Congenital absence of the lower sternum, defect of the abdominal wall, defects of the anterior diaphragm, pericardial defects and cardiac anomalies had been described as a syndrome by Cantrell et al. Developing mechanism of the syndrome was embryologically defined. These defects arise apparently from combined anomalies of developing of the dorsal mesoderm. The authors present two cases of Cantrell's syndrome developed in one day old and eight months old female patient in which angiocardiography was done at the Department of Radiology, Seoul National University Hospital.

Leukemia is the commonest malignancy in childhood, accounting for about one-third of all malignancies in thosebelow the age of 15 years. Authors analysed the chest PA findings of 177 cases of leukemia in childhood below theage of 15 years, diagnosed at Seoul National University Hospital in recent 5 years since 1978. The summaries ofthe analysis are as follows; 1. Among 177 cases, 121 cases were male and 56 cases were female. The malepredominant than the female with the ratio of 2.2:1. In 177 cases, 77% of the patients were under the 10 years ofage. 2. The incidence of subtype of leukemia was as follows; The acute form was 158 cases (89%), acute lymphocyticleukemia, 55%, acut myelocytic group, 31.6%, chronic myelocytic type, 10%. And acute lymphocytic leukemia was61.4% of acute form. 3. The incidence of abnormality in chest PA was 32.2%(57 cases) . Among these 57 cases,abnormality in lung field was 35 cases (19.8% of total patients) hilar and mediastinal lymph nodes enlargement was19 cases(10.7%). Pleural effusion was 13 cases (7.3%) and cardiac shadow enlargement in chest PA was 3 cases.

OBJECTIVES: The purpose of this study was to investigate recognition, dietary attitude and education needs for reducing sodium intakes of dietitian at customized home visiting health service (CHVHS). METHODS: The subjects were 75 dietitian at CHVHS. We investigated several variables (recognition, dietary attitude, education needs for reducing sodium intakes) and determined sodium intakes level of subjects as 'low', 'middle' and 'high' by Dish Frequency Questionnaire 25 (DFQ 25). Also, we assessed the differences in recognition, dietary attitude, sodium intake level and education needs by dietitian career period (under 3 yrs vs. over 3 yrs) at CHVHS. RESULTS: In recognition related reducing sodium intake, they showed 'checking a sodium content in nutrition labeling' score 2.5/4.0 and 'perception difference between sodium and salt' score 3.1/4.0. There was no difference in the recognition between under 3yrs' group and over 3yrs' group. In dietary attitude related reducing sodium intake, they showed 'palatability for salty taste' score 0.8/1.0, 'attitude in related soups' 0.7/1.0, 'attitude in related using natural spice' 0.6/1.0. There was a difference in 'attitude in related soups' between under 3yrs' group and over 3yrs' group (0.6 vs. 0.7). In sodium intake level by DFQ 25, they showed 'low group' 41.3%, 'middle group' 41.3% and 'high group' 17.3%. There was no difference in the distribution of sodium intake level by the career. In education needs related reducing sodium intakes, there were 'teaching experience' 93.3%, 'have a difficulty in teaching about reducing sodium intakes' 86.6%, and 'necessity of education for CHVHS dietitians' 100.0%. 'Needed education contents for CHVHS dietitians' were ranked as 'cooking way to reduce sodium intake' 58.7%, 'relation between hypertension and sodium' 17.3%, 'composing way to reduce sodium intake' 17.3%. There was a difference in needed education contents 'relationship between hypertension and sodium' (33.3% vs. 2.6%) and 'The cooking way to reduce sodium intake' (38.9% vs. 76.9%) by the career. CONCLUSIONS: The results suggested that a capacity training program for reducing sodium intake may be needed for dietitians at CHVHS to improve health of the community elderly. For effective training program related reducing sodium intake for dietitians at CHVHS, it may be necessary to consider the career period as dietitians at CHVHS.

