We recently encountered two cases of emphysematous pyelonephritis with abscess formation, which have recovered by only conservative treatment and percutaneous abscess drainage(PAD). Two patients had diabetes mellitus. in the first case, initial CT showed left intrarenal abscess with air-fluid level. In the second case, CT showed right perirenal abscess with air-fluid level. PAD was performed under CT and US guidances, respectively. Both patients were successfully treated with PAD and conservative treatment. Follow up after confirmation of abscess resolution by CT scan and contrast study through the catheter, we removed catheters two months and 40 days after PAD respectively.

PURPOSE: To implement evidence-based nursing, it is important to know where and how to find the best available evidence. This study was conducted to identify the results of a search from Ovid MEDLINE and to compare the results from Ovid MEDLINE with those from PubMed MEDLINE. METHODS: Four different approaches via Ovid MEDLINE were used to search for guidelines on preventing catheter-associated urinary tract infections. Outcomes of this study were the number of records and relevant literature, and the sensitivity and precision of the search methods via Ovid MEDLINE. RESULTS: The number of retrieved items ranged 23 to 6,005 and that of relevant studies, 5 to 8 of 8. Simple searches resulted in the highest sensitivity of 100.0%. When using MeSH terms and limits feature, the precision was highest (21.7%) among four approaches for literature searches. Simple searches in Ovid had higher sensitivity and lower precision than those in PubMed. CONCLUSION: Simple searches in Ovid may be inefficient for busy clinicians compared to PubMed. However, to ensure a comprehensive and systematic literature search, using Ovid MEDLINE in addition to PubMed is recommended.

Acute necrotizing encephalopathy is a recently established disease entity, proposed by Mizuguchi et al in 1995, that shows a characteristic symmetric and multifocal involvement of both thalamus, brainstem tegmentum, cerebral periventricular white matter, and cerebellar medulla. It is known to be prevalent in Japan and other Far Ease countries. The etiology of the acute necrotizing encephalopathy remains unknown. The typical course of acute necrotizing encephalopathy is the development of the irreversible neurologic symptoms related to brain lesions. The diagnosis can be made on the basis of the combination of a typical clinical profile and characteristic radiologic findings. We experienced a first case of acute necrotizing encephalopathy in a 9 month old boy in Korea. We report this case with the brief review of related literatures.

PURPOSE: The purpose of this study was to identify the changes in body weight, anxiety, depression, and quality of life in patients during a one month period following a radical gastrectomy. METHODS: Data were collected from 30 patients with gastric cancer from July 2, 2012 through December 31, 2012, at C University Hospital, Gwangju, South Korea. The body weight, anxiety, depression, and quality of life (QoL) of the patients were measured using structured questionnaires the day before surgery, day 7 postoperative, and 1 month postoperative. RESULTS: There were significant changes in the average body weight and the general health status/QoL, the functionality, and the symptoms of general QoL, and the functionality and the symptoms of specific QoL during the postoperative 1 month period (chi2=52.47-7.68, p<.001-.022). CONCLUSION: Oncology nurses should assess the nutritive conditions of gastrectomy patients carefully to avoid malnutrition during the postoperative 1 month period. Also, as the general and specific quality of life changes during the postoperative 1 month period, adequate support and sufficient attention should be provided to these patients to minimize the negative influence on quality of life, and to assist them in recovery to the preoperative state.

PURPOSE: To investigate the frequency of breakfast skipping and affecting factors in children in poverty. METHODS: The subjects were 644 children who were receiving the services of community child center or delivery meal-box. They were asked about their socio-demographic characteristics, health behaviors and breakfast-skipping status in the survey. RESULTS: Breakfast-skippers were 17.4% of the subjects. The count of negative health behaviors in breakfast skippers was higher than that in eaters, and the behaviors of exercise, screen time, alcohol experience, and smoking experience were worse in skippers than in eaters. The odds ratio of breakfast skipping was higher in female children (OR=2.53, p<.001), those with single father (OR=3.76, p<.001) or single mother (OR=1.83 p=.030), those with alcohol experience (OR=1.82, p=.022) and smoking experience (OR=2.80, p=.003), and under-weight ones (OR=2.54, p=.048). CONCLUSION: Breakfast skipping is associated with gender, family structure, alcohol experience, smoking experience and BMI. Children in poverty need comprehensive health promotion programs including interventions for breakfast skipping and health behaviors.
