2.Clinical pharmacokinetics of intravenous propofol

In order to investigate the pharmacokinetics of propofol, 8 adult patients, ASA grade Ⅰ-Ⅱ, scheduled for elective operation under general anesthesia,served as the subjects. After the injection bolus of propofol 2mg?kg~_(-1) was given, the venous blood samples were taken at various times to measure the plasma concentrations of propofol by high performance liquid chromatography. The data were analysed using PKBP-NI program. The results showed that the disposition of propofol was mathematically described by three compartment open motile, the distribution was rapid and extensive with slow balance among three compartments,and the eliminative duration was short. It is indicated that propofol is applicable for both induction and maintenance of anesthesia.
3.Characteristics of Gesell Developmental Schedules in Children with Movement Disorders

Objective To investigate the incidence and structure of mental disability in children with movement disorders. Methods 157 children with movement disorders (103 with cerebral palsy and 54 with mental retardation) were assessed with Gesell developmental schedules,and mental disability was identified as development quotient (DQ)<75. Results The incidence of intelligence disability was 92.2% in children with cerebral palsy, including 91.2% in spastic type, and 100% in dyskinetic, mixed or dystonic type. The development of gross motor was retarded in children with spastic cerebral palsy, and gross and fine motor in children with dyskinetic cerebral palsy, compared with those with mental retardation. Conclusion It is important to focus the mental development in children with movement disorders, especially the dyskinetic cerebral palsy. Gesell developmental schedules should be used carefully to assess the mental development in children with movement disorder.
4.Evaluation of teaching-in-English reform in five-year clinical tropical medicine program-Case analysis of curriculum reform of clinical medicine in Hainan Medical University

Objective:To investigate the model,quality as well as effect of teaching-in-English in five-year clinical medicine program of Hainan Medical University.Methods:The questionnaire was carried out among clinical medicine undergraduates of 2012-2015 grades in Hainan Medical University,to investigate studying time,studying habits and the impact of teaching in English.Additionally results of CET-4,CET-6 and overseas internship from undergraduates of 2012-2015 grade,as well as the result of phased medical licensing examination and post-graduate entrance examination from undergraduates of 2012 were accordingly collected from the Teaching Management Department.Results:For the Chinese students in international classes,the average time of self-study was 161.49 min,58.3% had preview before classes,and 90.7% had habit of review after classes.Thus the first time pass rate,total pass rate,first time excellent rate and total excellent rate of CET-4 and CET-6 of international classes were significantly higher than those of regular classes.The result of post-graduate entrance examination in 2016 showed that the score,pass rate and acceptance rate of international classes of 2012 grade were significantly higher those of regular classes (P < 0.01).Conclusions:Teaching-in-English reform in Hainan Medical University has achieved initial success.Chinese students from international classes are superior to those from regular classes in many aspects.However,there are still many problems,and effective measures should be implemented to promote teaching quality continuously.
5.Post-marketing reevaluation for potential quality risk and quality control in clinical application of traditional Chinese medicines.

The effective quality control in clinical practices is an effective guarantee for the authenticity and scientificity of the findings. The post-marketing reevaluation for traditional Chinese medicines (TCM) focuses on the efficacy, adverse reaction, combined medication and effective dose of drugs in the market by expanded clinical trials, and requires a larger sample size and a wider range of patients. Therefore, this increases the difficulty of quality control in clinical practices. With the experience in quality control in clinical practices for the post-marketing reevaluation for Kangbingdu oral for cold, researchers in this study reviewed the study purpose, project, scheme design and clinical practice process from an overall point of view, analyzed the study characteristics of the post-marketing reevaluation for TCMs and the quality control risks, designed the quality control contents with quality impacting factors, defined key review contents and summarized the precautions in clinical practices, with the aim to improve the efficiency of quality control of clinical practices. This study can provide reference to clinical units and quality control-related personnel in the post-marketing reevaluation for TCMs.