PURPOSE: We evaluated the surgical outcomes and complications of new pediatric intraocular lens in congenital cataract. METHODS: A retrospective study was carried out on 29 eyes of 22 patients diagnosed with congenital cataract that underwent optic capture following lens emulsification. New pediatric intraocular lens (LJC 575A, Lucid Korea, Seoul, Korea) implantation into the bag was performed with posterior capsulorrhexis and anterior vitrectomy. RESULTS: After follow-up of 17.79+/-7.08 months, 15 eyes showed a visual acuity of 0.5 or above, and 10 (71.4%) of the 14 eyes showed a visual acuity of 20/470 or above. As for complications, posterior synechia developed in 8 eyes, exudative membrane was found in 3 eyes, iris capture by IOL haptic was found in 1 eye, and slight opacification of posterior capsule was identified in 1 eye. CONCLUSIONS: Use of new 8 mm pediatric intraocular lens in congenital cataract may be considered selectively.

Fourteen cases of double chambered right ventricle were diagnosed angiographically and of these nine caseswere confirmed after operation and autopsy at Seoul National University Hospital in recent four years since 1979.The clinical and radiological findings with the emphasis on the cinecardiographic findings were analysed. Thesummaries of the analysis are as follows; 1. Among 14 cases, 6 cases were male and 8 cases were female. Agedistribution was from 4 years to 36 years. 2. In chest X-ray findings, pulmonary vascularity was increased in 8cases, decreased in 4 cases, and nomral in 2 cases. Cardiomegaly was observed in 8 cases and other showed normalheart size. 3. In cinecardiography, 11 cases had interventricular septal defect. Among these 11 cases, VSD locatedin proximal high pressusre chamber was in 2 cases and located in distal low presssure chamber was in 9 cases. 4.The location of aberrant muscle bundle in sinus portion of right ventricule was in 8 cases. In the rest cases, andaberrant muscle bundle was located below the infundibulum of right ventricle. 5. For accurate diagnosis anddifferential diagnosis with other congenital cardiac anomalies such as Tetralogy of Fallot or isolated pulmonicstenosis, biplane cineangiography and catheterization is an essential procedure.

STATEMENT OF PROBLEM: Within the elastic limit of the screw, the greater the preload, the tighter and more secure the screw joint. However, additional tensile forces can incur plastic deformation of the abutment screw when functional loads are superimposed on preload stresses, and they can elicit the loosening or fracture of the abutment screw. Therefore, it is necessary to find the optimum preload that will maximize fatigue life and simultaneously offer a reasonable degree of protection against loosening. Another critical factor in addition to the applied torque which can affect the amount of preload is the joint connection type between implant and abutment. PURPOSE: The purpose of this study was to evaluate the influence of tightening torque on the implant-abutment screw joint stability. MATERIAL AND METHODS: Respectively, three different amount of tightening torque (20, 30, and 40 Ncm) were applied to implant systems with three different joint connections, one external butt joint and two internal cones. The initial removal torque value and the postload (cyclic loading up to 100,000 cycles) removal torque value of the abutment screw were measured with digital torque gauge. Then rate of the initial and the postload removal torque loss were calculated for the comparison of the effect of tightening torques and joint connection types between implant and abutment on the joint stability. Results and CONCLUSION: 1. Increase in tightening torque value resulted in significant increase in initial and postload removal torque value in all implant systems (P < .05). 2. Initial removal torque loss rates in SS II system were not significantly different when three different tightening torque values were applied (P > .05), however GS II and US II systems exhibited significantly lower loss rates with 40 Ncm torque value than with 20 Ncm (P < .05). 3. In all implant systems, postload removal torque loss rates were lowest when the torque value of 30 Ncm was applied (P < .05). 4. Postload removal torque loss rates tended to increase in order of SS II, GS II and US II system. 5. There was no correlation between initial removal torque value and postload removal torque loss rate (P > .05).

For treatment of partially edentulous patients, a treatment using implant is widely used. Treatment method using implant are implant fixed prostheses and removable partial dentures, and for patients with severe bone resorption, removable implant overdenture with the effects of aesthetic and reducing cost can be used as treatment options. Specially, prosthesis with milled-bar and attachment has the effect of being splinted between implant fixtures, higher retention and stability than conventional removable partial denture. And it has the effect of improvement of aesthetic through lip support by denture base. In this case, the patient with severe alveolar bone resorption and partial edentulous maxilla and mandible was treated by implant-assisted removable partial denture using Milled-bar and ADD-TOC attachment. The esthetic was improved by removing the clasp because of effects of additional retention by using the attachment, and reducing palatal coverage of implant-assisted removable partial denture. The clinical results were satisfactory on the aspect of aesthetic and masticatory function.

