleura in Mediastinum.

Solitary fibrous tumors (SFTs) most often involve the pleura and also may encompass the peritoneum and nonserosal sites. They occur as solitary encapsulated tumors and pursue a relatively benign clinical course. The usual criteria for malignancy are high cellularity, mitotic activity (more than 4 per 10 high-power fields), cellular pleomorphism, hemorrhage and necrosis as well as infiltrative growth. We report a case of malignant SFT of pleura who presented with an anterior mediastinal mass. Grossly, it was a 10x8x6.5 cm sized, encapsulated and well-demarcated, solid neoplasm with areas of extensive necrosis. Microscopically, parallel or haphazard arrangement of spindle cells with variable degrees of collagenous background were noted. Storiform fascicle formation, hemangiopericytoma-like pattern, and epithelioid cell clusters were often intermingled. Nodular areas with high cellularity and mitotic activity (> or =10/10 HPFs) were scattered throughout the neoplasm, however no definite cellular pleomorphism was encountered. Tumor cells were immunoreactive for vimentin and CD-34, which distinguished them from the mesothelial cells. Electron microscopically, they revealed fibroblastic and myofibroblastic differentiation.

7.Chronic Maxillary Sinusitis Occurred after Reconstruction of Inferior Orbital Wall Fracture.

A 27-year-old man presented with symptoms of chronic postnasal drip and nasal obstruction, which had begun one year earlier. He had undergone an ophthalmologic operation due to inferior orbital wall fracture 12 years ago. During physical examination, mucopurulent discharge was observed from the right middle meatus. On the paranasal sinus computed tomography, a sheet-like foreign material was observed inside the right maxillary sinus and haziness was found in the right maxillary, ethmoid, and frontal sinuses. Endoscopic sinus surgery was performed and a silastic sheet, which had been inserted during previous surgery, was removed. After surgery, the patient's symptoms disappeared gradually. We report a case of maxillary sinusitis that occurred after orbital wall reconstruction with a silastic sheet and discuss about the efficacy of the silastic implant.

8.Analysis of Medical Care by Telemedicine.

Telemedicine is a health care system that contains new-media techniques and medical special knowledge, using information techniques such as audio, moving picture, and communication provided to patients in distant areas with medical service such as diagnosis, consultation, education. Especially during medical transportation, it is possible that transferred patient can be rapidly taken care of by telemedicine. Our study reports 65 cases between a doctor in a hospital on ground and a patient during medical evacuation by telephone, e-mail and facsimile. Sixteen of 65 cases (25%) were immediately transferred to a hospital by sailing or helicopter. Other 49 cases were successfully taken care of by telemedicine.Telemedicine is economic and reasonable solving medical problems that may occur during medical evacuation where access to medical facilities is impossible.

9.A Study on Factors and Correlations of Burnout and Job Satisfaction of Dietitians in Hospitals.

This study aimed to examine factors and the correlations of burnout and job satisfaction in hospital dietitians. Burnout was classified into three sub-concepts: emotional exhaustion, depersonalization, and reduced personal accomplishment. Job satisfaction was also classified into three sub-concepts, including relationships, self-realization, and advancement opportunities. A survey was conducted with dietitians working in general hospitals of over 400 beds, in the Seoul metropolitan area. The factors that significantly influenced emotional exhaustion were health, work hours, number of intern dietitians, self-esteem, level of responsibility, while the factors that influenced depersonalization were relationships with coworkers, and role ambiguity. Personal accomplishment was influenced by age, the number of staff dietitians, the general number of managed workers, and self-esteem. In terms of job satisfaction, relationship was affected by age and relationships with supervisors, while self-realization was influenced by work hours, health, the number of patient meals, and role ambiguity. Age, job regularity, business management, role ambiguity, and relationships with supervisors also affected advancement opportunities. Emotional exhaustion, depersonalization, and reduced personal accomplishments, which represented burnout, had negative relationships with satisfaction with relationships, self-realization and advancement opportunities. Dietitians with low job satisfaction were likely to experience burnout. These results suggest that burnout and job satisfaction are influenced by internal and external factors that are individual or organizational problems encountered by hospital dietitians. Therefore, managers as well as dietitians need to understand these factors and overcome them through communication and mutual support.

10.Primary Malignant Lymphoma of Lung: A Case Report.

Primary malignant lymphoma of lung is a very rare disease, only 0.34% of all malignant lymphomas. In our case, a 76 year old male patient had a solitary mass without lympha-denopathy at chest CT scan. He underwent right middle lobectomy through a posterolateral thoracotomy incision. Pathologic study confirmed a diagnosis of malignant lymphoma and chemotherapy was started by an oncologist.