6. Developmental toxicity of exposure to ambient fine particulate matter

Domestic and foreign studies have shown that fine particulate matter (PM2.5) pollution is a major factor affecting human health. In addition to the direct impact on the respiratory and cardiovascular systems, PM2.5 exposure during pregnancy can directly affect the growth and development of offspring, manifested as cardiopulmonary dysfunction and cognitive disorder, various adverse pregnancy outcomes, abnormal organ development, and congenital defects. However, the mechanism underlying the developmental toxicity of PM2.5 is still unclear. This article reviews PM2.5-induced developmental toxicity based on epidemiological investigation and toxicological studies, which will provide references for further studies.
7.Clinical effect of small needle -knife therapy with movement exercise on knee osteoarthritis

Objective To analyze the clinical effects of small needle -knife therapy with movement exercise on knee osteoarthritis (KOA).Methods The clinical data collected from all the patients with hospital care and other 60 patients with KOA treated in the outpatient department in People's Hospital of Changzhi from February 201 4 to August 201 4.The patients were divided into needle -knife group and hyaluronan group,30cases in each group.The needle -knife group was given small needle -knife therapy to each patient one or two times.The hyaluronan group was given weekly intra -articular hyaluronan treatment,once a week,5times in total.But all the patients must do the same exercise movement as treatment.The clinical efficacy evaluation went on 6 months to follow up the patients.The evaluation for each patient was made at 0.5,1 st,2nd,4th,and 6th months using the following measures:WOMAC (Western Ontario and McMaster University Osteoarthritis Index)global assessment and SF -36 (short form 36). Make a contrast to the efficacy of both therapies on the osteoarthritis of knee joint from pain -easing and functional recovery.And the results were synthetically analyzed.Results In terms of pain assessment,the outcomes obtained by WOMAC on the two groups was statistically effective in the phase of 1 -6 months (t =-5.952,P <0.001 ),while those by SF -36 showed differences between the first 0.5 month and 6th month (t =3.1 21 ,P <0.001 ,t =4.091 , P =0.01 1 ).In terms of the functional assessment,the figures obtained by the WOMAC manifested that the needle -knife group was significantly superior to the hyaluronan group at the second and sixth months (t =-8.579,P <0.001 ,t =-3.993,P =0.003),and SF -36 also displayed that after treatment,the objects from the two groups per-formed distinctively in the first half,first,and sixth month respectively.(t =6.1 32,P <0.001 ,t =5.063,P =0.0068, t =8.1 09,P =0.003).Conclusion Small needle knife with movement exercise can obviously improve activity -related function of knee joint and relief the pain for the patients with KOA.

9.Characteristics and risk factors of lymph node metastasis in colorectal neuroendocrine neoplasm

Objective To investigate the features and risk factors associated with lymph node metastasis in colorectal neuroendocrine neoplasm.Methods Clinical and pathological data of 63 patients with colorectal neuroendocrine neoplasm treated in Yinzhou People's Hospital between June 1997 to December 2012,were analyzed retrospectively.Comparisons of categorical data and univariate analysis of risk factors of lymph node metastasis were conducted by x2 test,multivariate analysis was conducted by Logistic regression analysis.Results Of the 63 patients the rate of regional lymph node metastasis was 30% (19/63) with 58% limited to para-intestinal lymph nodes in 11 cases,26% limited to mesenteric lymph nodes in 5 cases,and 16% limited to mesenteric root central lymph nodes in 3 cases.No metastasis exceeding central lymph nodes was observed.According to univariate analysis,tumor size,depth of invasion,ulceration in mucous membrane,invasion of lymphatic vessel and pathological grading suggested by WHO were related to regional lymph node metastasis (P < 0.01).Multivariate regression analysis revealed that tumor size,invasion of lymphatic vessel and pathological grading were independent risk factors for lymph node metastasis in colorectal neuroendocrine neoplasm (P < 0.05).Conclusions Colorectal neuroendocrine neoplasm with larger tumor size,invasion of lymphatic vessel or higher grade (G2,G3) has high risk of regional lymph node metastasis.
10.Drift correction in fMRI signal based on mathematical morphology

Objective To establish a new approach based on mathematical morphology that can effectively reduce the drifts in functional magnetic resonance imaging (fMRI) signals. Methods Based on investigation of the characteristic of drifted fMRI signals, a mathematical morphology method for baseline drift correction was presented. Results With both simulated data and real fMRI data, the results of the experiment showed that the mathematical morphology method can effectively correct the baseline drifts. Conclusion Both linear and nonlinear drifts can be removed with the proposed method without any statistical model assumption.