STATEMENT OF PROBLEM: Restoring and replacing teeth with fixed prostheses commonly used in dental practice. Because of improper oral hygiene care and inaccurate laboratory procedure, complications of fixed prostheses were found in the mouth of patients. Although many efforts have been continually made to obtain the data of long term prognosis of fixed prostheses, it was difficult to do it. PURPOSE: The purpose of this study was to evaluate the clinical status of fixed prostheses. MATERIAL AND METHODS: In order to assess the clinical status of fixed prostheses, a total of 161 individuals (aged 17-85, 99 women and 62 men with 1596 unit of fixed prostheses, and 1169 abutments) who first visited the Department of Prosthodontics, Pusan National University Hospital, between April to September, in 2007 were examined. RESULTS AND CONCLUSION: The results of this study were as follows: 1. Length of service of fixed prostheses was 8.6+/-0.6 years (mean), 10.0 years (median). 2. Location of fixed prostheses was found to have no statistically significant influence on longevity of fixed prostheses (P > .05). But, the success rate was high in posterior region and in mandible where the failure rate was high in combination (P < .05). 3. Longevity of fixed prostheses made of metal was longest (mean: 13.0+/-9.3, median: 14.0), gold, precious ceramic, non-precious ceramic trailing behind (P < .05). 4. Number of units in fixed prostheses was found to have no statistically significant influence on longevity of fixed prostheses (P > .05). But, the success rate was high in Single-unit and the failure rate was high in over 3-unit (P < .05). 5. Condition of opposing dentition was found to have no statistically significant influence on longevity of fixed prostheses (P > .05). But, the success rate was high in natural dentition (P < .05). 6. Defective margin (28.2%), dental caries (23.0%), periodontal disease (19.3%), periapical disease (16.9%) were frequent complications. In 30.1% of the cases, abutment state after removing fixed prostheses was needed to be extracted.

PURPOSE: The purpose of this study was to investigate the effect of size and shape of retention element on the bond strength of indirect composite resin and metal. MATERIAL AND METHOD: The metal disk specimens, each 6mm in diameter, were cast from Cr-Co alloy. They were divided into 8 groups by applied retention element: retention bead group B2 (phi 0.2mm), B4 (phi 0.4mm), B6 (phi 0.6mm), B8 (phi 0.8mm), retention crystal group C2 (0.2mm), C5 (0.5mm), C8 (0.8mm), and sandblasting group SB (110micrometer Al2O3 blasting) as control. Eighty-eight metal specimens were veneered with TESCERA(R) Indirect resin system. One specimen of each group was sectioned and the resin-metal bonding pattern at the interface was observed under measuring microscope. Other specimens were then tested for tensile bond strength on an Instron universal testing machine at a crosshead speed of 2mm/min. RESULTS: 1. Compared to sandblasting, beads or crystals increased the resin-metal bond strength (P<.05). 2. 0.2mm retention crystals were most effective in improving the resin-metal bond strength (P<.05). 3. 0.2mm beads showed the highest bond strength among retention bead groups, but there was no statistically significant difference (P>.05). 4. Retention crystals tend to be higher in bond strength than retention beads due to wider surface area. 5. The larger retention element, the larger the undercut for the mechanical retention, but the gap at resin-metal interface was also increased. CONCLUSION: Within the limitations of this study, 0.2mm retention crystals were most effective in improving the resin-metal bond strength.

Tricho-rhino-phalangeal syndrome (TRPS) is a hereditary disorder characterized by craniofacial and skeletal abnormalities. A mutation of the TRPS1 gene leads to TRPS type I or type III. A 20-year-old male patient visited our neurologic department with chronic fatigue. He presented with short stature, sparse hair, pear-shaped nose, and brachydactyly. Radiologic study showed short metacarpals, metatarsals with cone-shaped epiphyses, hypoplastic femur and hip joint. Panel sequencing for OMIM (Online Mendelian Inheritance in Man) listed genes revealed a de novo heterozygous frameshift mutation of c.1801_1802delGA (p.Arg601Lysfs*3) of exon 4 of the TRPS1 gene. The diagnosis of TRPS can be challenging due to the rarity and variable phenotype of the disease, clinicians should be aware of its characteristic clinical features that will lead a higher rate of diagnosis.
