2.Primary subacute Osteomyelitis in Children: A Retrospective Review

In our review of 36 hematogenous osteomyelitis nine cases were subacute form. Distal tibia was the most commonly involved location(four of nine patients). According to the modified Robert's classification cross-physeal lesions were the most common type(four of nine patients). In most of the cases the clinical findings and laboratory data were not much helpful for current diagnosis. Four patients had radiographic findings similar to those of tumors such as Ewing' s sarcoma, osteoid osteoma or bone cyst. Magnetic resonance imaging was useful in the identification of early lesions. In three patients Staphylococcus aureus was isolated by bacterial culture of biopsy specimen. In the remaining six patients diagnosis was made by histological evaluation. All patients except one were treated by curettage and antibiotics. In the other patient the disease showed a tendency of spontaneous healing without operative intervention and antibiotics. Cross-physeal lesion seemed to be a typical finding of the subacute osteomyelitis.

3.Congenital coronary arteriovenous fistulae

Six cases of congenital coronary arteriovenous fistulae were diagnosed radiographically and confirmed afteroperation at Seoul National University Hospital in recent three and a quarter years since 1979. The clinical andradiological findings with the emphasis on the cinecardiographic findings were analysed. The summaries of theanalysis are as follows; 1. Among 6 cases, 2 cases were male and 4 cases were female. Age dsitribution was from 3months to 19 years. 2. In chest x-ray findings, 5 cases had cardiomegaly. Among these 5 cases, increase ofpulmonary vascularity was in 3 cases. Chest X-ray finding of the rest 1 case was within normal limit. 3. Thecoronary arteriovenous fistula orginated from the right coronary artery in 5 cases and from the single left coronary artery in 1 case. In 5 cases, a coronary arteriovenous fistula drained into the right ventricle and in 1case, a fistula from a SA node branch of the right coronary artery into the right atrium. 4. In 5 cases, a mainright coronary artery was dilated. Among these 5 cases, aneurysmal dilatation of main right coronary artery was in2 cases, saccular aneurysmal dilatation of distal portion of right coronary artery was in 2 cases and in 1 case,diffuse dilatation without aneurysm formation of a main right coronary artery. In 1 case, SA node branch of theright coronary artery was dilated with aneurysm formation. 5. For accurate diagnosis of congenital coronaryarteriovenous fistula, biplane cinecardiography is an essential procedure.

4.Percutaneous Transluminal Angioplasty of Subclavian Artery: Case Report.

Percutaneous transluminal angioplasty(PTA) were performed in three patients with atherosclerotic stenosis of subclavian arteries. The arteries were successfully dilated without complications during the procedure. All patients were asymptomatic during follow-up periods ranging from eight months to fifteen months after PTA.

5.Sunitinib-induced hypothyroidism.

Sunitinib is an oral multi-targeted tyrosine kinase inhibitor that is effective for advanced renal cell carcinoma and imatinib-resistant gastrointestinal stromal tumor. Currently, sunitinib is being investigated in multiple other tumor types including non-small cell lung cancer, breast cancer, colon cancer, and endocrine tumors. Recent studies have shown that sunitinib induces thyroid dysfunction, mainly hypothyroidism during treatment. The incidence of sunitinib-induced hypothyroidism has been reported to be 36~85%. Although several hypotheses have been suggested, the mechanisms by which sunitinib induces hypothyroidism are not clear. As considered the fairly high incidence of sunitinib-induced hypothyroidism, thyroid function should be regularly monitored in all patients treated with sunitinib.

6.Correlation between Spontaneous Nystagmus and Vestibular Function Test Parameters according to Gender and Age in Vestibular Neuritis .

BACKGROUND AND OBJECTIVES: There are caloric test, SCEP, DP in Step velocity and VOR asymmetry in SHA test as parameters for evaluation in unilateral peripheral vestibulopathy in parameter of vestibular function test. The aim of this study is to assess the correlation between these parameters and intensity of spontaneous nystagmus in vestibular neuritis. MATERIALS AND METHOD: We evaluated 75 patients who were diagnosed as vestibular neuritis with 3 degree spontaneous nystagmus at vestibular function test lab from January 2000 to July 2005 in Dankook university hospital. The correlations between direction and intensity of spontaneous nystagmus and monothermal caloric test, SCEP and DP in step velocity, VOR asymmetry (0.01, 0.04, 0.16 Hz) in SHA test were analysed. And correlations according to sex, age were also analysed. RESULTS: The correlation coefficient between intensity of spontaneous nystagmus and monothermal caloric test was 0.60. The correlation coefficient between intensity of spontaneous nystagmus and SCEP was 0.35. The correlation coefficient between intensity of spontaneous nystagmus and Tc DP was 0.36 and it showed no significant correlation. The correlation coefficient between intensity of spontaneous nystagmus and VOR asymmetry (0.01, 0.04, 0.16) was 0.57, 0.46, 0.51, respectively and there was higher correlation in man, age under 60. CONCLUSION: Intensity of spontaneous nystagmus showed close relations to monothermal caloric test, VOR asymmetry of SHA, Tc DP, SCEP DP in sequence. In addition, young male patients tends to be closely related to intensity of spontaneous nystagmus.

Systemic lupus erythematosus, an autoimmune disease with multisystem involvement, has been reported to be associated with a number of gastrointestinal complications and symptoms such as nausea, vomiting, and abdominal pain. However, acute pancreatitis only rarely has been reported as a complication of SLE. We report a case of SLE presenting drug unrelated acute pancreatitis as a initial manifestation.

9.Relationship between HPV Infection and bcl-2 Protein Expression and Apoptosis in Invasive and In Situ Squamous Cell Carcinoma of the Uterine Cervix.

Human papillomavirus (HPV) 16/18 is a causative agent of uterine cervical carcinoma. HPV 16/18 can alter cell cycle regulation through apoptosis. Bcl-2 is an important regulatory gene of apoptosis. A study was done to evaluate the relation between HPV 16/18 and bcl-2 and apoptosis in 21 cases of carcinoma in-situ (CIS), 5 cases of microinvasive carcinoma and 23 cases of invasive squamous cell carcinoma. HPV 16/18 was detected by hybrid capture system (HCS), bcl-2 protein by immunohistochemical method and apoptosis by using the hematoxylin-eosin stained slide. The results were as follows: Expression of the bcl-2 protein was 43% (9/21) in CIS and 26% (6/23) in invasive carcinoma. Expression of the bcl-2 protein was 42% (5/12) in CIS with HPV 16/18 infection, 44% in CIS without HPV 16/18 infection, 20% (2/10) in invasive carcinoma with HPV 16/18 infection and 31% (4/13) in invasive carcinoma without HPV 16/18 infection. Mean apoptotic index (mAI) was 3.36 in CIS, 5.23 in microinvasive and 6.25 in invasive carcinoma. mAI was 3.66 in CIS with HPV 16/18 infection, 2.86 in CIS without HPV 16/18 infection, 6.18 in invasive carcinoma with HPV 16/18 infection and 6.30 in invasive carcinoma without HPV 16/18 infection. Based on these results, we conclude that there are no correlation between HPV infection and bcl-2, and between HPV infection and apoptosis in invasive and in situ carcinoma of the uterine cervix, and apoptosis is increased according to tumor progression.
